A Primer on Defense Mechanisms for Psychiatry Postgraduate Residents: A Psychodynamic Overview

Darshan Dharaiya Academic Bulletin of Mental Health, 2026, Vol. 0, pp. 1-7 https://doi.org/10.25259/abmh_18_2026

Defense mechanisms, one of the core constructs of psychodynamically oriented psychotherapy and a concept with broader relevance across psychiatric practice, remain difficult to grasp for many trainees. Given their clinical utility, this article aims to provide postgraduate psychiatry residents with an overview and foundational understanding of the concept, including its historical origins, established classifications, relationship to personality organization and psychopathology, and emerging neurobiological perspectives. A detailed discussion of individual defense and their technical use in psychotherapy is beyond the scope of this manuscript. A suggested reading list is provided for further understanding. Rather than serving as a treatment guide or manual, this article is intended as a sensitizer — an orientation toward understanding how the mind protects itself against conflicts, and why that understanding matters clinically.

Struggling for resonance: Mediopassive interaction in a long-term treatment of severe personality disorder.

Christian Sell; Stefan Pfänder Psychotherapy, 2026 https://doi.org/10.1037/pst0000633

Although clinicians often report significant emotional challenges in psychotherapy for personality disorders, we still know relatively little about how these difficulties are enacted and reorganized in moment-to-moment patient-therapist interaction. We examine whether interactional resonance and mediopassive agency can help illuminate relational difficulties and change in a good-outcome long-term psychodynamic psychotherapy involving a patient with severe interpersonal pathology, chronic depressive symptoms, and somatic complaints. The patient met the diagnostic criteria for borderline and avoidant personality disorders, as well as depressive and somatic symptom disorders. For much of the treatment, the therapist described sessions as highly distressing.Over time, sessions were experienced as less distressing and more productive. We examined whether this shift could be accounted for by changes in how patient and therapist took up and developed each other's contributions in interaction.Following a preliminary screening of therapist reports, case notes, and session recordings, we selected 10 sessions for multimodal conversation analysis.Using exploratory conversation analysis, we specified analytic indicators for interactional resonance and then compared five high-distress sessions with five lower distress sessions.High-distress sessions showed lower interactional resonance, whereas lower distress sessions showed higher resonance. In lower distress sessions, patient and therapist more often engaged in mediopassive interactions-that is, they jointly participated in the stepwise construction of meaning across turns.Psychotherapy for severe interpersonal pathology may involve working with persistently low levels of interactional resonance that constrain spontaneous, collaborative meaning-making.The findings further suggest that a dyad's capacity for interactional resonance may increase over the course of long-term treatment. Clinical Impact StatementQuestion: How can we explain the emotional challenges that often arise in psychotherapy with patients with severe personality disorder?Findings: Sessions with subjectively high emotional distress lacked interactional resonance, while lower distress sessions showed higher interactional resonance as well as more mediopassive agency-collaborative meaning-making that is not fully predictable or controllable by either participant.Meaning: Very low interactional resonance may be linked to severe emotional difficulties in therapeutic relationships, but long-term therapy may foster increases in a dyad's capacity for resonance.Next Steps: Future research should explore how therapists can foster the development of interpersonal resonance in patients with severe personality disorder and how clinical training might better prepare clinicians to effectively recognize and work with noninterpretive mechanisms of change.

Digitally Mediated Attachment Regulation: A Conceptual Clinical Model

Tobey Gross Psychotherapy and Counselling Journal of Australia, 2026 https://doi.org/10.59158/001c.171883

Minimal digital cues–read receipts, online status indicators, and response latency–can acquire disproportionate emotional significance for individuals with high attachment vulnerability. This paper proposes a conceptual model of digitally mediated attachment regulation, developed through psychodynamic and attachment-informed clinical reflection and illustrated by three cases (H10, S40, and T89) drawn from private practice. Whereas classical attachment triggers are largely episodic, the model suggests that contemporary digital affordances may reorganise attachment dynamics towards a near-continuous state of hyperactivation. The cases illustrate a recurring five-phase sequence: cue detection, threat-biased interpretation, affective destabilisation, regulatory enactment, and transient stabilisation. The paper conceptualises the smartphone not as a transitional object under the subject’s omnipotent control, but as a structurally unstable medium that continuously reflects the uncontrollable autonomy of the attachment figure. The configuration is offered as a clinical proposal, object-bound and meaning-driven, to be distinguished from generalised anxiety, behavioural addiction, and obsessive-compulsive phenomena, and inviting empirical investigation.

When the Self Feels Threatened: Devaluation Responses in Vulnerable Narcissism, A Case Study

Aanya Rachel Vinu International Journal of Innovations in Science Engineering and Management., 2026, Vol. 5, Issue 4, pp. 164-167 https://doi.org/10.69968/ijisem.2026v5i4164-167

This case study attempts to examine vulnerable narcissism through an in depth analy-sis of one adult male participant showing narcissistic traits without a formal clinical di-agnosis. Using a blind assessment approach, the participant completed the Self Con-cept Questionnaire, the Adjustment Inventory, and the Maudsley Personality Inventory, alongside semi structured interviews and behavioral observation, allowing indirect as-sessment of self concept, adjustment, and neuroticism without revealing the study's focus on narcissism. Psychodynamic and behaviorist frameworks were used to trace the participant's developmental history, including sibling comparison, high parental crit-icism, restrictive gender norms modeled within the family, and unmet academic aspira-tions within a competitive Indian secondary education system. Findings revealed a con-sistent pattern of reflexive devaluation in response to even minor perceived threats, fre-quently triggered by unprompted comparisons to others' achievements, alongside marked deficits in empathy and pronounced beliefs about entitlement to power and control. Gender based devaluation emerged as a distinct pattern, linked to the partici-pant's family history. These findings are discussed in relation to the broader literature on vulnerable narcissism, which has been comparatively understudied relative to grandiose narcissism, particularly regarding its expression in everyday, low stakes so-cial interactions rather than major relational threats.

LA CONSTRUCTION DE L'EMPATHIE FACE AUX MUTATIONS SOCIALES CONTEMPORAINES : CONSCIENCE, EXPRESSION ET REGULATION EMOTIONNELLES

Tamo Yannick; Célestin Pierre Mboua; NKELZOK KOMTSINDI Valère International Journal of Developmental Issues in Education and Humanities, 2026, Vol. 02, Issue 04 https://doi.org/10.5281/zenodo.23469864

Abstract This article offers a theoretical contribution which is conceptual, integrative, and model-oriented in nature, in the form of a critical literature review on empathy. Grounded in a psychodynamic perspective and situated at the intersection of phenomenological, existentialist, and structuralist frameworks, it examines the prevailing humanistic challenges across several sub-Saharan African countries. These nations are currently grappling with sociopolitical shifts, marked by instability in countries such as Mali, Guinea, Burkina Faso, Chad, Sudan, Niger, the Congo, and Cameroon, as well as natural disasters, all of which undermine interpersonal and intergroup (as well as intragroup) interactions. The primary objective is to explain how it is constructed and how it can be reconceptualized, not merely as a personality trait, but as an essential emotion and a vital skill for maintaining civil harmony during times of crisis. The aim is to demonstrate how it functions within a framework of intersubjectivity, rooted in the dynamics of our connection to others, as a means of regulating the psychological and physiological distress (such as stress, anxiety, anguish, depression, trauma, panic, addictions, mania, paranoia, etc.) that arises in contexts of social tension. It is developed as an internal capacity for empathy, structured around three developmental emotional competencies: emotional awareness, which emerges in infancy and grows in complexity into adulthood with the acquisition of a "theory of mind"; emotional expression, understood as a logical mode of verbal and non-verbal socio-affective communication; and emotional regulation, defined as a set of intra- and inter-individual processes used to avoid, transform, minimize, or de-intensify emotion. In conclusion, he establishes that a failure of empathy, characterized by a lack of acceptance of others, impaired awareness, and maladaptive coping, lies at the heart of maladjustment. Conversely, emotional empathy serves a homeostatic and anxiolytic function, operating on individual, collective, and contextual levels. It acts as a cathartic catalyst rooted in sharing, reciprocity, and altruistic awareness—a necessary condition for sustaining social cohesion within an anxiety-inducing environment.
