Purpose: This study aimed to compare the effectiveness of Compassion-Focused Therapy and Existential Therapy in reducing death anxiety and increasing illness acceptance among patients with cancer in remission. Methods and Materials: This applied study used a quasi-experimental pretest-posttest-follow-up design with a control group. The statistical population consisted of patients with common cancers in remission who referred to hospitals in Tehran during 2025–2026. Using purposive sampling, 45 eligible participants were selected and randomly assigned to Compassion-Focused Therapy, Existential Therapy, and control groups, with 15 participants in each group. Both intervention groups received eight 90-minute weekly sessions, while the control group received no psychological intervention. Data were collected using the Templer Death Anxiety Scale and the Acceptance of Illness Scale. The data were analyzed in SPSS version 26 using repeated-measures analysis of variance and Bonferroni-adjusted post hoc comparisons at a significance level of 0.05. Findings: Repeated-measures analysis of variance showed significant effects of time, group, and time-by-group interaction for death anxiety and illness acceptance (p < 0.001). Bonferroni comparisons indicated that both Compassion-Focused Therapy and Existential Therapy significantly reduced death anxiety and increased illness acceptance compared with the control group (p < 0.001). Compassion-Focused Therapy produced a significantly greater reduction in death anxiety than Existential Therapy (MD = 1.79, SE = 0.41, p < 0.001), whereas Existential Therapy produced a significantly greater increase in illness acceptance than Compassion-Focused Therapy (MD = 2.30, SE = 0.46, p < 0.001). The improvements were maintained at follow-up. Conclusion: Both Compassion-Focused Therapy and Existential Therapy were effective in improving psychological adjustment among patients with cancer in remission; however, Compassion-Focused Therapy was more effective in reducing death anxiety, whereas Existential Therapy was more effective in increasing illness acceptance.

Background Helping professionals face elevated risks of compassion fatigue and burnout, yet the daily processes through which emotional transformation occurs during therapeutic interventions remain poorly understood. This study examined how participants’ daily reflections shifted across the Emotion-Focused Training for Helping Professions (EFT-HP), analyzed through three complementary theoretical frameworks: Emotion-Focused Therapy's inner dialogue model (self-criticism, self-compassion, self-protection), Compassion-Focused Therapy's three-systems framework (threat, drive, soothing), and Ekman's basic emotion theory (primary emotions). Method Two hundred twenty-five helping professionals (88.0% female; M age = 38.7 years) from healthcare, social work, psychology, and education began the 14-day online EFT-HP intervention. Primary analyses focused on the 146 participants who completed at least 9 of the 14 days (completers), whose daily written reflections yielded 7,307 coded reflection units. Reflections were analyzed with directed content analysis and frequency quantification, applying non-exclusive, AI-assisted coding within three theoretical frameworks—EFT inner-dialogue categories, CFT emotion-regulation systems, and Ekman's discrete emotions—with a verbatim textual evidence excerpt retained for every assigned code. Results Across the intervention, participants’ reflections shifted from threat- and self-criticism-related content toward soothing and self-compassion. Within the EFT inner-dialogue framework, Self-Compassion was the most frequent category (17.9% of units) and was sustained throughout (Day-13 peak 30.8%), while Self-Criticism fell from 14.3% on Day 1 to 3.9% on Day 14 (73% relative decrease) and Self-Protection peaked on Day 6 (11.3%). Within the CFT emotion-regulation framework, the soothing system was the most frequently coded system overall (31.8% of units), followed by drive (20.4%); over the 14 days the threat system declined from 35.4% on Day 1 to 7.0% on Day 14 (80% relative decrease). Within Ekman's discrete-emotion framework, specific emotions were named less frequently: Fear declined from 2.7% to 0.8%, Anger peaked on Day 6 (15.0%), Joy rose to a Day-13 peak (14.8%), and Love was comparatively prominent (11.9% overall, Day-5 peak). Conclusions Across the 14-day intervention, participants’ daily reflections showed substantial shifts in emotional content, characterized by marked self-criticism and threat reduction alongside sustained self-compassion and soothing activation. Triangulated findings across three analytical frameworks: EFT inner dialogue, CFT emotion systems, and Ekman's primary emotions, are consistent with the emotional-transformation processes proposed by EFT-HP and with findings from previous controlled trials. Because the three frameworks were applied to the same reflection data, their convergence reflects analytic (theoretical) triangulation rather than independent confirmation.

Impacts of digital third-wave interventions for adults on wellbeing and mental health outcomes: A systematic review and meta-analysis of randomized controlled trials

To synthesise the efficacy of digital third-wave process-based interventions (e.g., ACT, Mindfulness, Compassion) for improving wellbeing and ameliorating mental health symptoms in adults. A prospectively registered systematic review (OSF: 10.17605/OSF.IO/5RMU2 ) and random-effects meta-analysis of randomised controlled trials (RCTs) was conducted across six databases through January 2026. Certainty of evidence was assessed using the GRADE framework. Interventions ( k = 147; N = 26,391) yielded small-to-moderate immediate post-intervention improvements in wellbeing ( k = 46, g = 0.36), depression ( k = 110, g = 0.44), anxiety ( k = 93, g = 0.40), stress ( k = 66, g = 0.46), quality of life ( k = 32, g = 0.30), and psychological flexibility ( k = 51, g = 0.41). Heterogeneity was substantial across most outcomes ( I2 > 75%). Benefits were observed across healthy (61 trials; n = 15,770), mental health (41 trials; n = 6,130), and medical samples (46 trials; n = 4,491), with population type not significantly moderating immediate post-intervention effects. Comparator type significantly moderated several analyses, with smaller effects against active than inactive controls. Guidance did not significantly moderate immediate post-intervention effects, although it predicted larger short-term depression effects ( QM = 4.17, p = .048). At short-term follow-up, effects remained statistically supported across all outcomes, while only depression, anxiety, and stress remained statistically supported at medium-term follow-up. GRADE certainty was moderate for most immediate post-intervention outcomes and low for quality of life. Digital third-wave interventions produced small-to-moderate average benefits across symptom-focused, wellbeing-focused, and process outcomes. Effects were smaller against active comparators, indicating that pooled benefits should be interpreted in relation to comparator intensity. Future research should prioritise rigorous active controls, longer follow-up, and direct tests of whether changes in psychological flexibility mediate improvements in symptoms and wellbeing.
