The COVID-19 Pandemic as a Natural Experiment: Advancing the Understanding of Stress, Rhythm Dysregulation, and Mental Health in Healthcare Workers

Noemi Maria Mereu; Roberto Demontis Clinical Practice and Epidemiology in Mental Health, 2026, Vol. 22, Issue 1, pp. e17450179512031 https://doi.org/10.2174/0117450179512031261020111641

Few events in recent history have forced clinicians and researchers to rethink mental health quite like COVID-19. In this commentary, we reflect on something that struck us while reading the work of Cantone et al. : the pandemic, for all its devastation, created a kind of unplanned observational window, a chance to study, at population scale, what happens when stress and rhythm disruption hit at the same time. Healthcare Workers (HCWs) were at the epicentre of this. They didn't just witness the crisis; they absorbed it, shift after shift, for months. What the data show is that their depressive symptoms and rhythm instability were not random; they appear systematically linked, and at levels one would more typically associate with mood disorder research. We discuss these findings through the lens of DYMERS (Dysregulation of Mood, Energy, and Social Rhythms Syndrome), a heuristic transdiagnostic construct that, in our view, captures something that standard categorical diagnoses tend to miss: the subthreshold, fluctuating psychopathology that sits just below the threshold of formal diagnosis but causes real impairment. The pandemic may not have created these vulnerabilities, but it may well have sharpened our ability to see them.

Trajectories of mental health and well-being across healthcare workers during COVID-19 pandemic: a Swiss longitudinal study

Chetna Bhatia; Grégory Moullec; Sandrine Corbaz-Kurth; Laure Vieux; Hannah Wozniak; Pauline Roos; … Frontiers in Health Services, 2026, Vol. 6, pp. 1883340 https://doi.org/10.3389/frhs.2026.1883340

This longitudinal study examined the trajectories of healthcare workers’ mental health from the first to the second COVID-19 wave (May 2020–December 2020) in a Swiss tertiary hospital, and their association with individual and organizational factors. For each mental health outcome (depression, anxiety, peritraumatic stress, and well-being), a separate analysis identified two trajectories: one with higher, persistent levels of symptoms and one that was resilient, with low-level symptoms. The persistent trajectory comprised about 25%–30% of participants for anxiety, depression and peritraumatic stress and 47% for well-being; because the classifications were outcome-specific, membership differed across outcomes (about 15% of participants were on a persistent trajectory for all four outcomes and 46% resilient for all four). We found that women, nurses, caregivers, non-managerial employees, and frontline COVID-19 workers were more likely to follow a persistent trajectory. Organizational risk factors were a high workload and an irregular work schedule. These results provide a basis for implementing risk-adapted prevention measures and resilience-promotion programs to protect health workers from the impact of major crises. Future research should address the needs of vulnerable health workers to provide evidence-based health promotion.

Eldhos Jacob; S Carishma; Srishankar Bairy; Basil B Mathews; P A Ajither Cureus, 2026, pp. e118040 https://doi.org/10.7759/cureus.118040

Introduction: Polysomnography technologists perform overnight sleep studies requiring continuous patient monitoring, technical management of diagnostic equipment, and sustained attention.Although night-shift work has been associated with adverse health outcomes among healthcare workers, the occupational health burden among polysomnography technologists remains poorly characterized in India. This study evaluated sleep quality, excessive daytime sleepiness, burnout-related symptoms, musculoskeletal symptoms, and selected cardiometabolic indicators and examined their associations with years of night-shift work experience. Methods: This cross-sectional observational study used an online structured questionnaire distributed through professional groups to polysomnography technologists working in private hospitals and sleep centers in South India.Participants were graduates with at least six months of night-shift experience.Of 196 responses, 16 were excluded for less than six months of night-shift experience and 30 for incomplete questionnaires, leaving 150 participants for analysis. The questionnaire included the Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), Copenhagen Burnout Inventory (CBI), and musculoskeletal assessment.Height and weight were measured by participants.Blood pressure was manually assessed, while fasting blood glucose and glycated hemoglobin (HbA1c) were obtained from recent laboratory reports.Poor sleep quality was defined as PSQI >5 and excessive daytime sleepiness as ESS >10.The CBI was analyzed continuously and categorized operationally as 0-49, 50-74, and 75-100.Years of night-shift work experience were analyzed as a continuous exposure and categorized as ≤5 versus >5 years.Multivariable logistic regression was adjusted for age, sex, and BMI.Results: Mean age was 29.97 ± 4.43 years, and BMI was 24.16 ± 3.19 kg/m².Mean PSQI, ESS, and CBI scores were 7.09 ± 2.95, 9.20 ± 3.39, and 45.99 ± 20.95, respectively.Poor sleep quality occurred in 97 (64.7%), excessive daytime sleepiness in 56 (37.3%), and musculoskeletal symptoms in 126 (84.0%).CBI scores were categorized as 0-49 in 86 (57.3%), 50-74 in 48 (32.0%), and 75-100 in 16 (10.7%).Hypertension-range blood pressure was identified in 21 (14.0%) and prediabetes in 11 (7.3%).Each additional year of night-shift work experience was associated with higher odds of poor sleep quality (adjusted odds ratio (aOR): 1.433, 95% CI: 1.118-1.835),excessive daytime sleepiness (aOR: 1.205, 95% CI: 1.001-1.450),CBI ≥50 (aOR: 1.810, 95% CI: 1.350-2.428),musculoskeletal symptoms (aOR: 1.372, 95% CI: 1.003-1.878),lower-back symptoms (aOR: 1.296, 95% CI: 1.051-1.598),neck symptoms (aOR: 1.272, 95% CI: 1.043-1.552),and hypertension-range blood pressure (aOR: 1.387, 95% CI: 1.052-1.829).No significant associations were observed with shoulder symptoms or prediabetes. Conclusion: Polysomnography technologists reported frequent poor sleep quality, excessive daytime sleepiness, burnout-related symptoms, and musculoskeletal symptoms.More years of night-shift work experience were associated with several adverse health outcomes after adjustment for age, sex, and BMI.These findings should be interpreted cautiously given the cross-sectional design and potential residual confounding.Prospective multicenter studies are warranted.

Rahul Joshi; Gayatri Marwah; Sainath Raman; Kristen Jones; Connie Cai Ru Gan; J.M. HOCKLEY Australian Critical Care, 2026, Vol. 39, Issue 6, pp. 101703 https://doi.org/10.1016/j.aucc.2026.101703

Background Paediatric intensive care unit (PICU) clinicians are particularly susceptible to workplace stress due to frequent ethical dilemmas (for example, perceptions of futile or unethical treatment), patient morbidity and mortality, sleep disruption, and emotional exhaustion, putting them at increased risk of anxiety, depression, and burnout. The psychological well-being of these healthcare professionals is vital for maintaining high standards of patient care and safety. Objective The aim of this study was to evaluate the feasibility of implementing a mindfulness-based program among PICU staff, with a focus on acceptability and barriers to participation. Methods Weekly 20-min guided group-practice sessions in mindfulness, with both in-person and virtual attendance options, were held over a 6-month period. These sessions were offered to medical, nursing, and allied health staff working in the PICU of a specialist paediatric hospital in Queensland, Australia, serving families from Queensland and northern New South Wales. The program was offered during a period of increased personal and occupational stress arising from the delivery of high-acuity care during the global COVID-19 pandemic. Findings The program was successfully delivered within the PICU setting, with most participants expressing interest in continuation of the program (72%). Qualitative responses suggested perceived individual and collective well-being benefits, alongside generally positive perceptions of the program. However, participants also identified important barriers to participation, including workload pressures and rostering constraints. Conclusions This study suggests that implementation of a mindfulness-based program within a PICU setting may be feasible while also highlighting important operational barriers to participation in high-acuity healthcare environments. Qualitative findings indicated generally positive staff perceptions and perceived acceptability of the program. Further research using longitudinal designs is recommended to better understand implementation considerations and potential longer-term outcomes associated with mindfulness-based interventions in PICU settings.

Mia Thompson; Alastair Cardno; Deepak Joshi Cureus, 2026, pp. e117755 https://doi.org/10.7759/cureus.117755

BackgroundIn Nepal, depression is one of the most common mental health conditions among adolescents, but mental health services for this population remain limited.Previous studies have identified barriers to accessing mental health services in Nepal; however, to our knowledge, no study has specifically examined the barriers faced by adolescents with depression. AimsThis study aimed to identify barriers to accessing mental health services for adolescents with depression in Pokhara Metropolitan City, Nepal, from the perspectives of mental healthcare providers, and to identify approaches to overcome these barriers. MethodSemi-structured interviews were conducted with nine healthcare professionals involved in delivering mental healthcare, including counsellors, psychologists, medical officers, and consultant psychiatrists.Participants were selected through purposive sampling according to predefined inclusion and exclusion criteria.Interviews were conducted in May 2023 at the participants' workplaces across Pokhara Metropolitan City. The interviews were audio-recorded and transcribed verbatim.Reflexive thematic analysis was used to develop codes and themes. ResultsBarriers to accessing mental health services included individual factors, primarily related to limited knowledge of mental health and available services; social factors, including stigma, particularly within families and the wider community; and systemic and structural factors, including limited service availability and an insufficient mental health workforce.Suggested strategies to overcome these barriers included raising mental health awareness in schools and through social media, as well as increasing the number of mental health-trained staff providing services within communities. ConclusionsRecommendations for future practice include integrating mental health education into Nepal's national school curriculum and expanding training programs for healthcare workers.Future research could explore perspectives from other regions of Nepal and include additional stakeholder groups, such as adolescents, parents of adolescents, and government staff.

Multimodal computational analysis of longitudinal stress profiles in healthcare workers

Sapir Gershov; Dayeon Cho; Dimitra Vergyri; Kacey Ferguson; Alan Taitz; Chase Adams; … npj Digital Medicine, 2026 https://doi.org/10.1038/s41746-026-03328-4

Healthcare workers (HCWs) face elevated risk for burnout and stress-related disorders, yet conventional self-report assessments are limited by professional stigma and underreporting. We evaluated whether multimodal computational analysis of brief, remotely collected stress narratives could distinguish longitudinal stress profiles. In a prospective six-week cohort study, N = 750 HCWs were invited to provide weekly naturalistic narratives describing recent stressful experiences and repeated measures of anxiety, depression, burnout, and subjective distress. Among 553 participants with sufficient repeated self-report data, multivariate longitudinal clustering identified two distinct profiles: Resilient ( n = 295) and Vulnerable ( n = 258). Linguistic, acoustic, and facial expression embeddings were extracted from the recordings and integrated using a hierarchical multimodal transformer. In a held-out test set, the model achieved an AUROC of 0.75, outperforming unimodal (linguistic embeddings; AUROC = 0.63) and bimodal (linguistic + acoustic; AUROC = 0.70) configurations. These findings provide proof of concept that multimodal embeddings extracted from brief stress narratives are associated with occupational stress profiles, potentially complementing traditional assessment methods and informing targeted prevention strategies.

Post-traumatic stress disorder: outline and updated insights focusing on occupational factors

Lea S. Lewandrowski; Doris Klingelhöfer; Dörthe Brüggmann; David Alexander Groneberg Journal of Occupational Medicine and Toxicology, 2026, Vol. 21, Issue 1 https://doi.org/10.1186/s12995-026-00530-8

BACKGROUND: Posttraumatic stress disorder (PTSD) is a stress-related disorder associated with substantial psychological distress and functional impairment, and it carries major relevance for occupational health because it frequently follows traumatic exposure in both high-risk professions and occupations not traditionally classified as high risk. This narrative review focuses on occupational risk factors, prevalence, work capacity, and implications for workers' compensation and return to work. METHODS: A non-systematic (narrative) literature search was conducted in PubMed/MEDLINE and Google Scholar for English- and German-language articles, with supplementary hand-searching of reference lists and institutional/guideline sources (e.g., WHO, AWMF). No protocol was registered, consistent with conventions for narrative reviews. Reporting follows the general principles of the Scale for the Assessment of Narrative Review Articles (SANRA), the checklist commonly used to appraise this review type, rather than PRISMA, which applies to systematic reviews. RESULTS: Occupational risk is particularly elevated in military service (including soldiers and veterans), public safety and disaster management, and occupations with structurally frequent severe incidents (e.g., transport, construction, industrial production), while unpredictable bystander events can also trigger PTSD in occupations not traditionally classified as high risk settings. Risk is shaped by trauma type, severity, and recurrence, as well as pre-traumatic, peri-traumatic, and post-traumatic factors. Prevalence varies substantially by context and is not directly comparable across studies, which differ markedly in population, exposure severity, diagnostic method, and assessment period. With this caveat, particularly high burden is reported in conflict-affected populations, healthcare workers during the COVID-19 pandemic, and trauma-exposed occupational groups such as military personnel, police officers, paramedics, train drivers, construction workers and industrial workers. Established treatments (trauma-focused psychotherapy, with pharmacotherapy as an adjunct) are distinct from early post-event interventions such as screening and Psychological First Aid, which aim to prevent chronicity rather than treat established illness. Routine single-session debriefing is not supported by the evidence. Occupational medicine must also address work capacity, return to work, compensation, and rehabilitation, since symptomatic improvement does not necessarily restore functional ability. CONCLUSION: Stronger workplace prevention, early screening after traumatic exposure, organizational support, and clearer compensation and return-to-work pathways are needed to reduce the long-term occupational consequences of PTSD. Evidence specifically comparing treatment of occupational versus non-occupational PTSD remains limited, and prevalence estimates should be interpreted in light of substantial heterogeneity across the underlying studies.
