Objective: To investigate the relationship between orthorexia dimensions, specifically healthy orthorexia (HeOr) and orthorexia nervosa (OrNe), and hematological inflammatory markers. Methods: This cross-sectional, analytical study included 218 healthy volunteers (73.9% male, median age: 25.0 years) who were evaluated between February 2026 and April 2026.Sociodemographic profiles and the Teruel Orthorexia Scale scores were obtained.Complete blood count parameters and inflammatory indices, including the monocyte-to-lymphocyte ratio (MLR) and systemic inflammation response index (SIRI), were analyzed. Results: HeOr scores demonstrated significant negative correlations with white blood cell count (r = -0.163,P = 0.016), neutrophil count (r = -0.160,P = 0.019), monocyte count (r = -0.244,P < 0.001), platelet count (r = -0.168,P = 0.013), MLR (r = -0.134,P = 0.049), and SIRI (r = -0.185,P = 0.006).Conversely, OrNe scores exhibited a significant negative correlation only with monocyte count (r = -0.171,P = 0.012) and showed no significant associations with other hematological parameters or systemic inflammatory indices. Conclusion: Individuals with higher HeOr tendencies display a healthier systemic inflammatory profile, likely reflecting beneficial dietary choices.Despite being characterized by more rigid cognitions and behaviors toward healthy eating, OrNe tendencies do not manifest a healthier inflammatory profile.

Conceptualisation of restrictive repetitive behaviours across autism and orthorexia nervosa

Jane Edge; Jac Neirin Airdrie; Maddy Greville-Harris; Samuel J. R. A. Chawner Frontiers in Psychiatry, 2026, Vol. 17, pp. 1936477 https://doi.org/10.3389/fpsyt.2026.1936477

Orthorexia Nervosa (ON) is widely regarded as an emerging eating disorder although it is not currently recognised as a formal diagnosis. It is characterised by a strict adherence to “pure” eating which becomes excessive and can lead to nutritional deficiencies, psychological distress, and impaired daily functioning. Restrictive eating behaviour in ON shows overlap with Restricted Repetitive Behaviours (RRBs) observed in autism. However, the conceptual relationship between ON and autism remains underexplored. This perspective article synthesises existing literature to explore potential overlapping and diverging mechanisms of RRBs across ON and autism. Intolerance for Uncertainty is highlighted as a possible shared mechanism that may contribute to RRBs observed in both ON and autism, whereas Cognitive Inflexibility and Perfectionism may mechanistically differentiate the conditions. We consider that autistic RRBs reflect neurodevelopmental divergence, whereas orthorexic RRBs may be influenced by evaluative belief-centred cognitions. We propose a conceptual model outlining how cognitive, affective and functional factors may interact to produce and maintain RRBs. Clarifying these distinctions is important in supporting clinicians to appropriately consider the drivers and mechanisms underlying RRBs across ON and autism, therefore supporting accurate diagnosis, formulation and treatment.

The association between orthorexia nervosa and social media use: A systematic review and meta-analysis

Anna-Marina Nakhl; Muhammad Ahmad Barakat; Jana Kotaich; Elssy Elleik; Ahmad Afyouni; Linda Abou Abbas PLOS mental health., 2026, Vol. 3, Issue 10, pp. e0000758 https://doi.org/10.1371/journal.pmen.0000758

Orthorexia nervosa (ON) is a newly described mental disorder characterized by an extreme obsession with the consumption of healthy foods, resulting in anxiety and impaired functionality. This systematic review and meta-analysis aims to synthesize the association between social media use and orthorexic tendencies among adolescents and adults. PubMed, Web of Science, Embase, Scopus, and PsycINFO were searched through January 2026. Original studies assessing the association between social media use and ON were included. Zero-order correlations between social media and orthorexia measures were pooled using Fisher’s z under a random-effects (REML) model; heterogeneity, subgroup and meta-regression, sensitivity and publication-bias analyses were performed; certainty was rated with GRADE. Of 33 included studies, 22 contributed to meta-analysis of the correlation between social media use and ON symptoms, yielding a small positive pooled correlation (r = 0.20, 95% CI 0.15–0.25; I² = 84.3%; very low certainty by GRADE). This association remained stable across leave-one-out, subgroup, and sensitivity analyses (r = 0.18–0.20), and after adjustment for potential publication bias (bias-adjusted r = 0.17). A secondary analysis of two studies with reconstructable 2 × 2 data suggested greater odds of ON among heavier users (OR = 1.57, 95% CI 1.25–1.97). Pooled ON prevalence differed sharply with the diagnostic threshold (39.9% at the ORTO-15 cutoff below 35 versus 65.5% below 40), reflecting extreme heterogeneity (I² > 90%) across studies. Narrative synthesis indicated that the type of content consumed was more consistently associated with ON than platform or duration of use alone Social media use is associated with greater orthorexic tendencies. However, because all included studies were cross-sectional and overall certainty of evidence was very low, causality and directionality cannot be established and reverse causality remains plausible. Longitudinal and experimental research is needed to clarify the direction of this relationship.

Determinants of healthy orthorexia and orthorexia nervosa: the roles of food security, dietary behaviors, and obesogenic environment

Tevfik Koçak; Tuba Eda Arpa Zemzemoğlu; Tuğba Türkkan; Kenan Bülbül; Birsen Yılmaz Frontiers in Nutrition, 2026, Vol. 13 https://doi.org/10.3389/fnut.2026.1919473

Orthorexia has recently gained attention as a two-dimensional construct centered on a shared health orientation. Recent conceptual developments distinguish healthy orthorexia (HeOr), defined as a non-pathological interest in healthy eating, from orthorexia nervosa (OrNe), characterized by rigid and compulsive food control accompanied by psychological and social impairment. This cross-sectional study investigated the associations of HeOr and OrNe with sociodemographic variables, dietary behaviors, and obesogenic environmental factors, and aimed to identify shared and distinct determinants of these tendencies with 1,015 adults aged 18–65 years. Orthorexia tendencies were assessed using the Teruel Orthorexia Scale. Additional measures included the Sustainable and Healthy Eating Behaviors Scale, Short Screening Questionnaire of Highly Processed Food Consumption, Adult Food Security Survey Module, Food Label Reading Attitude Scale, Food Choice Questionnaire, and the Obesogenic Environment Assessment Scale. In the adjusted regression models, HeOr was positively associated with sustainable and healthy eating behaviors ( β = 0.26), AFSS food-insecurity severity score ( β = 0.11), food label reading attitudes ( β = 0.07), health-related food choice motives ( β = 0.09), natural-ingredient preference ( β = 0.14), weight-control motives ( β = 0.15), ethical food choice motives ( β = 0.10), and obesogenic social determinants and effects ( β = 0.09). Conversely, affordability-related motives ( β = −0.08), emotional appeal ( β = −0.15), familiarity ( β = −0.10), and obesogenic cultural determinants/access to experts ( β = −0.12) were negatively associated with HeOr. For OrNe, positive associations were observed with sustainable and healthy eating behaviors ( β = 0.13), AFSS food-insecurity severity score ( β = 0.17), mood-related food choice motives ( β = 0.13), weight-control motives ( β = 0.26), familiarity ( β = 0.09), ethical food choice motives ( β = 0.18), and obesogenic social determinants and effects ( β = 0.30), whereas health-related food choice motives ( β = −0.12), affordability-related motives ( β = −0.08), and emotional appeal ( β = −0.24) showed negative associations. These findings suggest that orthorectic tendencies are associated with individual health motivations as well as broader behavioral and environmental contexts; however, because of the cross-sectional design, these associations should be interpreted as correlational and hypothesis-generating rather than causal.

Dermorexia (Cosmeticorexia): A Narrative Review of the Emerging Phenomenon of Obsessive Skincare Behavior in the Digital Age

Mohammed Saleh Al‐Dhubaibi; Ghada Farouk Mohammed; Lina Mohammed Atef; Saleh Salem Bahaj; Ahmed Mohammed Al‐Dhubaibi; Nada Sameh Aziz Aesthetic Surgery Journal Open Forum, 2026, Vol. 8, pp. ojag185 https://doi.org/10.1093/asjof/ojag185

Abstract The increasing use of visual social media has intensified societal pressure to achieve flawless skin and may contribute to excessive preoccupation with appearance and skincare. The emerging terms dermorexia and cosmeticorexia have been used to describe potentially compulsive skincare behaviors extending beyond conventional self-care. This narrative review examines the proposed concept of dermorexia, its conceptual overlap with recognized psychiatric conditions, potential risk factors and clinical manifestations, dermatological and psychological consequences, and implications for aesthetic practice. The available literature was conducted, focusing on compulsive skincare behaviors, social media–related body image concerns, body dysmorphic disorder (BDD), and obsessive-compulsive spectrum conditions. Relevant evidence was critically synthesized to assess the current conceptual and clinical basis of dermorexia. Dermorexia has conceptual similarities to orthorexia nervosa and may overlap with obsessive-compulsive and body dysmorphic psychopathology. Adolescents and young adults may represent a particularly vulnerable population. Proposed manifestations include excessive and prolonged skincare routines, compulsive product use, repetitive mirror checking, skin picking, and recurrent pursuit of cosmetic procedures. Potential dermatological consequences include epidermal barrier disruption, irritant or allergic contact dermatitis, and excoriation-related scarring. Psychological consequences may include anxiety, depressive symptoms, appearance-related distress, and functional impairment. However, empirical validation, standardized diagnostic criteria, and prevalence estimates are currently lacking Dermorexia/cosmeticorexia remains an unvalidated clinical construct rather than a formally recognized psychiatric diagnosis. Nevertheless, increasing awareness may help aesthetic clinicians identify maladaptive skincare and appearance-related behaviors and facilitate appropriate psychological referral. Further research is required to establish its epidemiology, diagnostic boundaries, and clinical significance.
