Functionalized Service of Adult Inpatient Psychiatry: A Review of the Current Evidence Base

Nilamadhab Kar Academic Bulletin of Mental Health, 2026, Vol. 0, pp. 1-8 https://doi.org/10.25259/abmh_14_2026

Mental health services are constantly evolving with the gradual introduction of functionalized care, which means medical intervention has become more focused on a particular clinical area or stage of the illness. This has progressed to having designated teams for inpatient psychiatry. Usually, psychiatrists manage their patients both in outpatient and inpatient settings while working in a particular geographical area or sector. Moving away from this model to a functionalized inpatient setting, where other psychiatrists take care of their patients, is seen as a breach in continuity of care. In this context, it was intended to review the current literature for the experience of clinicians and patients in these sectorized (medical continuity) or functionalized models of psychiatric inpatient care. A narrative review of the current literature from multiple databases was conducted. The results suggested that there are positive and negative observations in both models. It appeared that neither met the expectations of patients nor clinicians. Patients preferred to be seen by the same consultant in community and inpatient settings, but clinical outcomes were not significantly different in the two models. The functionalized care was mostly medical continuity rather than multidisciplinary. Any changes in the care models may be driven by local facilities and issues. There is a need to improve the quality of care through existing models before any changes. More studies are needed to improve the evidence on the effectiveness of these service models.

Rohan Mishra; Rounak Mandal; Ruhan Rana Hossein; Saed Hamza Molla; Sahanaj Parvin; Sahid Anowar; … The ESRF Research Journal for Undergraduate Medical Students, 2026, Vol. 4, Issue 1 https://doi.org/10.5281/zenodo.23368561

Abstract Introduction: Sleep disturbance is common among hospitalized patients and may adversely affect recovery and well-being. This study aimed to determine the proportion of patients hospitalized for at least 3 days, with unsatisfactory sleep, describe perceived sleep disruptors, and assess their association with socio-demographic, clinical and hospital-related factors. Materials and Methods: A hospital-based descriptive cross-sectional study was conducted among patients admitted to five randomly selected in-patient departments of a tertiary medical college, Kolkata, from 10 April to 20 May 2026. Eligible patients admitted for at least three days were recruited. Data were collected through face-to-face interviews using a pre-designed, pre-tested schedule. Perceived sleep quality and perceived sleep disruptors were assessed using 5-point Likert scales. Participants reporting fair, poor or very poor sleep were categorised as having unsatisfactory sleep. Associations were assessed using Chi-square/Fisher's exact test and Mann–Whitney U test, with p<0.05 considered significant. Results: A total of 183 patients were included in the final analysis. The median age was 40 years (IQR: 26–46.5), 50.8% were male, and median length of stay was 7 days (IQR: 4–13.5). Unsatisfactory sleep was reported by 101 (55.2%) participants, including 36 (19.7%) with fair, 39 (21.3%) with poor and 26 (14.2%) with very poor sleep. Acute medical conditions related to the current admission were the most commonly reported significant disruptor of sleep (77, 42.1%), followed by pre-existing medical conditions (45, 24.6%), noise (40, 21.9%) and light (38, 20.8%). Unsatisfactory sleep was significantly associated with perceived disruption from noise (p<0.001), light (p=0.006), frequent vital-sign monitoring (p=0.016), personal care routines (p=0.037), existing medical conditions (p=0.033), and acute medical conditions (p=0.010). Age, sex, department of admission, diagnosis and length of stay showed no significant association with sleep quality. Conclusion: More than half of hospitalized patients experienced unsatisfactory sleep. Clinical conditions and modifiable hospital-related factors, particularly noise, light and interruptions related to routine care, were associated with poorer sleep. Measures to minimize unnecessary nighttime disturbances may improve sleep among hospitalized patients.

The Psychological Impact of Hospital Admission on the Elderly: A Prospective Comparative Clinical Audit and Service Evaluation

Aqsa Sameen; Pirya Sarupani; Athina Angelopoulou Cureus, 2026, Vol. 18, Issue 10, pp. e117440 https://doi.org/10.7759/cureus.117440

Introduction Hospital admission in older adults can be associated with functional decline, loss of independence, delirium, disruption of routine, and psychological deterioration. Virtual Wards, also described as Hospital-at-Home models, provide acute clinical care in a patient's usual place of residence and may reduce some harms associated with inpatient admission. Although Virtual Wards are commonly evaluated in terms of bed capacity, admission avoidance, and clinical safety, their psychological impact remains less well explored. This audit compared depressive symptoms and self-rated mood in older adults managed in hospital with those treated through a Virtual Ward service. Methods A prospective comparative clinical audit and service evaluation was conducted within the Care of the Elderly directorate at East Cheshire NHS Trust. The audit included 100 patients aged ≥70 years: 50 hospital inpatients and 50 patients managed through Virtual Ward pathways. Eligible patients had received at least five days of inpatient or Virtual Ward care and had broadly comparable medical diagnoses. Patients with severe acute or life-threatening illness, those lacking capacity to participate, and those with communication barriers preventing completion of the assessment tools were excluded. Baseline age, sex, primary diagnosis, and Clinical Frailty Scale (CFS) characteristics were compared descriptively between cohorts. Depressive symptoms were assessed using the 15-item Geriatric Depression Scale (GDS-15), and self-rated mood was assessed using a five-point mood scale. Exploratory unadjusted between-group comparisons were performed for selected patient-level outcomes. Results Hospital inpatients demonstrated a greater depressive symptom burden than Virtual Ward patients. A GDS-15 score of ≥5 was recorded in 36 (72%) hospital inpatients compared with 15 (30%) Virtual Ward patients, corresponding to an unadjusted odds ratio of 6.00 (95% CI, 2.53-14.24; Fisher's exact p<0.001). Low self-rated mood, defined as a score of 1 or 2, was reported by 19 (38%) hospital inpatients compared with three (6%) Virtual Ward patients, with an unadjusted odds ratio of 9.60 (95% CI, 2.62-35.21; Fisher's exact p<0.001). Conversely, positive mood scores of 4 or 5 were reported by 32 (64%) Virtual Ward patients compared with 16 (32%) hospital inpatients. Baseline age and frailty distributions showed substantial descriptive overlap, although some variation in sex and primary diagnosis was present between cohorts. Conclusion Older adults receiving inpatient care demonstrated a greater burden of depressive symptoms and less positive self-rated mood than those managed through the Virtual Ward service. However, the non-randomised design, potential selection bias, and absence of adjustment for important baseline factors such as cognitive status, functional status, social support, and baseline psychological wellbeing mean that causation cannot be established. The findings should therefore be interpreted as an association between care setting and psychological wellbeing rather than evidence that Virtual Ward care itself improves mood. Further prospective evaluation using comprehensive baseline assessment, adjusted analyses, and post-discharge follow-up is required. Older adults who require hospital admission may also benefit from routine mood assessment and targeted measures to support psychological wellbeing.
