Background: Patients recovering from craniotomy in the Intensive Care Unit (ICU) are at risk of hemodynamic changes related to stress responses, neurological impairment, pain, and environmental stimuli. A combination of music therapy and hand massage may help maintain hemodynamic stability in these patients. Objective: To describe the application of combined music therapy and hand massage to support hemodynamic stability in post-craniotomy patients in the ICU. Methods: This evidence-based nursing intervention was implemented with three post-craniotomy patients admitted to the ICU of Abdul Wahab Sjahranie Regional General Hospital, Samarinda. The intervention consisted of 15 minutes of instrumental or classical music followed by 15 minutes of effleurage hand massage. Systolic and diastolic blood pressure, mean arterial pressure (MAP), heart rate (HR), respiratory rate (RR), and oxygen saturation (SpO₂) were measured before and after each intervention. The intervention was conducted over three days, with each patient’s clinical changes monitored. Results: The case study findings indicated greater stability in blood pressure, MAP, HR, RR, and SpO₂ following the intervention. Conclusion: Combined music therapy and hand massage may be a safe, non-pharmacological intervention to support nursing care and maintain hemodynamic stability in post-craniotomy patients. Further research is needed to assess its effects on other outcomes, including biochemical changes.
Introduction The Tibetan Buddhist tradition distinguishes between two broad categories of meditation: Sutra and Tantra, which differ in the methods and mental faculties they employ. The main neurophysiological studies of Tibetan Buddhist meditation have been conducted mostly on Sutra practices; while Tantric meditation remains less studied. Methods This publication presents a dual comparative study, using both between-groups and within-subjects design, of the neurophysiological (EEG) correlates characterizing Sutra and Tantra meditative practices, examining both the meditative states and the meditative traits. The study was carried out within the natural environment of Tibetan Buddhist monasteries in India, maintaining the socio-cultural context. Two groups of long-term practitioners (Buddhist monks of Gelug lineage), one experienced in the Tantra tradition (T-group, n = 21) and the other in the Sutra tradition (S-group, n = 20) participated in the study. Results A between-group cluster permutation test revealed that the groups differed in EEG alpha power during meditation, as well as in baseline resting conditions before and after meditation. Specifically, T-group demonstrated consistently lower alpha power compared to the S-group, regardless of the conditions. Thus, the observed differences are stable characteristics of the groups. The within-subjects analysis demonstrated that the conditions (meditation, and baseline resting states before and after meditation) differed only in the T-group: during meditation, an increase in individual alpha peak frequency (iAPF) was observed in the occipital leads, followed by a decrease in iAPF after meditation. Discussion We suggest that reported between-group differences represent enduring meditative traits developed through years of training in meditative techniques in the Sutra and Tantra traditions. Nevertheless, we cannot entirely exclude the possibility that these variations may also stem from certain cohort-specific characteristics unrelated to meditation. An examination of individual power spectra revealed that the absence of significant differences in EEG spectral power between conditions can be primarily attributed to the inter-individual variability in both groups. This variability, in turn, may stem from extensive expertise of highly experienced practitioners, who over years of training, develop individualized cognitive strategies for maintaining the meditative state. Our future research will focus on a more detailed quantitative analysis of inter-individual variability during meditations and on identifying additional between-group differences among practitioners in different traditions.
Preterm birth represents a risk for vulnerability across neurodevelopmental domains. However, its impact on early communication and cross-domain associations remains underexplored. This study compared preterm and full-term infants at 18 months on communicative, cognitive, linguistic, and motor abilities, and examined associations between pointing and these skills. Thirty-nine preterm and 28 full-term infants participated. Pointing (hand, index), vocal productions (vocalizations, babbling, words), and expression modalities (unimodal gestural, unimodal vocal, bimodal gestural-vocal) were elicited using the “decorated room” task and coded with ELAN. Cognitive, language (receptive, expressive), and motor (fine, gross) abilities were assessed with the BSID-III. Preterm infants produced fewer index points, babbling, and bimodal expressions, and showed lower scores across all BSID-III scales. Only in preterm infants, index pointing significantly correlated with developmental scores, particularly cognitive and fine-motor ones. Findings highlight interconnected cross-domain developmental trajectories in the preterm population, with index pointing as a key developmental marker at 18 months.
The role and responsibilities of pilots have been evolving as advanced automation systems are being integrated for flight operations. Operator state monitoring (OSM) helps ensure safety and improve performance by detecting and predicting degraded operator state during operations. Despite growing interest in OSM, no unified framework exists that integrates impairment definitions, sensor criteria, and interface design requirements. This work is presented as a conceptual analysis that establishes a foundational framework for operator state monitoring system (OSMS) development, addressing what an OSMS should monitor, what types of operator state changes should be detected, the characteristics of sensing and communication needed to support these functions and the operational considerations that should inform future development. A literature review establishes potential impairment and incapacitation events for pilots, identifies relevant physiological features to support detection of these events, and assess the possible impact of such events on flight operations to inform a system response. Existing work concentrates on military and automotive contexts and on workload, fatigue, and drowsiness rather than the acute-event end of the impairment continuum; the framework developed here spans the normal to impaired to incapacitated range required for in-flight operations. OSMSs should be developed based on a multi-modal data approach consisting of physiological features, pilot performance and activity, aircraft state and environmental data, as well as contain a means for pilots to self-report input. Heart, breathing, and behavioral features are affected across the widest range of events, making them broad-coverage candidates for sensor selection, though breadth of coverage is distinct from diagnostic specificity and no single feature is treated as a standalone indicator. The remaining recommendations address the engineering of sensing and processing for the flight deck. Design guidance for the alert and notification interface is presented alongside the recommendations. As OSMSs mature, this framework can serve as a critical reference for developing robust, reliable, and widely accepted solutions in aviation.
The underlying mechanisms linking childhood maltreatment (CM) to adverse health outcomes are unclear. Here, we estimated CM pleiotropy across human phenome, also assessing the role of socioeconomic factors. Specifically, we performed a phenome-wide association study (PheWAS) of CM-associated loci using electronic health records and genetic data from the All of Us Research Program (N = 98,551), FinnGen Project (N = 377,277), and Million Veteran Program (N = 449,042). To characterize CM pleiotropy, we also conducted genetic correlation (rg), latent causal variable analysis (LCV), and bidirectional Mendelian randomization (MR) analyses. Additionally, multi-trait conditional and joint analysis and multivariate MR (MVMR) were performed to estimate whether genetically inferred CM associations are partially explained by household income (HI, a proxy for socioeconomic factors). PheWAS revealed CM loci associations with 66 medical outcomes (p < 2.76 × 10 −6 ), including psychiatric, musculoskeletal, circulatory, neurologic, and respiratory conditions. Similarly, we observed 285 CM genetic correlations (p < 3.24 × 10 −5 ) with a range of medical domains. Among them, emotional disorders starting during childhood or adolescence showed the highest CM genetic overlap (rg=0.75). LCV analysis revealed CM genetic relationships with musculoskeletal, metabolic, respiratory, and eye-related outcomes. For these, MR analyses highlighted bidirectional and heterogeneous effects possibly due to dynastic confounders, and MVMR showed a reduced effect of CM genetic liability after accounting for HI. In conclusion, different pleiotropic mechanisms likely contribute to CM relationships with several health domains, which appear to be at least partially explained by socioeconomic factors.
به طور مشخص، مطالعه انجمنی فنومگسترده (PheWAS) بر روی جایگاههای مرتبط با CM با استفاده از پروندههای الکترونیک سلامت و دادههای ژنتیکی از برنامه پژوهشی All of Us (N = 98,551)، پروژه FinnGen (N = 377,277) و برنامه Million Veteran (N = 449,042) انجام شد.
