Obsessive-compulsive disorder (OCD) is a highly debilitating and often chronic condition. A large percentage of patients do not respond to first-line treatments with selective serotonin reuptake inhibitors (SSRIs) or cognitive behavioral therapy (CBT), underscoring the need for novel therapeutic approaches. In this context, promising innovative treatments have recently emerged for OCD; these include pharmacological options such as ketamine and psilocybin—which may exert rapid-acting effects—as well as neuromodulatory treatments such as repetitive transcranial magnetic stimulation (rTMS) and deep brain stimulation (DBS). In addition, growing evidence links early life stress with neuroinflammatory pathways and dysregulation of the hypothalamic–pituitary–adrenal (HPA) axis in OCD, opening novel treatment targets. While preliminary clinical trials of novel rapid-acting substances in OCD are encouraging, results are mixed and evidence of repeated dosing is scarce. This narrative review synthesizes current evidence on rapid-acting and neuromodulatory treatments for OCD, with a particular focus on stress-related mechanisms, offering new therapeutic possibilities as well as avenues for future research.

Reduced cortical thickness in obsessive-compulsive disorder and candidate molecular correlates

Yufei Chen; Haoran Li; Fenghua Long; 赵尤金; Robert K. McNamara; Fei Yue Li Translational Psychiatry, 2026 https://doi.org/10.1038/s41398-026-04510-8

Associations between obsessive-compulsive disorder (OCD) and cortical thickness (CTh) deficits have been inconsistent, and underlying candidate molecular correlates remain poorly understood. To address this, our study aims to investigate CTh alterations in OCD, evaluate potential clinical and methodological confounding factors, and explore the spatial associations of CTh alterations with molecular profiles. A meta-analysis comparing CTh differences between OCD patients and healthy controls was conducted, followed by subgroup and meta-regression analyses to evaluate the impact of clinical and methodological variables. Spatial associations between CTh differences in OCD and gene transcriptomic and neurotransmitter profiles were assessed. The results of meta-analysis revealed that OCD patients exhibited CTh deficits in left posterior cingulate cortex (PCC), left anterior cingulate cortex (ACC), and right pars opercularis compared with controls, although the left ACC cluster showed significant publication bias. Subgroup analyses demonstrated additional thinning in left inferior parietal lobule and right PCC in medicated and comorbid groups. Correlation analyses indicated that CTh alterations in OCD were associated with genes enriched in synaptic signaling and organization, which were highly expressed in excitatory neurons, inhibitory neurons, and glial cells, implicated in excitatory/inhibitory balance and signaling transmission efficiency. CTh alterations in OCD were also linked to multiple neurotransmitter systems, including mainly glutamate, 5-hydroxytryptamine, dopamine, and gamma-aminobutyric acid. Taken together, these findings suggest that OCD is associated with CTh deficits in regions affiliated with default mode and cingulo-opercular networks, which are influenced by medication and comorbidity, and these CTh alterations show spatial associations with genetic and neurotransmitter profiles primarily involving synaptic/glutamatergic pathways, providing hypothesis-generating insights into the neurobiological correlates of OCD.

The underlying mechanisms linking childhood maltreatment (CM) to adverse health outcomes are unclear. Here, we estimated CM pleiotropy across human phenome, also assessing the role of socioeconomic factors. Specifically, we performed a phenome-wide association study (PheWAS) of CM-associated loci using electronic health records and genetic data from the All of Us Research Program (N = 98,551), FinnGen Project (N = 377,277), and Million Veteran Program (N = 449,042). To characterize CM pleiotropy, we also conducted genetic correlation (rg), latent causal variable analysis (LCV), and bidirectional Mendelian randomization (MR) analyses. Additionally, multi-trait conditional and joint analysis and multivariate MR (MVMR) were performed to estimate whether genetically inferred CM associations are partially explained by household income (HI, a proxy for socioeconomic factors). PheWAS revealed CM loci associations with 66 medical outcomes (p < 2.76 × 10 −6 ), including psychiatric, musculoskeletal, circulatory, neurologic, and respiratory conditions. Similarly, we observed 285 CM genetic correlations (p < 3.24 × 10 −5 ) with a range of medical domains. Among them, emotional disorders starting during childhood or adolescence showed the highest CM genetic overlap (rg=0.75). LCV analysis revealed CM genetic relationships with musculoskeletal, metabolic, respiratory, and eye-related outcomes. For these, MR analyses highlighted bidirectional and heterogeneous effects possibly due to dynastic confounders, and MVMR showed a reduced effect of CM genetic liability after accounting for HI. In conclusion, different pleiotropic mechanisms likely contribute to CM relationships with several health domains, which appear to be at least partially explained by socioeconomic factors.

به طور مشخص، مطالعه انجمنی فنوم‌گسترده (PheWAS) بر روی جایگاه‌های مرتبط با CM با استفاده از پرونده‌های الکترونیک سلامت و داده‌های ژنتیکی از برنامه پژوهشی All of Us (N = 98,551)، پروژه FinnGen (N = 377,277) و برنامه Million Veteran (N = 449,042) انجام شد.

Haoxue Wang; Hong Fan; Rundong Liu; Kaiheng Zhu; Zhen Xiang; Shuai Zhao; … Translational Psychiatry, 2026 https://doi.org/10.1038/s41398-026-04429-0

Developmental dyslexia (DD) impacts learning achievement and psychosocial development and is associated with various environmental factors and also genetic factors. Here, we conducted a multi-center case-control study by screening 29,820 primary school students between 2–6 grades in four cities in China and enrolled 948 dyslexia cases who meet the criteria of dyslexia and 948 controls with matching age and sex. We comprehensively investigated the environmental factors using the eXtreme Gradient Boosting (XGBoost) machine learning model and Shapley additive explanations (SHAP) algorithm. The most important risk factors in dyslexia are the proactive learning habit, the age of child’s literacy, maternal education level, out-migrant workers, and prolonged screen exposure during pregnancy. The home literacy environment accounted for most of the variance within both groups. Then we examined shared polygenic risks between DD, attention deficit hyperactivity disorder, autism spectrum disorders, schizophrenia, major depressive disorder, and bipolar disorder using polygenic risk score (PRS) analyses. The results showed that the highest PRS DD quartile had a 2.13-fold risk than the lowest quartile with risk factors adjusted (95% CI: 1.08, 4.18). The individuals with the highest PRS ASD quartile and PRS SCZ quartile (OR: 2.13; 95% CI: 1.14, 3.98 and OR: 2.41; 95% CI: 1.23, 4.72) were associated with increasing dyslexia risks. This study identified the importance of screening personalized environmental factors among dyslexia and highlighted the shared genetic architecture between dyslexia and other disorders. These findings offered novel insight into the etiology of dyslexia, contributing to the advancement of early prevention strategies.
