Vygotsky's legacy in Brazilian inclusive education: (Trans)formation and singularity

Psychological Science and Education, 2026, Vol. 31, Issue 5, pp. 20-32 https://doi.org/10.17759/pse.2026310502

Context and relevance. The study investigates the contributions of Lev Vygotsky's cultural-historical theory to Brazilian special/inclusive education, particularly regarding the constitution of the subject with autism in school contexts. The theoretical framework is grounded in the concepts of drama, freedom and singularity, understanding the schooling process as a privileged pathway of human (trans)formation. Objective. To analyse how pedagogical knowledge impacts the constitution of the child with autism, based on the notion of constitutive singularity. Hypothesis. The schooling process anchored in critical-emancipatory pedagogical knowledge operates as a promoter of the subject's humanisation, in opposition to traditional clinical approaches that reduce the subject to diagnosis and to ableist perspectives that position them in a state of devaluation. Methods and materials. The research, adopting a cultural-historical approach, was configured as intervention-research with an eight-year-old student with autism, enrolled in the second year of primary education. Video recordings, audio recordings, photographs, field diaries and interviews with the teacher, the child and his mother were utilised. Data were analysed through the microgenetic approach. Results. The analysis of two paradigmatic episodes of story retelling through illustration and physical education class evidences that the child, upon inserting personal elements into school tasks, manifests his will and singularises himself, expressing the drama of the social personality as a struggle for existence. Conclusions. Understanding singularity as a dialectical, historical, cultural and dramatic process allows us to overcome pathologising views and to recognise that disability is not a destiny, but a condition that, in relations with others and with culture, can be transformed into potentiality.

Sleep and circadian disturbances in Alzheimer’s disease: clinical assessment, treatable syndromes, and limits of biomarker inference

Xuying Gan; Xilin Zhou; Shuolan Yan; Simin Yang; Zhihua Li Frontiers in Neurology, 2026, Vol. 17, pp. 1998439 https://doi.org/10.3389/fneur.2026.1998439

Sleep and circadian disturbances in Alzheimer’s disease (AD) affect patients and caregivers, but clinical action depends on the syndrome, measurement, and outcome under consideration. This narrative review links sleep assessment to indication-based care while distinguishing symptomatic benefit, physiological target engagement, and disease modification. A purposive literature search was supplemented by targeted checks of PubMed-indexed and original publisher records through 4 October 2026. Selected studies are described by population, AD ascertainment, comparator, and measured endpoint rather than assigned formal evidence-certainty grades. Actigraphy characterizes behavioral rest–activity patterns; melatonin timing and temperature rhythms address endogenous circadian physiology under controlled conditions. Polysomnography and quantitative electroencephalography characterize respiratory events, sleep architecture, and oscillatory activity. These measurements inform different questions and do not independently assign AD etiology or cognitive-functional stage. Clinical priorities include confirmed obstructive sleep apnea, cognition-adapted insomnia care, irregular sleep timing, medication-related effects, parasomnias, limb symptoms, and excessive daytime sleepiness. Interpretation incorporates hypoxemia, treatment exposure, comorbidity, caregiver support, and competing diagnoses. Randomized sleep benefits remain distinct from observational cognitive associations or exploratory imaging and cerebrospinal fluid changes. Human and animal studies of arousal circuits, protein dynamics, inflammation, and fluid transport provide mechanistic context, with conflicting results interpreted according to the actual experimental endpoint. The review provides a clinically organized account of selected evidence, not a comprehensive treatment ranking. Sleep care and cognitive-etiologic assessment should proceed in parallel, with follow-up matched to patient function, safety, and the specific treatment goal.

Genetic pleiotropy highlights immune mechanisms that link insomnia and C-reactive protein

Farah Abou Daya; Jesse Tyler Valliere; Matthew Maher; Richa Saxena SLEEP, 2026, pp. zsag254 https://doi.org/10.1093/sleep/zsag254

Insomnia is a prevalent sleep disorder with growing evidence implicating immune and inflammatory processes. Elevated C-reactive protein (CRP), a marker of systemic inflammation, is frequently observed in individuals with poor sleep, yet the genetic basis of this relationship remains unclear. This study aims to characterize the shared genetic architecture between insomnia and CRP using large-scale genomic data. CRP levels were significantly elevated in 130 611 individuals with insomnia compared with 327 375 controls in the UK Biobank. Genome-wide genetic correlation, pleiotropic analysis, colocalization, Mendelian randomization (MR), and functional annotation were used to characterize shared genetic architecture and biological pathways. Insomnia and CRP showed a significant positive genetic correlation. We identified 125 pleiotropic loci jointly associated with insomnia and CRP. Functional annotation implicated chromatin organization and nucleosome assembly, while pathway analyses highlighted immune signaling, cellular senescence, and programmed cell death. Colocalization analysis provided support for shared causal variants at six loci in or near APOE, HLA-B, NLGN1, AK5/DNAJB4, FAM114A2/GRIA1, and SATB1. Bidirectional MR provided stronger evidence for a directional relationship from insomnia liability to elevated CRP than in the reverse direction. This study uncovers shared genetic mechanisms linking insomnia and systemic inflammation. The directional genetic relationship from insomnia liability to elevated CRP suggests that sleep dysfunction may contribute to inflammatory processes.

Florian W. Sander; Etienne Sallard; Mauro Silva; Laurence Schneider; Karin Diserens; Christophe E. Graf; … Expert Review of Neurotherapeutics, 2026, pp. 1-18 https://doi.org/10.1080/14737175.2026.2723909

INTRODUCTION: Cognitive deficits are determinants of long-term disability and socio-professional reintegration across neurological disorders, including stroke, traumatic brain injury, multiple sclerosis, and neurodegenerative diseases. Despite evidence supporting cognitive neurorehabilitation, its implementation remains heterogeneous, and therapeutic parameters are insufficiently defined. AREAS COVERED: The authors reviewed the published literature from PubMed and Google Scholar from 1 January 2015 to 30 June 2026, supplemented by seminal publications. Clinical trials, systematic reviews, meta-analyses, and observational and proof-of-concept studies were prioritized. This brief narrative review examines restorative and compensatory cognitive neurorehabilitation for deficits of attention, executive functions, language and disorders of consciousness in people after stroke and traumatic brain injury (TBI), older adults, and those with neurological and neurodegenerative conditions, including multiple sclerosis, Alzheimer's and Parkinson's disease. Emphasis is placed on dose, frequency, adaptivity, personalization, and digital and technology-assisted interventions. Methodological challenges include heterogeneity of protocols, limited use of active control conditions, insufficient long-term follow-up, and the gap between cognitive test improvements and meaningful effects on participation. EXPERT OPINION: Advancing cognitive neurorehabilitation requires precision-driven approaches integrating digital therapeutics with clinical frameworks. Large-scale pragmatic trials, meaningful endpoints, and implementation strategies across the continuum of care will be essential to transform cognitive neurorehabilitation into a component of neurological practice.

Baseline Sleep Characteristics and Objective–Subjective Sleep Discordance in Early Alzheimer’s Disease Patients Prior to Lecanemab Therapy

Lecanemab

Yi-Seul Choo; Hyung‐Boo Kang; Seok-Yeol Yang; Hea Ree Park; Jae‐Joong Kim; Sang Won Seo; … Sleep Medicine Research, 2026, Vol. 17, Issue 3, pp. 198-208 https://doi.org/10.17241/smr.2026.03685

Background and Objective The baseline sleep phenotype of early Alzheimer’s disease (AD) patients entering lecanemab therapy is poorly characterized. Methods Forty-four consecutive early AD patients (mild cognitive impairment or mild dementia) awaiting lecanemab underwent attended polysomnography, the Epworth Sleepiness Scale, Insomnia Severity Index (ISI), Pittsburgh Sleep Quality Index (PSQI), and Beck Depression Inventory (BDI). Forty-four controls without documented cognitive disorder, referred for polysomnography by the study’s sleep specialist over the same period, were balanced on sex, age and apnea–hypopnea index. Comparisons used independent-samples tests with Benjamini–Hochberg false discovery rate correction within pre-specified families. Results Obstructive sleep apnea was prevalent (61.4% moderate-to-severe) despite a body mass index of 22.5 kg/m2; groups were balanced (all p>0.15). Early AD patients showed greater wake after sleep onset (114.0 min vs. 78.7 min; q=0.006), lower sleep efficiency (67.7% vs. 77.2%; q=0.006) and shorter total sleep time (284.0 min vs. 321.1 min; q=0.039) with comparable recording time, and lower REM arousal index (13.9/h vs. 21.3/h; q=0.012). Differences were moderate (|g| 0.49–0.72); sleep-stage distribution and total arousal index did not differ. ISI (3.8 vs. 13.8) and PSQI (4.1 vs. 10.0) were nonetheless lower in AD (both q<0.001; |g| 1.52–1.86); BDI did not differ. Conclusions Early AD patients awaiting lecanemab therapy had a high prevalence of sleep apnea and reduced objective sleep continuity despite reporting fewer subjective sleep complaints than controls, a contrast that may partly reflect the symptomatic sleep-clinic comparator. Objective sleep assessment before initiating lecanemab therapy may therefore be considered.

Synaptic Plasticity and Cognitive Enhancement: Bridging Neuropathological Cognitive Disorders to Emerging Neuropharmacological Strategies

Saad Misfer Alqahtani Discovery Medicine, 2026, Vol. 38, Issue 212 https://doi.org/10.24976/discov.med.202638212.212

Synaptic plasticity refers to the fundamental process through which synaptic connections become stronger or weaker in response to experience, thereby forming the basis of learning and memory. Disruption of this process is increasingly regarded as a common underlying mechanism of several cognitive and neuropsychiatric disorders, including Alzheimer’s disease, major depressive disorder, schizophrenia, and cognitive aging. This review focuses on synaptic plasticity as a unifying framework that connects neuropathological cognitive disorders to emerging neuropharmacological strategies, with particular attention to clinical translation and precision medicine approaches. The review has three interrelated aims: (i) to clarify the molecular mechanisms that regulate synaptic plasticity, (ii) to investigate how these mechanisms become dysregulated in cognitive disorders, and (iii) to evaluate emerging pharmacological strategies targeting synaptic plasticity for cognitive enhancement. Among the areas receiving special attention are glutamatergic and GABAergic neurotransmission, neurotrophic signaling pathways such as brain-derived neurotrophic factor (BDNF), intracellular cascades including mechanistic target of rapamycin (mTOR) and cyclic adenosine monophosphate (cAMP) response element-binding protein (CREB), and epigenetic regulators. With respect to their effects on long-term potentiation, synaptogenesis, and neural circuit function, novel therapeutic agents, including ampakines, N-methyl-D-aspartate (NMDA) receptor modulators, and allosteric gamma-aminobutyric acid (GABA) receptor modulators, are also assessed in this review. In both healthy individuals and disease populations, the review further considers the cognitive and neuroplastic effects of nootropics, psychedelics, and other neuromodulatory compounds. As key enablers of translational progress, the review highlights recent advances in brain-penetrant and target-selective agents, as well as improvements in drug delivery systems and biomarker development. This review aims to provide a comprehensive framework for developing safer, more effective, and more precise interventions for cognitive impairment by integrating neurobiological mechanisms, neuropathological insights, and therapeutic innovation.
