Symptom severity at first digital self-assessment is not associated with greater subsequent booking of professional care on a Saudi mental health platform: a cohort study of 5,524 users

Abdulmajeed A. Alkhamees Frontiers in Psychiatry, 2026, Vol. 17, pp. 1928345 https://doi.org/10.3389/fpsyt.2026.1928345

Background Digital mental health platforms routinely deliver self-administered symptom scales on the assumption that users with more severe symptoms are more likely to enter professional care. That assumption is rarely tested with linked participant-level data, and almost never in the Arabic-speaking Gulf. Objective To examine whether symptom severity at a user’s first self-assessment predicts booking a professional session within 90 days, across four symptom domains, on a Saudi commercial digital mental health platform. Methods Retrospective cohort analysis of 10,100 first assessments — depression, generalized anxiety, panic, and obsessive-compulsive scales — contributed by 5,524 unique users of the Talbinah platform and linked to demographic and session-booking records through a pseudonymous (SHA-256 hashed) user identifier. The primary outcome was any session booked within 90 days after the assessment. Severity was banded by the established cut-offs for the PHQ-9 and GAD-7, and by percentage of scale maximum for the panic and obsessive-compulsive measures, for which no severity thresholds are established. Per-scale multivariable logistic regression estimated adjusted odds ratios (aOR) for severity band (reference None/Minimal), adjusting for age, sex, and device type. Pre-specified sensitivity analyses used 30- and 180-day windows, an attended-session outcome, percentage-of-maximum banding applied to all four scales, and a subset with no prior booking (incident help-seeking). Reporting followed STROBE. Results Users were young (mean age 26.7 years, SD 8.1) and predominantly female (78.1%); 16.6% of first assessments were followed by a booking within 90 days. No severity band on any of the four scales was associated with higher odds of booking (severe vs None/Minimal: depression aOR 0.71, 95% CI 0.51–1.01; generalized anxiety 0.76, 0.49–1.16; panic 0.83, 0.52–1.31; obsessive-compulsive 1.01, 0.59–1.74). One severity contrast was statistically significant, in the opposite direction: moderate depression was associated with lower odds of booking (0.66, 95% CI 0.45–0.97). Older age (aOR ~1.2 per decade) and iOS device use (1.4–2.0) were associated with booking across scales. This absence of a positive severity–booking association was consistent across 30-, 90-, and 180-day windows, the attended-session outcome, and percentage-of-maximum banding; a nominal association for moderate anxiety seen under percentage banding did not persist with validated GAD-7 cut-offs. Estimates for the panic and obsessive-compulsive scales, which lack established severity thresholds, are exploratory. Conclusions On a Saudi digital mental health platform, greater self-reported symptom severity at first assessment was not followed by higher subsequent booking of professional care, consistently across specifications, and for moderate depression was associated with lower odds of booking; demographic and access-related attributes showed stronger associations with booking than symptom load did. Self-screening on its own appears insufficient as a pathway into care.

The association between orthorexia nervosa and social media use: A systematic review and meta-analysis

Anna-Marina Nakhl; Muhammad Ahmad Barakat; Jana Kotaich; Elssy Elleik; Ahmad Afyouni; Linda Abou Abbas PLOS mental health., 2026, Vol. 3, Issue 10, pp. e0000758 https://doi.org/10.1371/journal.pmen.0000758

Orthorexia nervosa (ON) is a newly described mental disorder characterized by an extreme obsession with the consumption of healthy foods, resulting in anxiety and impaired functionality. This systematic review and meta-analysis aims to synthesize the association between social media use and orthorexic tendencies among adolescents and adults. PubMed, Web of Science, Embase, Scopus, and PsycINFO were searched through January 2026. Original studies assessing the association between social media use and ON were included. Zero-order correlations between social media and orthorexia measures were pooled using Fisher’s z under a random-effects (REML) model; heterogeneity, subgroup and meta-regression, sensitivity and publication-bias analyses were performed; certainty was rated with GRADE. Of 33 included studies, 22 contributed to meta-analysis of the correlation between social media use and ON symptoms, yielding a small positive pooled correlation (r = 0.20, 95% CI 0.15–0.25; I² = 84.3%; very low certainty by GRADE). This association remained stable across leave-one-out, subgroup, and sensitivity analyses (r = 0.18–0.20), and after adjustment for potential publication bias (bias-adjusted r = 0.17). A secondary analysis of two studies with reconstructable 2 × 2 data suggested greater odds of ON among heavier users (OR = 1.57, 95% CI 1.25–1.97). Pooled ON prevalence differed sharply with the diagnostic threshold (39.9% at the ORTO-15 cutoff below 35 versus 65.5% below 40), reflecting extreme heterogeneity (I² > 90%) across studies. Narrative synthesis indicated that the type of content consumed was more consistently associated with ON than platform or duration of use alone Social media use is associated with greater orthorexic tendencies. However, because all included studies were cross-sectional and overall certainty of evidence was very low, causality and directionality cannot be established and reverse causality remains plausible. Longitudinal and experimental research is needed to clarify the direction of this relationship.

Cultural and Familial Context of Contamination-Related Obsessive-Compulsive Symptoms in an Adolescent: A Case Report

Pragya MITRA; Maneesha Upadhyay Cureus, 2026, pp. e118138 https://doi.org/10.7759/cureus.118138

Contamination-related obsessive-compulsive disorder (OCD) can cause substantial distress and functional impairment, particularly when compulsive behaviors become intertwined with familial and culturally shaped beliefs about purity and cleanliness.Understanding the distinction between culturally normative practices and clinically impairing compulsions is important for culturally sensitive assessment and treatment. This case report describes an 18-year-old woman with adolescent-onset, contamination-related obsessivecompulsive symptoms that developed at approximately 14-15 years of age during a period of academic and family stress. Her symptoms included persistent feelings of being unclean after toileting, prolonged bathing and toileting, repeated checking of undergarments after urination, counting rituals, avoidance of food and water to reduce elimination-related urges, and avoidance of physical contact in crowded or public settings.A distinctive feature of the presentation was its development within a familial environment in which purity and cleanliness were associated with bodily practices and social boundaries. The patient reported routine post-defecation bathing by family members, restrictions on physical contact with individuals from Scheduled Caste backgrounds, cleansing of the household after such contact, and repeated scrutiny of the adequacy of her own washing. These experiences were considered contextual influences that may have shaped the meaning and reinforcement of contamination concerns rather than direct causes of OCD.Her behaviors were distinguished from the family practices she described by their excessive duration, associated distress, impaired control, and functional interference. The baseline Yale-Brown Obsessive Compulsive Scale (Y-BOCS) score was 24, indicating severe symptoms.Treatment focused on psychoeducation, reduction of ritualized behaviors, distraction and urgemanagement strategies, toilet retraining, relaxation, and supportive psychotherapy, alongside concurrent pharmacological treatment.By sessions six to seven, checking and counting had ceased, bathing and toileting duration had markedly decreased, food and water avoidance had resolved, and previously avoided bus travel and crowded settings were tolerated. At session nine, approximately two months after baseline, the Y-BOCS score had decreased to 4, indicating symptoms in the subclinical range. The remaining sessions focused primarily on maintaining improvement and adjustment to college, with no recurrence of the principal rituals documented through the final session. This case highlights how familial and culturally transmitted concepts of purity may shape the content of contamination-related OCD symptoms.Substantial improvement occurred during a symptom-focused behavioral intervention without directly challenging the family's broader cultural framework. However, concurrent pharmacological treatment and the availability of only one follow-up Y-BOCS assessment limit conclusions about the independent effect of psychotherapy and the durability of improvement.

Obsessive-compulsive symptoms (OCS) experienced by patients with a diagnosis of obsessive-compulsive disorder (OCD) and schizophrenia

Amy M. Hoang; Sandro Sperandei; Vlasios Brakoulias International Journal of Mental Health, 2026, pp. 1-12 https://doi.org/10.1080/00207411.2026.2730806

Obsessive-compulsive symptoms (OCS) have been noted in a significant portion of patients with schizophrenia. Currently, there are limited studies involving patients with both obsessive-compulsive disorder (OCD) and schizophrenia which compare their clinical characteristics with those of patients with OCD without schizophrenia. This project aims to investigate the most common OCS experienced by patients with a diagnosis of both schizophrenia and OCD in comparison to those experienced by patients with OCD without schizophrenia. This retrospective cohort study involved patients referred to the Western Sydney Obsessive-Compulsive and Related Disorders Service. The likelihood of a patient with either OCD without schizophrenia or OCD with schizophrenia presenting with a subtype of OCS was assessed using a series of multiple logistic regression models which were adjusted for demographic variables. Model results were presented as odds ratios (OR) and 95% confidence intervals (CI). Within the cohort of 99 patients, one third had a diagnosis of schizophrenia or related disorders. When adjusted for age, sex, marital status, education, employment, and Yale-Brown Obsessive-Compulsive Scale (Y-BOCS) assessment, the group of patients with co-occurring schizophrenia were more likely to experience aggressive, religious, and miscellaneous obsessions and checking compulsions. However, they were less likely to present with contamination obsessions and cleaning compulsions. This study reports the OCS that are more commonly associated with patients with both OCD and schizophrenia compared to OCD alone within a clinical sample. Future areas of research include performing a similar logistic regression analysis process on a larger cohort to allow for more symptom models to converge.

“Just Right” Perceptions and Obsessive-Compulsive Symptoms in Tourette Syndrome and Chronic Tic Disorder: Implications for Tourettic Obsessive-Compulsive Disorder

Seok-Hyun Nam; Tae Won Park Journal of korean Academy of Child and Adolescent Psychiatry, 2026, Vol. 37, Issue 4, pp. 199-208 https://doi.org/10.5765/jkacap.260022

Objectives: Perceptions that something is not "just right" and feelings of incompleteness are forms of premonitory urges that extend beyond simple bodily sensations; they involve subjective interpretations of those sensations and may therefore be regarded as mental phenomena. These phenomena may precede tics but can also trigger compulsive behaviors.Investigating these phenomena is essential for understanding intermediate clinical phenotypes between tic disorders and obsessive-compulsive disorder (OCD). Methods:We retrospectively reviewed the medical records of 71 patients older than 10 years with tic disorders who attended the department of psychiatry at a hospital.Clinical assessments included the Korean versions of the Premonitory Urge for Tics Scale, Yale Global Tic Severity Scale, and (Children's) Yale-Brown Obsessive-Compulsive Scale. Results: Among patients with Tourette syndrome, 71.21% reported experiencing "just right" perceptions. Both the frequency and severity of "just right" perceptions were significantly greater in patients with tic disorders and comorbid OCD (n=8) than in those without comorbid OCD (n=63).In addition, the severity of premonitory urges showed a significant correlation with the severity of obsessivecompulsive symptoms. Conclusion: "Just right" perceptions are not only an intrinsic characteristic of tic disorders but are also associated with repetitive behaviors in OCD.These results provide preliminary clinical support for Tourettic OCD as an intermediate phenotype between tic disorders and OCD.
