Beyond Poor Sleep: Behavioral Correlates of Excessive Daytime Sleepiness and Academic Performance in Undergraduate Medical Students from Southern India: A Cross-sectional Study

Preethi Ashok Masali; Siddalingaiah HS Indian Journal of Sleep Medicine, 2026, Vol. 21, Issue 4, pp. 143-151 https://doi.org/10.5005/jp-journals-10069-0205

Aim and background: Excessive daytime sleepiness (EDS) is reported far less often than poor nocturnal sleep quality in Indian medical students. We aimed to estimate EDS prevalence and its independent correlates (primary), and to compare these with poor sleep quality correlates in the same cohort and relate both to examination performance (secondary). We hypothesized that sleep hygiene behavior, not global sleep quality category, would dominate.Materials and methods: Prospective cross-sectional study of 354 of 450 eligible MBBS students (78.7%) at a Southern Indian college, sampled by stratified proportional randomization.Under supervised standardized administration, they completed the Epworth Sleepiness Scale (ESS; EDS >10), Pittsburgh Sleep Quality Index (PSQI), and Sleep Hygiene Index (SHI), with screen time, stress, mood, lifestyle, clinical covariates, and the latest university examination percentage.Excessive daytime sleepiness correlates were examined by logistic regression with a pre-specified confounder-adjusted sensitivity model. Results: Excessive daytime sleepiness affected 21.5% [95% confidence interval (CI) 17.5-26.0],more often among poor than good sleepers (27.7% vs 18.6%).Adjusted [area under the receiver operating characteristic curve (AUC) 0.773, 0.714-0.832],only poorer sleep hygiene remained independent [odds ratio (OR) 1.12 per point, 1.07-1.18],unchanged after adjustment for all measured confounders (1.12, 1.06-1.17).Examination percentage did not differ by sleep status (67.33 ± 5.16 vs 68.07 ± 5.72; p = 0.229); academic stage and sex, not sleep, predicted marks (R² = 0.152).Conclusion: In this cross-sectional sample, EDS was associated more closely with sleep hygiene behavior than with global sleep quality category, and the two had distinct correlation profiles.Screening for sleepiness alongside sleep quality, and addressing bedtime device use, merit consideration in student health services. These are associations only; no direction of effect can be inferred.Clinical significance: Measuring EDS alongside global sleep quality identifies a partly distinct at-risk group, for whom brief counselling on bedtime device use may be more specific than generic sleep hygiene advice.

Sleep and circadian disturbances in Alzheimer’s disease: clinical assessment, treatable syndromes, and limits of biomarker inference

Xuying Gan; Xilin Zhou; Shuolan Yan; Simin Yang; Zhihua Li Frontiers in Neurology, 2026, Vol. 17, pp. 1998439 https://doi.org/10.3389/fneur.2026.1998439

Sleep and circadian disturbances in Alzheimer’s disease (AD) affect patients and caregivers, but clinical action depends on the syndrome, measurement, and outcome under consideration. This narrative review links sleep assessment to indication-based care while distinguishing symptomatic benefit, physiological target engagement, and disease modification. A purposive literature search was supplemented by targeted checks of PubMed-indexed and original publisher records through 4 October 2026. Selected studies are described by population, AD ascertainment, comparator, and measured endpoint rather than assigned formal evidence-certainty grades. Actigraphy characterizes behavioral rest–activity patterns; melatonin timing and temperature rhythms address endogenous circadian physiology under controlled conditions. Polysomnography and quantitative electroencephalography characterize respiratory events, sleep architecture, and oscillatory activity. These measurements inform different questions and do not independently assign AD etiology or cognitive-functional stage. Clinical priorities include confirmed obstructive sleep apnea, cognition-adapted insomnia care, irregular sleep timing, medication-related effects, parasomnias, limb symptoms, and excessive daytime sleepiness. Interpretation incorporates hypoxemia, treatment exposure, comorbidity, caregiver support, and competing diagnoses. Randomized sleep benefits remain distinct from observational cognitive associations or exploratory imaging and cerebrospinal fluid changes. Human and animal studies of arousal circuits, protein dynamics, inflammation, and fluid transport provide mechanistic context, with conflicting results interpreted according to the actual experimental endpoint. The review provides a clinically organized account of selected evidence, not a comprehensive treatment ranking. Sleep care and cognitive-etiologic assessment should proceed in parallel, with follow-up matched to patient function, safety, and the specific treatment goal.

Eldhos Jacob; S Carishma; Srishankar Bairy; Basil B Mathews; P A Ajither Cureus, 2026, pp. e118040 https://doi.org/10.7759/cureus.118040

Introduction: Polysomnography technologists perform overnight sleep studies requiring continuous patient monitoring, technical management of diagnostic equipment, and sustained attention.Although night-shift work has been associated with adverse health outcomes among healthcare workers, the occupational health burden among polysomnography technologists remains poorly characterized in India. This study evaluated sleep quality, excessive daytime sleepiness, burnout-related symptoms, musculoskeletal symptoms, and selected cardiometabolic indicators and examined their associations with years of night-shift work experience. Methods: This cross-sectional observational study used an online structured questionnaire distributed through professional groups to polysomnography technologists working in private hospitals and sleep centers in South India.Participants were graduates with at least six months of night-shift experience.Of 196 responses, 16 were excluded for less than six months of night-shift experience and 30 for incomplete questionnaires, leaving 150 participants for analysis. The questionnaire included the Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), Copenhagen Burnout Inventory (CBI), and musculoskeletal assessment.Height and weight were measured by participants.Blood pressure was manually assessed, while fasting blood glucose and glycated hemoglobin (HbA1c) were obtained from recent laboratory reports.Poor sleep quality was defined as PSQI >5 and excessive daytime sleepiness as ESS >10.The CBI was analyzed continuously and categorized operationally as 0-49, 50-74, and 75-100.Years of night-shift work experience were analyzed as a continuous exposure and categorized as ≤5 versus >5 years.Multivariable logistic regression was adjusted for age, sex, and BMI.Results: Mean age was 29.97 ± 4.43 years, and BMI was 24.16 ± 3.19 kg/m².Mean PSQI, ESS, and CBI scores were 7.09 ± 2.95, 9.20 ± 3.39, and 45.99 ± 20.95, respectively.Poor sleep quality occurred in 97 (64.7%), excessive daytime sleepiness in 56 (37.3%), and musculoskeletal symptoms in 126 (84.0%).CBI scores were categorized as 0-49 in 86 (57.3%), 50-74 in 48 (32.0%), and 75-100 in 16 (10.7%).Hypertension-range blood pressure was identified in 21 (14.0%) and prediabetes in 11 (7.3%).Each additional year of night-shift work experience was associated with higher odds of poor sleep quality (adjusted odds ratio (aOR): 1.433, 95% CI: 1.118-1.835),excessive daytime sleepiness (aOR: 1.205, 95% CI: 1.001-1.450),CBI ≥50 (aOR: 1.810, 95% CI: 1.350-2.428),musculoskeletal symptoms (aOR: 1.372, 95% CI: 1.003-1.878),lower-back symptoms (aOR: 1.296, 95% CI: 1.051-1.598),neck symptoms (aOR: 1.272, 95% CI: 1.043-1.552),and hypertension-range blood pressure (aOR: 1.387, 95% CI: 1.052-1.829).No significant associations were observed with shoulder symptoms or prediabetes. Conclusion: Polysomnography technologists reported frequent poor sleep quality, excessive daytime sleepiness, burnout-related symptoms, and musculoskeletal symptoms.More years of night-shift work experience were associated with several adverse health outcomes after adjustment for age, sex, and BMI.These findings should be interpreted cautiously given the cross-sectional design and potential residual confounding.Prospective multicenter studies are warranted.
