Beyond screen time: artificial intelligence, algorithmic media and the future of child health

Venugopal Reddy I. International Journal of Contemporary Pediatrics, 2026, Vol. 13, Issue 11, pp. 2379-2385 https://doi.org/10.18203/2349-3291.ijcp20263830

Digital media has evolved from passive screen viewing into an algorithmically curated ecosystem of social platforms, short form video, games, generative artificial intelligence (AI), conversational agents, connected toys and educational technologies. These systems can support learning, creativity, communication, disability access and healthcare; however, engagement optimized design, commercial surveillance and synthetic content create risks that are not adequately captured by screen-time measures alone. This narrative review examines the effects of contemporary digital ecosystems on child and adolescent health and outlines implications for pediatric practice. PubMed/MEDLINE, Google Scholar and guidance from the World Health Organization, United Nations Children's Fund, UNESCO and the American Academy of Pediatrics were reviewed, with emphasis on evidence published from 2019 to June 2026. Associations have been reported between problematic digital media use and sleep disruption, reduced physical activity, visual and musculoskeletal symptoms, obesity related behaviours, anxiety, depressive symptoms, cyberbullying, disordered eating and impaired attention. Causal interpretation remains difficult because effects are heterogeneous, often small, bidirectional and strongly modified by age, content, context, displacement, family relationships and pre-existing vulnerability. AI adds distinct concerns: persuasive personalization, inaccurate health information, anthropomorphic dependency, deepfakes, sexual exploitation, biased outputs, privacy loss and erosion of learning through uncritical outsourcing. Benefits are also substantial when technologies are developmentally appropriate, evidence-based, inclusive and used with adult support. Pediatric counselling should shift from counting hours alone toward assessing the child's digital ecosystem, sleep, function, content, compulsivity, safety and relationships. Effective protection requires shared family practices, school digital literacy, safety by design, age-appropriate assurance, transparent algorithms, data minimization and independent research. The goal is not a screen free childhood but a child centred digital environment that supports health, agency and development.

Abstract Numerous studies have explored disordered eating (DE) in athletes, yet their findings frequently conflict. This research focused on examining the prevalence of DE among HK Chinese athletes. A sample of 100 athletes participated in the study, which employed a cross-sectional design using the EAT-26 questionnaire. The results revealed a prevalence rate of 4.0%, notably lower than global averages. Athletes with dieting experience scored higher on EAT-26 compared to those without (8.08 ± 9.03 vs. 4.92 ± 5.73, p < 0.05). Lower body image (BI ≤ 5 out of 9) was associated with higher EAT-26 scores compared to higher body image (BI > 5) (9.91 ± 8.89 vs. 4.21 ± 5.51, p < 0.05). Pearson’s correlation analysis demonstrated that age and EAT-26 scores showed a weak negative correlation (r = -0.234, p < 0.05), while self-perceived BI had a moderate negative correlation with EAT-26 scores (r = -0.344, p < 0.01). Regression analysis identified age (β = -0.339, p < 0.001) and self-perceived BI (β = -0.226, p < 0.05) as predictors of EAT-26 scores. Results align with international studies indicating higher scores among younger athletes and those with dieting history, suggesting tailored health education programs for different groups may be beneficial. Level of Evidence: Level V, Descriptive Study.

Eating disorders (ED) continue to be a concern worldwide. There is a growing focus on problematic discourse around ideal bodies as well as on how exposure to related content and spaces online might influence disordered eating and other ED symptoms. A qualitative evidence synthesis using thematic synthesis methods was conducted to explore young people’s lived experiences of online ED content. Studies were included if they provided, from 2007 onwards, views about online content from young people from OECD countries aged 11–25 years old who identified as having had an eating disorder, disordered eating or eating difficulties. This article focuses on findings from 13 included studies whereby three key themes were identified: Comparing – how online content was used to evaluate others and oneself; Curating – how young people manage this content; and Community – spaces for connection over shared experience. Participants across the studies described the complex ‘double-edged’ nature of online spaces, which can offer inclusion but can also be experienced as harmful. The synthesis outlined a lack of supportive offline mental health-promoting spaces as part of the context of young people’s online content use.

Screening-based low energy availability risk, disordered eating risk, and their co-occurrence among female volleyball players: a cross-sectional study

Low energy availability (LEA) and disordered eating (DE) are important health concerns in female athletes, yet their occurrence and co-occurrence in team-sport contexts, particularly volleyball, remain underexplored. This cross-sectional online survey study examined the proportions of athletes classified as being at risk of LEA and DE overall and across competition levels, and assessed the co-occurrence between the two risk classifications in female volleyball players in Türkiye, using the Low Energy Availability in Females Questionnaire (LEAF-Q) and Eating Attitudes Test-26 (EAT-26). It also examined correlations between EAT-26 total score and LEAF-Q total and domain scores. A total of 213 athletes, recruited via non-probability convenience sampling, completed the LEAF-Q and EAT-26 to assess LEA risk and DE risk, respectively. In this sample, 33.3% of athletes were classified as being at risk of LEA, with no difference across competition levels ( p = 0.825). A total of 17.4% were classified as being at risk of DE, with variation across competition levels and the highest proportion among Turkish Women’s Volleyball Second League athletes ( p = 0.034). LEA risk and DE risk co-occurred in 10.3% of the sample, and DE risk was associated with higher odds of LEA risk (unadjusted OR = 3.80, 95% CI: 1.82–7.92). EAT-26 total score was positively correlated with LEAF-Q total and gastrointestinal function scores ( p < 0.001) and injury history score ( p = 0.004), but not menstrual function score ( p = 0.133). These results suggest that LEA risk was commonly identified in this sample, DE risk was comparatively less common, and their co-occurrence was only partial, highlighting the value of screening for both risks rather than assuming one from the other.
