Background Functional tic-like behaviors (FTLBs) have emerged as a prominent clinical presentation within the context of the COVID-19 pandemic. Although FTLBs share phenomenological overlap with Tourette Syndrome and other tic disorders (collectively Tourette Syndrome, TS), evidence suggests that differences exist in demographics, phenomenology, clinical course, psychiatric comorbidity, and response to intervention. Given the rapid rise in the incidence of FTLBs—along with their complex clinical presentation—we provide a systematic review and integrated synthesis of the extant literature. Methods We conducted a systematic review following PRISMA guidelines. PubMed and PsycINFO databases searches were completed in June 2026, using terms related to functional tics and TS. Inclusion criteria were English-language studies reporting clinical data on FTLB with sample sizes of 8 or greater. Case reports, non-human studies, and commentaries were excluded. Thirty-nine studies were included. Data were extracted on demographics, phenomenology, clinical course, psychiatric comorbidity, and treatment responsiveness. Since multiple reports originated from the same data repositories, information was synthesized qualitatively by study sample and conceptual domain. Results The results of our review suggest that FTLBs predominantly affect adolescents and young adults and exhibit a strong female predominance—with higher than anticipated representation of gender-diverse individuals. Compared with TS, the onset of FTLB symptoms was abrupt and characterized by complex motor movements and vocalizations that cause functional impairment. While there is a considerable overlap between FTLBs and TS, there were distinctions in patterns of psychiatric comorbidities. Pharmacological and cognitive behavioral therapy (CBT) provided some benefit for individuals with FTLBs, but the therapeutic target of these interventions was not specified. Conclusions While sharing some similarities, FTLBs have a distinct clinical presentation from TS. There is some evidence to inform treatment approaches, but findings are complicated by the retrospective nature of most extant reports. Future research is essential to characterize longitudinal trajectories, underlying mechanisms, and evaluation of targeted interventions in randomized clinical trials.

“I swear it’s not Tourette’s!” - it is time to rethink coprolalia in Tourette syndrome

Kirsten Müller‐Vahl; Christelle Nilles; Nanette Mol Debes; Natalia Szejko; Tammy Hedderly Frontiers in Psychiatry, 2026, Vol. 17 https://doi.org/10.3389/fpsyt.2026.1948442

Coprolalia is a relatively rare type of complex vocal tic in people with Tourette syndrome (TS). In the general population, however, it is believed that coprolalia is the most typical and common symptom resulting in the stereotype of TS as the “swearing disease”. Swearing, however, also occurs as a “normal” expression of negative emotions and thus occurs more often in people with impulse control problems. It is believed that there is a continuum reaching from voluntary to involuntary swearing. Besides coprolalia, in TS, coprophenomena may also occur in the form of motor tics, called copropraxia. Furthermore, other socially inappropriate behaviors have been described such as making socially inappropriate comments and actions, called Non Obscene Socially Inappropriate symptoms or NOSIS. Only recently, coprolalia has been identified as a core symptom in functional tic-like behaviors (FTLB), a relatively new and escalating presentation of a functional movement disorder (FMD). Today it is well-established that FTLB and TS represent two distinct clinical entities, although FTLB is a common comorbidity in TS. The overlap of diseases and disorders with the more incongruent presentations of FMD are well-established clinical concepts in other neurological disorders. This raises the clinically important question as to whether coprolalia as a complex tic can be differentiated at all from “coprolalia-like” words as FTLB. Here, we want to introduce a novel concept regarding the classification of coprolalia, copropraxia, and NOSIS. We propose that it is important to consider the presence of obscene and/or socially inappropriate words, behaviors and/or insults as being more supportive of FTLB or a combination of both TS plus comorbid FTLB as opposed to being considered to be complex tic phenomena, since coprolalia and copropraxia are rare in TS. Given what we know about FTLB today, the symptom construct of NOSIS is probably not a separate symptom entity in TS. We therefore suggest that NOSIS should be preferable considered as comorbid FTLB. We believe that this novel re-conceptualization will lead us toward some explanations around inconsistent findings related to coprophenomena in TS. Furthermore, it is of high clinical relevance, since it opens wider management and treatment strategies including psychological interventions for people suffering from coprolalia, copropraxia, and NOSIS and will hopefully prevent these patients from inappropriate or even harmful treatments including invasive surgical therapies when being mistakenly classified as severely affected and treatment resistant patients with TS.

“Just Right” Perceptions and Obsessive-Compulsive Symptoms in Tourette Syndrome and Chronic Tic Disorder: Implications for Tourettic Obsessive-Compulsive Disorder

Seok-Hyun Nam; Tae Won Park Journal of korean Academy of Child and Adolescent Psychiatry, 2026, Vol. 37, Issue 4, pp. 199-208 https://doi.org/10.5765/jkacap.260022

Objectives: Perceptions that something is not "just right" and feelings of incompleteness are forms of premonitory urges that extend beyond simple bodily sensations; they involve subjective interpretations of those sensations and may therefore be regarded as mental phenomena. These phenomena may precede tics but can also trigger compulsive behaviors.Investigating these phenomena is essential for understanding intermediate clinical phenotypes between tic disorders and obsessive-compulsive disorder (OCD). Methods:We retrospectively reviewed the medical records of 71 patients older than 10 years with tic disorders who attended the department of psychiatry at a hospital.Clinical assessments included the Korean versions of the Premonitory Urge for Tics Scale, Yale Global Tic Severity Scale, and (Children's) Yale-Brown Obsessive-Compulsive Scale. Results: Among patients with Tourette syndrome, 71.21% reported experiencing "just right" perceptions. Both the frequency and severity of "just right" perceptions were significantly greater in patients with tic disorders and comorbid OCD (n=8) than in those without comorbid OCD (n=63).In addition, the severity of premonitory urges showed a significant correlation with the severity of obsessivecompulsive symptoms. Conclusion: "Just right" perceptions are not only an intrinsic characteristic of tic disorders but are also associated with repetitive behaviors in OCD.These results provide preliminary clinical support for Tourettic OCD as an intermediate phenotype between tic disorders and OCD.
