Background Patient education is an important component of nursing care, influencing treatment outcomes, quality of life, and healthcare costs. However, educational practices are often generalized and may not address patients’ individual needs, health literacy, or socio-cultural contexts. Individualized patient education tailors content to patients’ characteristics, goals, and capabilities, and enhances adherence, satisfaction, and clinical outcomes. Existing tools mainly focus on general aspects of patient education, such as educational quality or provider competencies, and no comprehensive patient-centered tool has been established to assess the individualized nature of patient education. This may constrain systematic assessment and improvement of individualized educational practices. Therefore, this protocol aims to conceptualize individualized patient education, develop a measurement tool, and evaluate its psychometric properties. Methods This two-phase exploratory sequential mixed-methods study will be conducted in educational hospitals affiliated with Tabriz University of Medical Sciences, northwest Iran. Phase 1 will involve hybrid concept analysis, including a literature review and directed qualitative study using semi-structured interviews with patients with chronic diseases and their family members until data saturation, followed by directed content analysis to clarify the concept, identify provisional dimensions, and generate initial items, which will be evaluated quantitatively. Phase 2 will assess the tool’s psychometric properties. Face and content validity will be evaluated using qualitative feedback and the Impact Score, Content Validity Ratio, Content Validity Index, and Modified Kappa coefficient. Construct validity will be examined using exploratory and confirmatory factor analyses in SPSS v.24 and AMOS v.24, with independent samples of at least 200 participants per analysis. Reliability will be assessed using Cronbach’s alpha, McDonald’s omega, and test-retest stability. Interpretability and feasibility will also be evaluated. Discussion If the tool demonstrates adequate validity and reliability, it may provide a standardized measure for assessing individualized educational interventions and support quality improvement, patient engagement, and evidence-informed health education strategies.
Traditional statistical models face the challenges of insufficient modeling capabilities and difficulty in interpreting complex, nonlinear, and high-dimensional data. Therefore, this paper combines fuzzy logic and deep learning methods to construct a framework model for identifying key factors for high-quality employment for college students. The model in this paper uses multi-channel encoding technology. We use the attention mechanism to fuse the features obtained from multi-channel encoding and input the self-organizing fuzzy inference structure for regression and classification analysis. The system can transform numerical prediction results into a semantic rule-driven decision-making process, which can significantly improve the model’s modeling capabilities and interpretability for uncertainty. Our experiments were conducted on 5,623 graduates. Experimental results on the data show that our model achieved an average mean square error of 0.831, a coefficient of determination of 0.892, and a classification accuracy of 0.865, which are significantly higher than traditional statistical and machine learning models. The corresponding experiments demonstrate the key role of cross-modal fusion and fuzzy reasoning modules in improving the results. Feature importance analysis shows that factors such as internship duration, GPA, and research participation have a key impact on high-quality employment, which is consistent with expert consensus.
Stress is a major contributor to physical and mental health disorders, underscoring the need for reliable and interpretable detection systems. We present an interpretable multimodal deep learning framework that integrates electrocardiogram (ECG), electrodermal activity (EDA), and respiration signals using modality-specific neural architectures complemented by feature-based classical models. To ensure robustness and generalizability, the framework was evaluated under both subject-independent and subject-dependent protocols. Results show that multimodal fusion consistently outperformed single modalities, achieving 94.8% accuracy, 93.9% F1-score, and 0.97 AUC under subject-independent validation, compared with 86–91% accuracy for individual signals. In subject-dependent experiments, performance reached 97.2% accuracy and 96.8% F1-score. Comparative analysis with recent state-of-the-art methods demonstrated the competitiveness of our approach, while ablation studies confirmed the contribution of each modality and fusion strategy. Statistical significance testing with ( p < 0.05) further validated the reliability of these gains. Importantly, interpretability analyses using Grad-CAM and attention visualization provided physiological insights, revealing that the model emphasizes clinically meaningful regions such as ECG R-peaks and phasic EDA fluctuations. By uniting high performance, robustness, and interpretability, this work advances stress detection research and offers a physiologically grounded framework for continuous mental health monitoring.
The number of people seeking gender-affirming medical care and gender-affirming hormone therapy (GAHT) is increasing. As sex steroids shape cardiometabolic physiology, GAHT is expected to meaningfully alter cardiometabolic risk. The aim of this review was to systematically determine and narratively synthesize evidence on the cardiometabolic effects of GAHT and identify clinically relevant knowledge gaps. We searched PubMed for English-language primary human studies published through November 26, 2025. Of 346 screened records, 96 unique original investigations were included. Most studies had less than 3 years of follow-up, the longest being 20 years in one electronic health record-based study. In transgender women, BMI was often reported as stable with higher fat and lower lean mass compared to baseline; insulin resistance tended to increase while fasting glucose and HbA1c were usually longitudinally stable. Lipid changes often showed reductions in total and low-density lipoprotein cholesterol as well as triglyceride levels. Blood pressure generally remained stable or decreased. In transgender women, VTE incidence was higher than in both cisgender women and men, whereas stroke and MI incidence was higher primarily in comparison with cisgender women. In transgender men, BMI was increased or stable and lean mass increased with variable fat mass changes and some long-term increases in visceral and liver fat. Glucose usually remained stable while insulin resistance findings were mixed. Lipid profiles commonly shifted toward lower high-density lipoprotein cholesterol and higher low-density lipoprotein cholesterol and triglycerides. Blood pressure findings were mixed with some cohorts showing an increase. In transgender men, MI incidence was higher than in cisgender women but generally not higher than in cisgender men; evidence for other cardiovascular outcomes was limited and inconsistent. Overall, evidence on systemic inflammation, renal and hepatic outcomes is sparse. In conclusion, GAHT is associated with quantifiable cardiometabolic changes, but magnitude and clinical relevance vary by regimen, population, baseline risk and study follow-up duration. Heterogeneous regimens, study methods, and confounding contribute to mixed findings. Major gaps persist with limited long-term prospective data, sparse mechanistic insights, underrepresentation of adolescents and non-European ancestry populations, and insufficient information on cardiometabolic outcomes with ageing. Sex- and gender identity-stratified evidence synthesis: Distinguishes feminizing and masculinizing GAHT while presenting parallel outcome analyses for transgender women and transgender men, enabling more clinically precise interpretation of cardiometabolic effects. Systematic characterization of heterogeneity: Integrates regimen-, population-, and context-specific findings, including discordant results, into structured domain-specific evidence tables, improving transparency and interpretability across a heterogeneous literature base. Novel evidence mapping framework: Introduces a domain-level framework quantifying both evidence volume (number of primary studies) and directional concordance of findings, visualized within a sex-stratified cardiometabolic risk continuum to identify areas of consistency, uncertainty, and evidence gaps.
Background: Indirect exposure to disasters and violence through continuous news and social media is increasingly common among adolescents. However, instruments designed to assess media-induced indirect trauma in this population remain limited and conceptually heterogeneous. Objective: This systematic review aims to synthesize the characteristics and psychometric evidence of instruments measuring media-induced indirect trauma and evaluate their suitability for adolescents. Methods: This review of outcome measurement instruments (OMIs) was conducted using the COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) methodology and reported in accordance with the Preferred Reporting Items for Systematic reviews and Meta-Analyses−COSMIN for OMIs 2024 guideline. Eight electronic databases were searched for peer-reviewed articles published between 1 January 2000 and 22 October 2025, in English or Korean. Reference list screening was also performed. Eligible studies used or developed self-report instruments assessing media-induced vicarious, secondary, or indirect trauma responses to potentially traumatic events. Methodological quality was evaluated using the COSMIN Risk of Bias checklist, and certainty of evidence was graded using a modified Grading of Recommendations Assessment, Development and Evaluation approach. Results: Seven instruments were evaluated, varying in conceptual focus, recall periods, and contextual framing. Evidence for structural validity and internal consistency was generally moderate, whereas content validity was rated very low across all instruments. Overall, while evidence for internal structure is relatively stronger, substantial gaps in content validity and longitudinal measurement properties limit confident application in adolescents. Conclusion: Future research should prioritize adolescent-inclusive validation; systematic content validity evaluations; and rigorous reporting of test–retest reliability, measurement error, responsiveness, and interpretability.
COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) Preferred Reporting Items for Systematic reviews and Meta-Analyses−COSMIN for OMIs Grading of Recommendations Assessment, Development and Evaluation درجهبندی گردید.
Objective The Win Ratio (WR) is a novel approach to analyze composite outcomes that accounts for the relative priority of each outcome. In this study, we evaluated use of the WR for composite outcomes in suicide prevention studies to overcome challenges associated with low base rates of primary outcomes.Method We illustrate the utility of the WR in suicide prevention studies through two re-analyses: 1) data from the Emergency Department Safety Assessment and Follow-up Evaluation (ED-SAFE) trial, and 2) Medicaid claims data comparing youths with concussions to those with orthopedic injuries. Statistical power of the WR is illustrated through simulations. Results In the ED-SAFE trial, with ordered outcomes of suicide death, suicide attempt, interrupted attempt, aborted attempt, and preparatory acts, the active intervention had a WR of 1.26 (95% CI: 1.04–1.53, p = 0.018) compared to treatment as usual. For the Medicaid data, using a matched sample of 42,313 youths we found that youths with orthopedic injuries had a WR of 1.10, 95% CI (1.07, 1.12), p < 0.001 relative to concussed youths for the ordered outcomes of suicide; deliberate self-harm/suicide attempt; psychosis, externalizing, or internalizing disorders; migraines or sleep problems; and post-baseline concussions. Both results are consistent with the originally published findings. Conclusions The Win Ratio preserves the clinical hierarchy of events and offers a robust analytic approach to account for multiple outcomes. By addressing challenges related to low event rates, the WR can enhance study efficiency and interpretability, supporting its broader use in suicide prevention research.
Emergency Department Safety Assessment and Follow-up Evaluation (ED-SAFE)، و ۲) دادههای ادعاهای Medicaid که در آن جوانان با ضربه مغزی با جوانان دارای آسیبهای ارتوپدی مقایسه شدند.
Emotion recognition offers a route to quantifying interaction quality in early childhood education, yet existing methods suffer from unimodal input, poor real-time performance and weak generalization. This study proposes a speech-text multimodal model built on a Mamba encoder with a GatedxLSTM fusion mechanism, in which frame pooling and temporal masking resolve the temporal misalignment between modalities and a learnable gate adjusts modality contribution. Experiments on IEMOCAP and FAU-AIBO, together with an in-house KTC-Pilot corpus of authentic kindergarten interactions used for ecological validation, show that the approach outperforms mainstream baselines in F1, WA and UA. Three child-speech-aware adaptations, namely pitch and formant augmentation, silence-aware frame pooling and disfluency-tolerant text encoding, close the domain gap with authentic kindergarten dialogue, and modality-dropout training, cross-modal reconstruction of missing text and gating regularisation maintain robustness under incomplete inputs. A contamination audit against the CLAP pretraining corpora confirms that the gains do not arise from data leakage. A downstream evaluation using an Interaction Quality Index calibrated against expert observer ratings achieves a Pearson correlation of 0.692 with expert scores. An on-device deployment study on two embedded platforms further shows that the quantised framework sustains an end-to-end latency of 377 ms per utterance at a real-time factor of 0.075 and a board power of 9.6 W while retaining an F1 score of 0.706 on authentic kindergarten audio, and a stage-wise data-handling design keeps raw audio, spectral features and transcripts inside the classroom node so that only a scalar interaction quality value is written to persistent storage. Ablation and visualization analyses confirm the contribution of each structural component to recognition performance and interpretability.
Depression represents a complex, multifactorial disorder influenced by the interplay of biological, behavioral, and social determinants. While extensive health surveys, including the National Health and Nutrition Examination Survey (NHANES), gather comprehensive multidomain data, existing studies tend to analyze these domains independently, overlooking the potential to identify integrated, data-driven patterns of depression risk. We have developed SYNERGY-VAE, an interpretable and multimodal deep learning framework that is designed to identify latent health subgroups and facilitate the transparent prediction of depression risk, utilizing NHANES-derived multimodal data from a large analytic cohort that spans five distinct domains from 2005 to 2018. SYNERGY-VAE utilizes a variational autoencoder to learn a shared latent representation derived from five modalities of NHANES, specifically demographic, dietary, examination, laboratory, and questionnaire data. Clustering within this latent space revealed subpopulations exhibiting distinct health signatures. To enhance interpretability, we triangulated insights utilizing encoder weights, standardized mean differences, and permutation feature importance (PFI). Machine learning classifiers were trained within each cluster employing the top 30 PFI-ranked features, with performance evaluated through the receiver operating characteristic area under the curve (AUC) in a 70/30 train-test split. Three latent clusters emerged, each demonstrating markedly different observed prevalence rates of depression within the analytic sample, ranging from 6.8% to 10.9%. Cluster-specific models consistently surpassed pooled models in performance, with the highest predictive accuracy identified in Cluster 2 utilizing XGBoost (AUC = 0.839, 95% CI:0.804–0.874). The importance of features varied across clusters, indicating unique depression risk profiles specific to each subgroup. SYNERGY-VAE demonstrates the power of generative, explainable deep learning for the identification of latent health phenotypes, thereby furthering the objectives of precision mental health. By integrating high-dimensional, multimodal data and facilitating transparent subgroup discovery, our model may inform future stratified and context-aware screening research and contribute to precision psychiatry-oriented research in large health survey datasets.
Abstract Rapid advancements in education assessment and natural language processing have fundamentally transformed the methodologies employed in multi-dimensional writing assessment. Neural network approaches have shown remarkable effectiveness, obviating the need for handcrafted feature engineering. However, the potential of pre-trained language models for constructing multi-dimensional automated essay scoring (AES) models still needs to be explored. The present study addresses critical challenges, including the resilience of AES models to subtle input perturbations leading to prediction inaccuracies and the elucidation of the intricate mechanisms within deep learning frameworks. We propose an integrated approach leveraging multi-task learning and the pre-trained model Bidirectional Encoder Representations from Transformers (BERT), augmented with adversarial training via the Fast Gradient Sign Method (FGSM), aiming to enhance scoring accuracy and stability in multi-dimensional AES tasks. Findings reveal that BERT-Integrated-MTL-FGSM significantly outperforms existing baselines. Integrating Explainable Artificial Intelligence (XAI) elucidates the complex evaluation processes within the deep learning black box, offering potentially diagnostic feedback for students and assisting educators in delivering individualized instruction guided by word-level attributions across diverse analytic rating scales. These insights enhance the robustness, generalizability, and interpretability of multi-dimensional AES models, thus paving an interdisciplinary pathway for multi-dimensional writing assessment and instruction.
