Background Chronic pelvic pain (CPP) and related genital pain are disabling and multifaceted, but understudied in transgender and gender-diverse (TGD) people. Methods In this scoping review, we mapped evidence on prevalence, aetiology, assessment, and management across gender-affirming hormone therapy (GAHT), surgical, and community contexts. Following Joanna Briggs Institute methodology and Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) reporting guidance, we searched ten databases from database inception to 30 July 2025, supplemented by citation chasing, and including primary-data reports of duration-defined CPP (persistent or recurrent pelvic pain for at least three months) or related pelvic and genital pain within a broad chronic pain framework. The search protocol was registered prospectively on the Open Science Framework (osf.io/mk6cp). Two reviewers independently screened records and extracted published data. Findings 24 reports representing 23 unique cohorts and 5527 TGD participants were included. Only one study explicitly estimated duration-defined CPP prevalence, reporting 17·7% among TGD adults assigned female at birth. Broader pelvic and genital pain outcomes ranged from 9·6% to 72·2% across five studies, but should not be interpreted as duration-defined CPP prevalence. Four non-mutually exclusive domains were identified: hormonal and tissue-mediated, pelvic floor and musculoskeletal, surgical and anatomical, and psychosocial and trauma-related. Assessment and diagnostic pathways were heterogeneous. Management evidence was very limited and derived almost entirely from observational studies, programme audits, and case reports; only one short randomised trial evaluated an intervention. Interpretation Our findings support gender-affirming, multidisciplinary evaluation while highlighting the need for standardised CPP definitions, validated TGD-specific patient-reported measures, prospective cohorts, and adequately powered interventional trials. Funding None.

Mattia Fienga; Uwe J.F. Tietge Cardiovascular Diabetology – Endocrinology Reports, 2026, Vol. 12, Issue 1 https://doi.org/10.1186/s40842-026-00336-x

The number of people seeking gender-affirming medical care and gender-affirming hormone therapy (GAHT) is increasing. As sex steroids shape cardiometabolic physiology, GAHT is expected to meaningfully alter cardiometabolic risk. The aim of this review was to systematically determine and narratively synthesize evidence on the cardiometabolic effects of GAHT and identify clinically relevant knowledge gaps. We searched PubMed for English-language primary human studies published through November 26, 2025. Of 346 screened records, 96 unique original investigations were included. Most studies had less than 3 years of follow-up, the longest being 20 years in one electronic health record-based study. In transgender women, BMI was often reported as stable with higher fat and lower lean mass compared to baseline; insulin resistance tended to increase while fasting glucose and HbA1c were usually longitudinally stable. Lipid changes often showed reductions in total and low-density lipoprotein cholesterol as well as triglyceride levels. Blood pressure generally remained stable or decreased. In transgender women, VTE incidence was higher than in both cisgender women and men, whereas stroke and MI incidence was higher primarily in comparison with cisgender women. In transgender men, BMI was increased or stable and lean mass increased with variable fat mass changes and some long-term increases in visceral and liver fat. Glucose usually remained stable while insulin resistance findings were mixed. Lipid profiles commonly shifted toward lower high-density lipoprotein cholesterol and higher low-density lipoprotein cholesterol and triglycerides. Blood pressure findings were mixed with some cohorts showing an increase. In transgender men, MI incidence was higher than in cisgender women but generally not higher than in cisgender men; evidence for other cardiovascular outcomes was limited and inconsistent. Overall, evidence on systemic inflammation, renal and hepatic outcomes is sparse. In conclusion, GAHT is associated with quantifiable cardiometabolic changes, but magnitude and clinical relevance vary by regimen, population, baseline risk and study follow-up duration. Heterogeneous regimens, study methods, and confounding contribute to mixed findings. Major gaps persist with limited long-term prospective data, sparse mechanistic insights, underrepresentation of adolescents and non-European ancestry populations, and insufficient information on cardiometabolic outcomes with ageing. Sex- and gender identity-stratified evidence synthesis: Distinguishes feminizing and masculinizing GAHT while presenting parallel outcome analyses for transgender women and transgender men, enabling more clinically precise interpretation of cardiometabolic effects. Systematic characterization of heterogeneity: Integrates regimen-, population-, and context-specific findings, including discordant results, into structured domain-specific evidence tables, improving transparency and interpretability across a heterogeneous literature base. Novel evidence mapping framework: Introduces a domain-level framework quantifying both evidence volume (number of primary studies) and directional concordance of findings, visualized within a sex-stratified cardiometabolic risk continuum to identify areas of consistency, uncertainty, and evidence gaps.

Michael Colwill; Richard Pollok; Chandni Radia; Leighton Seal; Andrew P. Poullis Clinical and Experimental Gastroenterology, 2026, Vol. Volume 19, pp. 1-12 https://doi.org/10.2147/ceg.s633494

Purpose: Transgender and gender non-conforming (TGNC) individuals are under-represented in inflammatory bowel disease (IBD) research, and little is known about their diagnostic experiences, disease burden, mental health comorbidity, or quality of life (QoL). We therefore aimed to characterise clinical features, diagnostic timelines, comorbidity, and QoL among TGNC people with IBD and to contextualise key outcomes using a matched cisgender cohort. Patients and Methods: We conducted a nationwide cohort study within the IBD BioResource population. Using questionnaires and linked BioResource data, we collected information on demographics, clinical history, gender-affirming care, comorbidity, and QoL. TGNC participants were matched to cisgender controls by sex and age category for a contextual and exploratory comparison. Analyses were performed using regression models. Results: Thirty-nine TGNC participants responded (46.2% Crohn’s disease, 43.6% ulcerative colitis, 10.3% IBD-unclassified). Median time from symptom onset to diagnosis was 10.5 months (interquartile range 6.0– 18.0), 62.3% reported prior hospitalisation and 28.2% prior IBD surgery. Psychiatric comorbidity was frequent (79%). Gender-affirming hormone therapy was reported in 30.7% and prior gender-affirming surgery in 17.9%. Matched analyses demonstrated a trend towards longer diagnostic delays and higher rates of hospitalisation and IBD-related surgery in TGNC participants. Psychiatric comorbidity was significantly higher (79.5% vs 56.8%, p =0.033). QoL impairment was substantial and broadly comparable between groups. Conclusion: TGNC individuals with IBD demonstrated a high burden of psychiatric comorbidity and signals of longer diagnostic timelines and poorer disease control compared with cisgender peers. These findings highlight potential disparities across IBD care pathways and underscore the need for larger, dedicated studies. Keywords: ulcerative colitis, Crohn’s disease, inflammatory bowel disease, transgender persons, quality of life
