This quantitative comparative investigation evaluated and contrasted levels of state-trait anxiety and generalized anxiety symptoms between early and late adolescent cohorts (N = 100). A cross-sectional comparative survey design was employed using standardized psychometric instruments: the 40-item State-Trait Anxiety Inventory (STAI) and the Generalized Anxiety Disorder-7 scale (GAD-7). Non-parametric Mann-Whitney U tests and chi-square independence analyses revealed no statistically significant differences in overall anxiety scores between early and late adolescents across all dimensions (p > .26). However, over 47% of participants across both groups met clinical severity cut-offs ?10on GAD-7). State and trait anxiety demonstrated a robust positive association (? = 0.81, p < .001). The findings underscore an urgent imperative for universal, stage-independent psychological screening and preventive mental health frameworks within educational environments.
Background: Brief psychological treatment programmes in primary care are increasingly used to address common mental disorders (CMDs), yet patient outcomes remain heterogeneous. Routine symptom measures may help identify patients who are more or less likely to achieve clinically meaningful change and may support treatment planning within stepped-care systems. This study examined associations between baseline depressive and anxiety symptom severity predicted reliable improvement and reliable recovery following brief psychological treatment in a real-world primary care programme. Methods: This secondary analysis used routinely collected data from 512 adults treated in a real-world primary care psychological intervention programme. Participants were assessed with the Patient Health Questionnaire-9 (PHQ-9) and Generalized Anxiety Disorder-7 (GAD-7) before and after brief psychological treatment. Baseline demographic and clinical variables, including age, sex, number of sessions, and PHQ-9 and GAD-7 scores, were examined according to reliable improvement and reliable recovery status. Logistic regression analyses examined associations between baseline variables and treatment outcomes. Results: Reliable improvement was observed in 73.6% of participants, whereas 53.3% achieved reliable recovery. Higher baseline anxiety severity was associated with reliable improvement (odds ratio [OR] = 1.16, 95% confidence interval [CI]: 1.08–1.23), whereas lower depressive severity predicted reliable recovery (OR = 0.92, 95% CI: 0.88–0.96). Fewer treatment sessions were associated with both reliable improvement and reliable recovery. Age and sex were not significantly associated with outcomes. Conclusions: Baseline anxiety and depressive symptom severity may be relevant for understanding variability in clinically meaningful outcomes following brief psychological interventions in primary care. Patients presenting with greater depressive symptom burden may benefit from closer monitoring and more intensive or extended treatment pathways within stepped-care models.
Background Digital mental health platforms routinely deliver self-administered symptom scales on the assumption that users with more severe symptoms are more likely to enter professional care. That assumption is rarely tested with linked participant-level data, and almost never in the Arabic-speaking Gulf. Objective To examine whether symptom severity at a user’s first self-assessment predicts booking a professional session within 90 days, across four symptom domains, on a Saudi commercial digital mental health platform. Methods Retrospective cohort analysis of 10,100 first assessments — depression, generalized anxiety, panic, and obsessive-compulsive scales — contributed by 5,524 unique users of the Talbinah platform and linked to demographic and session-booking records through a pseudonymous (SHA-256 hashed) user identifier. The primary outcome was any session booked within 90 days after the assessment. Severity was banded by the established cut-offs for the PHQ-9 and GAD-7, and by percentage of scale maximum for the panic and obsessive-compulsive measures, for which no severity thresholds are established. Per-scale multivariable logistic regression estimated adjusted odds ratios (aOR) for severity band (reference None/Minimal), adjusting for age, sex, and device type. Pre-specified sensitivity analyses used 30- and 180-day windows, an attended-session outcome, percentage-of-maximum banding applied to all four scales, and a subset with no prior booking (incident help-seeking). Reporting followed STROBE. Results Users were young (mean age 26.7 years, SD 8.1) and predominantly female (78.1%); 16.6% of first assessments were followed by a booking within 90 days. No severity band on any of the four scales was associated with higher odds of booking (severe vs None/Minimal: depression aOR 0.71, 95% CI 0.51–1.01; generalized anxiety 0.76, 0.49–1.16; panic 0.83, 0.52–1.31; obsessive-compulsive 1.01, 0.59–1.74). One severity contrast was statistically significant, in the opposite direction: moderate depression was associated with lower odds of booking (0.66, 95% CI 0.45–0.97). Older age (aOR ~1.2 per decade) and iOS device use (1.4–2.0) were associated with booking across scales. This absence of a positive severity–booking association was consistent across 30-, 90-, and 180-day windows, the attended-session outcome, and percentage-of-maximum banding; a nominal association for moderate anxiety seen under percentage banding did not persist with validated GAD-7 cut-offs. Estimates for the panic and obsessive-compulsive scales, which lack established severity thresholds, are exploratory. Conclusions On a Saudi digital mental health platform, greater self-reported symptom severity at first assessment was not followed by higher subsequent booking of professional care, consistently across specifications, and for moderate depression was associated with lower odds of booking; demographic and access-related attributes showed stronger associations with booking than symptom load did. Self-screening on its own appears insufficient as a pathway into care.
Background Depression and anxiety symptoms are common among college students. Behavioral and psychosocial indicators may support preventive campus mental health work, but risk-stratification frameworks require external validation. Methods We developed a construct-level model-explanation framework using questionnaire data from a development sample. The cleaned dataset included 1,267 students; internal-validation analyses included 1,096 students for depression risk and 1,095 for anxiety risk. Risk was defined as a Patient Health Questionnaire-9 (PHQ-9) or Generalized Anxiety Disorder-7 (GAD-7) score of 10 or higher. The primary Feature set C random-forest (RF) pipelines integrated demographic, traditional, and emerging behavioral and psychosocial indicators. Fold-specific preprocessing and percentile cutoffs were used for internal validation. Locked pipelines were applied without retraining, predictor reselection, optimization, recalibration, or threshold adjustment to an independent university sample of 1,468 students. Results External area under the receiver operating characteristic curve (AUROC), area under the precision–recall curve (PR-AUC), and Brier scores were 0.813 (95% CI, 0.776–0.848), 0.402 (0.325–0.491), and 0.090 (0.083–0.098), respectively, for depression and 0.809 (0.765–0.851), 0.205 (0.144–0.300), and 0.063 (0.056–0.070), respectively, for anxiety. Ordinary participant-level bootstrap was the primary approach, with stratified bootstrap as a sensitivity analysis. Depression calibration was close to the prevalence baseline (Brier 0.091; intercept −0.668; slope 1.401), whereas anxiety showed miscalibration (Brier 0.063 vs. baseline 0.051; intercept −1.227; slope 1.102). The models supported external discrimination and risk ranking but did not validate individual absolute-risk probabilities or clinical thresholds. Conclusion Behavioral and psychosocial indicators supported externally validated risk stratification in an independent university sample. Anxiety-risk findings should be interpreted as preliminary because the number of positive cases was limited. Further evaluation is needed before wider implementation.
Background Clinical research on psychedelic-assisted therapy (PAT) has expanded substantially in recent years, particularly for post-traumatic stress disorder (PTSD) and depressive disorders. Contemporary studies have generally evaluated psychedelic administration within controlled therapeutic settings that include psychological support, preparation, and integration. However, variability in clinical response and methodological challenges have highlighted the importance of individual, relational, and contextual factors, often described as “set and setting,” in understanding therapeutic outcomes. The role of psychotherapy and the broader therapeutic context therefore remains an important area of investigation. Objective This scoping review aimed to map the contemporary evidence on PAT, characterize the clinical and psychotherapeutic approaches used with psilocybin, MDMA, LSD, and DMT, summarize reported clinical and safety findings, and identify methodological, mechanistic, ethical, and implementation considerations across existing protocols. Methods A structured search of PubMed, PsycINFO, Scopus, and ClinicalTrials.gov was conducted on 23 February 2026. Sources published or publicly available between January 2020 and November 2025 were eligible. The review included peer-reviewed and relevant non-peer-reviewed sources addressing psychedelic-assisted interventions, including primary clinical studies, reviews, clinical trial registrations, regulatory documents, and other relevant contextual sources. Records were managed using Zotero and screened against predefined Population, Concept, and Context criteria. Findings were synthesized descriptively according to source type and evidence domain. Results The mapped evidence was most developed for MDMA-assisted therapy in PTSD and psilocybin-assisted interventions for depressive disorders. Randomized clinical trials reported reductions in PTSD and depressive symptoms following MDMA- and psilocybin-based interventions, respectively, while emerging clinical evidence also investigated LSD-based treatment for generalized anxiety disorder. Evidence for DMT-assisted therapy remained comparatively preliminary, with several available sources consisting of trial registrations or industry-reported information. Across the literature, preparation, supervised psychedelic administration, and integration were common therapeutic components, although the structure and intensity of psychotherapy varied substantially. Functional unblinding, expectancy effects, heterogeneity of psychotherapeutic approaches, and limitations in long-term safety data were recurring concerns. Ethical issues included informed consent during altered states of consciousness, therapist competence and boundaries, cultural considerations, and equitable access. Conclusion PAT represents a rapidly developing area of clinical research, but the evidence base remains heterogeneous across compounds, indications, therapeutic models, and levels of evidential maturity. Future research should improve the standardization and reporting of psychotherapeutic interventions, strengthen methods for assessing blinding and expectancy, establish consistent long-term safety monitoring, and develop robust ethical and equity frameworks. Clear distinction between peer-reviewed clinical findings and preliminary or contextual evidence will also be essential as PAT research progresses toward broader clinical implementation.
Background Correctional workers (CWs), are frequently exposed to traumatic workplace events, placing them at higher risk of experiencing mental health disorders, including major depressive disorder (MDD), generalized anxiety disorder (GAD), and posttraumatic stress disorder (PTSD). Despite high prevalence of mental health challenges, CWs underutilize mental health services due to stigma, irregular work schedules, and accessibility issues. While cognitive behavioral therapy (CBT) is effective, it poses accessibility challenges. Electronic CBT (eCBT) offers a scalable, accessible alternative, but use among CWs remains underexplored. Objective This study aimed to evaluate the efficacy of diagnosis-specific eCBT programs designed for CWs in reducing symptoms of MDD, GAD, and PTSD compared to treatment as usual (TAU). Methods A randomized controlled trial (RCT) was conducted with 84 CWs in Ontario, assigned to either eCBT (n=40) or TAU (n=44). The eCBT program provided diagnosis-specific modules with synchronous personalized feedback from care providers over 12-weeks via the Online Psychotherapy Tool (OPTT). Symptom severity was measured at baseline, week-6, and post-treatment using validated scales for depression, anxiety, PTSD, and quality-of-life. Data analysis included paired samples t-tests, mixed linear models, and effect size calculations (Cohen’s d). Results The eCBT group exhibited significant reductions in symptom severity for depression, anxiety, and PTSD from baseline to post-treatment compared to TAU (p<0.01), with a moderate effect size (Cohen’s d=0.68). Symptom severity decreased by 61.37% for eCBT versus 23.29% in TAU. Mixed linear models confirmed a significant treatment-by-time interaction, favoring eCBT (p<0.01). However, no significant differences in quality-of-life improvements were observed between groups. Conclusions Diagnosis-specific eCBT programs are effective in reducing symptoms of depression, anxiety, and PTSD for CWs. The asynchronous, accessible format of eCBT addresses current key barriers. Future research should explore strategies to improve adherence and increase the accessibility of services for CWs.
Objective Although the State–Trait Anxiety Inventory (STAI) is a well-established instrument for anxiety assessment, its length may limit feasibility in large-scale, longitudinal, and time-sensitive research settings. Existing abbreviated forms were mainly created for adults worldwide, including Türkiye. Accordingly, this study aimed to develop and psychometrically evaluate abbreviated versions of the STAI-State (STAI-S) and STAI-Trait (STAI-T) for Turkish adolescents. Method A two-phase psychometric validation design was implemented across two independent cohorts. Exploratory factor analysis (EFA) was initially conducted in Sample 1, comprising adolescents recruited from an outpatient child and adolescent psychiatry clinic ( n = 420), to identify the most parsimonious item configurations. Subsequently, confirmatory factor analysis (CFA) and criterion-related validity analyses were performed in Sample 2, consisting of public high school students ( n = 277). Measurement invariance across clinical/community status, sex, and age, comparisons with alternative factor models, and receiver operating characteristic analyses against the Generalized Anxiety Disorder-7 were examined. Results EFA supported a stable two-factor structure for both six-item scales, accounting for 53.7% of the total variance for the STAI-S and 57.3% for the STAI-T. Internal consistency was high for both abbreviated instruments ( α = 0.804). CFA indicated mixed but broadly acceptable fit for the two-factor solutions (STAI-S: χ 2 /df = 3.66, CFI = 0.979, SRMR = 0.033, RMSEA = 0.098; STAI-T: χ 2 /df = 3.28, CFI = 0.987, SRMR = 0.028, RMSEA = 0.091). Standardized factor loadings ranged from 0.61 to 0.92 across models. Invariance of item thresholds and factor loadings held across clinical/community status, sex, and age in analyses treating the items as ordered-categorical. The shortened versions also showed a high correlation with the original 20-item scales and preserved their criterion-related performance. Concurrent validity was supported by correlations with the Generalized Anxiety Disorder-7 and the Satisfaction with Life Scale. Conclusion The findings provide preliminary evidence supporting the reliability and structural validity of the six-item STAI-S and STAI-T in Turkish adolescents. The abbreviated forms appear suitable for research use, whereas their clinical utility requires confirmation in independent samples.
Severe mental illness (SMI) often limits participation in traditional therapy due to motivational and cognitive deficits. We examined short-term clinical changes during use of a personalized video-based mobile app ( MARO )—providing psychoeducation, emotion regulation, cognitive restructuring, and behavioral activation—in community-dwelling individuals with SMI and explored whether the timing of minimal human coaching was associated with these outcomes. This 8-week exploratory randomized crossover trial compared an Immediate-Coaching group (app + coaching → app only) and a Delayed-Coaching group (app only → app + coaching); both groups used MARO throughout the study. Among 40 randomized participants, 27 completed all assessments. Depression (Patient Health Questionnaire-9), anxiety (Generalized Anxiety Disorder-7), and positive automatic thoughts (Automatic Thoughts Questionnaire–Positive) were assessed at baseline and after 4 and 8 weeks. Both groups showed decreases in PHQ-9 and GAD-7 scores over the 8-week intervention period, while ATQ-P scores increased. Overall PHQ-9 scores were lower and ATQ-P scores were higher in the Immediate-Coaching group than in the Delayed-Coaching group. These preliminary findings support further evaluation in larger, adequately powered controlled studies of personalized video-based mobile interventions as a potentially scalable digital–human approach to community-based care for individuals with severe mental illness. Trial Registration: CRIS, KCT0011492.
OBJECTIVE: Evaluate test-retest reliability, internal consistency, and construct validity of the Norwegian version of the Pediatric Family Needs Questionnaire (FNQ-P). DESIGN: Test-retest reliability, internal consistency, and construct validity. Subject/patients: Parents/caregivers of children (6-17 years) with ABI in the chronic phase who participated in the Child in Context Intervention (CICI) study. METHODS: Internal consistency was assessed with Cronbach's alpha, while test-retest reliability was explored with intraclass correlation coefficients (ICC), t-tests, and coefficients of variation. Standard error of measurement and minimal detectable change were also assessed. Construct validity in relation to Matrices and Similarities from WISC-V, Health and Behavior Inventory (HBI), Behavior Rating Inventory of Executive Function-2 (BRIEF-2), Pediatric Quality of Life Inventory (PedsQL), Tool to Measure Parenting Self-Efficacy (TOPSE), Patient Health Questionnaire-9 (PHQ-9), and Generalized Anxiety Disorder Scale-7(GAD-7) was explored with correlation coefficients. RESULTS: Test-retest reliability of the total mean FNQ-P score was good (ICC = 0.84). Internal consistency was excellent with a Cronbach's alpha of 0.98. Construct validity was confirmed. The only significant correlations were weak correlations with parent anxiety (GAD-7) and child's emotional functioning (PedsQL). CONCLUSION: The Norwegian FNQ-P has good psychometric properties and can be used to obtain reliable and valid data regarding family needs.
Background: As the war in Ukraine has become protracted, civilians have been forced to adapt to chronic danger, uncertainty, and repeated military threat. This prospective study examined mental health symptom change during a pivotal six-month period in 2023, encompassing the high hopes surrounding the rise of the Ukrainian counteroffensive and the subsequent disappointment of its stall. Methods: We conducted a two-wave online longitudinal study of 208 Ukrainian civilian adults (80.8% female; mean age 29.4 years) assessed in June 2023 and November 2023. Primary outcomes were generalised anxiety, depression, and somatic symptoms. We further examined whether symptom change was associated with baseline sleep disturbance, trait resilience, and sociodemographic factors. Subjective threat perception, confidence in military success, and objective indicators of military threat were included as contextual variables. Results: Confidence in military success declined significantly across the two waves, while objective indicators of military threat, including artillery attacks and air raid alarms, increased. Subjective threat perception remained stable. Despite this unfavourable context, generalised anxiety, depression, and somatic symptoms decreased on average over time. Individual differences in symptom change were partly associated with baseline characteristics. Higher baseline sleep disturbance predicted smaller reductions in anxiety and depression. Female sex was associated with smaller reduction in depression, and older age with greater reduction in anxiety. Trait resilience and socioeconomic factors were not significantly associated with symptom change. Conclusion: The findings suggest that anxiety, depression, and somatic symptoms may decrease over time even under worsening objective wartime conditions. This pattern is more consistent with partial adaptation under chronic threat than with improvement in external circumstances. Sleep disturbance emerged as a robust predictor of less favourable symptom change, highlighting sleep as a potentially important target for mental health support under prolonged war-related stress.
BACKGROUND: The whole-university approach to mental health emphasises that well-being should be a strategic priority across all aspects of higher education. However, there is limited evidence on which aspects of university life institutions should target to maximise well-being. AIM: To identify factors associated with student mental health and well-being and provide an evidence base for developing initiatives that protect and promote this. METHODS: The Nurture-U survey was conducted across five universities in the UK from 2022 to 2024. This cross-sectional sample of the survey data assessed student well-being and mental health at the start of the academic year. Hierarchical linear regression models examined the relative association between demographic, family history, university and lifestyle variables and self-reported depression, measured using the Patient Health Questionnaire-9 (PHQ-9), anxiety, measured using the Generalised Anxiety Disorder-7 (GAD-7) and well-being, measured using the Short Warwick-Edinburgh Mental Well-being Scale (SWEMWBS). RESULTS: A total of 14 172 students completed the survey. Over a quarter self-reported probable anxiety or depression based on established screening thresholds. Regression analysis indicated that younger age, identifying as female or lesbian, gay, bisexual, transgender, queer/questioning, intersex or asexual (LGBTQIA+) and reporting a disability were associated with poorer mental health. There was minimal evidence of mental health impact associated with university factors, such as course of study or university attended. There was a strong association with exercise and self-care. CONCLUSIONS: We have provided evidence that exercise and self-care may be areas that universities can target when considering a whole-university approach to student mental health and well-being. Increasing opportunities for students from all backgrounds, means and abilities to adopt healthy behaviours is vital to supporting university students.
Huge citizens expose to social media during a novel coronavirus disease (COVID-19) outbroke in Wuhan, China. We assess the prevalence of mental health problems and examine their association with social media exposure. A cross-sectional study among Chinese citizens aged ≥18 years old was conducted during Jan 31 to Feb 2, 2020. Online survey was used to do rapid assessment. Total of 4872 participants from 31 provinces and autonomous regions were involved in the current study. Besides demographics and social media exposure (SME), depression was assessed by The Chinese version of WHO-Five Well-Being Index (WHO-5) and anxiety was assessed by Chinese version of generalized anxiety disorder scale (GAD-7). multivariable logistic regressions were used to identify associations between social media exposure with mental health problems after controlling for covariates. The prevalence of depression, anxiety and combination of depression and anxiety (CDA) was 48.3% (95%CI: 46.9%-49.7%), 22.6% (95%CI: 21.4%-23.8%) and 19.4% (95%CI: 18.3%-20.6%) during COVID-19 outbroke in Wuhan, China. More than 80% (95%CI:80.9%-83.1%) of participants reported frequently exposed to social media. After controlling for covariates, frequently SME was positively associated with high odds of anxiety (OR = 1.72, 95%CI: 1.31–2.26) and CDA (OR = 1.91, 95%CI: 1.52–2.41) compared with less SME. Our findings show there are high prevalence of mental health problems, which positively associated with frequently SME during the COVID-19 outbreak. These findings implicated the government need pay more attention to mental health problems, especially depression and anxiety among general population and combating with “infodemic” while combating during public health emergency.
