Video and digital games: Historical controversies, benefits, and risks for children and adolescents

Hanna David Journal of Child Development, Exceptionality and Education, 2026, pp. 47-59 https://doi.org/10.5281/zenodo.22768009

Video and digital games have been the focus of sustained public and scientific debate since their commercial emergence in the late 1970s, particularly regarding their effects on children and adolescents. Early public and scientific discourse frequently portrayed gaming as a source of physical injury, addiction, aggression, social isolation, academic problems, and moral decline. However, subsequent empirical research has produced a substantially more differentiated picture of both the benefits and risks of gaming. This review examines the historical foundations of controversies surrounding video and digital games and synthesizes contemporary evidence concerning their cognitive, emotional, social, therapeutic, and physical effects. . Research indicates that moderate, purposeful gaming may improve visuospatial abilities, attentional control, problem-solving, mood regulation, social connection, and selected educational outcomes. Gaming-based interventions have also demonstrated therapeutic potential, particularly for depressive symptoms and attention-related difficulties among young people. At the same time, important risks remain, especially when gaming becomes excessive, compulsive, or disruptive to everyday functioning. These include repetitive-strain injuries, sleep disruption, problematic gaming, and adverse mental-health and academic outcomes. Evidence concerning violent games and serious real-world aggression remains more contested, with recent research emphasizing relatively small effects and the importance of individual and contextual factors. Overall, the evidence suggests that the developmental consequences of gaming for children and adolescents depend less on gaming itself than on the duration, content, context, and pattern of engagement.

Determinants of Insomnia among Undergraduate Female Students in a Rural Area of Haryana: A Mixed-method Study

Ilakkiya P; Ramesh Kumar Verma; Anita Punia; Sree Sathvik; Jitender Kaushik; Sandeep Rao; … Indian Journal of Sleep Medicine, 2026, Vol. 21, Issue 4, pp. 158-163 https://doi.org/10.5005/jp-journals-10069-0202

Aim and background: Sleep is a biological necessity essential for physical and mental well-being, yet many young adults, particularly college students, experience sleep disruptions due to major life changes and poor sleep hygiene. This study aimed to identify the determinants of insomnia among undergraduate female students in a rural area of Haryana.Materials and methods: A mixed-method study was conducted between May 2024 and May 2025 involving 340 undergraduate female students.Quantitative data were collected using a semi-structured socio-demographic questionnaire and the insomnia severity index (ISI).Qualitative insights were gathered through in-depth interviews (IDI) with 15 students identified with clinically significant insomnia.Statistical analysis was performed using SPSS software version 26, with significance set at p < 0.05.Results: The prevalence of clinically significant insomnia in the cohort was 5.6% (n = 19).Quantitative analysis revealed that the participant's state of origin was the only statistically significant socio-demographic determinant (p = 0.031).Students from states other than Haryana reported a significantly higher insomnia prevalence (12.5%) compared to those from Haryana (4.2%).Factors such as age, socio-economic status, and type of occupancy showed no significant correlation.Qualitative themes highlighted academic performance, overuse of electronic devices, social environment, and psycho-social stress as primary triggers. Conclusion: While the prevalence of insomnia is relatively low, it is significantly driven by academic transitions, excessive screen time, and communal living challenges.A notable awareness-action gap exists, as students recognize the importance of sleep but prefer self-management or mindfulness over professional clinical help.

Beyond screen time: artificial intelligence, algorithmic media and the future of child health

Venugopal Reddy I. International Journal of Contemporary Pediatrics, 2026, Vol. 13, Issue 11, pp. 2379-2385 https://doi.org/10.18203/2349-3291.ijcp20263830

Digital media has evolved from passive screen viewing into an algorithmically curated ecosystem of social platforms, short form video, games, generative artificial intelligence (AI), conversational agents, connected toys and educational technologies. These systems can support learning, creativity, communication, disability access and healthcare; however, engagement optimized design, commercial surveillance and synthetic content create risks that are not adequately captured by screen-time measures alone. This narrative review examines the effects of contemporary digital ecosystems on child and adolescent health and outlines implications for pediatric practice. PubMed/MEDLINE, Google Scholar and guidance from the World Health Organization, United Nations Children's Fund, UNESCO and the American Academy of Pediatrics were reviewed, with emphasis on evidence published from 2019 to June 2026. Associations have been reported between problematic digital media use and sleep disruption, reduced physical activity, visual and musculoskeletal symptoms, obesity related behaviours, anxiety, depressive symptoms, cyberbullying, disordered eating and impaired attention. Causal interpretation remains difficult because effects are heterogeneous, often small, bidirectional and strongly modified by age, content, context, displacement, family relationships and pre-existing vulnerability. AI adds distinct concerns: persuasive personalization, inaccurate health information, anthropomorphic dependency, deepfakes, sexual exploitation, biased outputs, privacy loss and erosion of learning through uncritical outsourcing. Benefits are also substantial when technologies are developmentally appropriate, evidence-based, inclusive and used with adult support. Pediatric counselling should shift from counting hours alone toward assessing the child's digital ecosystem, sleep, function, content, compulsivity, safety and relationships. Effective protection requires shared family practices, school digital literacy, safety by design, age-appropriate assurance, transparent algorithms, data minimization and independent research. The goal is not a screen free childhood but a child centred digital environment that supports health, agency and development.

Assessment of Sleep Quality and Modifiable Disruptors in Postoperative Paediatric Patients: A Clinical Audit

Paediatric: A Clinical Audit

Zeenia Merali; Mohammed Hazara Cureus, 2026, Vol. 18, Issue 10, pp. e118296 https://doi.org/10.7759/cureus.118296

BackgroundSleep disruption is common during postoperative hospitalisation and may be influenced by pain, overnight clinical care, and environmental factors. This quality improvement project assessed sleep quality among postoperative paediatric inpatients and identified potentially modifiable contributors to sleep disturbance. MethodologyA clinical audit was conducted on a paediatric inpatient ward between April and June 2026.Children aged 0-18 years admitted following elective or emergency orthopaedic surgery and requiring at least one overnight postoperative stay were eligible.Parents or carers completed a questionnaire after the child's first postoperative night assessing sleep quality, nighttime awakenings, causes of sleep disturbance, ward noise and lighting, clustering of overnight care, assessment of sleep needs, and parent/carer sleep.Responses were analysed descriptively using frequencies and percentages. ResultsIn total, 19 questionnaires were included: 14 from children accommodated in multi-bed bays and five from children in individual cubicles. In bays, eight of 14 (57.1%)children were reported to have fair sleep and six (42.9%) woke more than five times overnight. In cubicles, two of five (40.0%) children were reported to have good sleep and one (20.0%)woke more than five times.Nursing observations were reported as a contributor to sleep disturbance in 10 of 14 (71.4%)children in bays and four of five (80.0%) in cubicles.Pain contributed to sleep disturbance in 10 of 19 (52.6%) children overall. Most respondents reported that the ward was quiet overnight and that lights were kept low. However, only seven of 19 (36.8%) families reported being asked about their child's sleep needs.Parent/carer sleep was also frequently disrupted. ConclusionsPostoperative sleep disruption was common, with nursing observations and pain among the most frequently reported contributors.Despite generally favourable perceptions of overnight noise and lighting, opportunities for improvement were identified in assessment of individual sleep needs, coordination of overnight care where clinically appropriate, and optimisation of pain management.A subsequent audit cycle following implementation of these measures will assess their impact on sleep-related outcomes.

Psychological support for depression and anxiety across chronic illness contexts: a comparative evaluation of three general-purpose AI chatbots

Wei Xiao; Xuwei Cao; Wangahe Jin; Xiaoyi Chen; Jun Cai; 駱旭琛; … Frontiers in Psychiatry, 2026, Vol. 17 https://doi.org/10.3389/fpsyt.2026.1962615

Background Digital mental health tools, including general-purpose artificial intelligence (AI) chatbots, may provide accessible and low-threshold psychological support for people living with chronic conditions. However, their value in this population depends not only on recognizing depression or anxiety, but also on understanding how psychological distress is shaped by disease-specific experiences and when self-guided support should transition toward professional care. We evaluated the appropriateness, contextual sensitivity, safety, and consistency of psychological support provided by three widely accessible AI chatbots across contrasting chronic illness contexts. Methods We conducted a standardized simulated-patient benchmarking study comparing ChatGPT, Gemini, and Doubao. Twenty scenarios were developed across type 2 diabetes mellitus (T2DM) and chronic spontaneous urticaria (CSU), representing matched mild and moderate presentations of depression and anxiety together with condition-specific distress related to diabetes self-management and CSU-related flare uncertainty. Each scenario was independently submitted to all three systems in two testing rounds, yielding 120 responses. Blinded raters assessed overall psychological support appropriateness, recognition and emotional support, chronic-illness contextualization, psychological support and referral, psychosomatic balance and safety, and major clinical or safety errors. Repeated testing assessed response consistency. Results Overall psychological support appropriateness differed across systems (P = 0.002), with 90.0% of ChatGPT, 75.0% of Gemini, and 55.0% of Doubao responses meeting the prespecified appropriateness threshold. Recognition and emotional support was consistently strong across systems, whereas greater differences emerged in chronic-illness contextualization (P = 0.001), psychological support and referral (P = 0.002), and psychosomatic balance and safety (P<0.001). All responses to mild presentations were classified as psychologically appropriate, compared with 58.3% of responses to moderate presentations, indicating that performance became less reliable when digital support required stronger escalation toward professional assessment. Contextualization was also less consistent in CSU, particularly when responses needed to incorporate flare unpredictability, pruritus, sleep disruption, and illness uncertainty. Major clinical or safety errors occurred in 0%, 10.0%, and 15.0% of first-round ChatGPT, Gemini, and Doubao responses, respectively. Test–retest consistency was moderate and varied between systems. Conclusions General-purpose AI chatbots show potential as accessible, self-guided digital mental health resources for people living with chronic conditions, particularly for recognizing distress and providing initial supportive guidance. Their current limitations emerge when effective support requires disease-specific contextualization, balanced interpretation of psychological and somatic factors, or escalation as symptoms become more severe. These findings identify important boundaries for the use of conversational AI in chronic-illness mental health care and suggest that implementation should emphasize context-sensitive guidance, severity-responsive referral, and clear integration with rather than substitution for professional care.

Self-reported Sleep Quality and Perceived Sleep Disruptors among Hospitalized Patients in a Tertiary Care Hospital: A Cross-sectional Study

Rohan Mishra; Rounak Mandal; Ruhan Rana Hossein; Saed Hamza Molla; Sahanaj Parvin; Sahid Anowar; … The ESRF Research Journal for Undergraduate Medical Students, 2026, Vol. 4, Issue 1 https://doi.org/10.5281/zenodo.23368561

Abstract Introduction: Sleep disturbance is common among hospitalized patients and may adversely affect recovery and well-being. This study aimed to determine the proportion of patients hospitalized for at least 3 days, with unsatisfactory sleep, describe perceived sleep disruptors, and assess their association with socio-demographic, clinical and hospital-related factors. Materials and Methods: A hospital-based descriptive cross-sectional study was conducted among patients admitted to five randomly selected in-patient departments of a tertiary medical college, Kolkata, from 10 April to 20 May 2026. Eligible patients admitted for at least three days were recruited. Data were collected through face-to-face interviews using a pre-designed, pre-tested schedule. Perceived sleep quality and perceived sleep disruptors were assessed using 5-point Likert scales. Participants reporting fair, poor or very poor sleep were categorised as having unsatisfactory sleep. Associations were assessed using Chi-square/Fisher's exact test and Mann–Whitney U test, with p<0.05 considered significant. Results: A total of 183 patients were included in the final analysis. The median age was 40 years (IQR: 26–46.5), 50.8% were male, and median length of stay was 7 days (IQR: 4–13.5). Unsatisfactory sleep was reported by 101 (55.2%) participants, including 36 (19.7%) with fair, 39 (21.3%) with poor and 26 (14.2%) with very poor sleep. Acute medical conditions related to the current admission were the most commonly reported significant disruptor of sleep (77, 42.1%), followed by pre-existing medical conditions (45, 24.6%), noise (40, 21.9%) and light (38, 20.8%). Unsatisfactory sleep was significantly associated with perceived disruption from noise (p<0.001), light (p=0.006), frequent vital-sign monitoring (p=0.016), personal care routines (p=0.037), existing medical conditions (p=0.033), and acute medical conditions (p=0.010). Age, sex, department of admission, diagnosis and length of stay showed no significant association with sleep quality. Conclusion: More than half of hospitalized patients experienced unsatisfactory sleep. Clinical conditions and modifiable hospital-related factors, particularly noise, light and interruptions related to routine care, were associated with poorer sleep. Measures to minimize unnecessary nighttime disturbances may improve sleep among hospitalized patients.

Feasibility of implementing a mindfulness-based program for paediatric intensive care unit staff

Rahul Joshi; Gayatri Marwah; Sainath Raman; Kristen Jones; Connie Cai Ru Gan; J.M. HOCKLEY Australian Critical Care, 2026, Vol. 39, Issue 6, pp. 101703 https://doi.org/10.1016/j.aucc.2026.101703

Background Paediatric intensive care unit (PICU) clinicians are particularly susceptible to workplace stress due to frequent ethical dilemmas (for example, perceptions of futile or unethical treatment), patient morbidity and mortality, sleep disruption, and emotional exhaustion, putting them at increased risk of anxiety, depression, and burnout. The psychological well-being of these healthcare professionals is vital for maintaining high standards of patient care and safety. Objective The aim of this study was to evaluate the feasibility of implementing a mindfulness-based program among PICU staff, with a focus on acceptability and barriers to participation. Methods Weekly 20-min guided group-practice sessions in mindfulness, with both in-person and virtual attendance options, were held over a 6-month period. These sessions were offered to medical, nursing, and allied health staff working in the PICU of a specialist paediatric hospital in Queensland, Australia, serving families from Queensland and northern New South Wales. The program was offered during a period of increased personal and occupational stress arising from the delivery of high-acuity care during the global COVID-19 pandemic. Findings The program was successfully delivered within the PICU setting, with most participants expressing interest in continuation of the program (72%). Qualitative responses suggested perceived individual and collective well-being benefits, alongside generally positive perceptions of the program. However, participants also identified important barriers to participation, including workload pressures and rostering constraints. Conclusions This study suggests that implementation of a mindfulness-based program within a PICU setting may be feasible while also highlighting important operational barriers to participation in high-acuity healthcare environments. Qualitative findings indicated generally positive staff perceptions and perceived acceptability of the program. Further research using longitudinal designs is recommended to better understand implementation considerations and potential longer-term outcomes associated with mindfulness-based interventions in PICU settings.

BACKGROUND: Poor sleep quality is a global health concern among nurses, particularly in high-intensity settings. Unpredictable workloads, extended shifts and high patient acuity compromise sleep quality, negatively affecting nurses' well-being and patient outcomes. AIM: To describe the perceived sleep quality of nurses working in high-intensity settings. STUDY DESIGN: A qualitative descriptive design was used. Purposive sampling was used to select participants who met the inclusion criteria. Data were collected through three semi-structured focus group discussions with 21 nurses working in high-intensity units of an academic hospital in Gauteng, South Africa. Thematic analysis was used to analyse data manually, and the COREQ guidelines were followed. FINDINGS: Three major themes emerged: barriers to quality sleep, consequences of poor sleep and coping mechanisms. Participants described environmental, occupational, organisational and domestic barriers to quality sleep, including community noise, long working hours, staff shortages, inadequate resources and family responsibilities. Poor sleep was associated with emotional exhaustion, impaired concentration and perceived patient safety risks. Participants reported using pharmacological and non-pharmacological strategies to manage fatigue and sleep difficulties. CONCLUSIONS: This study highlights that sleep disruption among nurses working in high-intensity settings is shaped by environmental, occupational, organisational, community and family-related factors. Poor sleep was perceived to be associated with emotional exhaustion, impaired concentration, patient safety concerns and reliance on adaptive and maladaptive coping strategies to manage fatigue and insomnia. These findings suggest that sleep disruption is a systemic issue requiring organisational and context-sensitive interventions to improve nurses' well-being and patient care outcomes. RELEVANCE TO CLINICAL PRACTICE: Improving nurses' well-being and patient safety requires interventions that address the organisational, environmental and social contributors to sleep disruption. Supportive practice environments, adequate staffing, fatigue management initiatives and psychosocial support may improve sleep quality, reduce emotional exhaustion and clinical errors and enhance nurse well-being and patient care outcomes.
