Objective: Poor insight in obsessive-compulsive disorder (OCD) has been proposed to reflect a more neurodevelopmentally rooted subtype. We examined whether minor physical anomalies (MPAs) and neurological soft signs (NSS), two peripheral markers of aberrant neurodevelopment, distinguish OCD patients according to insight. Methods: Sixty-seven adults with Diagnostic and Statistical Manual of Mental Disorders, fifth edition OCD were assessed with the Yale-Brown Obsessive Compulsive Scale, the Brown Assessment of Beliefs Scale (BABS), the Schedule for the Assessment of Insight, the Waldrop MPA scale and the Neurological Evaluation Scale.Patients were classified as having good (BABS ≤ 12; n = 37) or poor (BABS ≥ 13; n = 30) insight.Firth penalized logistic regression identified independent predictors. Results: The groups were comparable in age, sex, marital status, employment and comorbidity, whereas poor insight was associated with lower education, past suicidal behaviour and greater symptom severity.Total, craniofacial and peripheral MPA scores were higher in the poor-insight group, with an excess of ocular, auricular and oral anomalies.Low-set ears, epicanthus and high-arched palate were the most discriminating.Total NSS and all subdomains except complex motor sequencing were approximately doubled, with large effects for sensory integration and motor coordination.MPA burden predicted poor insight independently of symptom severity, while NSS was the dominant predictor when severity was excluded (apparent area under the receiver operating characteristic curve 0.97; optimism-corrected 0.95).Conclusion: These convergent findings suggest a neurodevelopmental pattern in poor-insight OCD.Given the cross-sectional design, modest sample and absence of a control group, they should be regarded as hypothesis-generating.
Contamination-related obsessive-compulsive disorder (OCD) can cause substantial distress and functional impairment, particularly when compulsive behaviors become intertwined with familial and culturally shaped beliefs about purity and cleanliness.Understanding the distinction between culturally normative practices and clinically impairing compulsions is important for culturally sensitive assessment and treatment. This case report describes an 18-year-old woman with adolescent-onset, contamination-related obsessivecompulsive symptoms that developed at approximately 14-15 years of age during a period of academic and family stress. Her symptoms included persistent feelings of being unclean after toileting, prolonged bathing and toileting, repeated checking of undergarments after urination, counting rituals, avoidance of food and water to reduce elimination-related urges, and avoidance of physical contact in crowded or public settings.A distinctive feature of the presentation was its development within a familial environment in which purity and cleanliness were associated with bodily practices and social boundaries. The patient reported routine post-defecation bathing by family members, restrictions on physical contact with individuals from Scheduled Caste backgrounds, cleansing of the household after such contact, and repeated scrutiny of the adequacy of her own washing. These experiences were considered contextual influences that may have shaped the meaning and reinforcement of contamination concerns rather than direct causes of OCD.Her behaviors were distinguished from the family practices she described by their excessive duration, associated distress, impaired control, and functional interference. The baseline Yale-Brown Obsessive Compulsive Scale (Y-BOCS) score was 24, indicating severe symptoms.Treatment focused on psychoeducation, reduction of ritualized behaviors, distraction and urgemanagement strategies, toilet retraining, relaxation, and supportive psychotherapy, alongside concurrent pharmacological treatment.By sessions six to seven, checking and counting had ceased, bathing and toileting duration had markedly decreased, food and water avoidance had resolved, and previously avoided bus travel and crowded settings were tolerated. At session nine, approximately two months after baseline, the Y-BOCS score had decreased to 4, indicating symptoms in the subclinical range. The remaining sessions focused primarily on maintaining improvement and adjustment to college, with no recurrence of the principal rituals documented through the final session. This case highlights how familial and culturally transmitted concepts of purity may shape the content of contamination-related OCD symptoms.Substantial improvement occurred during a symptom-focused behavioral intervention without directly challenging the family's broader cultural framework. However, concurrent pharmacological treatment and the availability of only one follow-up Y-BOCS assessment limit conclusions about the independent effect of psychotherapy and the durability of improvement.
Rates of response to repetitive transcranial magnetic stimulation (rTMS) in obsessive-compulsive disorder (OCD) vary widely. This systematic review and meta-analysis evaluated whether neuroimaging-guided targeting personalization, from fixed-coordinate neuronavigation to individually connectivity-personalized targeting, moderates clinical response, safety, and neural mechanisms in OCD. Following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) 2020 guidelines, PubMed, the Cochrane Library, and ScienceDirect were searched through July 2026 for randomized and non-randomized studies of neuroimaging-guided transcranial magnetic stimulation (TMS) in OCD. Risk of bias (RoB) was assessed using Cochrane RoB2 for RCTs (randomized controlled trials) and ROBINS-I (Risk of Bias in Non-randomized Studies of Interventions) for non-randomized studies. Random-effects inverse-variance and risk-ratio models pooled Yale-Brown Obsessive Compulsive Scale (Y-BOCS) scores and responder rates; non-poolable data were synthesized narratively. Nine study cohorts across eight publications met inclusion criteria (five RCTs and four open-label/single-arm). Pooled analysis of four sham-controlled RCTs showed no significant benefit of neuroimaging-guided TMS on Y-BOCS total score (MD = −0.64, 95% CI = −3.60, 2.32) or responder rate (RR = 1.46, 95% CI = 0.68, 3.15), with moderate heterogeneity. However, trials using individually connectivity-personalized pre-supplementary motor area (pre-SMA) targeting consistently trended toward benefit, while fixed-coordinate neuronavigated trials trended toward null or sham-favoring results. A narrative synthesis of non-poolable and non-randomized studies showed larger, mostly positive within-group reductions in Y-BOCS scores (25%-48%), particularly among patients with moderate disease severity, alongside mechanistic evidence linking individualized connectivity-based targets to symptom improvement. TMS was well tolerated overall, with mild, transient headache being the most common adverse event and no serious safety concerns identified. Overall, neuroimaging-guided TMS showed no pooled benefit in OCD; however, individually connectivity-personalized targeting may be more beneficial than fixed-coordinate approaches, warranting adequately powered trials that directly compare targeting-personalization strategies.
Digital cognitive behavioral therapy (CBT) has emerged as a promising strategy to improve access to evidence-based treatment for pediatric obsessive-compulsive disorder (OCD). However, its comparative efficacy relative to conventional interventions remains uncertain. This systematic review and meta-analysis evaluated the effectiveness of digital CBT in children and adolescents with OCD. PubMed, Scopus, and Web of Science were systematically searched from inception to July 2026 for randomized controlled trials (RCTs) comparing digital CBT with face-to-face CBT, waitlist, or treatment as usual. The primary outcome was Children's Yale-Brown Obsessive Compulsive Scale (CY-BOCS) scores post-treatment and at 3- and 6-month follow-up. Secondary outcomes included Clinical Global Impression-Severity (CGI-S), Clinical Global Impression-Improvement (CGI-I), Children's Global Assessment Scale (CGAS), Child Obsessive-Compulsive Inventory (ChOCI-C/P), and Family Accommodation Scale (FAS). Random-effects meta-analyses were performed using mean differences (MDs) with 95% confidence intervals (CIs). Six RCTs involving 404 participants were included. Digital CBT showed no significant difference from comparator interventions in reducing CY-BOCS scores post-treatment (MD = −2.09, 95% CI −4.92 to 0.74), at 3 months (MD = 1.15, 95% CI −0.74 to 3.04), or at 6 months (MD = 0.01, 95% CI −1.58 to 1.60). Similarly, no significant differences were observed for CGI-S, CGI-I, CGAS, ChOCI-C, ChOCI-P, or FAS. Moderate-to-high heterogeneity was present for several outcomes; however, sensitivity analyses resolved heterogeneity in most secondary outcomes. Digital CBT demonstrated comparable clinical effectiveness to conventional interventions for pediatric OCD, supporting its role as an accessible alternative when face-to-face CBT is unavailable. Larger, high-quality trials are needed to determine the optimal digital delivery modalities and long-term effectiveness.

This study investigated the relationship between celebrity worship and body dysmorphic symptoms among 1,081 adolescent female K-pop fans. We examined body image disturbance and self-objectification as mediators. Participants completed the Body Dysmorphic Disorder Yale-Brown Obsessive Compulsive Scale-Self report, the Celebrity Attitude Scale-23, the Self-Objectification Questionnaire, and the Multidimensional Body-Self Relations Questionnaire-AS. The structural equation model demonstrated significant direct and indirect pathways among the study variables, confirming the mediating roles of both variables and the overall adequacy of model fit. This suggests that intense identification with celebrities is associated with heightened vulnerability to body dysmorphic symptoms among adolescents, reflecting the internalisation of unrealistic beauty ideals and appearance-focused cognitive and emotional processes, particularly in cultures that emphasise feminine beauty. Interventions that enhance media literacy and critical thinking about beauty norms are essential, especially as societal structures intensify appearance-related pressures.
Obsessive-compulsive symptoms (OCS) have been noted in a significant portion of patients with schizophrenia. Currently, there are limited studies involving patients with both obsessive-compulsive disorder (OCD) and schizophrenia which compare their clinical characteristics with those of patients with OCD without schizophrenia. This project aims to investigate the most common OCS experienced by patients with a diagnosis of both schizophrenia and OCD in comparison to those experienced by patients with OCD without schizophrenia. This retrospective cohort study involved patients referred to the Western Sydney Obsessive-Compulsive and Related Disorders Service. The likelihood of a patient with either OCD without schizophrenia or OCD with schizophrenia presenting with a subtype of OCS was assessed using a series of multiple logistic regression models which were adjusted for demographic variables. Model results were presented as odds ratios (OR) and 95% confidence intervals (CI). Within the cohort of 99 patients, one third had a diagnosis of schizophrenia or related disorders. When adjusted for age, sex, marital status, education, employment, and Yale-Brown Obsessive-Compulsive Scale (Y-BOCS) assessment, the group of patients with co-occurring schizophrenia were more likely to experience aggressive, religious, and miscellaneous obsessions and checking compulsions. However, they were less likely to present with contamination obsessions and cleaning compulsions. This study reports the OCS that are more commonly associated with patients with both OCD and schizophrenia compared to OCD alone within a clinical sample. Future areas of research include performing a similar logistic regression analysis process on a larger cohort to allow for more symptom models to converge.
Objectives: Perceptions that something is not "just right" and feelings of incompleteness are forms of premonitory urges that extend beyond simple bodily sensations; they involve subjective interpretations of those sensations and may therefore be regarded as mental phenomena. These phenomena may precede tics but can also trigger compulsive behaviors.Investigating these phenomena is essential for understanding intermediate clinical phenotypes between tic disorders and obsessive-compulsive disorder (OCD). Methods:We retrospectively reviewed the medical records of 71 patients older than 10 years with tic disorders who attended the department of psychiatry at a hospital.Clinical assessments included the Korean versions of the Premonitory Urge for Tics Scale, Yale Global Tic Severity Scale, and (Children's) Yale-Brown Obsessive-Compulsive Scale. Results: Among patients with Tourette syndrome, 71.21% reported experiencing "just right" perceptions. Both the frequency and severity of "just right" perceptions were significantly greater in patients with tic disorders and comorbid OCD (n=8) than in those without comorbid OCD (n=63).In addition, the severity of premonitory urges showed a significant correlation with the severity of obsessivecompulsive symptoms. Conclusion: "Just right" perceptions are not only an intrinsic characteristic of tic disorders but are also associated with repetitive behaviors in OCD.These results provide preliminary clinical support for Tourettic OCD as an intermediate phenotype between tic disorders and OCD.
