Aim: Stigma related to mental illness remains a major barrier to help-seeking and equitable care. Among medical students, stigma is clinically relevant because it may influence both future patient management and students’ own willingness to seek professional support. This study examined two complementary stigma domains, public stigma endorsement and anticipated stigma with concealment/avoidance, and their association with psychiatry clerkship exposure.Materials and Methods: This cross-sectional study included 354 undergraduate medical students at Selçuk University Faculty of Medicine. Psychiatry clerkship exposure was defined as pre-clerkship (Years 1–4) versus clerkship (Years 5–6). Stigma-related constructs were measured using a 29-item researcher-developed questionnaire organized a priori into Public Stigma Endorsement (PSE) and Anticipated Stigma with Concealment/Avoidance (ASA). Internal consistency was high (Cronbach’s α = 0.884 for PSE; 0.900 for ASA). Group differences were evaluated using appropriate univariable tests. Separate multiple linear regression models were fitted for PSE and ASA, adjusting for sociodemographic characteristics, smoking, psychiatric service use, contact with mental illness (self/family/social environment), family history of stigma, and stigma awareness/knowledge.Results: PSE scores did not differ by clerkship exposure in univariable analyses and were not independently associated with clerkship status in adjusted models. Male sex was an independent correlate of higher PSE. In contrast, ASA was higher among clerkship students and remained independently associated with clerkship exposure after adjustment (B = 2.75, 95% CI 0.44–5.05, p = 0.020). Middle socioeconomic status (vs low) was also associated with higher ASA (B = 3.37, 95% CI 0.67–6.07, p = 0.014). Model fit was significant for PSE and borderline for ASA.Conclusion: Psychiatry clerkship exposure was not associated with reduced public stigma endorsement but was linked to higher anticipated stigma and concealment/avoidance. These findings underscore the need for educational strategies that explicitly target distinct stigma dimensions, including self-referential anticipated stigma that may impede help-seeking among medical trainees.

Purpose To evaluate trends in psychotropic prescription rates (PRs) in a Peruvian general hospital during and after the COVID-19 pandemic. Materials and methods We conducted an interrupted time-series (ITS) study using routinely collected clinical data from hospitalized patients with psychiatric diagnoses between September 2020 and December 2024. Psychotropic PRs, defined as the number of weekly psychotropic prescriptions per 1,000 psychiatric consultations, were calculated overall and by subclass: antidepressants (N06A), antipsychotics (N05A), anxiolytics (N05B), and mood stabilizers (N05AN01/N03A). For ITS analysis, the study period was divided into six predefined phases (two waves, three relaxation phases, and one post-pandemic phase) based on COVID-19 mortality and transmission trends in Peru. Segmented regression models were applied to estimate phase-specific level and slope changes. Results A total of 6,151 patients were included. The overall psychotropic PR was higher during the post-pandemic period compared with the pandemic period (1,308 vs. 1,254; p = 0.023). ITS analyses showed a significant downward trend in overall psychotropic prescribing during phase 1 (post-first wave stabilization, β = −25.53; 95% CI = −43.82 to −7.01; p = 0.007), followed by a structural break in phase 2 (second pandemic wave) with an abrupt level increase (β = 295.53; 95% CI = 60.48 to 530.57; p = 0.014) and slope reversal (β = 23.79; 95% CI = 3.33 to 44.26; p = 0.016), after which prescribing stabilized. A similar temporal pattern was observed for antidepressant PR. Anxiolytic PR displayed a transient upward slope change in phase 2 (β = 14.15; 95% CI = 1.80 to 26.50; p = 0.01), whereas antipsychotic and mood stabilizer prescribing remained stable across phases. Conclusions Overall psychotropic prescribing showed phase-specific disruption followed by structural rebound and subsequent stabilization across the COVID-19 and post-pandemic periods. These findings provide real-world evidence and underscore the need to monitor psychiatric prescribing and mental health service adaptation in middle-income settings.

Attention-guided hybrid transformer-ensemble framework for emotional vulnerability detection in cancer patient and caregiver narratives

Md. Siam Ansary Discover Informatics, 2026, Vol. 1, Issue 1 https://doi.org/10.1007/s44564-026-00028-9

Understanding the emotional vulnerability of cancer patients and caregivers is critical for improving psychological support and clinical intervention strategies. With the rapid growth of online health communities, large volumes of textual narratives describing treatment experiences and emotional struggles have become available for computational analysis. However, accurately capturing emotional intensity and contextual sentiment in such narratives remains challenging due to the complex linguistic patterns present in health-related discussions. This study proposes an attention-guided hybrid transformer-ensemble learning framework for emotional vulnerability detection in cancer-related social media posts. The proposed architecture integrates domain-adapted contextual representations generated by BioBERT with linguistic intensity features to capture both semantic context and emotional strength. An attention-based feature fusion mechanism dynamically combines these heterogeneous features to produce enriched representations of patient narratives. The fused representations are subsequently utilized by multiple optimized machine learning classifiers, including XGBoost, LightGBM, Random Forest, and CatBoost. Hyperparameter optimization is performed using the Optuna framework to improve predictive performance. Furthermore, ensemble learning strategies such as soft voting, hard voting, and stacking are employed to combine the predictions of the base classifiers. Experiments conducted on a held-out split of a publicly available dataset of 10,391 posts from cancer patients and caregivers demonstrate that ensemble approaches consistently outperform individual classifiers on this data. The stacking ensemble achieves an accuracy of 0.76 and an ROC-AUC of 0.91, outperforming its own individual base classifiers but not a fine-tuned BioBERT baseline (accuracy 0.77, ROC-AUC 0.92) evaluated on the same split; a TF-IDF with linear SVM baseline is also competitive (accuracy 0.763). The framework’s practical advantage over full BioBERT fine-tuning is therefore computational rather than purely predictive: it reuses frozen BioBERT features and adds a lightweight, interpretable attention-fusion and tree-ensemble stage at a fraction of the training cost, while an ablation study confirms that the attention-based fusion of contextual and lexical-intensity signals meaningfully outperforms either signal alone or their simple concatenation. The proposed framework thus provides a scalable, lower-cost computational approach that can support mental health monitoring and early intervention systems in online healthcare communities, at a modest accuracy cost relative to full transformer fine-tuning.

Neuroimaging-Guided Transcranial Magnetic Stimulation for Obsessive-Compulsive Disorder: A Systematic Review and Meta-Analysis

Rates of response to repetitive transcranial magnetic stimulation (rTMS) in obsessive-compulsive disorder (OCD) vary widely. This systematic review and meta-analysis evaluated whether neuroimaging-guided targeting personalization, from fixed-coordinate neuronavigation to individually connectivity-personalized targeting, moderates clinical response, safety, and neural mechanisms in OCD. Following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) 2020 guidelines, PubMed, the Cochrane Library, and ScienceDirect were searched through July 2026 for randomized and non-randomized studies of neuroimaging-guided transcranial magnetic stimulation (TMS) in OCD. Risk of bias (RoB) was assessed using Cochrane RoB2 for RCTs (randomized controlled trials) and ROBINS-I (Risk of Bias in Non-randomized Studies of Interventions) for non-randomized studies. Random-effects inverse-variance and risk-ratio models pooled Yale-Brown Obsessive Compulsive Scale (Y-BOCS) scores and responder rates; non-poolable data were synthesized narratively. Nine study cohorts across eight publications met inclusion criteria (five RCTs and four open-label/single-arm). Pooled analysis of four sham-controlled RCTs showed no significant benefit of neuroimaging-guided TMS on Y-BOCS total score (MD = −0.64, 95% CI = −3.60, 2.32) or responder rate (RR = 1.46, 95% CI = 0.68, 3.15), with moderate heterogeneity. However, trials using individually connectivity-personalized pre-supplementary motor area (pre-SMA) targeting consistently trended toward benefit, while fixed-coordinate neuronavigated trials trended toward null or sham-favoring results. A narrative synthesis of non-poolable and non-randomized studies showed larger, mostly positive within-group reductions in Y-BOCS scores (25%-48%), particularly among patients with moderate disease severity, alongside mechanistic evidence linking individualized connectivity-based targets to symptom improvement. TMS was well tolerated overall, with mild, transient headache being the most common adverse event and no serious safety concerns identified. Overall, neuroimaging-guided TMS showed no pooled benefit in OCD; however, individually connectivity-personalized targeting may be more beneficial than fixed-coordinate approaches, warranting adequately powered trials that directly compare targeting-personalization strategies.

Sleep Quality and Sleep Timing in Adolescent Gymnasts: A Cross-Sectional Study

Inês Alves Pereira; Beatriz Martins; Joana Branco; Rosário Ferreira Cureus, 2026, Vol. 18, Issue 9, pp. e116922 https://doi.org/10.7759/cureus.116922

Objectives: Given the limited pediatric-specific evidence in this area, we aimed to examine the association between federated-level gymnastics participation and sleep quality and sleep timing in adolescents. The global Pittsburgh Sleep Quality Index (PSQI) score was defined as the primary outcome, sleep-timing measures as secondary outcomes, and individual PSQI component analyses as exploratory. Methods: This observational, cross-sectional comparative study included adolescents aged 12 to 16 years participating in federated gymnastics and controls who did not participate in federated sports.Demographic variables included age, sex, and parental educational level, while training-related variables included weekly training duration, number of training sessions, and training schedule.Sleep quality was assessed using the PSQI, and bedtime, wake-up time, sleep duration, and sleep latency were derived from the corresponding PSQI items.Multiple linear regression models included age, sex, maternal educational level, and paternal educational level as covariates. Results: The study included 60 gymnasts and 37 controls, with baseline differences in age, sex distribution, and parental educational level between groups.Gymnasts trained a median of 12.0 hours/week (Q1-Q3: 8.25 to 20.0) over five sessions/week (Q1-Q3: 4 to 6), with a median training-session end time of 20:30 (range: 18:00 to 22:00).In adjusted analyses, gymnast status was associated with later bedtime (B=0.392hours, 95% CI 0.043 to 0.740, p=0.028), whereas no statistically significant association was observed between study group and global PSQI score (B=0.311,95% CI -0.720 to 1.343, p=0.550).Increasing age was associated with later bedtime (B=0.231hours, p=0.004) and shorter sleep duration (B=-0.398hours, 95% CI -0.560 to -0.236, p<0.001).Among gymnasts, training-session end time was not associated with global PSQI score (B=0.141,95% CI -0.344 to 0.625, p=0.563;R²=0.006).Overall, 19/97 participants (19.6%) met the conventional PSQI >5 threshold for poor sleep quality, although an optimal adolescent-specific clinical cutoff has not been definitively established. Conclusion: No statistically significant difference in adjusted global PSQI score was detected between federated adolescent gymnasts and controls.Gymnast status was associated with later bedtime.Approximately one in five participants met the conventional PSQI >5 threshold for poor sleep quality.Given the cross-sectional design, these findings should be interpreted as associations, and larger longitudinal studies incorporating objective sleep measures are warranted.

Dancing to wellbeing: a three-wave longitudinal mixed-methods study of Rock “n” Roll Dancesport and psychosocial outcomes in Chinese university students

Yuanmei He; Jialin Wu Frontiers in Public Health, 2026, Vol. 14 https://doi.org/10.3389/fpubh.2026.1928623

Background Mental health challenges among university students have intensified globally, with rising anxiety and social isolation reported across diverse educational contexts. These trends demand innovative, accessible, and non-stigmatizing approaches that complement traditional clinical services. This study examined whether participation in Rock “n” Roll Dancesport (Jive and Cha-Cha) was prospectively associated with improvements in self-efficacy, anxiety symptoms, and heart rate variability (HRV) among Chinese university students over 6 months. Methods A three-wave longitudinal mixed-methods design was employed. Quantitative assessments were conducted at baseline (T1, n = 152), 3 months (T2, n = 136), and 6 months (T3, n = 131). Measures included the General Self-Efficacy Scale (GSES), Social Self-Efficacy Scale (SSES), GAD-7 for anxiety symptoms, and resting HRV. Linear mixed-effects models examined the effects of time, baseline dance frequency group (high: ≥3 sessions/week vs. low: 1–2 sessions/week), and their interaction on each outcome, controlling for age, gender, and academic year. Semi-structured interviews ( n = 32) and 50 + hours of observation explored participants’ lived experiences. Results Mixed-effects models revealed significant main effects of frequency group across all outcomes: high-frequency participants showed higher GSES ( β = 3.60, p < 0.001), higher SSES ( β = 8.62, p < 0.001), lower GAD-7 ( β = −2.80, p < 0.001), and higher HRV ( β = 7.64 ms, p < 0.001). Significant Time × Frequency Group interactions indicated that high-frequency participants showed greater improvements over 6 months in SSES (T2: β = 2.40, p = 0.001; T3: β = 2.37, p = 0.001), GSES (T3: β = 0.99, p = 0.007), and HRV (T3: β = 3.10, p = 0.004). Qualitative analysis identified four interconnected experiential themes: rhythmic entrainment as affective catalyst, embodied mastery as a source of self-efficacy, synchronized partnership as a context for trust, and meaning-making as narrative integration. Conclusion More frequent Rock “n” Roll Dancesport participation was prospectively associated with improvements in self-efficacy, anxiety symptoms, and HRV over 6 months. The convergence of quantitative trajectories and qualitative narratives supports a directional interpretation consistent with participation temporally preceding psychological gains, while acknowledging that self-selection and reciprocal effects remain plausible. These findings suggest that structured, arts-based physical activities may serve as promising, non-stigmatizing contexts for resource-building among university students.

Background Globally, suicide is the fourth highest cause of adolescent mortality ( Suicide : https://www.who.int/news‐room/fact‐sheets/detail/suicide ). The effects of post‐primary school‐based suicide prevention (PSSP) on adolescent suicidal thoughts and behaviours (STBs) have not been comprehensively synthesised. We aim to estimate the population effect for PSSP interventions on adolescent STBs and explore how intervention effects vary based on intervention and contextual moderators. Methods Searches of PsycINFO, Medline, Education Source, ERIC, Web of Science, and the Cochrane Central Register of Controlled Trials identified cluster randomised trials examining the effectiveness of PSSP on adolescent STBs. The Cochrane Risk of Bias tool assessed bias. Crude and adjusted back‐transformed odds ratios (ORs) were calculated. Multilevel random‐effects models accounted for dependencies of effects. Univariate meta‐regression explored variability of intervention and contextual moderators on pooled effects. Results There were 19 and 12 effects for suicidal ideation (SI) and suicide attempts (SA). Compared with controls, interventions were associated with 13% (OR = 0.87, 95%CI [0.78, 0.96]) and 34% (OR = 0.66, 95%CI [0.47, 0.91]) lower crude odds reductions for SI and SA, respectively. Effects were similar for adjusted SI (OR = 0.85, 95%CI [0.75, 0.95]) and SA (OR = 0.72, 95%CI [0.59, 0.87]) models. Within‐study (0.20–9.10%) and between‐study (0–51.20%) heterogeneity ranged for crude and adjusted SA models and SI heterogeneity was 0%. Moderator analyses did not vary SA effects ( p s > .05). Conclusions This meta‐analysis contributes to the PSSP evidence‐base by demonstrating that PSSP targeting STBs as both primary intervention outcomes and with other health and well‐being outcomes reduced SI and SA among 33,155 adolescents attending 329 schools, compared to controls. The number needed to treat estimates suggests the potential of reducing the incidence of SA and SI in one adolescent by implementing PSSP in 1–2 classrooms, supporting PSSP as a clinically relevant suicide prevention strategy. Although moderator analyses were nonsignificant and contained a small number of trials, larger SA effect sizes support particular effectiveness for interventions of a duration of ≤1 week, involving multiple stakeholders and with a 12‐month follow‐up.

آثار برای مدل‌های تعدیل‌شده SI (OR = 0.85, 95%CI [0.75, 0.95]) و SA (OR = 0.72, 95%CI [0.59, 0.87]) مشابه بود.
