Background Suicidal behaviors among adolescents are a serious global public health concern and are closely linked to both mental health and everyday lifestyle behaviors. Suicidal ideation, planning, and attempts lie along a spectrum of increasing severity. However, how psychological distress and health-related behaviors interact to influence suicide risk is complex and not yet fully understood. This study aimed to identify key mental health and lifestyle factors associated with different stages of adolescent suicidal behaviors using interpretable machine learning methods. Methods Data were obtained from the 2021 Youth Risk Behavior Surveillance System (YRBS), a nationally representative survey of U.S. high school students. After data cleaning and multiple imputation, 16,946 adolescents aged 14–18 years were included in the analysis. Extreme Gradient Boosting (XGBoost) models were developed to predict suicidal ideation, suicide planning, and suicide attempts. Model performance was assessed using the area under the receiver operating characteristic curve (AUC) and related metrics. Feature importance was examined using gain-based measures, while SHapley Additive exPlanations (SHAP) and partial dependence plots were used to explore the direction, strength, and nonlinear effects of key predictors. Results All three XGBoost models showed satisfactory predictive performance. Mental health factors, particularly persistent feelings of sadness and self-rated mental health, were the strongest predictors across all stages of suicidal behaviors. Lifestyle behaviors, including sleep duration, physical activity, diet, screen time, and substance use, also contributed meaningfully to risk prediction, although their relative importance differed by outcome. SHAP analyses indicated that greater sadness and poorer mental health were strongly associated with higher suicide risk, whereas adequate sleep and higher levels of physical activity were associated with lower predicted risk. Substance use, including alcohol, cigarette, and marijuana use, played an important role in predicting suicide attempts than suicidal ideation or planning. Partial dependence analyses showed nonlinear and threshold effects, especially for sadness, mental health status, and sleep duration. Conclusion Mental health factors, especially persistent sadness and overall psychological well-being, were among the most prominent factors associated with suicidal behaviors among adolescents, while lifestyle behaviors showed important and stage-specific associations. Interpretable machine learning approaches help identify complex and nonlinear patterns associated with adolescent suicide risk and offer valuable insights to guide targeted, evidence-based prevention efforts.

Background Adolescent suicide attempts remain difficult to predict, in part because research has concentrated on a narrow set of risk factors. We examined whether self-reported plain-water consumption is associated with past-year suicide attempt among U.S. adolescents, whether that association is graded across levels of intake, and whether a zero-intake response adds information beyond established risk factors. Methods We analyzed the 2017, 2019 and 2021 cycles of the U.S. national Youth Risk Behavior Survey (n = 36,779). Missing data, including the plain-water item, were multiply imputed (40 datasets) and pooled by Rubin’s rules, accounting for the survey design. The association was estimated unadjusted, then adjusted for demographics, each of six risk domains, and all domains together. Least absolute shrinkage and selection operator (LASSO) models, tuned under two metrics and two penalty rules, provided variable selection and held-out performance; odds ratios come from unpenalized models. Results Reporting no plain water was uncommon (3.6%) and associated with past-year suicide attempt (survey-weighted odds ratio 2.14, 95% CI 1.70–2.71), in each cycle. The association was confined to the zero category: the adjusted odds ratio for none was 2.55 (1.91–3.39) against 3 times/day, whereas among those reporting any plain water there was no gradient (1.01, 0.97–1.06). It remained associated after adjustment for each domain separately (1.45–2.01) but attenuated to 1.34 (0.97–1.85) under simultaneous adjustment. LASSO retained it in every model at the discrimination optimum and, under the one-standard-error rule, in five of six domains but not the full model. Adding it changed held-out discrimination by at most 0.0011. Conclusions Reporting no plain-water consumption in the past seven days was consistently associated with higher odds of a past-year suicide attempt among U.S. adolescents. However, the association was confined to the zero-water category, attenuated after simultaneous adjustment for all risk domains, and did not improve prediction. These findings suggest that the zero-water response may function as a behavioral marker of broader risk rather than as an independent risk factor.

Background Adolescent suicide attempt is a major public health concern linked to psychosocial distress, school environment, health-risk behaviors, and physical activity patterns. This study aimed to develop an explainable XGBoost model for adolescent suicide-attempt status and to determine whether physical inactivity and low physical education (PE) participation could serve as informative school-based indicators. Methods Data were obtained from the Global School-based Student Health Survey conducted in Argentina in 2018, Anguilla in 2016, and Jamaica in 2017. The Argentina dataset was used for model training, while Anguilla and Jamaica were combined as an independent validation cohort. The outcome was self-reported suicide attempt during the past 12 months. Candidate predictors included demographic, anthropometric, dietary, psychosocial, violence- and injury-related, tobacco-related, sexual behavior, physical activity, and PE-related variables. An XGBoost model was developed using nested cross-validation, staged hyperparameter tuning, and Bayesian fine-tuning. Model performance was evaluated by the area under the receiver operating characteristic curve (AUC). SHAP and partial dependence plots were used for interpretation. Results A total of 27,710 adolescents were included; 5,165 (18.6%) reported at least one suicide attempt. The final model achieved an independent validation AUC of 0.7928 (95% CI: 0.7790–0.8066). The top 18 SHAP-ranked predictors retained a cumulative 10-fold cross-validated AUC of 0.7865. Loneliness was the most influential predictor, followed by sex, insomnia, early cigarette use, PE days, physical attack, school peer kindness, and physical activity. Physical inactivity and low PE participation showed nonlinear associations with predicted suicide-attempt risk. Conclusions Loneliness and insomnia represented the psychosocial vulnerability profile, while physical inactivity and limited PE participation emerged as potentially informative school-based indicators requiring further longitudinal investigation. These findings support risk-stratified prevention strategies integrating early psychological identification with inclusive, socially supportive physical activity and PE programs.

Global Perspectives on Adolescent Suicide and Self-Harm: Findings and Insights From the ASCAPAP Grand Forum

Miae Oh; Ji Sun Hong; Hee Jeong Yoo Journal of korean Academy of Child and Adolescent Psychiatry, 2026, Vol. 37, Issue 4, pp. 167-176 https://doi.org/10.5765/jkacap.260019

Adolescent suicide and self-harm have become pressing global public health crises. At the 2026 Asian Society of Child and Adolescent Psychiatry and Allied Professions Grand Forum on Suicide and Self-Harming Behavior in Adolescents Worldwide, researchers, clinicians, and policymakers from multiple countries presented current evidence and field experiences. This study synthesizes the forum's major presentations and discussions in the thematic areas. The forum emphasizes that South Korea continues to report one of the highest adolescent suicide rates among Organisation for Economic Co-operation and Development (OECD) countries and that suicide-related behaviors and non-suicidal self-injury have reached concerning levels across Asia. The cross-national evidence presented at the forum indicated a marked post-pandemic increase in adolescent suicide attempts and self-harm, with a disproportionate burden on adolescents and young women. The speakers further emphasized that aggregate national statistics could obscure the heightened risk faced by the most vulnerable subgroups.Major risk factors included psychiatric comorbidities, diminished sense of belonging, social media use, algorithm-driven content exposure, and socioeconomic disparities in healthcare access. The forum emphasized that for many adolescents, self-harm reflected not a wish to die but an attempt to escape overwhelming psychological pain.Proposed solutions centered on a paradigm shift toward primary prevention, integrated school-government-professional systems incorporating AI-based early warning, the "Safety by Design" principle for social media, community-based safe spaces, and strengthening families and communities.Adolescent suicide prevention is not solely a medical issue but a collective societal responsibility that requires international collaboration.
