Abstract Background Borderline personality disorder (BPD) is a severe mental illness associated with emotional and functional impairment, elevated suicide risk, and high individual and societal costs. BPD is under-researched in the general population, especially among males, and from a developmental perspective. We examined the prevalence and development of BPD symptoms in young men and explored the longitudinal associations with risk-taking behaviors. Methods Data came from the prospective-longitudinal Cohort Study on Substance Use Risk Factors (C-SURF), a population sample of young men. Participants were screened for BPD risk using the McLean Screening Instrument (MSI-BPD) at mean age 25 (2016–2018; n = 5,516) and again at mean age 28 (2019–2020; n = 5,367). Risk-taking behaviors (e.g., interpersonal violence, gambling, substance use) were self-reported at both time points. Multilevel regression models examined within-person changes in BPD symptoms, including self-harm, and risk-taking behaviors over time. Results 7.4% of participants had a high risk of BPD, defined by endorsing 7 + MSI-BPD items at least once. Among those with 7 + items endorsed at the first assessment, 29.5% still endorsed 7 + items three years later. Despite a general within-person decline in BPD symptoms with age, participants at initially high risk for BPD continued to exhibit elevated levels of risk-taking behaviors after three years. Conclusion BPD symptomatology affects a substantial proportion of young men. Despite its modest stability over time, it is associated with persistent engagement in various risk-taking behaviors. Interventions targeting BPD symptoms in male populations may offer a promising avenue towards mitigating substantial psychosocial burden associated with these behaviors.
Abstract Purpose Forced migration increases vulnerability to exploitation. Globally, around 50 million people are affected by human trafficking, which often involves trauma with negative mental health effects. Forced migrants already face elevated risks of PTSD, depression, and anxiety, yet the specific association of trafficking on their mental health remains unclear. Methods Symptoms of posttraumatic stress disorder (PTSD), depression, anxiety, and overall stress were assessed using standardized screening instruments (PC-PTSD-5, PHQ-4, and the Distress Thermometer). Experiences of human trafficking were measured with a modified version of the Adult Human Trafficking Screening Tool (AHTST) among forced migrants in Germany. Missing data were handled using multiple imputation. Logistic regression analyses were conducted to examine the association between human trafficking and positive screenings for the respective mental health conditions. Results A total of N = 1108 forced migrants participated in the study. Depending on the imputation model, between 49% and 56% of all participants screened positive for PTSD, 60% to 71% for depression, and 41% to 52% for an anxiety disorder. Among participants who screened positive for these mental health conditions, a positive screen for human trafficking was associated with a 30–40% higher probability of screening positive for PTSD and a 15% to 20% higher probability of screening positive for depression or anxiety disorder. Conclusions Forced migrants with a history of human trafficking are at elevated risk for PTSD, depression, and anxiety than those without, underscoring the need for early identification and tailored psychosocial support.
Few events in recent history have forced clinicians and researchers to rethink mental health quite like COVID-19. In this commentary, we reflect on something that struck us while reading the work of Cantone et al. : the pandemic, for all its devastation, created a kind of unplanned observational window, a chance to study, at population scale, what happens when stress and rhythm disruption hit at the same time. Healthcare Workers (HCWs) were at the epicentre of this. They didn't just witness the crisis; they absorbed it, shift after shift, for months. What the data show is that their depressive symptoms and rhythm instability were not random; they appear systematically linked, and at levels one would more typically associate with mood disorder research. We discuss these findings through the lens of DYMERS (Dysregulation of Mood, Energy, and Social Rhythms Syndrome), a heuristic transdiagnostic construct that, in our view, captures something that standard categorical diagnoses tend to miss: the subthreshold, fluctuating psychopathology that sits just below the threshold of formal diagnosis but causes real impairment. The pandemic may not have created these vulnerabilities, but it may well have sharpened our ability to see them.
Introduction/Objective Depression is a disabling mental health disorder affecting over 300 million people worldwide. In Tunisia, population-level data on its prevalence and determinants remain limited. Our study aimed to estimate the prevalence of depression in the Tunisian population and identify associated socio-demographic and clinical factors. Methods Data were drawn from the mental health component of the Tunisian Health Examination Survey (2016), a nationally representative study conducted among 9,209 individuals aged ≥15 years. Self-reported, physician-diagnosed lifetime depression was the main outcome. Associations with socio-demographic characteristics and clinical conditions, including rheumatoid arthritis, diabetes, and menopause, were examined. Results The study population comprised 4360 men (49.2%) and 4849 women (51.8%), with a mean age of 41.4±17.4 years. The prevalence of self-reported lifetime depression was 4.7%. Multivariate analysis identified female sex (OR=1.53, 95% CI [1.11–2.10]) and age groups 30–64 years (OR=1.81, 95% CI [1.20–2.73]) and ≥65 years (OR=1.82, 95% CI [1.06–3.14]) as factors significantly associated with higher prevalence. Conversely, marriage (OR=0.59, 95% CI [0.42–0.84]) and the absence of rheumatoid arthritis (OR=0.48, 95% CI [0.38–0.60]) were associated with a lower prevalence of the disorder. Discussion The findings indicate that self-reported depression in Tunisia is significantly influenced by gender and aging, consistent with global epidemiological trends. The strong association with physical comorbidities, particularly rheumatoid arthritis, underscores the necessity of integrated mental health screening for patients with chronic illnesses. Furthermore, the protective association of marriage suggests that social and family cohesion remain vital factors in the Tunisian sociocultural context. Conclusion Depression in Tunisia is shaped by gender, aging, and physical comorbidities. Targeted prevention and integrated screening strategies are needed, especially for vulnerable demographic and clinical groups.
The underlying mechanisms linking childhood maltreatment (CM) to adverse health outcomes are unclear. Here, we estimated CM pleiotropy across human phenome, also assessing the role of socioeconomic factors. Specifically, we performed a phenome-wide association study (PheWAS) of CM-associated loci using electronic health records and genetic data from the All of Us Research Program (N = 98,551), FinnGen Project (N = 377,277), and Million Veteran Program (N = 449,042). To characterize CM pleiotropy, we also conducted genetic correlation (rg), latent causal variable analysis (LCV), and bidirectional Mendelian randomization (MR) analyses. Additionally, multi-trait conditional and joint analysis and multivariate MR (MVMR) were performed to estimate whether genetically inferred CM associations are partially explained by household income (HI, a proxy for socioeconomic factors). PheWAS revealed CM loci associations with 66 medical outcomes (p < 2.76 × 10 −6 ), including psychiatric, musculoskeletal, circulatory, neurologic, and respiratory conditions. Similarly, we observed 285 CM genetic correlations (p < 3.24 × 10 −5 ) with a range of medical domains. Among them, emotional disorders starting during childhood or adolescence showed the highest CM genetic overlap (rg=0.75). LCV analysis revealed CM genetic relationships with musculoskeletal, metabolic, respiratory, and eye-related outcomes. For these, MR analyses highlighted bidirectional and heterogeneous effects possibly due to dynastic confounders, and MVMR showed a reduced effect of CM genetic liability after accounting for HI. In conclusion, different pleiotropic mechanisms likely contribute to CM relationships with several health domains, which appear to be at least partially explained by socioeconomic factors.
به طور مشخص، مطالعه انجمنی فنومگسترده (PheWAS) بر روی جایگاههای مرتبط با CM با استفاده از پروندههای الکترونیک سلامت و دادههای ژنتیکی از برنامه پژوهشی All of Us (N = 98,551)، پروژه FinnGen (N = 377,277) و برنامه Million Veteran (N = 449,042) انجام شد.
Franco Basaglia's centenary, celebrated on March 11, 2024, invites reflection on the historical significance of his legacy. His work in defense of people with mental disorders represents his fight for the protection of human dignity and the value of life. Pejoratively called “the philosopher Basaglia,” he transformed psychiatric practices by pushing the boundaries of medicine to address profound human rights issues. Basaglia adopted an existential-analytical stance that underpinned his commitment to psychiatric reform and the management of mental health practice, inspiring changes in public policies around the world. This commemorative editorial presents a historical overview of Franco Basaglia's contributions to psychiatric reform and their implications for mental health care.

People with restricted mobility in humanitarian settings face extreme barriers to healthcare access, leading to functional decline, social isolation, and significant mental health distress [44]. In dense urban Lebanese refugee camps, hazardous infrastructure and the rising burden of non-communicable diseases (NCDs) further exacerbate the vulnerability of homebound and bed-ridden individuals and their informal caregivers [30]. This study evaluates the operational feasibility and outcomes of a multidisciplinary nurse-led home-based care (HBC) model designed to address these physical and psychosocial needs. This convergent mixed-methods descriptive study investigated a nurse-led HBC program in the Shatila and Bourj el Barajneh refugee camps in Beirut, Lebanon, between June 2021 and October 2023. We analysed routinely collected operational data for 105 patient-caregiver dyads (verbal retrospective consent) and conducted semi-structured in-depth interviews with a purposive subsample of 16 dyads as 32 participants 16 patients and 16 informal caregivers (written prospective consent) to explore participant perceptions. Quantitative outcomes were analysed for the n=79 dyads who completed the full intervention. The program achieved a high retention rate of 82%. Quantitative findings demonstrated significant improvements in mental health, with a reduction in depression and anxiety scores for both patients and caregivers (both p < 0.001). Enrolment was accompanied by statistically significant reductions in depression (PHQ-9) and anxiety (GAD-7) scores among both patients and caregivers (both \(p<0.001\) ). Statistically significant gains were observed in physical dependency (Basic Activities of Daily Living, p= 0.003), alongside documented wound healing and maintenance of high medication adherence. Qualitative data revealed that the program’s primary value lay in alleviating profound social isolation, restoring dignity, and providing emotional and practical support, which participants often prioritized alongside clinical nursing interventions. Multidisciplinary nurse-led HBC is a feasible and acceptable model for delivering care to highly vulnerable populations in complex humanitarian settings. While the single-arm design precludes definitive causal attribution, observed trajectories indicate that in contexts of chronic distress, the psychosocial and relational components of care are as vital as clinical nursing intervention for fostering resilience in both patients and caregivers.
BACKGROUND: Although the Danish health registers provide rich data for epidemiological mental health research, the comparability of findings is hindered by different definitions of mental disorders across studies. We aimed to describe different approaches taken by researchers to define mental disorders using Danish register data and quantify the impact of selecting different definitions on estimates of prevalence and incidence. METHOD: We undertook a scoping review to identify the approaches to define mental disorders using Danish register data and produced a narrative synthesis. We compiled tables of potential definitions for any mental disorder and specific types of mental disorders using nationwide Danish registers. Using these definitions, we quantified (i) the number of individuals fulfilling each definition during 2018 and 2018-2022 and (ii) the estimated cumulative incidence. RESULTS: Our search identified 2185 papers, 1155 of which used Danish register data to define mental disorders to describe the study population, an exposure, or an outcome. Our estimates of the number of individuals fulfilling each definition and the estimated cumulative incidences indicated considerable variation with the data sources and variables used to define mental disorders within the Danish national registers. For example, the cumulative incidence of any mental disorder at 80 years among females varied between 29.2% when only psychiatric unit patient register data were used and 62.1% when somatic patient, prescription, cause of death, and health services register data were added. CONCLUSION: The different data sources and variables used to define mental disorders can result in substantial variation in the numbers of identified cases. Studies should clearly document all variables and values used to define their disorders of interest, as well as describe the disorder they aim to capture, discussing the strengths and limitations of their approach and the likely impacts on the sensitivity and specificity of including or omitting data from other available registers.
Abstract The increasing use of visual social media has intensified societal pressure to achieve flawless skin and may contribute to excessive preoccupation with appearance and skincare. The emerging terms dermorexia and cosmeticorexia have been used to describe potentially compulsive skincare behaviors extending beyond conventional self-care. This narrative review examines the proposed concept of dermorexia, its conceptual overlap with recognized psychiatric conditions, potential risk factors and clinical manifestations, dermatological and psychological consequences, and implications for aesthetic practice. The available literature was conducted, focusing on compulsive skincare behaviors, social media–related body image concerns, body dysmorphic disorder (BDD), and obsessive-compulsive spectrum conditions. Relevant evidence was critically synthesized to assess the current conceptual and clinical basis of dermorexia. Dermorexia has conceptual similarities to orthorexia nervosa and may overlap with obsessive-compulsive and body dysmorphic psychopathology. Adolescents and young adults may represent a particularly vulnerable population. Proposed manifestations include excessive and prolonged skincare routines, compulsive product use, repetitive mirror checking, skin picking, and recurrent pursuit of cosmetic procedures. Potential dermatological consequences include epidermal barrier disruption, irritant or allergic contact dermatitis, and excoriation-related scarring. Psychological consequences may include anxiety, depressive symptoms, appearance-related distress, and functional impairment. However, empirical validation, standardized diagnostic criteria, and prevalence estimates are currently lacking Dermorexia/cosmeticorexia remains an unvalidated clinical construct rather than a formally recognized psychiatric diagnosis. Nevertheless, increasing awareness may help aesthetic clinicians identify maladaptive skincare and appearance-related behaviors and facilitate appropriate psychological referral. Further research is required to establish its epidemiology, diagnostic boundaries, and clinical significance.
OBJECTIVES: Depression, which is the most common comorbidity in breast cancer (BC) patients, has adverse effects on patients’ quality of life, disease progress, and survival.METHODS: The protocol of this study was registered in PROSPERO (registration No. CRD42019121494). We electronically searched published studies through January 2019 with the aim of finding articles that investigated the prevalence of depression among BC survivors. Web of Science, Scopus, PubMed/MEDLINE, Science Direct, and Google Scholar were searched to obtain relevant published studies. This review included 14 cross-sectional and 4 cohort studies published from 2000 to 2018. We used a random-effects model to conduct the meta-analysis and generated a summary estimate for the pooled prevalence with 95% confidence intervals (CIs). A subgroup analysis was also conducted based on the depression assessment tool used and the study design.RESULTS: The total sample size of the studies contained 2,799 women with BC, including 1,228 women who were diagnosed with depression. The pooled prevalence of depression among Iranian women with BC was 46.83% (95% CI, 33.77 to 59.88) with significant heterogeneity (I2 =98.5%; p<0.001). The prevalence of depression ranged from 14.00% (95% CI, 4.91 to 23.09) to 95.90% (95% CI, 91.97 to 99.83). The results of the subgroup analyses suggested that the depression assessment tool, year of publication, and study design were sources of heterogeneity.CONCLUSIONS: Our findings indicate a high prevalence of depression among BC patients, underscoring the urgent need for clinicians and health authorities to provide well-defined social and psychological supportive care programs for these patients.
