Beyond the Common Versus Specific Binary: A Culturally Situated Responsive Calibration Model for Counselling

Jonathon Massey Psychotherapy and Counselling Journal of Australia, 2026 https://doi.org/10.59158/001c.173000

Counselling researchers have spent decades arguing about why counselling works. One philosophical camp locates the active ingredient in specific techniques tied to specific disorders. The other locates it in factors common to all bona fide therapies, chiefly the relationship between counsellor and client. This article develops a process-based perspective on the debate, examining how relational and technical elements are used within an unfolding therapeutic interaction. Drawing on the equivalence findings, the contemporary alliance literature, the unresolved question of causal mediation, the routine outcome monitoring evidence, and Australian decolonising scholarship, I propose a culturally situated responsive calibration model. The model reframes the therapist’s continuous, feedback-informed adjustment of relational and technical elements, made within the client’s cultural frame of wellbeing, as a proposed proximal mechanism of change. Calibration is not a new observation about good practice. Building on established responsiveness theory, the proposed contribution is to organise testable episodes of clinical judgement around feedback, action, and the client’s definition of wellbeing. Four testable propositions are advanced, including tests of whether calibration adds explanatory value beyond established process and treatment measures. Implications for practice, training, and measurement are outlined. The model is offered as a conceptual framework awaiting empirical testing, not as an established finding.

Background: Post-traumatic stress disorder (PTSD) is prevalent among children and adolescents, with cognitive behavioral therapy (CBT) and eye movement desensitization and reprocessing (EMDR) widely used treatment options. This study compared the effects of CBT and EMDR in children and adolescents using a meta-analysis approach. Methods: PubMed, EMBASE, CENTRAL, CINAHL, ProQuest Dissertations & Theses, and PsycInfo were searched in March 2025. Randomized controlled trials that compared the effectiveness of CBT and EMDR on PTSD symptoms in children and adolescents aged 6–18 years who had been exposed to traumatic events were included. The Cochrane Risk of Bias version 2 was used to assess the methodological quality of the included studies. A random-effects model was applied to calculate the standardized mean difference (SMD) along with 95% confidence interval (CI), and the I2 statistic was used to quantify statistical heterogeneity. Results: Five trials (248 participants) were included. There was no statistically significant difference in PTSD symptoms between the CBT and EMDR groups, whether reported by children (SMD: 0.18; 95% CI: –0.07–0.43; I2 = 0%) or by parents (SMD: 0.23; 95% CI: –0.02–0.48; I2 = 0%). For anxiety, the pooled estimate did not reach statistical significance (SMD: 0.44; 95% CI: –0.19–1.07; I2 = 69%). Depression symptoms were higher in the CBT group than in the EMDR group (SMD: 0.44; 95% CI: 0.13–0.76; I2 = 0%).Conclusion: The comparative effects of CBT and EMDR in reducing PTSD and anxiety symptoms remain inconclusive. Although CBT was associated with higher depression symptoms compared with EMDR, the current evidence is based on a small number of trials, and the pooled estimates should be interpreted with caution.

Family caregivers often experience anxiety and depression. Technology-based Acceptance and Commitment Therapy (technology-based ACT) provides new treatment options, but its effectiveness for family caregivers needs further research. To explore the effectiveness of technology-based ACT in relieving family caregivers’ anxiety and depression symptoms. Search databases of PubMed, Web of Science, Cochrane Library, CINAHL, and Embase from inception to January 6, 2026. Then, use the Cochrane Risk of Bias Assessment Tool, Version 2 to evaluate study quality. Finally, conduct statistical analysis with Stata 17.0 and Review Manager 5.4. A total of 1067 papers were initially retrieved, among which 7 met the inclusion criteria. The meta-analysis showed a statistically significant improvement in anxiety symptoms (SMD = -0.42; 95% CI -0.64 to -0.19; p < 0.001). For depressive symptoms, the fixed-effect analysis indicated a small statistically significant improvement (SMD = -0.28; 95% CI -0.51 to -0.06; p = 0.014); however, this effect was not statistically significant under the random-effects sensitivity analysis, indicating limited robustness to model choice. Exploratory subgroup analyses suggested greater improvements in anxiety among caregivers of care recipients with relatively mild conditions, while studies”’ using the GAD-7 showed a larger estimated effect than those using other anxiety measures. Greater improvements in depressive symptoms were observed among caregivers of care recipients with relatively mild conditions and in interventions lasting ≥ 8 weeks. Given the small number of studies, these subgroup findings should be interpreted cautiously. Technology-based ACT may improve anxiety symptoms among family caregivers, while the evidence for depressive symptoms is less robust and should be interpreted cautiously. Given the limited number of included RCTs and participants, these findings should be considered preliminary and require confirmation in larger, adequately powered trials.
