Development and Validation of Caring Based HIV Counseling Training Program for Nurses: A Delphi Methodology

Christine Buela International Journal of Education, Research, and Innovation Perspectives, 2026, Vol. 2, Issue 10 https://doi.org/10.5281/zenodo.23840800

Human immunodeficiency virus (HIV) infection remains a growing public health concern in the Philippines, where new infections rose by about 550% between 2010 and 2024 and diagnosis gaps persist in CALABARZON. Nurses are often the first and most sustained point of contact in HIV testing and care, yet knowledge gaps, stigmatizing attitudes, and procedure-focused training may limit their ability to provide compassionate, person-centered counseling. This study aims to develop and validate a Caring-Based HIV Counseling Training Program for Nurses in Laguna, Philippines, using a methodological research design and a modified Delphi technique. Registered nurses from selected government and private healthcare facilities in Laguna will complete a researcher-developed training-needs assessment questionnaire covering six competency domains: HIV-related knowledge; counseling and therapeutic communication; caring behaviors and person-centered practice; ethical and legal responsibilities; cultural sensitivity and stigma reduction; and linkage to prevention, treatment, care, and support. The questionnaire will undergo expert review, pilot testing, and reliability testing using Cronbach’s alpha. The needs-assessment findings, together with a review of current literature and guidelines, will inform a preliminary training program, which a multidisciplinary panel of approximately 15 to 20 experts will evaluate over successive anonymous Delphi rounds for relevance, clarity, comprehensiveness, cultural appropriateness, feasibility, and applicability. Consensus is defined in advance as at least 80% expert agreement, a median rating of at least 3.00, and an interquartile range of no more than 1.00. Content validity will be established through an Item-Level Content Validity Index of at least .78 and a Scale-Level Content Validity Index (average) of at least .90, overall agreement will be measured with Kendall’s coefficient of concordance, and the experts’ qualitative comments will undergo directed content analysis. The study is expected to produce an expert-validated, contextually relevant training program that strengthens nurses’ capacity to deliver compassionate, stigma-free HIV counseling, with its effectiveness to be tested in a future pilot or quasi-experimental study.

Posttraumatic Stress Symptoms, Experiences of Trauma, and Coping among Pakistani Frontline Nurses Caring for COVID-19 Patients: A Qualitative Study

Omama Taneem; Zunaira Arshad; Areeba Bano Pakistan Journal of Positive Psychology, 2026, Vol. 3, Issue 9, pp. 101-110 https://doi.org/10.67785/pjpp.9.407

Nurses delivered much of the direct care to COVID-19 patients and were repeatedly exposed to death, infection risk, and shortages of equipment, yet little is known about how Pakistani nurses experienced and managed posttraumatic stress symptoms (PTSS). This qualitative study explored PTSS, experiences of trauma, perceived psychological impact, and coping strategies among nurses who cared for COVID-19 patients during peak pandemic waves. Eight registered nurses working in COVID wards and intensive care units of public hospitals in Islamabad and Rawalpindi were recruited through purposive sampling and took part in individual semi-structured interviews conducted in Urdu and basic English. Data were analysed using Braun and Clarke’s thematic analysis. Four themes were generated: (a) symptoms related to trauma (altered sleep, changes in temperament, emotional reactivity, diminished interest in social life, flashbacks); (b) threats and stress due to COVID-19 (increased workload, direct threat to self and others, sense of isolation, lack of knowledge and equipment); (c) psychological impact of trauma (increased mental health concerns, burnout and compassion fatigue, elevated death anxiety, lack of optimism about the future); and (d) strategies to cope (sharing workload with the team, emotional support from colleagues). Seven of eight nurses described flashbacks, and accounts spanned several DSM-5 symptom clusters, although no diagnostic assessment was used. Peer teamwork and emotional support were the only coping resources reported, and no participant had received formal counselling. Peer solidarity seems to have been an informal resource of resilience where organizational support was absent and institutional psychological services should be built around it.

Emotional intelligence (EI) has emerged as a relevant professional competency in nursing, yet its relationship with career growth remains underexplored in Saudi Arabia. This study examined the association between total emotional intelligence and perceived career growth among nurses in the Jazan Health Cluster. A cross-sectional correlational design was employed with 295 registered nurses in the Jazan Health Cluster recruited via convenience sampling. EI was measured using the Schutte Self-Report Emotional Intelligence Test, and career growth was measured using the Career Growth Scale. Pearson correlation and multiple linear regression served as the primary analytical strategies. The sample was predominantly female (78.0%), Saudi (67.1%), and BSN-qualified (74.9%), with a mean age of 36.28 years (SD = 7.94). Mean total EI was 127.98 (SD = 9.96), and mean total career growth was 53.73 (SD = 6.66). EI was positively correlated with career growth ( r = .351, 95% CI [0.246, 0.447], p < .001). Exploratory analysis showed that total EI was most strongly correlated with Career Capacity ( r = .438). Social Skills and Emotion Utilization showed broad positive patterns across the non-salary career-growth domains, whereas total EI was not associated with Salary Increase ( r = − .008, 95% CI [-0.122, 0.106]). The association between the total scores remained after covariate adjustment (B = 0.213, standardized β = 0.319, HC3 p < .001; adjusted R² = 0.220). Total emotional intelligence was positively associated with perceived career growth among nurses in Saudi Arabia after covariate adjustment but was not associated with perceived salary growth. Exploratory findings suggest that emotional competencies may be more relevant to developmental and relational aspects of career growth than to financial advancement. The findings may inform broader professional-development strategies aligned with Saudi Arabia’s Vision 2030 healthcare workforce priorities.

Burnout among nurses threatens healthcare quality and workforce sustainability. This study examined associations between perceived skill development, institutional support, resilience, job satisfaction, and burnout among nurses in Saudi Arabia, testing resilience as a mediator linking organizational resources to outcomes. In this cross-sectional study, 190 registered nurses from three hospitals in Riyadh completed validated instruments: Professional Development Scale in Nursing, Survey of Perceived Organizational Support, Connor–Davidson Resilience Scale-25, Index of Work Satisfaction, and Copenhagen Burnout Inventory. Hierarchical regression and regression-based mediation (PROCESS Model 4; 5,000 bootstrap samples) were conducted, controlling for age, gender, experience, hospital type, and nationality. Reporting follows STROBE guidelines. Skill development (β = 0.38, p < 0.001) and institutional support (β = 0.34, p < 0.001) were independently associated with resilience (R 2 = 0.412). Resilience was positively associated with job satisfaction (r = 0.48) and negatively with burnout (r = −0.54). Indirect effects via resilience were statistically significant for the paths from skill development to job satisfaction (0.15,95% CI [0.09,0.22]), skill development to burnout (−0.23,95% CI [−0.32, −0.15]), institutional support to job satisfaction (0.14,95% CI [0.09,0.21]), and institutional support to burnout (−0.19,95% CI [−0.28, −0.12]). Nationality was not significantly associated with resilience (β = 0.03, p = 0.660). Higher perceived skill development and institutional support were associated with higher resilience, which in turn correlated with improved job satisfaction and reduced burnout. These findings suggest that strengthening organizational resources is associated with higher nurse resilience and, in turn, with more favorable job satisfaction and burnout profiles; given the cross-sectional design, causal or directional claims cannot be made.

Sexual prejudice and associated factors among clinical nurses in Türkiye: A cross-sectional study

Ezgi Şişman; Mehtap Güngör; Umut Gunay; Aila Gareayaghi; Hanife Yilmaz; İlay Dalkıran; … PLoS ONE, 2026, Vol. 21, Issue 10, pp. e0360301 https://doi.org/10.1371/journal.pone.0360301

Aim/objective This study aimed to assess the level of homophobia and to identify its independent predictors among practicing clinical nurses working in a tertiary university hospital in Türkiye. Background Homophobia among healthcare professionals undermines equitable care for sexual minorities and remains underexplored in Türkiye, particularly among practicing nurses. In settings with limited institutional support for sexual minority health, identifying determinants of sexual prejudice among healthcare professionals can inform training needs. Design A cross-sectional survey design was used. Methods One hundred registered nurses providing direct patient care completed the Turkish version of the Hudson and Ricketts Homophobia Scale and a structured questionnaire assessing sociodemographic characteristics, knowledge, etiological beliefs, interpersonal contact, and comfort working with LGB patients and colleagues. Homophobia scores were dichotomized using a median split, and binary logistic regression was performed to identify independent predictors Results The mean homophobia score was 102.14 (SD = 24.73). In multivariate analysis, endorsing biological explanations for homosexuality (OR = 0.22), having a homosexual acquaintance (OR = 0.19) and greater comfort working with LGB colleagues (OR = 0.41) were independently associated with lower odds of homophobia, whereas holding a bachelor’s degree was associated with higher odds (OR = 4.24). Conclusions Homophobia scores among nurses were shaped by educational, interpersonal, cognitive, and affective factors. These findings underscore the need for structured, evidence-based, and inclusive nursing education and institutional reform to promote equitable and ethical care for sexual minority patients.
