Background Helping professionals face elevated risks of compassion fatigue and burnout, yet the daily processes through which emotional transformation occurs during therapeutic interventions remain poorly understood. This study examined how participants’ daily reflections shifted across the Emotion-Focused Training for Helping Professions (EFT-HP), analyzed through three complementary theoretical frameworks: Emotion-Focused Therapy's inner dialogue model (self-criticism, self-compassion, self-protection), Compassion-Focused Therapy's three-systems framework (threat, drive, soothing), and Ekman's basic emotion theory (primary emotions). Method Two hundred twenty-five helping professionals (88.0% female; M age = 38.7 years) from healthcare, social work, psychology, and education began the 14-day online EFT-HP intervention. Primary analyses focused on the 146 participants who completed at least 9 of the 14 days (completers), whose daily written reflections yielded 7,307 coded reflection units. Reflections were analyzed with directed content analysis and frequency quantification, applying non-exclusive, AI-assisted coding within three theoretical frameworks—EFT inner-dialogue categories, CFT emotion-regulation systems, and Ekman's discrete emotions—with a verbatim textual evidence excerpt retained for every assigned code. Results Across the intervention, participants’ reflections shifted from threat- and self-criticism-related content toward soothing and self-compassion. Within the EFT inner-dialogue framework, Self-Compassion was the most frequent category (17.9% of units) and was sustained throughout (Day-13 peak 30.8%), while Self-Criticism fell from 14.3% on Day 1 to 3.9% on Day 14 (73% relative decrease) and Self-Protection peaked on Day 6 (11.3%). Within the CFT emotion-regulation framework, the soothing system was the most frequently coded system overall (31.8% of units), followed by drive (20.4%); over the 14 days the threat system declined from 35.4% on Day 1 to 7.0% on Day 14 (80% relative decrease). Within Ekman's discrete-emotion framework, specific emotions were named less frequently: Fear declined from 2.7% to 0.8%, Anger peaked on Day 6 (15.0%), Joy rose to a Day-13 peak (14.8%), and Love was comparatively prominent (11.9% overall, Day-5 peak). Conclusions Across the 14-day intervention, participants’ daily reflections showed substantial shifts in emotional content, characterized by marked self-criticism and threat reduction alongside sustained self-compassion and soothing activation. Triangulated findings across three analytical frameworks: EFT inner dialogue, CFT emotion systems, and Ekman's primary emotions, are consistent with the emotional-transformation processes proposed by EFT-HP and with findings from previous controlled trials. Because the three frameworks were applied to the same reflection data, their convergence reflects analytic (theoretical) triangulation rather than independent confirmation.

CDigital therapeutics (DTx) are software-based medical devices that deliver evidence-based therapeutic interventions directly to patients. While the clinical use of DTx is expanding rapidly, regulatory fragmentation and the absence of harmonized legal definitions hinder their assessment and reimbursement policies, as well as their integration into healthcare systems. There is currently a lack of comprehensive overviews of DTx clinical development, which are essential for informing policy, research and regulatory priorities. This paper provides an updated overview of randomised clinical trials involving DTx. 254 eligible trials on DTx, (defined according to ISO/TR 11147:2023) registered through January 2025 were identified through a search on C linicalTrials.gov . Mobile applications represent the most prevalent technology (65%), followed by web-based tools and virtual reality (20%). Psychiatric disorders are the primary therapeutic target (52% of the studies) with insomnia, depression, and anxiety being the most addressed conditions. Trials were primarily conducted in North America (41%) and Europe (32%), where Germany, Spain, and Italy represent the most active countries. The analysis of clinical trials registrations on ClinicalTrials.gov reveals a heterogenous geographical distribution of DTx development but also a clear direction in the type of conditions for which they are mostly indicated, namely psychiatric and nervous system disorders. The fragmented policies on DTx reimbursement, even among EU countries, and the different capacity of governments to fund DTx research and development emerge as key factors, along with cultural and ethical issues, in determining the willingness of the industry to invest in these devices.

Healing our intergenerational wounds: a praxis of community healing envisioned by refugee young adults engaging in participatory avatar-based community circles

Vivetha Thambinathan; Lloy Wylie; Deluxna Ganesh; Luxana Ramesh; Elizabeth Anne Kinsella International Journal for Equity in Health, 2026, Vol. 25, Issue 1 https://doi.org/10.1186/s12939-026-02993-x

Refugee communities across the world have long gathered to connect and support one another during times of crisis, and this has been well-documented. Despite the vast amount of literature pertaining to coping and resiliency among refugee communities, there is limited knowledge about refugee community healing. Approximately one in three refugees experiences depression, anxiety, or post-traumatic stress that can persist long after displacement. Dominant approaches within Western, Educated, Industrialized, Rich, Democratic (WEIRD) psychologies tend to conceptualize trauma through an individualistic lens, often overlooking social, historical, and collective dimensions of suffering. This can result in siloed findings with limited relevance to refugee communities. Addressing this gap, this paper reports on the third phase of a Decolonizing Participatory Action Research (PAR) study examining historical trauma and intergenerational healing among second-generation Tamil refugee young adults in Toronto. This community-driven study foregrounds a praxis of healing envisioned with, by, and for refugee communities. Two research questions guided the inquiry: (1) What elements are important for community healing among Tamil young adults? and (2) What do community healing practices and therapeutic interventions look like in this context? Using storytelling and participatory avatar-based community circles, alongside reflexive thematic analysis, seven key elements of community healing were identified: (1) agency and self-determination; (2) historical trauma-informed approaches; (3) cultural safety; (4) intergenerational connectedness and belonging; (5) destigmatized access and delivery; (6) holistic and authentic healing practices; and (7) truth-telling as justice. Findings highlight community healing as a dynamic, dialectical process, requiring both resistance to intersecting systems of oppression and the cultivation of radical hope for justice and liberation. Findings also emphasize the importance of culturally grounded, community-led interventions in promoting mental well-being. This study contributes innovative, participatory methodology and actionable insights for practitioners and policymakers, advancing a shift toward liberatory approaches to refugee health.

Holding multiplicity: narrative and trauma-informed couples therapy for dissociative identity disorder

Annemarie Sohn; Eman Tadros; Josh Bolle Sexual & Relationship Therapy, 2026, pp. 1-24 https://doi.org/10.1080/14681994.2026.2741916

Couples therapy involving a couple with Dissociative Identity Disorder (DID) promotes the incorporation of relational treatment modalities that have historically been regarded as adjunctive in DID care. Through narrative therapy, trauma-informed practice, and a neurodiversity-affirming framework, the case study addresses individual dissociative experiences and relational dynamics while centering client strengths, agency, and meaning-making. In contrast to integration, the therapy respected and honored multiplicity, supporting partners in constructing empowering narratives about their experiences and shared futures. The findings emphasize the need for therapists to master complex therapeutic interventions that enable co-constructed healing for misunderstood populations. Policy and clinical implications promote couples therapy for those with or systems impacted by DID, the enhancement of trauma training for relational therapists, and the encouragement of treatment methodologies that elucidate intimate partnerships as avenues of healing. This case contributes to emerging discourse positioning couples therapy as essential rather than ancillary in comprehensive DID treatment.

The economic burden of Borderline Personality Disorder: direct and indirect costs, and evidence from evaluations of interventions

Monica Ghoyal; Greta Josefine Dertwinkel; Katie Nicol; Ruchika Gajwani; Manuela Deidda Frontiers in Psychiatry, 2026, Vol. 17 https://doi.org/10.3389/fpsyt.2026.1835343

Background Borderline Personality Disorder (BPD) is characterized by longstanding patterns of difficulty in functioning, and when left untreated, BPD can impose a substantial financial burden on individuals and society, in terms of higher healthcare costs as well as broader societal costs. Aims To conduct a critical review of the direct and indirect costs related to BPD. Additionally, to critically evaluate the economic evidence on interventions for BPD. Methods A comprehensive search strategy was conducted across PsycINFO, PubMed, and SCOPUS. Studies were screened in two phases: an initial title and abstract review, followed by full-text screening against a predefined eligibility criteria. Results Across 23 studies, high direct and indirect costs were consistently reported. Economic evaluations primarily examined established psychological interventions for BPD, such as Dialectical Behavior Therapy (DBT), Cognitive Behavior Therapy (CBT), and Schema-Focused Therapy (SFT), comparing costs and quality-of-life outcomes, with most demonstrating cost savings through reduced inpatient and emergency service use. Conclusions This review highlights the substantial economic burden of BPD, driven by high service use, productivity losses, and unmet needs. While interventions such as DBT, CBT and SFT show potential cost-effectiveness, inconsistent cost definitions and the omission of key components, including informal care and long-term impairment, likely lead to an underestimation of true costs. More comprehensive and standardized economic evaluations are needed.

Moderated Online Social Therapy (MOST) in early intervention for psychosis services: a pilot randomised controlled study

Sophie Mae Harrington; Maeve Dwan-O'Reilly; Scott Mahon; Conor Gavin; Megan Cowman; Anna O’Mahony Sinnott; … Irish Journal of Psychological Medicine, 2026, pp. 1-11 https://doi.org/10.1017/ipm.2026.10222

INTRODUCTION: Psychosis is a significant cause of reduced social and occupational function in those aged 18-35 years. This study investigates the feasibility of enhancing currently available Early Intervention for Psychosis (EIP) care using a moderated online intervention comprising therapist-supported online therapeutic interventions and peer-to-peer social mentoring and networking. METHODS: The trial explores data relevant to establishing the feasibility of delivering MOST to patients enrolled in EIP services and estimation of effect sizes for pre-post measures administered to inform power calculations for a full trial. RESULTS: A total of 35 patients with early psychosis were recruited during the study period, with 17 (48.6%) retained in the study at 6 months. In the intervention arm, 65.4% of participants engaged with MOST for 5 weeks or more but only 15.4% continued to engage at six months. At six months, calculation of effect sizes for pre-post changes indicated small to moderate improvements in social and occupational function, small changes in clinical symptom severity and no changes in cognitive performance. DISCUSSION: Findings partially supported the feasibility of conducting a trial of MOST in Irish EIP services. Recruitment and study procedures were successfully completed, while retention and sustained engagement suggested that intervention duration, delivery, and positioning within the care pathway may require consideration for further study of this promising intervention in the Irish context.
