Background and Purpose: Non-suicidal self-injury (NSSI) is a global concern in adolescent mental health. Rural boarding middle school students, as a unique and vulnerable population, remain understudied regarding the potential heterogeneity and longitudinal trajectories of NSSI. This study aimed to identify latent classes of NSSI among this group, examine their transition patterns over time, and evaluate the effectiveness of an emotion management group intervention. Methods: Study 1 cross-sectionally surveyed 927 adolescents; we split samples into non-NSSI and NSSI subgroups and ran latent profile analysis (LPA) only among participants with NSSI ( n =285). Study 2 conducted a 6-month follow-up of 550 participants and applied latent transition analysis (LTA) to examine class shifts and predictive factors. Study 3 was a randomized controlled trial in which 30 students from the High-NSSI received an 8-week emotion-management group intervention. Intervention effects were evaluated using repeated-measures ANCOVA with demographic covariates. Results: Within the NSSI subgroup ( n = 285), two classes were identified: a High-NSSI class ( n = 21, 7.4%) and a Low-NSSI class ( n = 264, 92.6%). Poor sleep quality and bullying victimization independently predicted High-NSSI membership. Longitudinal analysis revealed high stability in the Low-NSSI class (91%) and high fluidity in the High-NSSI class (83% transitioning to low-risk). The emotion-management intervention significantly reduced both NSSI frequency and negative affect. Discussion: These findings demonstrate that clinically meaningful NSSI subtypes exist among rural boarding adolescents and that sleep quality and bullying are key modifiable risk factors. The results support a stepped-care approach in school settings, where the High-NSSI class requires intensive intervention and the Low-NSSI class may benefit from less intensive, universal supports. The emotion-management group intervention is an effective school-based program for the most severe NSSI subtype. Keywords: non-suicidal self-injury, latent profile analysis, latent transition analysis, emotion management, group intervention
Background Non-suicidal self-injury (NSSI) is common during adolescence and is associated with substantial psychological burden and elevated subsequent suicide risk. Physical activity (PA) may be a modifiable correlate of NSSI, but prospective evidence on potential emotional mechanisms and social boundary conditions remains limited. This study examined whether baseline PA was associated with subsequent NSSI, whether anxiety symptoms accounted for part of that association, and whether baseline social support moderated the relevant pathways. Methods A school-based two-wave cohort of 756 Chinese adolescents and young students (mean age = 17.36 years, SD = 2.58; 51.7% female) was followed for 6 months. At Time 1 (T1), participants reported PA, social support, anxiety symptoms, NSSI, and demographic characteristics; anxiety and NSSI were reassessed at Time 2 (T2). Autoregressive regression models adjusted for baseline anxiety or NSSI as appropriate, together with age, gender, and urban/rural residence. Indirect effects were estimated with 5,000 bootstrap resamples, and interaction terms were used to examine first- and second-stage moderation. Results Higher T1 PA was associated with lower T2 NSSI after adjustment for T1 NSSI. The indirect association through T2 anxiety was −0.12 [95% CI (−0.18, −0.06)], representing 48.0% of the reported total association. T1 social support significantly moderated the T1 PA-T2 anxiety association ( p = 0.003); simple-slope estimates indicated a stronger inverse PA-anxiety association at higher social support. Social support did not significantly moderate the T2 anxiety-T2 NSSI association ( p = 0.281). Conclusion The findings are consistent with a model in which greater PA is prospectively associated with lower NSSI partly through lower subsequent anxiety, while social support may shape the PA-anxiety link. Because anxiety and NSSI were measured concurrently at follow-up, the indirect effect should not be interpreted as definitive causal mediation. Multi-wave and intervention studies are needed to establish temporal ordering and causality.
Objective Non-suicidal self-injury (NSSI) is an urgent and substantial public health concern among adolescents. This study is aimed at identifying the independent psychological predictors of NSSI and developing and validating a logistic regression-based risk discrimination model focused on coping styles. Methods A cross-sectional study involving 206 adolescents (104 with NSSI and 102 healthy controls) was conducted. All the participants provided general demographic data, and completed the Experiences in Close Relationships Inventory (ECR), the Childhood Trauma Questionnaire-Short Form Version (CTQ-SF), and the Coping Styles Questionnaire (CSQ). Univariate and multivariate logistic regression analyses were performed to identify the independent predictors of NSSI. A nomogram was constructed in accordance with the final multivariate model. Model performance was evaluated using receiver operating characteristic (ROC) curves, calibration plots, the Hosmer-Lemeshow (HL) test, and a decision curve analysis (DCA). Results Multivariate logistic regression revealed that a higher level of self-blame (OR = 1.062; 95% CI: 1.023–1.102; P = 0.002) was an independent risk factor, whereas a better problem-solving capacity (OR = 0.911; 95% CI: 0.867–0.957; P < 0.001) was an independent protective factor for adolescents with NSSI. Attachment dimensions and childhood trauma did not retain independent significance after adjustment. The nomogram showed excellent discrimination (AUC = 0.951, 95% CI: 0.922–0.980), good calibration (HL test P > 0.05), and superior clinical net benefit across a wide threshold (0.01–0.99). Conclusion Targeting self-blame and enhancing problem-solving skills may be effective styles for the prevention of adolescent NSSI. As a preliminary finding, the nomogram developed in our study is a useful tool for exploratory differentiation.
Background Non-suicidal self-injury (NSSI) is common among adolescents and often remains concealed because of shame and stigma. Voluntary disclosure can provide access to support, but emotional and interpersonal concerns shape the decision to disclose. This review will synthesize disclosure experiences, barriers, and facilitators reported by adolescents who have voluntarily disclosed NSSI at least once. Methods Primary qualitative studies and separately extractable qualitative components of mixed-methods studies involving adolescents aged 10–19 years will be eligible. Searches will cover PubMed, Embase, APA PsycINFO, CINAHL, Scopus, and Web of Science Core Collection. China National Knowledge Infrastructure, Wanfang Data, and the Chinese Biomedical Literature Database will also be searched. Coverage will extend from inception through 31 March 2026, inclusive, without language restrictions; formal searches have not yet been executed. Gray literature, reference lists, and forward citations will also be searched. Two human reviewers will independently select studies, appraise them using the Joanna Briggs Institute (JBI) qualitative checklist, and extract findings. Findings will be synthesized using JBI meta-aggregation, with confidence assessed using ConQual. Discussion The synthesis will examine disclosure decisions, recipients' responses, and subsequent experiences from adolescents' perspectives. Findings may inform clinical, school, and family support for voluntary NSSI disclosure. Background: Sleep disturbances impact outcomes in major depressive disorder (MDD). This cross-sectional study evaluated subjective sleep quality and habits in adolescents with MDD and their associations with suicide attempt (SA) and non-suicidal self-injury (NSSI).Methods: Sixty-eight adolescents aged 13–18 years with MDD were classified as NSSI-only, SA-only, and combined. Clinical severity and depressive symptoms were measured with Clinical Global Impression– Severity scale and Children’s Depression Rating Scale–Revised (CDRS-R). Self-harm and suicidality were evaluated using Inventory of Statements About Self-Injury and Columbia-Suicide Severity Rating Scale. Pittsburgh Sleep Quality Index (PSQI) and Children’s Sleep Habits Questionnaire (CSHQ) assessed past-month sleep quality and past-week sleep habits. Results: Participants had poor sleep quality (89.7%) and disturbed sleep habits (83.8%), without group differences. CSHQ scores correlated moderately with lifetime NSSI frequency (r = .36, P = .003), whereas PSQI scores correlated weakly (r = .28, P = .023). PSQI scores were modestly related to past-month suicidal ideation severity (r = .28, P = .022). In hierarchical regressions controlling for sedative medications and CDRS-R scores, CSHQ (β = 0.299, P = .022) and PSQI (β = 0.248, P = .047) accounted for incremental variance in lifetime NSSI frequency. Conclusion: Sleep problems are common among adolescents with MDD, suggesting a clinical feature rather than a subgroup-specific trait. Sleep disturbance was associated with lifetime NSSI burden beyond depression severity, indicating a clinically relevant assessment domain. Prospective longitudinal studies should clarify temporal and bidirectional relationships with self-harm outcomes.

Background: This cross-sectional study examined the frequency of lifetime suicidal ideation, suicide attempts, and nonsuicidal self-injury (NSSI), and factors associated with lifetime suicidal ideation among candidates for bariatric surgery. Methods: Seventy consecutive adults referred for psychiatric evaluation before a planned bariatric procedure underwent a DSM-5-based unstructured clinical interview and completed standardized measures of suicide-related behaviors, depressive and anxiety symptoms, eating behavior, and self-injury. Group comparisons and exploratory multivariable logistic regression analyses were performed. Results: Lifetime suicidal ideation was reported by 22 participants (31.4%), and 11 (15.7%) reported a history of NSSI. Psychiatric disorders were identified in 21 participants (30.0%). In the exploratory multivariable model, unemployment (odds ratio [OR], 6.17; 95% CI, 1.63–23.38), medical comorbidity (OR, 3.41; 95% CI, 1.03–11.30), and emotional eating (OR per unit increase, 1.73; 95% CI, 1.04–2.87) remained associated with lifetime suicidal ideation. Conclusion: Lifetime suicidal ideation, NSSI, and psychiatric morbidity were common in this single-center sample. Unemployment, medical comorbidity, and emotional eating were associated with lifetime suicidal ideation in an exploratory model. These findings support comprehensive psychiatric assessment before bariatric surgery and require confirmation in larger prospective studies.
Background Self-harm, including non-suicidal self-injury (NSSI), represents a major public health concern among adolescents. Although numerous risk and protective factors have been identified in the literature, their utility for assessment and management remains limited due to their large number and the variability in the strength of their associations. In Iran, in particular, there is a notable lack of synthesis, related to adolescent NSSI, especially regarding protective factors. To address this gap, this protocol outlines an ongoing systematic review aiming to provide a comprehensive overview of the recent (2015 onward) Persian and English peer-reviewed literature on NSSI and self-harm among adolescents in Iran, with a specific focus on studies investigating risk and protective factors. Methods & analysis The protocol is registered with PROSPERO (registration number CRD420261286909), and the systematic literature review, following PRISMA-P guidelines, will include Persian and English-language studies from 2015 onwards examining risk and protective factors for non-suicidal self-injury (NSSI) in Iranian adolescents (10–19 years), as well as robustness of findings. Eligible studies include quantitative, qualitative, and mixed-methods designs, identified through PubMed, PsycINFO, Scopus, Embase, Google Scholar, SID, and Magiran. Data will be extracted using a standardized form, and study quality will be assessed with the Mixed Methods Assessment Tool (MMAT). Findings will be synthesized narratively, summarizing associations between risk or protective factors and NSSI, organized by clinically meaningful categories such as psychopathology, individual, family, peer, and demographic factors.
Adolescent suicide and self-harm have become pressing global public health crises. At the 2026 Asian Society of Child and Adolescent Psychiatry and Allied Professions Grand Forum on Suicide and Self-Harming Behavior in Adolescents Worldwide, researchers, clinicians, and policymakers from multiple countries presented current evidence and field experiences. This study synthesizes the forum's major presentations and discussions in the thematic areas. The forum emphasizes that South Korea continues to report one of the highest adolescent suicide rates among Organisation for Economic Co-operation and Development (OECD) countries and that suicide-related behaviors and non-suicidal self-injury have reached concerning levels across Asia. The cross-national evidence presented at the forum indicated a marked post-pandemic increase in adolescent suicide attempts and self-harm, with a disproportionate burden on adolescents and young women. The speakers further emphasized that aggregate national statistics could obscure the heightened risk faced by the most vulnerable subgroups.Major risk factors included psychiatric comorbidities, diminished sense of belonging, social media use, algorithm-driven content exposure, and socioeconomic disparities in healthcare access. The forum emphasized that for many adolescents, self-harm reflected not a wish to die but an attempt to escape overwhelming psychological pain.Proposed solutions centered on a paradigm shift toward primary prevention, integrated school-government-professional systems incorporating AI-based early warning, the "Safety by Design" principle for social media, community-based safe spaces, and strengthening families and communities.Adolescent suicide prevention is not solely a medical issue but a collective societal responsibility that requires international collaboration.
