Danielle Pretty; Michael Norwood; Tamara Ownsworth; Rachel Gregory; Elizabeth Kendall European Journal of Adapted Physical Activity, 2026, Vol. 18, pp. 16 https://doi.org/10.5507/euj.2025.010

Modified dance for people with traumatic brain injury (TBI) is an emerging adjunct neurorehabilitation technique. The aim of this review is to describe the characteristics of dance programs developed for people with TBI, and to examine how stakeholders perceive these programs. Eight databases (MEDLINE, Scopus, Embase, CINAHL, Web of Science, SPORTDiscus, PsycINFO, and ProQuest Dissertations and Theses) were searched for studies on dance and TBI. Seven eligible articles were identified, involving a total of 22 dance participants with TBI and a further three people with TBI in a control group. The most common dance techniques used were dance movement therapy, improvisation, meditative movement, and mirroring. Modifications to programs included one-on-one delivery, individual tailoring with strategies to assist with cognitive or mobility barriers, and support from clinicians or assistants. Overall, the dance programs were feasible to deliver and acceptable to participants. Three studies revealed mixed evidence of efficacy in terms of motor fitness outcomes (e.g., balance and mobility), and there was insufficient evidence for efficacy in terms of cognition, neuroplasticity, and psychosocial outcomes. The review makes recommendations to enhance the safety and sustainability of dance for the neurorehabilitation population and to improve the standardized reporting of program design and execution to advance the field.

INTRODUCTION: Cognitive deficits are determinants of long-term disability and socio-professional reintegration across neurological disorders, including stroke, traumatic brain injury, multiple sclerosis, and neurodegenerative diseases. Despite evidence supporting cognitive neurorehabilitation, its implementation remains heterogeneous, and therapeutic parameters are insufficiently defined. AREAS COVERED: The authors reviewed the published literature from PubMed and Google Scholar from 1 January 2015 to 30 June 2026, supplemented by seminal publications. Clinical trials, systematic reviews, meta-analyses, and observational and proof-of-concept studies were prioritized. This brief narrative review examines restorative and compensatory cognitive neurorehabilitation for deficits of attention, executive functions, language and disorders of consciousness in people after stroke and traumatic brain injury (TBI), older adults, and those with neurological and neurodegenerative conditions, including multiple sclerosis, Alzheimer's and Parkinson's disease. Emphasis is placed on dose, frequency, adaptivity, personalization, and digital and technology-assisted interventions. Methodological challenges include heterogeneity of protocols, limited use of active control conditions, insufficient long-term follow-up, and the gap between cognitive test improvements and meaningful effects on participation. EXPERT OPINION: Advancing cognitive neurorehabilitation requires precision-driven approaches integrating digital therapeutics with clinical frameworks. Large-scale pragmatic trials, meaningful endpoints, and implementation strategies across the continuum of care will be essential to transform cognitive neurorehabilitation into a component of neurological practice.

The Effects of Mindfulness-Based Interventions on Proprioception: A Bayesian Multilevel Meta-Analysis of Evidence from Randomized Controlled Trials

Proprioception is crucial for physical activities, motor control, balance, and coordination, yet strategies to enhance it remain debated. Mindfulness-based interventions (MBIs) may improve proprioception by enhancing body awareness through attention regulation and body-mind synergy. MBIs offer low intensity, low cost, and good repeatability. This study hypothesized that MBIs improve proprioception indirectly by enhancing sensory awareness and attention regulation. We used Bayesian multilevel meta-analysis to quantify the effects of MBIs on proprioception from studies over the past two decades. Following PRISMA guidelines, we searched six databases for studies published between 2005 and 2025. Bayesian models quantified the effects of MBIs, and moderation analysis explored factors influencing the effect size. We included 22 studies (n = 1397) yielding 112 effect sizes. Following our sign convention, a negative Hedges’ g denoted improved proprioceptive accuracy. The main effect model showed that MBIs improved proprioceptive accuracy (g = −0.70; 95% HDI = [−0.96, −0.44]; BF = 4290.00). Subgroup analysis revealed that Tai Chi had the largest credible effect (g = −0.82), and older adults showed the greatest improvement (g = −0.84). However, a significant multilevel Egger test indicated small-study effects, and the model-averaged bias-corrected interval included zero, so the magnitude of the pooled effect remained uncertain. This meta-analysis indicated that MBIs may improve proprioceptive accuracy, with Tai Chi showing the most consistent evidence, particularly in older adults; however, significant small-study effects and high heterogeneity meant the magnitude is uncertain and may be smaller than the unadjusted estimate suggests. The low-intensity nature of MBIs and their promotion of neuroplasticity through sensory awareness and attention regulation may optimize proprioception, offering insights for neurorehabilitation and integrated rehabilitation programs.

A study protocol for international consensus guidelines on music therapy for paediatric posterior fossa syndrome

James L. Burns; Jonathan Pool; Karen Twyford; Annette Baron; Melanie Thomas; Anna Menén Sánchez; … PLoS ONE, 2026, Vol. 21, Issue 9, pp. e0358571 https://doi.org/10.1371/journal.pone.0358571

Posterior fossa syndrome (PFS) is a common complication following surgical resection of paediatric posterior fossa tumours, characterised by mutism, ataxia, emotional disturbance, and cognitive impairment. Despite significant rehabilitation needs, there is limited evidence regarding effective therapeutic approaches, including the role of music therapy. Music therapy is uniquely positioned to address the multidimensional needs of children with PFS, yet the low incidence and clinical heterogeneity of the syndrome preclude traditional empirical research designs. International consensus guidelines are therefore needed to consolidate clinical knowledge and support consistent, high-quality practice. This protocol describes a two-phase study to develop international consensus guidelines for music therapy practice with children with PFS. Phase 1 involves a pre-Delphi consultation with three regional Advisory Panels (North America, Europe, Australia), each comprising family representatives, music therapists, allied health professionals, and medical practitioners. These panels will identify clinical priorities, family concerns, and interdisciplinary outcomes to inform the development of the survey instrument. Phase 2 uses an international Delphi process with a minimum of 30 experienced paediatric music therapists, proceeding through three to five iterative rounds of item generation, rating, and refinement. Consensus is defined a priori as ≥80% agreement. Quantitative analysis will be conducted in R; qualitative data will be analysed thematically. The study will be conducted and reported in line with DELPHISTAR recommendations. The resulting consensus guidelines will support developmentally responsive, family-centred music therapy assessment and intervention for children with PFS, while identifying priority areas for future research in paediatric neuro-oncology and neurorehabilitation.

The effect of different observation modalities in motor imagery: Immersive virtual reality produces stronger desynchronization than video and real-world presentations

Daniel Carrillo Martín; Mateo Fortich; José Ignacio Serrano; Maria Dolores del Castillo Journal of Neural Engineering, 2026, Vol. 23, Issue 5, pp. 056042 https://doi.org/10.1088/1741-2552/aeabf4

OBJECTIVE: The combination of action observation and motor imagery (AO+MI) has emerged as a promising neurorehabilitation strategy to promote sensorimotor plasticity without real movement, making it particularly valuable for individuals whose motor capacity is limited or absent. Improving AO+MI protocols requires understanding whether different observation stimuli engage the sensorimotor system to varying degrees, as the quality of cortical activation during each training trial may directly influence cumulative neuroplastic outcomes. The present study aimed to determine how the observation modality influences the activation of the sensorimotor cortex during synchronous AO+MI. APPROACH: Here, we investigated the effects of three observation modalities: immersive virtual reality (iVR), two-dimensional video and real-world observation of a robotic agent, on sensorimotor cortical engagement, using Event-Related Desynchronization (ERD) as a neurophysiological marker. Electroencephalographic (EEG) signals were recorded from fifteen healthy participants across three sessions, one per modality, while they observed a humanoid robot performing arm elevation movements and simultaneously engaged in motor imagery of the same movements. ERD was computed for alpha and beta frequency bands over eight electrodes covering frontocentral, central, and centroparietal regions. MAIN RESULTS: Statistical analyses revealed significant modality-dependent differences in sensorimotor cortical engagement across multiple frequency bands, electrode locations, and movement types. Immersive virtual reality consistently elicited the strongest desynchronization, with robust effects in the beta band over frontocentral and right centroparietal regions, and in the alpha band over the right central motor cortex. SIGNIFICANCE: These results demonstrate that the modality of action observation significantly shapes sensorimotor cortical engagement during AO+MI. Our findings also support the integration of immersive virtual reality as a neurophysiological grounded tool for optimizing AO+MI-based neurorehabilitation protocols and brain-computer interface applications.
