Na Zhang; Mengqian Wu; Kaijun Han; Manlan He; Lu Chen Frontiers in Public Health, 2026, Vol. 14, pp. 1903876 https://doi.org/10.3389/fpubh.2026.1903876

Background Sleep duration is an important component of healthy aging, yet gender differences in short and long sleep among older adults remain insufficiently understood. This study aimed to examine gender differences in sleep duration among Chinese older adults and quantify the contributions of socioeconomic, behavioral, functional, and health-related factors to these differences. Methods Data were obtained from 9,590 adults aged ≥65 years who participated in the 2018 wave of the Chinese Longitudinal Healthy Longevity Survey. Sleep duration was self-reported and categorized as short (<6 h), normal (6–8 h), or long (>8 h). Multinomial logistic regression was used to identify factors associated with short and long sleep, with weighted analyses and Poisson regression performed as sensitivity analyses. Separate Fairlie decomposition analyses were conducted for short and long sleep to quantify the contributions of observed characteristics to gender differences. Results Overall, 44.95% of participants had abnormal sleep duration, with a higher prevalence among women than men (46.87% vs. 42.66%, p < 0.001). Compared with men, women had a higher relative risk of short sleep (RRR = 1.399, 95% CI: 1.204–1.625) but a lower relative risk of long sleep (RRR = 0.856, 95% CI: 0.754–0.972). The association with short sleep remained significant in the weighted analysis (RRR = 1.530, 95% CI: 1.210–1.935), whereas the gender difference in long sleep was attenuated (RRR = 0.821, 95% CI: 0.661–1.021). For short sleep, observed characteristics explained 21.65% of the gender disparity. Education (22.75%), depressive symptoms (11.07%), IADL limitation (6.02%), and self-assessed health (4.82%) contributed positively to the disparity, whereas smoking status (−22.57%), drinking status (−12.45%), and exercise (−4.87%) partially offset it. The gender difference in long sleep was negligible, with substantial offsetting contributions across factors. Conclusion Gender differences in sleep duration among Chinese older adults were primarily concentrated in short sleep, with women experiencing a higher risk. Socioeconomic, behavioral, psychological, and functional characteristics contributed to this disparity, although most of the short-sleep difference remained unexplained. Gender-sensitive interventions addressing educational disadvantage, depressive symptoms, functional limitations, and health behaviors may help reduce inequalities in sleep duration among older adults.

Background Sleep disturbances and oral health problems are both prevalent public health concerns with substantial impacts on quality of life and systemic health. However, population-based evidence examining the independent associations between multiple dimensions of oral health and multiple sleep outcomes remains limited. This study investigated the associations between oral health indicators and sleep disturbances among Canadian adults using nationally representative survey data. Methods We utilized data from adults aged 18 years and older who completed the 2017–2018 Canadian Community Health Survey (CCHS). Four oral health indicators were assessed: self-reported oral health, oral pain, bleeding gums, and dry mouth. Four sleep outcomes were examined: short sleep duration, insomnia symptoms, unrefreshing sleep, and difficulty staying awake. Multivariable logistic regression models were used, with progressive adjustment for sociodemographic factors, health status, and health behaviors. Results All four oral health indicators were significantly associated with most sleep outcomes in the full adjusted model. For short sleep duration, the strongest association was observed for poor oral health (AOR: 1.623). For insomnia symptoms, dry mouth (AOR: 1.904) and oral pain (AOR: 1.826) exhibited the strongest associations. For unrefreshing sleep, oral pain (AOR: 1.937) and dry mouth (AOR: 1.808) were associated with higher odds of reporting unrefreshing sleep. For difficulty staying awake, dry mouth (AOR: 1.720) and oral pain (AOR: 1.701) showed the strongest associations. Conclusion Oral health problems are associated with multiple dimensions of sleep disturbance. These findings highlight the need for integrated care and complementary screening between dental and sleep medicine.

Although khat chewing is common among university students in Yemen, evidence on dose–response relationships between khat chewing frequency and sleep duration, perceived effects, and academic performance remains limited. This study examined these associations among medical students at Thamar University, Yemen. A cross-sectional study was conducted among undergraduate medical students. Data were collected using a structured questionnaire. Khat chewing frequency was categorized as non-use, 1–2 days/week, 3–4 days/week, and 5–7 days/week. Multivariable logistic and linear regression analyses were performed to assess associations, and ordinal trend analyses were used to evaluate dose–response relationships. Adjusted odds ratios (AOR) and regression coefficients (β) with 95% confidence intervals (CI) were reported. Statistical significance was set at p < 0.05. Increasing khat chewing frequency was independently associated with higher odds of short sleep duration (< 6 h). Compared with non-chewers, the adjusted odds of short sleep increased progressively, reaching an AOR of 7.83 (95% CI: 3.70–16.57; P < 0.001) among daily chewers.A significant linear dose–response relationship was also observed (AOR = 1.97, 95% CI: 1.54–2.52; P < 0.001). Perceived-effects scores increased progressively with khat-chewing frequency, with adjusted mean differences of 3.36, 3.73, and 4.94 for 1–2, 3–4, and 5–7 days/week, respectively (all P ≤ 0.001), with a significant linear trend (adjusted β = 1.61, 95% CI: 1.29–1.92; P < 0.001). In contrast, academic performance showed no significant association or linear dose–response relationship with khat chewing (adjusted β = 0.22, 95% CI: −0.46–0.89; P = 0.532). Khat chewing frequency showed clear dose–response associations with short sleep duration and perceived effects among medical students. However, these associations did not translate into measurable differences in academic performance. These findings suggest that the effects of frequent khat use are more evident in sleep and perceived behavioural outcomes than in academic achievement. These findings are consistent with known stimulant-related effects and may extend to similar student populations in comparable settings.
