BackgroundPrescription digital therapeutics (PDTs) may be addressed through clinical policies, pharmacy benefits, purchaser-selected catalogs, or professional-care payment rules.A single covered/not-covered designation can obscure differences in scope, distribution, and financial responsibility. This study examined how these distinctions appear in a public-source corpus. MethodsAn exploratory cross-sectional health policy and market access study used an October 4, 2026 snapshot.Twenty-two regulatory-anchored therapeutic program families were classified as general-purpose-device programs or software-primary systems.Adaptive public-source searches assembled policy, benefit, program, and financial records.Operational product-scope statements were separated from general frameworks, historical documents, and manufacturer-reported access.Descriptive counts and sensitivity analyses were generated from source-linked data; national coverage prevalence and covered lives were not estimated. ResultsThe dataset contained 35 operational source records.Twelve current-posted position documents from 10 publishing families supported 57 product-scope mappings across 21 of the 22 programs: 54 restrictive, two supporting conditional clinical eligibility, and one class-based mapping with unresolved delivery-setting applicability.Restrictive mappings comprised 45 medical/clinical-policy and nine pharmacy records; 48 were named and six class-derived.Anthem's product-specific guideline contributed 15 restrictive mappings and addressed both listed applications and associated management services.Conditional clinical eligibility involved Luminopia (Luminopia, Inc., Cambridge, MA, USA) and CureSight (NovaSight Ltd., Airport City, Israel); the intravaginal biofeedback class mapped to leva (Axena Health, Inc., Newton, MA, USA) remained setting-uncertain.Deduplication across repeated family-product-route positions retained 53 mappings. The general-purpose-device-only analysis retained 30 restrictive mappings and excluded both conditionaleligibility mappings and the setting-uncertain mapping.Ten additional pathway records distinguished exception guidance, Medicare professional-care payment, optional digital catalogs, non-drug list architecture, and manufacturer reports.Fifteen financial observations were heterogeneous: a currentdisplayed $99 EndeavorRx (Akili Interactive Labs, Inc., Boston, MA, USA) prescription price, an expired $99 Rejoyn (Otsuka America Pharmaceutical, Inc., Rockville, MD, USA) offer, a $25 leva evaluation fee, and a $2,100 economic-model input were not interchangeable. ConclusionsFDA authorization and billing codes did not establish favorable medical-necessity decisions in this corpus.Product-specific restrictions, exemplified by Anthem's October 2026 guideline, coexisted with conditional pathways elsewhere.Clinical access assessment should verify the governing policy, plan adoption, benefit route, and patient cost before treatment. The broader product definition retained clinical-eligibility pathways and an uncertain class mapping excluded by the general-purpose-device-only analysis. These findings characterize selected documents, not national coverage prevalence or realized patient access.

CDigital therapeutics (DTx) are software-based medical devices that deliver evidence-based therapeutic interventions directly to patients. While the clinical use of DTx is expanding rapidly, regulatory fragmentation and the absence of harmonized legal definitions hinder their assessment and reimbursement policies, as well as their integration into healthcare systems. There is currently a lack of comprehensive overviews of DTx clinical development, which are essential for informing policy, research and regulatory priorities. This paper provides an updated overview of randomised clinical trials involving DTx. 254 eligible trials on DTx, (defined according to ISO/TR 11147:2023) registered through January 2025 were identified through a search on C linicalTrials.gov . Mobile applications represent the most prevalent technology (65%), followed by web-based tools and virtual reality (20%). Psychiatric disorders are the primary therapeutic target (52% of the studies) with insomnia, depression, and anxiety being the most addressed conditions. Trials were primarily conducted in North America (41%) and Europe (32%), where Germany, Spain, and Italy represent the most active countries. The analysis of clinical trials registrations on ClinicalTrials.gov reveals a heterogenous geographical distribution of DTx development but also a clear direction in the type of conditions for which they are mostly indicated, namely psychiatric and nervous system disorders. The fragmented policies on DTx reimbursement, even among EU countries, and the different capacity of governments to fund DTx research and development emerge as key factors, along with cultural and ethical issues, in determining the willingness of the industry to invest in these devices.

Florian W. Sander; Etienne Sallard; Mauro Silva; Laurence Schneider; Karin Diserens; Christophe E. Graf; … Expert Review of Neurotherapeutics, 2026, pp. 1-18 https://doi.org/10.1080/14737175.2026.2723909

INTRODUCTION: Cognitive deficits are determinants of long-term disability and socio-professional reintegration across neurological disorders, including stroke, traumatic brain injury, multiple sclerosis, and neurodegenerative diseases. Despite evidence supporting cognitive neurorehabilitation, its implementation remains heterogeneous, and therapeutic parameters are insufficiently defined. AREAS COVERED: The authors reviewed the published literature from PubMed and Google Scholar from 1 January 2015 to 30 June 2026, supplemented by seminal publications. Clinical trials, systematic reviews, meta-analyses, and observational and proof-of-concept studies were prioritized. This brief narrative review examines restorative and compensatory cognitive neurorehabilitation for deficits of attention, executive functions, language and disorders of consciousness in people after stroke and traumatic brain injury (TBI), older adults, and those with neurological and neurodegenerative conditions, including multiple sclerosis, Alzheimer's and Parkinson's disease. Emphasis is placed on dose, frequency, adaptivity, personalization, and digital and technology-assisted interventions. Methodological challenges include heterogeneity of protocols, limited use of active control conditions, insufficient long-term follow-up, and the gap between cognitive test improvements and meaningful effects on participation. EXPERT OPINION: Advancing cognitive neurorehabilitation requires precision-driven approaches integrating digital therapeutics with clinical frameworks. Large-scale pragmatic trials, meaningful endpoints, and implementation strategies across the continuum of care will be essential to transform cognitive neurorehabilitation into a component of neurological practice.

No evidence of increased gaming-related problems with long-term use of a video game therapeutic: Exploratory endpoint findings from a randomized controlled trial

Digital therapeutics for mental health often face low patient engagement, which limits their clinical impact. Interventions that deliver treatment using a video game medium may improve engagement and therapeutic efficacy, but the potential emergence of gaming-related problems remains a safety question and a concern among clinical stakeholders. We examined whether long-term adjunctive use of the video game therapeutic Meliora was associated with changes in gaming-related problems in a three-arm randomized controlled trial. Participants were adults with interview-confirmed major depressive disorder without significant gaming or gambling problems at baseline. The intention-to-treat cohort ( n = 1001) had a mean age of 33.4 years (SD 9.3), and 64% were female. From baseline to post-intervention (week 12), Game Addiction Scale (GAS-7) scores decreased in the Meliora group (mean change, −0.63 points; 95% CI, −0.91 to −0.34; dz = −0.24; Holm-adjusted p < 0.001), whereas no change was observed in the Sham group, which used a highly similar video game therapeutic (mean change, −0.02 points; 95% CI, −0.25 to 0.28; dz = −0.01; Holm-adjusted p = 0.900). Between-group analyses showed more negative GAS-7 change in the Meliora group than in the Sham group (adjusted mean difference, −0.19 points; 95% CI, −0.30 to −0.08; d = −0.13; Holm-adjusted p = 0.001), indicating a relatively greater reduction with Meliora, whereas changes were similar between the Meliora and TAU groups (adjusted mean difference = 0.04 points; 95% CI, −0.08 to 0.15; d = 0.03; Holm-adjusted p = 0.509). Intervention usage time, experienced immersion, and their interaction were not significantly associated with changes in GAS-7 scores in either the Meliora or Sham group. Using a ≥ 25% deterioration threshold, deterioration occurred in 10.4% of participants in the Meliora group, 15.3% in the Sham group, and 9.8% in the TAU group, with no statistically significant between-group differences. Complementary per-protocol analyses among participants with greater intervention exposure were consistent with the ITT findings. These findings provide no evidence that long-term use of a video game therapeutic was associated with increased gaming-related problems in appropriately screened patients using interventions with safeguards against excessive use.
