Fadillah Nur Fajri; Anisa A'in Jurnal Riset Keperawatan dan Kesehatan, 2026, Vol. 3, Issue 5, pp. 425-432 https://doi.org/10.71203/jrkk.v3i5.191

Background: Patients recovering from craniotomy in the Intensive Care Unit (ICU) are at risk of hemodynamic changes related to stress responses, neurological impairment, pain, and environmental stimuli. A combination of music therapy and hand massage may help maintain hemodynamic stability in these patients. Objective: To describe the application of combined music therapy and hand massage to support hemodynamic stability in post-craniotomy patients in the ICU. Methods: This evidence-based nursing intervention was implemented with three post-craniotomy patients admitted to the ICU of Abdul Wahab Sjahranie Regional General Hospital, Samarinda. The intervention consisted of 15 minutes of instrumental or classical music followed by 15 minutes of effleurage hand massage. Systolic and diastolic blood pressure, mean arterial pressure (MAP), heart rate (HR), respiratory rate (RR), and oxygen saturation (SpO₂) were measured before and after each intervention. The intervention was conducted over three days, with each patient’s clinical changes monitored. Results: The case study findings indicated greater stability in blood pressure, MAP, HR, RR, and SpO₂ following the intervention. Conclusion: Combined music therapy and hand massage may be a safe, non-pharmacological intervention to support nursing care and maintain hemodynamic stability in post-craniotomy patients. Further research is needed to assess its effects on other outcomes, including biochemical changes.

Previous research has suggested that culturally mismatched interventions may cause resistance and reduce the effectiveness of music-based interventions. This pilot randomized exploratory study examined the implementation and preliminary outcomes of a music-based intervention incorporating large language model (LLM)-generated, culturally adapted verbal suggestions among Chinese undergraduate students. A total of 113 participants were randomly assigned to three multicomponent conditions: (1) Chinese film music accompanied by LLM-generated verbal suggestions involving Eastern imagery, (2) a standardized music therapy recording with conventional verbal guidance, or (3) piano music without verbal guidance. Following a 40-minute cognitively demanding task, participants completed pre- and post-intervention measures of state anxiety, positive and negative affect, and heart rate. Baseline-adjusted ANCOVAs showed no group differences in positive affect, F (2, 109) = 1.03, p = .360, η p 2 = .019, or state anxiety, F (2, 109) = 0.68, p = .511, η p 2 = .012. A significant group effect was found for negative affect, F (2, 109) = 4.60, p = .012, η p 2 = .078; the Eastern imagery group had lower adjusted post-intervention scores than both comparison groups (Holm-adjusted ps = .026). Heart rate also differed across groups, F (2, 109) = 10.56, p < .001, η p 2 = .162, with lower adjusted values in the standardized therapy group than in the other two groups. The findings identify potentially meaningful response patterns but do not establish efficacy or the independent effect of culturally adapted verbal suggestions because the conditions differed in musical material and duration. Larger, controlled trials are required. (ClinicalTrials.gov, NCT07359950).

Combined aromatherapy and music or sound for emotional distress: a systematic review of controlled studies published between 2019 and 2026

Haojian Zhai; Xinyue Chen; Dong Lv; Amy Jing-Wen Yin; Wenge Zhang; Yunkai Wang; … Frontiers in Psychology, 2026, Vol. 17 https://doi.org/10.3389/fpsyg.2026.1955987

Background Emotional distress is common around clinical procedures, cancer supportive care, and health-professions training. Aromatherapy and music are easy to deliver, yet it remains unclear when combining them is worthwhile and whether the combination offers more than either component alone. Objective To synthesize experimental studies of aromatherapy combined with music or sound published from January 2019 through September 7, 2026; identify the populations and settings in which benefits were most consistent; and separate benefit over inactive care from incremental benefit over a single sensory intervention. Methods We searched PubMed, Embase, CINAHL, APA PsycINFO, Scopus, Web of Science Core Collection, CENTRAL, CNKI, and Dentistry & Oral Sciences Source from January 1, 2019, to September 7, 2026. Randomized trials and controlled nonrandomized or quasi-experimental studies were eligible when they included an identifiable arm combining aromatherapy with music or sound and reported emotional-distress or related physiological outcomes. Two reviewers independently screened records, extracted data, and assessed risk of bias. Clinical and methodological heterogeneity precluded meta-analysis; findings were synthesized narratively using SWiM guidance. Results Twenty-four studies involving 2,530 participants from 12 countries or regions were included. Of these, 21 were randomized studies and three were controlled nonrandomized or quasi-experimental studies. The evidence covered procedural and perioperative care, dentistry, nursing education and occupational stress, and cancer or palliative care. Results were most consistent in pediatric dentistry, magnetic resonance imaging, nursing examinations or skills testing, and some repeated cancer-support programs. Two lithotripsy trials and the adult endodontic study did not show a clear benefit. Although combined treatment often outperformed inactive care, it frequently failed to outperform aromatherapy or music alone. Adverse events, acceptability, and economic outcomes were poorly reported. Conclusion Combined aromatherapy and music or sound showed the most consistent reductions in anxiety or distress in pediatric dentistry, MRI examinations, nursing examinations or skills training, and some perioperative and cancer-support settings. Clear benefit was not demonstrated in lithotripsy or endodontic care; in general surgery, pregnancy non-stress testing, coronary angiography, palliative care, and outpatient oncology, the combination generally did not outperform a single modality. Future adequately powered factorial trials should standardize the interventions and use prespecified between-group comparisons, longer follow-up, and systematic reporting of adverse events and costs. Effect of Live Classical Music in Homeless and Hospital Settings

Joseph S Wang; Nader Wehbi; Natalie Briggs; Helena Yip; Esteban Hernandez Parra Cureus, 2026, pp. e117394 https://doi.org/10.7759/cureus.117394

IntroductionThe therapeutic effects of classical music in reducing stress have been well documented, particularly in healthcare settings. However, classical music has also been used to discourage unhoused individuals from remaining in public spaces. For example, loud classical music is sometimes played at gas stations and train stations to deter unhoused individuals from lingering in these areas. Our study sought to answer the question of whether exposure to live classical music in an unhoused or homeless population would lead to a similar reduction in anxiety levels compared with staff, patients, and patients' families in a hospital setting. MethodsBrief anonymous surveys were based on the State-Trait Anxiety Inventory (STAI), a validated anxiety questionnaire.A shortened five-item survey was administered to participants seated in the cafeteria of the hospital and also in dining areas of facilities serving the unhoused.A single musician played the violin/viola for 60 minutes, while audience members were asked to rate themselves on a scale of 1 to 4 in categories including content, worried, tense, calm, and upset before and after listening to at least 15 minutes of classical music. ResultsTwo groups were surveyed, including the Hospital Group (N = 47) as the comparison group and the Unhoused Group (N = 66).A Student's t-test was used for analysis.Classical music was associated with improved feelings in both groups. When comparing change scores between groups, improvements were similar for "calm" (p = 0.092), "tense" (p = 0.678), "upset" (p = 0.183), and "content" (p = 0.123)."Worried" feelings decreased significantly more in the Unhoused Group than in the Hospital Group (p = 0.037).When stratified by gender or duration of housing insecurity, there was no difference in the improvement scores. ConclusionsThis observational study suggests that exposure to live classical music is associated with a self-reported short-term improvement in anxiety levels in the unhoused population.

The Therapeutic Impact of Traditional Indian Instrumental Music on Labour Pain and Anxiety in Primigravida Mothers: A Quasi-Experimental Study

Paulina Kumari; Janet Prima Miranda; Amisha S Amin; Vijayalaxmi Subramanium Journal of Health and Allied Sciences NU, 2026, Vol. 0, pp. 1-9 https://doi.org/10.25259/jhasnu_81_2026

Objectives Childbirth is a universally intense physiological and psychological event. For primigravida women, the lack of prior experience often leads to heightened anxiety and lower pain tolerance during the first stage of labour. This study evaluates the efficacy of a specialized instrumental sound therapy as a non-pharmacological nursing intervention to manage these outcomes. Material and Methods A quasi-experimental pre-test/post-test control group design was utilized at a tertiary care hospital in Mangaluru. Sixty-eight primigravida women were recruited via purposive sampling and divided into intervention ( n = 34) and control ( n = 34) groups. The intervention consisted of two 30-min sessions of traditional Indian instrumental music administered via headphones during the latent phase (2-4 cm dilatation) with a 30-min rest interval. Pain was assessed using the Numerical Pain Rating Scale (NPRS), and anxiety was measured via the Visual Analogue Scale for Anxiety (VASA). Results The study of 68 primigravida mothers found that following the music intervention, the experimental group showed highly significant reductions in mean pain score (6.44–5.00) and anxiety (4.50–3.59) score, whereas scores in the control group naturally increased as labour progressed. Post-test comparisons confirmed that traditional Indian instrumental music significantly lowered pain and anxiety levels compared to standard care, establishing it as an effective non-pharmacological labour aid. Conclusion Specialized instrumental music is a highly effective, low-cost tool for improving maternal comfort and psychological stability during labour. These findings support the integration of structured music therapy into standard obstetric care protocols to enhance the birthing experience in modern, home-like labour environments.

A study protocol for international consensus guidelines on music therapy for paediatric posterior fossa syndrome

James L. Burns; Jonathan Pool; Karen Twyford; Annette Baron; Melanie Thomas; Anna Menén Sánchez; … PLoS ONE, 2026, Vol. 21, Issue 9, pp. e0358571 https://doi.org/10.1371/journal.pone.0358571

Posterior fossa syndrome (PFS) is a common complication following surgical resection of paediatric posterior fossa tumours, characterised by mutism, ataxia, emotional disturbance, and cognitive impairment. Despite significant rehabilitation needs, there is limited evidence regarding effective therapeutic approaches, including the role of music therapy. Music therapy is uniquely positioned to address the multidimensional needs of children with PFS, yet the low incidence and clinical heterogeneity of the syndrome preclude traditional empirical research designs. International consensus guidelines are therefore needed to consolidate clinical knowledge and support consistent, high-quality practice. This protocol describes a two-phase study to develop international consensus guidelines for music therapy practice with children with PFS. Phase 1 involves a pre-Delphi consultation with three regional Advisory Panels (North America, Europe, Australia), each comprising family representatives, music therapists, allied health professionals, and medical practitioners. These panels will identify clinical priorities, family concerns, and interdisciplinary outcomes to inform the development of the survey instrument. Phase 2 uses an international Delphi process with a minimum of 30 experienced paediatric music therapists, proceeding through three to five iterative rounds of item generation, rating, and refinement. Consensus is defined a priori as ≥80% agreement. Quantitative analysis will be conducted in R; qualitative data will be analysed thematically. The study will be conducted and reported in line with DELPHISTAR recommendations. The resulting consensus guidelines will support developmentally responsive, family-centred music therapy assessment and intervention for children with PFS, while identifying priority areas for future research in paediatric neuro-oncology and neurorehabilitation.
