This study examined whether metacognitive strategy training strengthens the contribution of an AI-based Socratic reflection system to pre-service teachers’ reflective practice. Study 1 used a 2 (metacognitive training: present vs. absent) × 2 (tool condition: AI-based Socratic scaffolding vs. traditional digital tools) randomized between-subjects design ( N = 60; n = 15 per cell). Written reflection quality at post-test was analyzed by ANCOVA with pre-test performance as the covariate. The metacognitive-training main effect was significant, F (1, 55) = 33.89, p < 0.001, η 2 p = 0.381, as was the tool-condition main effect, F (1, 55) = 9.63, p = 0.003, η 2 p = 0.149. More importantly, the metacognition × tool interaction was significant, F (1, 55) = 10.91, p = 0.002, η 2 p = 0.165. Under metacognitive training, AI-based Socratic scaffolding produced a significant model-estimated advantage over traditional digital tools (Δ M = 0.56, p < 0.001), whereas the tool difference was not significant without metacognitive training (Δ M = −0.03, p = 0.821). Study 2 examined dialogue logs from the two AI conditions. Participants who received metacognitive training generated a substantially larger proportion of higher-order prompts than participants who received placebo training (44.1% vs. 12.2%), t(28) = 7.14, p < 0.001, d = 2.61. These process data are consistent with the proposed account that metacognitive preparation changes how learners engage with AI, although they do not constitute a formal mediation test. The findings suggest that, within this configured Socratic AI environment, metacognitive preparation is an important condition for realizing higher-quality reflective practice.

Chronic insomnia frequently co-exists with nocturia, contributing to impaired sleep quality and reduced health-related quality of life. A recent randomized cross‑over study found that daridorexant improved sleep duration, insomnia severity, daytime functioning, and nocturnal voiding in patients with chronic insomnia and nocturia. To complement these quantitative outcomes, the study incorporated qualitative free‑text surveys capturing patients’ lived experiences, addressing an existing gap in patient‑reported insights for this multifaceted condition. Here, we report these qualitative findings assessed at baseline and post‑treatment. This double-blind, placebo-controlled, two-way crossover trial of patients with insomnia disorder and comorbid nocturia included 60 adults ≥ 55 years with ≥ 3 months of insomnia symptoms, an Insomnia Severity Index ≥ 13, and ≥ 3 nocturnal voids per night. Patients were randomized to daridorexant 50 mg or placebo for 4 weeks, followed by crossover after a washout period. Free‑text surveys at baseline and after each treatment period captured patient‑reported impacts of their conditions on daily life and perceived changes during treatment. At baseline, patients reported broad negative impacts across physical, emotional, cognitive, and daily functioning domains. During treatment, more patients reported improvements in sleep (87% vs. 50%) and nocturia (79% vs. 32%) on daridorexant than on placebo, and improvements in functioning, particularly physical, cognitive, emotional, and daily activities. Patients reported meaningful improvements with daridorexant 50 mg, including better sleep, reduced nocturia, and enhanced physical, cognitive, and emotional functioning. Daridorexant consistently elicited more positive patient‑reported changes than placebo. These findings highlight the value of free‑text surveys for capturing patient‑centered outcomes. NCT05597020.

Objective Insomnia causes daytime impairment and reduced quality of life. We evaluated the comparative efficacy and safety of Traditional Chinese Medicine (TCM) treatment strategies for insomnia. Methods Nine databases (PubMed, Embase, Web of Science, Cochrane CENTRAL, ClinicalTrials.gov , CNKI, Wanfang, VIP, SinoMed) were searched through January 2026 for randomized controlled trials (RCTs). Primary outcomes were ISI and PSQI scores; secondary outcomes included clinical efficacy and adverse events (AEs). Data were synthesized using frequentist network meta-analysis. Results Twelve RCTs involving 1,260 participants were included, evaluating five strategy nodes: TCM alone, TCM plus Western medicine (WM), WM alone, placebo, and CBT-I. For ISI, TCM plus WM ranked highest (SUCRA 89.7%) and significantly improved scores versus placebo (OR 2.84, 95% CI 1.42–5.68) and WM (OR 1.96, 95% CI 1.08–3.56). For PSQI, TCM plus WM showed the highest probability of improvement (SUCRA 71.9%) with greater reduction versus placebo (MD −2.31, 95% CI −3.65 to −0.97). For clinical efficacy, SUCRA values were 86.6% for TCM plus WM, 77.2% for TCM, 20.9% for WM, and 15.3% for placebo. For AEs (higher SUCRA indicating better safety), SUCRA values were 94.8% for WM, 43.9% for TCM plus WM, 31.1% for placebo, and 30.2% for TCM. Sensitivity analysis excluding secondary insomnia confirmed robust findings. Conclusion Integrated TCM-WM and TCM therapies demonstrate clinical efficacy and acceptable safety for insomnia management. Systematic review registration The review protocol was prospectively registered in the International Platform of Registered Systematic Review and Meta-analysis Protocols (INPLASY) (Registration number: INPLASY202680041; DOI: 10.37766/inplasy2026.8.0041 ).

۶۸) و WM (OR ۱.
