Effects of acceptance and commitment therapy on psychological and health-related outcomes in stroke patients: a systematic review and meta-analysis of randomized controlled trials

Xiaohuan Li; Huang Weijin; Ying Gao; Yu'e Wu; Ping Wang Frontiers in Neurology, 2026, Vol. 17, pp. 1953111 https://doi.org/10.3389/fneur.2026.1953111

Background Stroke is a leading cause of disability worldwide and is frequently accompanied by psychological and functional impairments. Acceptance and Commitment Therapy (ACT), a third-wave behavioral intervention focusing on psychological flexibility, has been increasingly applied in stroke rehabilitation. Although previous reviews have suggested beneficial effects of ACT in stroke patients, whether these benefits persist in the short term after the intervention ends, and the certainty of the evidence, remain unclear. Methods A systematic review and meta-analysis was conducted according to PRISMA guidelines and registered in PROSPERO (CRD420261325108). Randomized controlled trials evaluating ACT in stroke patients were included. Pooled effects were calculated using random-effects models, with separate analyses for post-intervention and follow-up outcomes. Sources of heterogeneity were explored using subgroup analyses and meta-regression. Risk of bias was assessed using the Cochrane Risk of Bias 2 tool, and certainty of evidence was evaluated using the GRADE framework. Results Twenty-two RCTs involving 1,975 participants were included. ACT may be associated with reductions in depressive symptoms (post-intervention: SMD = −1.32, 95% CI: −1.80 to −0.84; follow-up: SMD = −0.76, 95% CI: −1.07 to −0.45) and anxiety symptoms (post-intervention: SMD = −1.36, 95% CI: −1.90 to −0.82; follow-up: SMD = −0.51, 95% CI: −1.42 to 0.40, not significant); depressive symptoms remained statistically significant at follow-up. Heterogeneity was considerable for both outcomes at post-intervention ( I 2 = 93.68% for depression and 93.60% for anxiety), so the pooled estimates represent average effects across studies. ACT may also be associated with improvements in psychological flexibility, quality of life, and sleep quality; however, these outcomes were based on a limited number of studies and showed considerable heterogeneity ( I 2 = 74.68 to 92.26%), so they should be interpreted cautiously. However, no statistically significant effects were observed for activities of daily living, neurological function, or hope. ACT did not appear to increase dropout rates in the available studies ( K = 16; RR = 0.99, 95% CI: 0.58–1.69). Meta-regression analyses indicated that intervention duration may not be a significant moderator of treatment effects. The certainty of evidence ranged from very low to moderate, mainly due to risk of bias and inconsistency. Conclusion Acceptance and Commitment Therapy may be a useful adjunctive intervention for improving psychological outcomes in people with stroke. However, evidence for functional recovery remains insufficient. Future high-quality RCTs with standardized protocols, larger samples, and longer follow-up are needed to confirm the long-term effectiveness of ACT. From reducing cognitive load to sustaining participation: psychological mechanisms, evidence, and stage-matched pedagogy for university tennis beginners

Jing Wang; Paramasivam Muthusamy Frontiers in Psychology, 2026, Vol. 17 https://doi.org/10.3389/fpsyg.2026.1940031

University tennis beginners face simultaneous technical, perceptual, tactical, and psychological demands that may exceed their attentional resources, challenge perceived competence, and diminish engagement. This mini-review synthesizes evidence on cognitive load, self-efficacy, autonomous motivation, multidimensional engagement, ecological dynamics, and beginner tennis pedagogy to explain how instructional design can support both skill acquisition and sustained participation. Evidence on equipment scaling and game-based learning indicates that low-compression balls, smaller courts, simplified rules, and cooperative tasks can increase the number of playable exchanges, enjoyment, and early confidence, although direct tests of cognitive load reduction are scarce and university-specific evidence remains limited. Constraints-led practice can then connect stroke production with perception, positioning, decision-making, and tactical intention, whereas selective technical instruction is most useful when a diagnosable movement feature restricts accuracy, consistency, or transfer. On this basis, this review proposes an evidence-informed, stage-matched (readiness-informed) conceptual framework that organizes individually supported instructional functions into a diagnostic, skill-specific, overlapping, and reversible structure rather than an empirically validated sequence: Stage 1 reduces entry barriers and builds credible mastery experiences; Stage 2 develops adaptive exploration and transfer; and Stage 3 refines limiting technical features while progressively restoring standard conditions. Engagement is positioned as a proposed mediating process linking psychological mechanisms to learning and participation. The framework extends evaluation beyond immediate performance toward retention, transfer, confidence under challenge, and sustained participation as an important but currently undertested outcome. Controlled and longitudinal studies are needed to evaluate the proposed mechanisms, pathways, and boundary conditions across diverse university settings.

Testing a dual-process-inspired questionnaire-primary residual framework for personality prediction: an exploratory study with static line drawings

Lifen Weng; Yiyang Shi Frontiers in Psychology, 2026, Vol. 17 https://doi.org/10.3389/fpsyg.2026.1873771

Comprehensive self-report personality inventories can impose substantial response burden, motivating the development of more efficient assessment strategies and the exploration of complementary information sources. Guided by Dual-Process Theory, this exploratory study examined a questionnaire-primary residual fusion framework in which a reduced questionnaire response vector served as the main predictive source, while static abstract line drawings were evaluated as a bounded auxiliary source of information. A total of 100 participants completed the 187-item Sixteen Personality Factor Questionnaire (16PF) and provided unprompted abstract line drawings. To account for the small sample size and reduce optimistic bias, we used participant-level repeated cross-validation, fold-local questionnaire item selection, standardized image preprocessing, cross-fitted residual learning, and nested selection of the visual correction scale. The reduced questionnaire provided a useful but imperfect held-out approximation of the full-form 16PF reference scores, with factor-level Pearson correlations ranging from 0.34 to 0.78. In contrast, the static drawing modality did not provide stable incremental predictive value. Nested validation selected a zero visual correction in 21 of 25 outer folds, and the fixed scale originally proposed for visual correction (0.05) produced a slightly higher mean absolute error than the questionnaire-only condition (1.2120 vs. 1.2100). An exploratory analysis using teacher-provided observer ratings likewise showed no advantage for visual fusion over questionnaire-only prediction. These findings indicate that Dual-Process Theory can motivate a testable asymmetric framework for integrating explicit and non-verbal information, but the present data do not support static final line drawings as a reliable source of incremental personality information. Future research should examine richer process-level drawing signals, including stroke timing, pauses, pressure, and trajectory dynamics, in larger independent samples.

Cardiometabolic diseases in transgender populations

Mattia Fienga; Uwe J.F. Tietge Cardiovascular Diabetology – Endocrinology Reports, 2026, Vol. 12, Issue 1 https://doi.org/10.1186/s40842-026-00336-x

The number of people seeking gender-affirming medical care and gender-affirming hormone therapy (GAHT) is increasing. As sex steroids shape cardiometabolic physiology, GAHT is expected to meaningfully alter cardiometabolic risk. The aim of this review was to systematically determine and narratively synthesize evidence on the cardiometabolic effects of GAHT and identify clinically relevant knowledge gaps. We searched PubMed for English-language primary human studies published through November 26, 2025. Of 346 screened records, 96 unique original investigations were included. Most studies had less than 3 years of follow-up, the longest being 20 years in one electronic health record-based study. In transgender women, BMI was often reported as stable with higher fat and lower lean mass compared to baseline; insulin resistance tended to increase while fasting glucose and HbA1c were usually longitudinally stable. Lipid changes often showed reductions in total and low-density lipoprotein cholesterol as well as triglyceride levels. Blood pressure generally remained stable or decreased. In transgender women, VTE incidence was higher than in both cisgender women and men, whereas stroke and MI incidence was higher primarily in comparison with cisgender women. In transgender men, BMI was increased or stable and lean mass increased with variable fat mass changes and some long-term increases in visceral and liver fat. Glucose usually remained stable while insulin resistance findings were mixed. Lipid profiles commonly shifted toward lower high-density lipoprotein cholesterol and higher low-density lipoprotein cholesterol and triglycerides. Blood pressure findings were mixed with some cohorts showing an increase. In transgender men, MI incidence was higher than in cisgender women but generally not higher than in cisgender men; evidence for other cardiovascular outcomes was limited and inconsistent. Overall, evidence on systemic inflammation, renal and hepatic outcomes is sparse. In conclusion, GAHT is associated with quantifiable cardiometabolic changes, but magnitude and clinical relevance vary by regimen, population, baseline risk and study follow-up duration. Heterogeneous regimens, study methods, and confounding contribute to mixed findings. Major gaps persist with limited long-term prospective data, sparse mechanistic insights, underrepresentation of adolescents and non-European ancestry populations, and insufficient information on cardiometabolic outcomes with ageing. Sex- and gender identity-stratified evidence synthesis: Distinguishes feminizing and masculinizing GAHT while presenting parallel outcome analyses for transgender women and transgender men, enabling more clinically precise interpretation of cardiometabolic effects. Systematic characterization of heterogeneity: Integrates regimen-, population-, and context-specific findings, including discordant results, into structured domain-specific evidence tables, improving transparency and interpretability across a heterogeneous literature base. Novel evidence mapping framework: Introduces a domain-level framework quantifying both evidence volume (number of primary studies) and directional concordance of findings, visualized within a sex-stratified cardiometabolic risk continuum to identify areas of consistency, uncertainty, and evidence gaps.

Precision cognitive neurorehabilitation: dose, adaptivity, digital therapeutics and implementation

Florian W. Sander; Etienne Sallard; Mauro Silva; Laurence Schneider; Karin Diserens; Christophe E. Graf; … Expert Review of Neurotherapeutics, 2026, pp. 1-18 https://doi.org/10.1080/14737175.2026.2723909

INTRODUCTION: Cognitive deficits are determinants of long-term disability and socio-professional reintegration across neurological disorders, including stroke, traumatic brain injury, multiple sclerosis, and neurodegenerative diseases. Despite evidence supporting cognitive neurorehabilitation, its implementation remains heterogeneous, and therapeutic parameters are insufficiently defined. AREAS COVERED: The authors reviewed the published literature from PubMed and Google Scholar from 1 January 2015 to 30 June 2026, supplemented by seminal publications. Clinical trials, systematic reviews, meta-analyses, and observational and proof-of-concept studies were prioritized. This brief narrative review examines restorative and compensatory cognitive neurorehabilitation for deficits of attention, executive functions, language and disorders of consciousness in people after stroke and traumatic brain injury (TBI), older adults, and those with neurological and neurodegenerative conditions, including multiple sclerosis, Alzheimer's and Parkinson's disease. Emphasis is placed on dose, frequency, adaptivity, personalization, and digital and technology-assisted interventions. Methodological challenges include heterogeneity of protocols, limited use of active control conditions, insufficient long-term follow-up, and the gap between cognitive test improvements and meaningful effects on participation. EXPERT OPINION: Advancing cognitive neurorehabilitation requires precision-driven approaches integrating digital therapeutics with clinical frameworks. Large-scale pragmatic trials, meaningful endpoints, and implementation strategies across the continuum of care will be essential to transform cognitive neurorehabilitation into a component of neurological practice.
