Background Insomnia and poor sleep are highly prevalent among university students and are linked to impaired mental and physical health, reduced cognitive functioning, and lower academic performance. Existing interventions for insomnia include cognitive behavioural therapy for insomnia (CBT-I), mindfulness, and sleep hygiene. CBT-I is widely recognised as effective but remains difficult to access for the general population and lack scalability, long-term follow-up, and feasibility in non-clinical settings. Pragmatic, group-based approaches that can be delivered within the university and are tailored to help students develop self-endorsed routines and competencies for better sleep, creating sustainable changes that extend beyond the program period, is needed. Methods This study is a pragmatic, two-arm randomised controlled trial evaluating the effectiveness of a group-based sleep program for university students with insomnia symptoms. A total of 118 full-time students aged 18–30 meeting diagnostic criteria for chronic insomnia disorder will be randomized (1:1) to either the intervention or a wait-list control group. The intervention comprises 12 weekly 90-minute group sessions followed by nine monthly follow-up meetings, facilitated by trained coaches. The program integrates core elements of CBT-I with motivational and coping strategies, delivered in an interactive and peer-supported format. The primary outcome is change in insomnia severity measured by the Bergen Insomnia Scale. Secondary outcomes include subjective and objective sleep measures, mental health, cognitive performance, academic attainment, health behaviours, physical fitness, and biological markers. Assessments will be conducted at baseline, post-program (12 weeks), and 12 months. A mixed methods process evaluation will assess fidelity, acceptability, mechanisms of impact, and contextual influences. Discussion This trial will be among the first pragmatic evaluations of a structured, group-based sleep intervention tailored to university students with insomnia. Key strengths include the randomised controlled design, comprehensive outcome assessment, 12-month follow-up, and embedded process evaluation. If effective, the program could provide a feasible, scalable, and cost-effective model to improve sleep and therein health, and academic performance in young adults in higher education.

Low-intensity mind–body exercise for mental health in ageing societies: Tai Chi as a scalable intervention across depression, anxiety, insomnia, loneliness, and cognitive decline

Population ageing is increasing the number of older adults who experience overlapping depressive symptoms, anxiety, insomnia, loneliness, and cognitive vulnerability, often alongside multimorbidity, pain, frailty, and functional limitation. This structured narrative review evaluates Tai Chi as a scalable, low-intensity mind–body exercise for these interconnected problems. We synthesised 171 publications identified through PubMed and citation chaining, including Tai Chi-specific clinical studies, broader mind–body evidence, guidelines, and implementation literature. Evidence is most consistent for reducing depressive symptoms, improving sleep, and improving balance, with possible benefits for selected cognitive outcomes. Evidence is weaker for diagnosed depressive or anxiety disorders, loneliness as a primary outcome, long-term cognitive trajectories, and dementia incidence. Tai Chi should therefore be positioned as an adjunct to established care, not as a replacement for pharmacotherapy, psychotherapy, cognitive behavioral therapy for insomnia (CBT-I), or multidomain dementia-risk reduction. Proposed pathways include autonomic regulation, sleep stabilisation, behavioral activation, balance confidence, cognitive–motor integration, and group participation; however, most mechanism evidence is indirect or exploratory. Translation requires clearly specified protocols, active comparators, blinded outcome assessments where feasible, long-term follow-up, systematic adverse-event reporting, cost-effectiveness analyses, and equity-focused implementation. Tai Chi is the most credible as an adaptable component of stepped or integrated care for older adults, provided that programmes are standardised, safely delivered, and evaluated against clinically significant outcomes.

Cognitive behavioral therapy for insomnia (CBTI) is a first-line treatment, but the quality and engagement of CBTI information on short-video platforms remain uncertain. We evaluated 293 videos from Douyin, Kwai, RedNote, and Bilibili using the Global Quality Scale, modified DISCERN, Video Information Quality Instrument, Journal of the American Medical Association (JAMA) benchmarks, and a Composite Quality Index (CQI). The primary outcome was ln(1 + weighted engagement per publication day). Multivariable models adjusted for publication days, follower count, video length, and author identity using HC3 robust inference. Bilibili had the highest mean JAMA and Global Quality Scale scores, whereas Douyin had the highest median weighted engagement per day. A 10-point CQI increase was positively associated with engagement on Douyin (β = 0.469, 95% CI [0.107,0.831], P = 0.011). However, the global CQI-by-platform interaction was nonsignificant ( P = 0.067), providing insufficient evidence that the association differed across platforms. JAMA slopes and the global interaction were nonsignificant. Alternative score weightings produced similar secondary unadjusted findings, and view-normalized CQI associations were nonsignificant in Kwai and Bilibili. The platforms differed in information transparency, quality, and engagement, but no platform performed best across all dimensions. Popularity should not be considered a proxy for CBTI information quality.

(Journal of the American Medical Association - JAMA)
