Beyond Poor Sleep: Behavioral Correlates of Excessive Daytime Sleepiness and Academic Performance in Undergraduate Medical Students from Southern India: A Cross-sectional Study

Preethi Ashok Masali; Siddalingaiah HS Indian Journal of Sleep Medicine, 2026, Vol. 21, Issue 4, pp. 143-151 https://doi.org/10.5005/jp-journals-10069-0205

Aim and background: Excessive daytime sleepiness (EDS) is reported far less often than poor nocturnal sleep quality in Indian medical students. We aimed to estimate EDS prevalence and its independent correlates (primary), and to compare these with poor sleep quality correlates in the same cohort and relate both to examination performance (secondary). We hypothesized that sleep hygiene behavior, not global sleep quality category, would dominate.Materials and methods: Prospective cross-sectional study of 354 of 450 eligible MBBS students (78.7%) at a Southern Indian college, sampled by stratified proportional randomization.Under supervised standardized administration, they completed the Epworth Sleepiness Scale (ESS; EDS >10), Pittsburgh Sleep Quality Index (PSQI), and Sleep Hygiene Index (SHI), with screen time, stress, mood, lifestyle, clinical covariates, and the latest university examination percentage.Excessive daytime sleepiness correlates were examined by logistic regression with a pre-specified confounder-adjusted sensitivity model. Results: Excessive daytime sleepiness affected 21.5% [95% confidence interval (CI) 17.5-26.0],more often among poor than good sleepers (27.7% vs 18.6%).Adjusted [area under the receiver operating characteristic curve (AUC) 0.773, 0.714-0.832],only poorer sleep hygiene remained independent [odds ratio (OR) 1.12 per point, 1.07-1.18],unchanged after adjustment for all measured confounders (1.12, 1.06-1.17).Examination percentage did not differ by sleep status (67.33 ± 5.16 vs 68.07 ± 5.72; p = 0.229); academic stage and sex, not sleep, predicted marks (R² = 0.152).Conclusion: In this cross-sectional sample, EDS was associated more closely with sleep hygiene behavior than with global sleep quality category, and the two had distinct correlation profiles.Screening for sleepiness alongside sleep quality, and addressing bedtime device use, merit consideration in student health services. These are associations only; no direction of effect can be inferred.Clinical significance: Measuring EDS alongside global sleep quality identifies a partly distinct at-risk group, for whom brief counselling on bedtime device use may be more specific than generic sleep hygiene advice.

Assessing growth mindset in undergraduate medical students: translation, cross-cultural adaptation, and evaluation of the Chinese version of the implicit theories of intelligence scale

Wei Hong; Gang Chen; Chan Choong Foong Frontiers in Medicine, 2026, Vol. 13, pp. 1969574 https://doi.org/10.3389/fmed.2026.1969574

Objectives Cross-cultural equivalence translations of the widely used Implicit Theories of Intelligence Scale (ITIS) into Chinese are limited. Past research has focused on Western educational contexts, and Eastern cultural perspectives have been overlooked. This study aimed to translate and validate the Chinese version of the ITIS, and assess the growth mindset in undergraduate medical students. Methods This study was conducted among final-year undergraduate medical students in Fujian, China. Following forward and back-translation of the Implicit Theories of Intelligence Scale (ITIS), cognitive interviews were conducted with students to assess the clarity and comprehensibility of the translated items, followed by an online survey distributed through medical schools. Construct validity, convergent validity, and discriminant validity were evaluated using confirmatory factor analysis. Internal consistency was assessed using Cronbach's alpha and composite reliability (CR), while test–retest reliability was evaluated separately for the Fixed Mindset and Growth Mindset dimensions using the intraclass correlation coefficient (ICC). Results A total of 444 responses were obtained. Confirmatory factor analysis supported a two-factor structure with satisfactory model fit ( χ 2 = 52.293, df = 17, CMIN/DF = 3.076, CFI = 0.984, NFI = 0.977, RFI = 0.962, IFI = 0.984, TLI = 0.974, RMSEA = 0.068, SRMR = 0.059). Standardized factor loadings were satisfactory, while CR values were 0.908 for Fixed Mindset and 0.910 for Growth Mindset, and AVE values were 0.713 and 0.717, respectively, supporting convergent validity. Discriminant validity was supported by the Fornell–Larcker criterion, AVE values exceeding the corresponding maximum shared variance values, and HTMT ratios below 0.90. Test–retest reliability differed between the two dimensions: the Fixed Mindset dimension demonstrated moderate temporal stability (ICC = 0.662, 95% CI: 0.443–0.806), whereas the Growth Mindset dimension showed limited temporal stability (ICC = 0.159, 95% CI: −0.158–0.446). The mean scores for Fixed Mindset items ranged from 2.64 (SD = 1.09) to 2.86 (SD = 1.56), whereas those for Growth Mindset items ranged from 3.81 (SD = 1.16) to 3.96 (SD = 1.21), indicating relatively stronger endorsement of fixed-mindset beliefs in this sample. Conclusions The Chinese version of the ITIS is a valid and reliable instrument for assessing the intelligence mindset of medical students.

Rohan Mishra; Rounak Mandal; Ruhan Rana Hossein; Saed Hamza Molla; Sahanaj Parvin; Sahid Anowar; … The ESRF Research Journal for Undergraduate Medical Students, 2026, Vol. 4, Issue 1 https://doi.org/10.5281/zenodo.23368561

Abstract Introduction: Sleep disturbance is common among hospitalized patients and may adversely affect recovery and well-being. This study aimed to determine the proportion of patients hospitalized for at least 3 days, with unsatisfactory sleep, describe perceived sleep disruptors, and assess their association with socio-demographic, clinical and hospital-related factors. Materials and Methods: A hospital-based descriptive cross-sectional study was conducted among patients admitted to five randomly selected in-patient departments of a tertiary medical college, Kolkata, from 10 April to 20 May 2026. Eligible patients admitted for at least three days were recruited. Data were collected through face-to-face interviews using a pre-designed, pre-tested schedule. Perceived sleep quality and perceived sleep disruptors were assessed using 5-point Likert scales. Participants reporting fair, poor or very poor sleep were categorised as having unsatisfactory sleep. Associations were assessed using Chi-square/Fisher's exact test and Mann–Whitney U test, with p<0.05 considered significant. Results: A total of 183 patients were included in the final analysis. The median age was 40 years (IQR: 26–46.5), 50.8% were male, and median length of stay was 7 days (IQR: 4–13.5). Unsatisfactory sleep was reported by 101 (55.2%) participants, including 36 (19.7%) with fair, 39 (21.3%) with poor and 26 (14.2%) with very poor sleep. Acute medical conditions related to the current admission were the most commonly reported significant disruptor of sleep (77, 42.1%), followed by pre-existing medical conditions (45, 24.6%), noise (40, 21.9%) and light (38, 20.8%). Unsatisfactory sleep was significantly associated with perceived disruption from noise (p<0.001), light (p=0.006), frequent vital-sign monitoring (p=0.016), personal care routines (p=0.037), existing medical conditions (p=0.033), and acute medical conditions (p=0.010). Age, sex, department of admission, diagnosis and length of stay showed no significant association with sleep quality. Conclusion: More than half of hospitalized patients experienced unsatisfactory sleep. Clinical conditions and modifiable hospital-related factors, particularly noise, light and interruptions related to routine care, were associated with poorer sleep. Measures to minimize unnecessary nighttime disturbances may improve sleep among hospitalized patients.

Rethinking How National Health Programmes Are Taught: Team-Based Decision Learning Versus Mini-Interactive Lectures Among Undergraduate Medical Students

Minu Mohan; Diya T Jose; Binu Areekal Cureus, 2026, pp. e117822 https://doi.org/10.7759/cureus.117822

Purpose: To compare the learning outcomes, learner engagement, and student satisfaction associated with team-based decision learning (TBDL), an adaptation of team-based learning, and small group discussion supplemented by a mini-interactive lecture (SGD-MIL) for teaching National Health Programmes to Phase III Bachelor of Medicine and Bachelor of Surgery (MBBS) students. Methods: A non-randomised quasi-experimental crossover study was conducted among 56 Phase III MBBS students at Government Medical College, Ernakulam, India across six National Health Programme topics.Students received TBDL for three topics and SGD-MIL for three other topics, with the teaching method reversed in the other posting batch.Immediate post-test scores were analysed topic-wise, and a linear mixed-effects model was used to estimate the overall effect of teaching method while accounting for differences across topics and repeated observations within students; teaching method and topic were included as fixed effects, with student specified as a random intercept.Short-term retention, learner engagement, and satisfaction were also assessed. Results: TBDL produced higher immediate post-test scores for the National Tuberculosis Elimination Programme (NTEP), National AIDS Control Programme (NACP), and Reproductive and Child Health (RCH), but not for the National Programme for Prevention and Control of Non-Communicable Diseases (NP-NCD), National Vector Borne Disease Control Programme on Malaria (NVBDCP -Malaria), or National Vector Borne Disease Control Programme on Vector Borne Diseases Other Than Malaria (NVBDCP -Other VBDs).In a mixed-effects model using 296 scores from 55 students, the adjusted mean was 1.07 points higher with TBDL (95% CI: 0.53-1.61;P < 0.001), with a significant teaching method × topic interaction (P < 0.001).Short-term retention did not differ (P = 0.721), while TBDL was associated with higher engagement across all five domains. Conclusion: TBDL improved immediate learning for selected National Health Programme topics and consistently enhanced learner engagement, while short-term retention was comparable between methods. These findings support TBDL as a complementary, decision-oriented strategy for selected community medicine topics, particularly where learners must apply programme knowledge to realistic implementation decisions.

Psychiatry Clerkship Exposure and Mental Illness Stigma in Medical Students: Public Stigma Endorsement and Anticipated Stigma with Concealment/Avoidance

Fatih Ekici; Muhammet Cahit Atılgan; Samet Kaya; Mustafa Esad Tezcan; Yavuz Selvı Genel Tıp Dergisi, 2026, Vol. 36, Issue 2026, pp. 1-8 https://doi.org/10.54005/geneltip.1893733

Aim: Stigma related to mental illness remains a major barrier to help-seeking and equitable care. Among medical students, stigma is clinically relevant because it may influence both future patient management and students’ own willingness to seek professional support. This study examined two complementary stigma domains, public stigma endorsement and anticipated stigma with concealment/avoidance, and their association with psychiatry clerkship exposure.Materials and Methods: This cross-sectional study included 354 undergraduate medical students at Selçuk University Faculty of Medicine. Psychiatry clerkship exposure was defined as pre-clerkship (Years 1–4) versus clerkship (Years 5–6). Stigma-related constructs were measured using a 29-item researcher-developed questionnaire organized a priori into Public Stigma Endorsement (PSE) and Anticipated Stigma with Concealment/Avoidance (ASA). Internal consistency was high (Cronbach’s α = 0.884 for PSE; 0.900 for ASA). Group differences were evaluated using appropriate univariable tests. Separate multiple linear regression models were fitted for PSE and ASA, adjusting for sociodemographic characteristics, smoking, psychiatric service use, contact with mental illness (self/family/social environment), family history of stigma, and stigma awareness/knowledge.Results: PSE scores did not differ by clerkship exposure in univariable analyses and were not independently associated with clerkship status in adjusted models. Male sex was an independent correlate of higher PSE. In contrast, ASA was higher among clerkship students and remained independently associated with clerkship exposure after adjustment (B = 2.75, 95% CI 0.44–5.05, p = 0.020). Middle socioeconomic status (vs low) was also associated with higher ASA (B = 3.37, 95% CI 0.67–6.07, p = 0.014). Model fit was significant for PSE and borderline for ASA.Conclusion: Psychiatry clerkship exposure was not associated with reduced public stigma endorsement but was linked to higher anticipated stigma and concealment/avoidance. These findings underscore the need for educational strategies that explicitly target distinct stigma dimensions, including self-referential anticipated stigma that may impede help-seeking among medical trainees.

Dose–response relationship between khat chewing frequency and sleep duration, perceived effects, and academic performance among medical students in Yemen

Abdalkhaliq Ahmad Obadi; Abdulelah H. Al‐Adhroey; Mohammed Abdullah Al Amad; Hamed Abdullah Salman; Ahmed Mohammed Alsaedi; Asad Ali Al-Yamani; … Scientific Reports, 2026 https://doi.org/10.1038/s41598-026-72988-9

Although khat chewing is common among university students in Yemen, evidence on dose–response relationships between khat chewing frequency and sleep duration, perceived effects, and academic performance remains limited. This study examined these associations among medical students at Thamar University, Yemen. A cross-sectional study was conducted among undergraduate medical students. Data were collected using a structured questionnaire. Khat chewing frequency was categorized as non-use, 1–2 days/week, 3–4 days/week, and 5–7 days/week. Multivariable logistic and linear regression analyses were performed to assess associations, and ordinal trend analyses were used to evaluate dose–response relationships. Adjusted odds ratios (AOR) and regression coefficients (β) with 95% confidence intervals (CI) were reported. Statistical significance was set at p < 0.05. Increasing khat chewing frequency was independently associated with higher odds of short sleep duration (< 6 h). Compared with non-chewers, the adjusted odds of short sleep increased progressively, reaching an AOR of 7.83 (95% CI: 3.70–16.57; P < 0.001) among daily chewers.A significant linear dose–response relationship was also observed (AOR = 1.97, 95% CI: 1.54–2.52; P < 0.001). Perceived-effects scores increased progressively with khat-chewing frequency, with adjusted mean differences of 3.36, 3.73, and 4.94 for 1–2, 3–4, and 5–7 days/week, respectively (all P ≤ 0.001), with a significant linear trend (adjusted β = 1.61, 95% CI: 1.29–1.92; P < 0.001). In contrast, academic performance showed no significant association or linear dose–response relationship with khat chewing (adjusted β = 0.22, 95% CI: −0.46–0.89; P = 0.532). Khat chewing frequency showed clear dose–response associations with short sleep duration and perceived effects among medical students. However, these associations did not translate into measurable differences in academic performance. These findings suggest that the effects of frequent khat use are more evident in sleep and perceived behavioural outcomes than in academic achievement. These findings are consistent with known stimulant-related effects and may extend to similar student populations in comparable settings.
