Probable Rapid Eye Movement Sleep Behavior Disorder Associated With Escitalopram in a Patient With Schizoaffective Disorder: A Case Report

Moza K Bin Kamil AlShamsi; Mehnaz Ali; Hajar Medhat Youssef Cureus, 2026 https://doi.org/10.7759/cureus.118869

Rapid eye movement (REM) sleep behavior disorder (RBD) is a parasomnia in which the normal muscle atonia of REM sleep is lost, and patients act out their dreams.Serotonergic antidepressants such as selective serotonin reuptake inhibitors (SSRIs) are a recognized trigger, but in psychiatric patients the nocturnal behaviors are easily attributed to stress or to the primary illness. We report the case of a 46-year-old man with schizoaffective disorder, depressive type, maintained on aripiprazole 15 mg daily, who was started on escitalopram 10 mg at bedtime (day one); the dose was reduced to 5 mg on day 77.Around day 430, about 14 months after starting escitalopram and while taking 5 mg, he began to walk in his sleep and act out his dreams, including punching movements and, on one occasion, walking while holding a clothing iron, with no recollection on waking. The episodes occurred late in the night, with his eyes closed, two to three times a month and were accompanied by sleep paralysis and daytime sleepiness. His mental status examination and routine blood tests were unremarkable. He declined polysomnography and brain imaging, so a diagnosis of probable RBD, possibly associated with escitalopram, was made on clinical grounds; a non-REM parasomnia could not be excluded. On day 616, escitalopram was stopped and clonazepam 0.5 mg at bedtime was started at the same time. The episodes had stopped by day 665.Clonazepam was discontinued on day 772, and at the latest follow-up on day 1962 there had been no recurrence.Because clonazepam was started when escitalopram was withdrawn, the improvement cannot be attributed to stopping escitalopram alone, although the absence of recurrence after clonazepam was discontinued supports a role for the antidepressant.Clinicians who prescribe serotonergic antidepressants should take a sleep history from patients and their bed partners, arrange polysomnography where it is available, and review the suspected drug.

Processing event nominalizations in developing readers: eye-tracking evidence from bilingual and dyslexic readers

Maria Vender; Ilaria Venagli; Andrea Nardon; Sabrina Piccinin; Serena Dal maso; Chiara Melloni Frontiers in Language Sciences, 2026, Vol. 5, pp. 1912839 https://doi.org/10.3389/flang.2026.1912839

Introduction Event nominalizations are a central feature of academic language: they compress complex events into nominal structures, increasing informational density while reducing the transparency of thematic-role assignment. The present eye-tracking study examined whether nominalizations impose greater processing demands than corresponding active and passive constructions in developing readers, and whether these effects vary across readers with different linguistic and literacy profiles. Methods Seventy-nine fifth- and sixth-grade children participated in the study: 28 typically developing monolingual Italian children, 21 monolingual Italian children with developmental dyslexia, and 30 typically developing bilingual children with migrant background and Italian as the societal language. Participants silently read short passages in which the same target event was expressed either as a nominalization, a passive construction, or an active construction, and then answered comprehension questions while their eye movements were recorded. Accuracy, standard eye-movement measures, and scanpaths were analyzed to capture comprehension outcomes and online reading strategies. Results Nominalizations did not significantly reduce comprehension accuracy, but they affected online processing during both initial reading and the question-answering phase. Scanpath dissimilarity was higher for nominalizations than for both verbal constructions during initial reading, and than for passives during question-answering in correctly answered trials. In these trials, nominalizations were also associated with a higher probability of partial rescanning and a lower probability of minimal rescanning relative to active texts, indicating greater reliance on rereading when answering questions correctly. Moreover, children with dyslexia showed a generalized increase in processing effort, whereas bilingual children displayed an intermediate profile, with increased reliance on rereading during question-answering. Discussion These findings indicate that nominalizations significantly influence online reading processes even when questions are answered correctly, suggesting additional demands in retrieving information about event participants. Overall processing patterns differed across reader groups, and children with dyslexia showed the greatest effort in both online reading and question-answering.

Driver Responses to Mismatches Between Expected and Experienced Automated Vehicle Driving Styles

M. J. Lee; Brandon J. Pitts International Journal of Human-Computer Interaction, 2026, pp. 1-25 https://doi.org/10.1080/10447318.2026.2739750

Automated vehicles (AV) can exhibit diverse driving behaviors that deviate from driver expectations. More research is needed to understand drivers’ preferred driving styles when navigating safety-critical situations as well as their responses to unexpected AV driving behaviors. Forty-eight participants experienced simulated automated driving scenarios where an AV navigated a construction zone while exhibiting four different predefined driving styles that varied in terms of cautiousness and efficiency. After each trial, participants provided feedback to adjust the AV’s driving style toward their preferences, which was successfully implemented in 80% cases. Drivers’ trust, workload, voluntary takeover decisions, eye movements, and skin conductance were measured. Overall, participants preferred driving styles balancing cautiousness and efficiency. When AV behavior matched driver preferences, they reported higher trust and more stable visual scanning patterns, while unexpected behaviors were associated with broader ocular scanning. Findings can inform the development of adaptive systems that intelligently modify AV behaviors.

Eye Movement Desensitisation and Reprocessing (EMDR) for ICU-related psychological distress among adult intensive care survivors: a scoping review

Shamini Satkunam; Irene Dimitriou; Madiha Shaikh European Journal of Psychotraumatology, 2026, Vol. 17, Issue 1, pp. 2732465 https://doi.org/10.1080/20008066.2026.2732465

Background: Psychological morbidity is common following admission to adult intensive care units (ICUs), with many patients experiencing post-traumatic stress disorder (PTSD), anxiety, and depression as part of post-intensive care syndrome. Eye Movement Desensitisation and Reprocessing (EMDR) is an established trauma-focused psychological therapy; however, its evidence base within critical care populations remains emerging. Objectives: This scoping review aimed to map the extent, characteristics, and findings of the available literature on the use of EMDR for ICU-related psychological distress. Methods: A scoping review was conducted in accordance with Joanna Briggs Institute methodology and reported following the PRISMA-ScR guidelines. The review aimed to systematically identify and synthesise evidence relating to the application and reported outcomes of EMDR in ICU survivors. Results: Seven publications representing six independent study populations were included. Participants included adult ICU patients experiencing PTSD symptoms, delirium-related psychological disturbances, and COVID-19-related distress. EMDR was delivered using either standard protocols or the Recent Traumatic Episode Protocol (R-TEP). Across the evidence base, EMDR was associated with reductions in trauma-related distress, although evidence for improvements in anxiety and depression was less consistent. Where qualitative data were available, participants additionally reported improved integration and processing of ICU experiences. Overall, methodological heterogeneity, overlapping samples, small sample sizes, and limited controlled evidence restrict the strength of conclusions that can be drawn. Conclusions: This review highlights growing but preliminary evidence supporting the feasibility and potential utility of EMDR, including R-TEP, for ICU-related psychological distress. While findings are promising, the current evidence base remains insufficient to determine effectiveness. Future research should prioritise adequately powered controlled studies, standardised outcome measures, longer-term follow-up, and more diverse ICU populations to strengthen the evidence base and inform clinical implementation.

Systematic review and meta-analysis: the effectiveness of cognitive behavioral therapy and eye movement desensitization and reprocessing in children and adolescents

Irada Meoon; Aungsana Khlaisuk; Siriyupa Nansunanon; Teerapon Dhippayom European Journal of Psychotraumatology, 2026, Vol. 17, Issue 1, pp. 2731855 https://doi.org/10.1080/20008066.2026.2731855

Background: Post-traumatic stress disorder (PTSD) is prevalent among children and adolescents, with cognitive behavioral therapy (CBT) and eye movement desensitization and reprocessing (EMDR) widely used treatment options. This study compared the effects of CBT and EMDR in children and adolescents using a meta-analysis approach. Methods: PubMed, EMBASE, CENTRAL, CINAHL, ProQuest Dissertations & Theses, and PsycInfo were searched in March 2025. Randomized controlled trials that compared the effectiveness of CBT and EMDR on PTSD symptoms in children and adolescents aged 6–18 years who had been exposed to traumatic events were included. The Cochrane Risk of Bias version 2 was used to assess the methodological quality of the included studies. A random-effects model was applied to calculate the standardized mean difference (SMD) along with 95% confidence interval (CI), and the I2 statistic was used to quantify statistical heterogeneity. Results: Five trials (248 participants) were included. There was no statistically significant difference in PTSD symptoms between the CBT and EMDR groups, whether reported by children (SMD: 0.18; 95% CI: –0.07–0.43; I2 = 0%) or by parents (SMD: 0.23; 95% CI: –0.02–0.48; I2 = 0%). For anxiety, the pooled estimate did not reach statistical significance (SMD: 0.44; 95% CI: –0.19–1.07; I2 = 69%). Depression symptoms were higher in the CBT group than in the EMDR group (SMD: 0.44; 95% CI: 0.13–0.76; I2 = 0%).Conclusion: The comparative effects of CBT and EMDR in reducing PTSD and anxiety symptoms remain inconclusive. Although CBT was associated with higher depression symptoms compared with EMDR, the current evidence is based on a small number of trials, and the pooled estimates should be interpreted with caution.

The Role of Emotional Regulation Mechanisms and Early Trauma in the Development of Personality Disorders

Valbone Ramaj; Donard Ramaj North American journal of psychology, 2026, Vol. 28, Issue 4 https://doi.org/10.65696/001c.171643

Impairments in emotional regulation and adverse experiences in early childhood are considered key factors in the development of personality disorders. However, existing theoretical approaches remain fragmented, failing to provide an interdisciplinary synthesis of the neurobiological, psychological, and interpersonal mechanisms within a single explanatory framework. This study aimed to reveal the relationship between deficits in affective regulation and early traumatisation as mutually reinforcing factors in the development of personality pathology, substantiating this relationship at multiple levels. This narrative review with elements of conceptual synthesis is based on 41 sources from PubMed, Scopus, Web of Science, and Google Scholar covering the period from 2020 to 2025. These sources include meta-analyses, longitudinal cohort studies, randomised controlled trials, and qualitative investigations from Kosovo, Norway, Lebanon, Canada, and other countries. According to the analysed studies, borderline personality disorder is characterised by limbic hyperreactivity associated with emotional abuse, with an odds ratio=11.02; conversely, antisocial personality disorder is characterised by prefrontal hypoactivation and a blunted cortisol response. The reviewed sources indicate that epigenetic modifications of the FK506 binding protein 5 (FKBP5) gene and accelerated biological ageing according to the GrimAge clock (β=0.12; p=0.03) constitute components of a cascading molecular mechanism linking early trauma with neurostructural changes. The literature analysis showed that emotional dysregulation forms a continuum ranging from affective overreactivity (cluster B) and overcontrol (cluster C) to incongruence (cluster A). Mentalisation deficits, self-compassion, and social support were described as mediators of the ‘trauma-personality pathology’ relationship. According to the results of meta-analyses, trauma-informed dialectical behaviour therapy reduces symptoms of borderline personality disorder with an effect size of g=-0.82. Group schema therapy demonstrates an effect size of g=0.859, and eye movement desensitisation shows d=0.65 for psychological symptoms. The four-level substantiation – carried out at molecular-neurobiological, developmental-interpersonal, psychological, and clinical-phenomenological levels – reveals the basis for a stratified approach.
