Purpose: This study aimed to compare the effectiveness of Compassion-Focused Acceptance and Commitment Therapy and Schema-Integrated Acceptance and Commitment Therapy on body image and sexual relationship quality in women with breast cancer in Hamadan. Methods and Materials: This applied quasi-experimental study employed a pretest–posttest design with a two-month follow-up, two experimental groups, and one active control group. The statistical population consisted of women with breast cancer attending the Health Donors Association of Hamadan Province during 2025–2026. Sixty eligible women were selected through convenience sampling and randomly assigned to three groups of 20 participants. Following attrition, the final sample comprised 50 women, including 17 in the Compassion-Focused Acceptance and Commitment Therapy group, 16 in the Schema-Integrated Acceptance and Commitment Therapy group, and 17 in the control group. The experimental groups received 10 weekly 90-minute group sessions of their respective interventions, while the control group received five weekly 60-minute health and life-skills education sessions. Data were collected using the Objectified Body Consciousness Scale and the Sexual Quality of Life–Female Questionnaire. Data were analyzed in SPSS version 26 using multivariate and univariate analyses of covariance and Bonferroni-adjusted pairwise comparisons. Findings: Multivariate analysis showed a significant overall effect of treatment condition on the combined dependent variables, Pillai’s Trace = 0.592, F(4, 90) = 9.468, p < 0.001, partial η² = 0.296. Significant group effects were found for body image, F(2, 45) = 20.691, p < 0.001, partial η² = 0.482, and sexual relationship quality, F(2, 45) = 16.250, p < 0.001, partial η² = 0.419. Bonferroni comparisons indicated that both interventions were significantly more effective than the control condition on both outcomes. Compassion-Focused Acceptance and Commitment Therapy was also significantly more effective than Schema-Integrated Acceptance and Commitment Therapy for body image, p = 0.006, and sexual relationship quality, p = 0.031. Conclusion: Both interventions were effective in improving body image and sexual relationship quality among women with breast cancer, although Compassion-Focused Acceptance and Commitment Therapy demonstrated greater effectiveness and may therefore be particularly useful in psycho-oncological rehabilitation following breast cancer treatment.
Purpose: This study aimed to determine the effectiveness of Acceptance and Commitment Therapy (ACT) in improving the psychological well-being of patients diagnosed with cancer. Materials and Methods: This quasi-experimental study employed a pretest–posttest design with a control group and a follow-up assessment. The statistical sample consisted of 60 patients with cancer who were selected through purposive sampling and assigned to three groups of 20 participants. Participants in the intervention groups received an ACT-based intervention consisting of eight 45-minute sessions delivered over one month, whereas the control group received no psychological intervention during the study period. Psychological well-being was assessed using Ryff’s 84-item Psychological Well-Being Scale (1989) at the pretest, posttest, and follow-up stages. The collected data were analyzed using inferential statistical procedures in SPSS version 25. Findings: The inferential analysis demonstrated a statistically significant difference between the intervention and control groups in posttest psychological well-being scores. Participants who received Acceptance and Commitment Therapy showed significantly higher levels of psychological well-being than those in the control group. The intervention produced a very large effect size (η² = 0.935), indicating that a substantial proportion of the variance in post-intervention psychological well-being was attributable to the therapeutic intervention. The follow-up analysis further showed that the improvement in psychological well-being was maintained after the completion of treatment, suggesting the stability and durability of the intervention effects over time. Conclusion: Acceptance and Commitment Therapy is an effective psychological intervention for enhancing psychological well-being among patients with cancer. By promoting psychological flexibility, acceptance of difficult internal experiences, present-moment awareness, and engagement in value-oriented behaviors, ACT may help patients cope more adaptively with the psychological consequences of cancer. Therefore, this intervention may be incorporated into psychosocial rehabilitation and supportive care programs for patients with cancer.
Abstract Background Repetitive negative thinking (RNT) is a transdiagnostic process relevant to common mental health problems in primary care. RNT-ACT is a brief acceptance and commitment therapy-based protocol targeting RNT in two sessions. This pilot study assessed the feasibility of RNT-ACT compared with therapist-supported internet-based cognitive behavioural therapy (iCBT) in Swedish primary care. Methods Thirteen participants were randomised: 7 to RNT-ACT and 6 to iCBT. The primary feasibility outcomes were recruitment rate, intervention non-completion, and patient satisfaction measured with the Client Satisfaction Questionnaire (CSQ-8). Symptom and process measures were secondary outcomes. Secondary outcomes were assessed at baseline and at 1- and 3-month follow-ups. Analyses were descriptive and exploratory, based on available observations without imputation of missing data. Results Thirteen of 20 eligible individuals were enrolled (65%), corresponding to 35% of all 37 individuals screened. All participants in the RNT-ACT group completed the intervention, whereas 4 of 6 participants in the iCBT group did not complete the intervention. Mean CSQ-8 scores were 27.8 (SD 3.7; n = 5) in the RNT-ACT group and 23.0 (SD 0.0; n = 2) in the iCBT group. Conclusions The small sample size and high iCBT non-completion limit between-group comparisons. Further investigation of recruitment, treatment engagement, and patient and therapist experiences is needed before determining whether and how to proceed to a full-scale RCT. Trial registration ClinicalTrials.gov NCT05946382 registered July 14, 2023.
Background Stroke is a leading cause of disability worldwide and is frequently accompanied by psychological and functional impairments. Acceptance and Commitment Therapy (ACT), a third-wave behavioral intervention focusing on psychological flexibility, has been increasingly applied in stroke rehabilitation. Although previous reviews have suggested beneficial effects of ACT in stroke patients, whether these benefits persist in the short term after the intervention ends, and the certainty of the evidence, remain unclear. Methods A systematic review and meta-analysis was conducted according to PRISMA guidelines and registered in PROSPERO (CRD420261325108). Randomized controlled trials evaluating ACT in stroke patients were included. Pooled effects were calculated using random-effects models, with separate analyses for post-intervention and follow-up outcomes. Sources of heterogeneity were explored using subgroup analyses and meta-regression. Risk of bias was assessed using the Cochrane Risk of Bias 2 tool, and certainty of evidence was evaluated using the GRADE framework. Results Twenty-two RCTs involving 1,975 participants were included. ACT may be associated with reductions in depressive symptoms (post-intervention: SMD = −1.32, 95% CI: −1.80 to −0.84; follow-up: SMD = −0.76, 95% CI: −1.07 to −0.45) and anxiety symptoms (post-intervention: SMD = −1.36, 95% CI: −1.90 to −0.82; follow-up: SMD = −0.51, 95% CI: −1.42 to 0.40, not significant); depressive symptoms remained statistically significant at follow-up. Heterogeneity was considerable for both outcomes at post-intervention ( I 2 = 93.68% for depression and 93.60% for anxiety), so the pooled estimates represent average effects across studies. ACT may also be associated with improvements in psychological flexibility, quality of life, and sleep quality; however, these outcomes were based on a limited number of studies and showed considerable heterogeneity ( I 2 = 74.68 to 92.26%), so they should be interpreted cautiously. However, no statistically significant effects were observed for activities of daily living, neurological function, or hope. ACT did not appear to increase dropout rates in the available studies ( K = 16; RR = 0.99, 95% CI: 0.58–1.69). Meta-regression analyses indicated that intervention duration may not be a significant moderator of treatment effects. The certainty of evidence ranged from very low to moderate, mainly due to risk of bias and inconsistency. Conclusion Acceptance and Commitment Therapy may be a useful adjunctive intervention for improving psychological outcomes in people with stroke. However, evidence for functional recovery remains insufficient. Future high-quality RCTs with standardized protocols, larger samples, and longer follow-up are needed to confirm the long-term effectiveness of ACT. This study aimed to develop a Japanese version of the Sleep Problem Acceptance Questionnaire (SPAQ-J) and assess its reliability and validity. The SPAQ-J was developed through a forward-backward translation procedure with permission from the original authors. The sample included 861 adults with insomnia who completed questionnaires on experiential avoidance, insomnia symptoms, dysfunctional beliefs about sleep, sleep-related safety behaviors, insomnia catastrophizing, and pre-sleep arousal. Confirmatory factor analysis confirmed that the SPAQ-J retained the two-factor structure of the original version (CFI = 0.972, TLI = 0.958, RMSEA = 0.082, SRMR = 0.059): Activity Engagement and Willingness. Although internal consistency was adequate (Activity Engagement: α = 0.868, ω = 0.870; Willingness: α = 0.895, ω = 0.895), test-retest reliability was below the recommended threshold (Activity Engagement: ICC = 0.586, Willingness: ICC = 0.589). In terms of construct validity, Willingness moderately correlated with all scales, whereas Activity Engagement showed moderate-to-weak correlations. Hierarchical multiple regression analysis revealed that adding the SPAQ-J subscales significantly enhanced the explanatory power. Measurement invariance analysis across sex and age groups showed no significant changes in the fit indices, indicating stability. These findings suggest that the SPAQ-J is a valid and partially reliable measure of psychological flexibility related to sleep problems in the Japanese population.
Psychological flexibility and inflexibility are core processes in the Acceptance and Commitment Therapy (ACT) model and are consistently associated with well-being and emotional distress. However, the validity of widely used measures, particularly the Acceptance and Action Questionnaire – II (AAQ-II), has been questioned due to potential overlap with indicators of distress and neuroticism. This study examined the psychometric properties of the Acceptance and Action Questionnaire – III (AAQ-III) and the PsyFlex in a general population sample from Colombia ( N = 785) using network psychometrics. We estimated the dimensionality of each scale via Exploratory Graph Analysis (EGA) and evaluated replicability via bootstrap EGA (bootEGA). Discriminant validity was examined using a joint EGA that included the AAQ-III, PsyFlex, and measures of emotional distress (DASS-21) and neuroticism (BFI-2-Neuroticism). Convergent validity was assessed through correlations with the Multidimensional Psychological Flexibility Inventory – Short Form. EGA identified unidimensional structures for both instruments, and bootEGA confirmed perfect structural consistency and item stability. The joint EGA yielded five distinct communities, with the AAQ-III and PsyFlex forming separate communities from emotional symptoms and neuroticism, providing structural evidence consistent with the discriminant validity of both measures. Both instruments showed the expected patterns of correlations with flexibility and inflexibility processes. These findings provide initial validation evidence that the Spanish versions of the AAQ-III and PsyFlex are psychometrically sound measures of psychological inflexibility and flexibility.
To synthesise the efficacy of digital third-wave process-based interventions (e.g., ACT, Mindfulness, Compassion) for improving wellbeing and ameliorating mental health symptoms in adults. A prospectively registered systematic review (OSF: 10.17605/OSF.IO/5RMU2 ) and random-effects meta-analysis of randomised controlled trials (RCTs) was conducted across six databases through January 2026. Certainty of evidence was assessed using the GRADE framework. Interventions ( k = 147; N = 26,391) yielded small-to-moderate immediate post-intervention improvements in wellbeing ( k = 46, g = 0.36), depression ( k = 110, g = 0.44), anxiety ( k = 93, g = 0.40), stress ( k = 66, g = 0.46), quality of life ( k = 32, g = 0.30), and psychological flexibility ( k = 51, g = 0.41). Heterogeneity was substantial across most outcomes ( I2 > 75%). Benefits were observed across healthy (61 trials; n = 15,770), mental health (41 trials; n = 6,130), and medical samples (46 trials; n = 4,491), with population type not significantly moderating immediate post-intervention effects. Comparator type significantly moderated several analyses, with smaller effects against active than inactive controls. Guidance did not significantly moderate immediate post-intervention effects, although it predicted larger short-term depression effects ( QM = 4.17, p = .048). At short-term follow-up, effects remained statistically supported across all outcomes, while only depression, anxiety, and stress remained statistically supported at medium-term follow-up. GRADE certainty was moderate for most immediate post-intervention outcomes and low for quality of life. Digital third-wave interventions produced small-to-moderate average benefits across symptom-focused, wellbeing-focused, and process outcomes. Effects were smaller against active comparators, indicating that pooled benefits should be interpreted in relation to comparator intensity. Future research should prioritise rigorous active controls, longer follow-up, and direct tests of whether changes in psychological flexibility mediate improvements in symptoms and wellbeing.
Background: This study examined the characteristics and recovery outcomes of acceptance-based interventions for individuals with trauma experience in Korea, using the psychological flexibility model as an analytic framework. Methods: Five Korean databases were searched, yielding 2,156 records; from these, fifteen intervention studies were selected. Intervention content was rated for each of the six processes and summarized using the triflex categories of open, aware, and active. Results: The program composition differed according to lineage. Five of the eight acceptance and commitment therapy-based programs addressed all six processes as distinct sessions, while six of the seven mindfulness-based programs were organized around awareness and did not address cognitive defusion or self as context. Committed action was the process least often identified as a distinct session. Recovery was operationalized mainly as symptom reduction, and acceptance showed the most consistent change, although this was measured less often. Conclusions: When developing acceptance-based interventions, processes that follow awareness training and recovery indicators beyond symptoms reduction warrant greater attention.
Family caregivers often experience anxiety and depression. Technology-based Acceptance and Commitment Therapy (technology-based ACT) provides new treatment options, but its effectiveness for family caregivers needs further research. To explore the effectiveness of technology-based ACT in relieving family caregivers’ anxiety and depression symptoms. Search databases of PubMed, Web of Science, Cochrane Library, CINAHL, and Embase from inception to January 6, 2026. Then, use the Cochrane Risk of Bias Assessment Tool, Version 2 to evaluate study quality. Finally, conduct statistical analysis with Stata 17.0 and Review Manager 5.4. A total of 1067 papers were initially retrieved, among which 7 met the inclusion criteria. The meta-analysis showed a statistically significant improvement in anxiety symptoms (SMD = -0.42; 95% CI -0.64 to -0.19; p < 0.001). For depressive symptoms, the fixed-effect analysis indicated a small statistically significant improvement (SMD = -0.28; 95% CI -0.51 to -0.06; p = 0.014); however, this effect was not statistically significant under the random-effects sensitivity analysis, indicating limited robustness to model choice. Exploratory subgroup analyses suggested greater improvements in anxiety among caregivers of care recipients with relatively mild conditions, while studies”’ using the GAD-7 showed a larger estimated effect than those using other anxiety measures. Greater improvements in depressive symptoms were observed among caregivers of care recipients with relatively mild conditions and in interventions lasting ≥ 8 weeks. Given the small number of studies, these subgroup findings should be interpreted cautiously. Technology-based ACT may improve anxiety symptoms among family caregivers, while the evidence for depressive symptoms is less robust and should be interpreted cautiously. Given the limited number of included RCTs and participants, these findings should be considered preliminary and require confirmation in larger, adequately powered trials.
Objectives: Internet Gaming Disorder (IGD) is associated with emotional distress and functional impairment in adolescents. This study evaluated the effectiveness of Acceptance and Commitment Group Therapy (ACGT) in reducing problematic gaming symptoms and associated depression, anxiety, and stress among high school students. Methods: In this randomized controlled trial, 40 adolescents with problematic gaming symptoms, identified using a prespecified 7-item Game Addiction Scale (GAS-7) screening threshold of ≥14, were randomly assigned to either ACGT (n=20) or a control group (n=20).Participants in the intervention group received eight weekly sessions of ACGT, whereas the control group received no structured intervention.Outcomes were assessed at baseline, post-intervention, and three-month follow-up using GAS-7 and the Depression Anxiety Stress Scale-Youth version. Results: Participants who received the intervention demonstrated reductions in problematic gaming symptoms that were maintained at follow-up.The median GAS-7 scores decreased from 16.5 at baseline to 11.0 post-intervention and 11.5 at the three-month follow-up, whereas scores increased in the control group (all p<0.001).In the adjusted analyses, a significant "treatment group×baseline GAS-7" interaction was observed at both post-intervention (F[1,33]=10.36;p=0.003) and follow-up (F[1,33]=6.79;p=0.014), indicating that between-group differences varied according to baseline problematic gaming severity.Secondary emotional outcomes were exploratory and should be interpreted with caution. Conclusion: A group-based intervention that incorporated Acceptance and Commitment Therapy was associated with a sustained reduction in problematic gaming symptoms among adolescents. These findings support the need for further evaluation of this approach in larger randomized trials with active control conditions and diagnostic confirmation of IGD.
