Rapid eye movement (REM) sleep behavior disorder (RBD) is a parasomnia in which the normal muscle atonia of REM sleep is lost, and patients act out their dreams.Serotonergic antidepressants such as selective serotonin reuptake inhibitors (SSRIs) are a recognized trigger, but in psychiatric patients the nocturnal behaviors are easily attributed to stress or to the primary illness. We report the case of a 46-year-old man with schizoaffective disorder, depressive type, maintained on aripiprazole 15 mg daily, who was started on escitalopram 10 mg at bedtime (day one); the dose was reduced to 5 mg on day 77.Around day 430, about 14 months after starting escitalopram and while taking 5 mg, he began to walk in his sleep and act out his dreams, including punching movements and, on one occasion, walking while holding a clothing iron, with no recollection on waking. The episodes occurred late in the night, with his eyes closed, two to three times a month and were accompanied by sleep paralysis and daytime sleepiness. His mental status examination and routine blood tests were unremarkable. He declined polysomnography and brain imaging, so a diagnosis of probable RBD, possibly associated with escitalopram, was made on clinical grounds; a non-REM parasomnia could not be excluded. On day 616, escitalopram was stopped and clonazepam 0.5 mg at bedtime was started at the same time. The episodes had stopped by day 665.Clonazepam was discontinued on day 772, and at the latest follow-up on day 1962 there had been no recurrence.Because clonazepam was started when escitalopram was withdrawn, the improvement cannot be attributed to stopping escitalopram alone, although the absence of recurrence after clonazepam was discontinued supports a role for the antidepressant.Clinicians who prescribe serotonergic antidepressants should take a sleep history from patients and their bed partners, arrange polysomnography where it is available, and review the suspected drug.
BACKGROUND: Understanding prescription trends and costs of psychotropic drugs is essential to inform mental healthcare policy and practice. However, comprehensive national evidence across psychotropic drug classes is limited, and little is known on how prescription volumes changed during the COVID-19 pandemic. AIMS: We examined prescription trends and costs of psychotropic drugs in the UK and assessed changes in prescription volumes following the onset of the COVID-19 pandemic. METHOD: We analysed national Prescription Cost Analysis data from 2013 to 2023, for six major psychotropic drug classes. Annual prescription volumes, total costs and average costs per drug were estimated. Linear regression was used to assess temporal trends and changes in prescription volume during the pandemic years. Costs were adjusted for inflation to 2023 prices. RESULTS: Between 2013 and 2023, psychotropic prescriptions grew 4% (95% CI 3.9-4.2) annually. Prescriptions for antidepressants, antipsychotics, stimulants and anti-dementia drugs increased, but not for hypnotics and anxiolytics and drugs used in substance dependence. In contrast, total costs declined by approximately £328 million, and the average cost per drug dropped from £11 to £5.9. Prescription volume remained unchanged during the pandemic years, except for anti-dementia medications, which showed an annual reduction of 17.2% (95% CI 7.8-26.6) relative to the underlying temporal trend during the study period. CONCLUSIONS: Our observed rising prescription trend is consistent with global evidence on availability and use of psychotropic drugs. Furthermore, we found a decrease both in total drug costs and average costs per drug.
Background: Individuals with complex trauma may continue to experience emotional distress, rumination, and maladaptive response patterns despite pharmacological treatment. Integrative interventions addressing affective responses, maladaptive patterns, and adaptive functioning warrant further clinical investigation. Case presentation: This report describes a 23-year-old woman diagnosed with complex trauma who her psychiatrist referred because she was not responding adequately to prescribed antidepressant and anti-anxiety medication. She received an eight-session intervention delivered in three phases: one session of Wiring Affect with ReAttach (W.A.R.A.®) on Day 1; one session of Brain-Switch 2.0® on Day 2 to address maladaptive brain patterns; and six subsequent ReAttach® sessions, emphasizing Cognitive Bias Modification (CBM) 3, followed by CBM 1 and CBM 2. Clinical observations: Following the Day 1 W.A.R.A.® session, the client reported a quieter mind, reduced rumination, greater calmness, and sleeping that night without sleeping medication. Brain-Switch 2.0® was administered on Day 2, which further enhanced her self-agency. Subsequent ReAttach® sessions integrated her sense of identity and improved boundary setting with her parents. At one-month post-therapy follow-up, the client reported improved management of rumination, greater emotional regulation, anxiety within normal ranges, preserved cognitive functioning despite ongoing family difficulties, and greater satisfaction with herself. Pre–post quantitative electroencephalography (qEEG) data showed changes in several recorded measures. The qEEG data indicated the absence of previous mental sluggishness, a clinically significant improvement in the trauma-related marker and emotional regulation within the normative range, and the absence of a depression-related marker.
Background Due to their shorter half-lives, paroxetine and venlafaxine often induce symptoms of discontinuation. There is still a lack of a thorough macroscopic analysis of the historical development and structure of this particular area. Methods The literature on this topic was collected from Web of Science and Scopus (2000–2025) and analyzed using bibliometric analysis. Descriptive and collaboration analyses utilized the merged dataset, whereas citation networks were constructed exclusively using Web of Science data. All data synthesis and visualizations were done in R using the bibliometrix, igraph, and ggplot2 packages, applying giant component extraction and the Fruchterman-Reingold force-directed layout for network optimization. Results A total of 903 documents were included. The number of annual publications reached its maximum around 2007 and since then has remained stable. The publications were primarily concentrated in North America and Europe, with the United States and the United Kingdom being the leading contributors. The clinical manifestations reported as “anxiety” and “withdrawal” were still basic themes identified in the results of the keyword analysis and thematic analysis. Interestingly, the research emphasis is now moving away from case reports of early phenomenology to “tapering” and “pharmacokinetics” (underlying strategies). Co-citation networks revealed a persistent theoretical impact from the pioneering psychopharmacologists and early specialized studies. Conclusions Venlafaxine and paroxetine withdrawal studies have evolved from clinical understanding to mechanistic investigation and protocol creation. Embedding some of the tapering ideas within a general treatment paradigm may offer a potential pathway to support clinical outcomes.
Electronic cognitive behavioral therapy (e-CBT) is a scalable intervention for anxiety and depressive disorders. While its efficacy against inactive controls is well documented, its comparative effectiveness against first-line pharmacotherapy remains critically underexplored. We conducted a systematic review and meta-analysis (International Prospective Register of Systematic Reviews (PROSPERO): CRD420261413686) of randomized controlled trials (RCTs) comparing e-CBT with waitlist, active psychotherapy, usual care, or medication in adults with anxiety and depression.Effect sizes were pooled using random-effects models with restricted maximum likelihood (REML) and Hartung-Knapp-Sidik-Jonkman (HKSJ) adjustments.Outcomes were assessed using Hedges' g and risk ratios (RR), and evidence certainty was evaluated via the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) framework.Twenty-two RCTs were included in the analysis.Compared to waitlist/attention controls (k, number of independent contrasts = 11), e-CBT demonstrated a large, statistically significant reduction in depression severity (Hedges' g, standardized mean difference = 0.91; 95% CI: 0.63 to 1.18).However, re-expression onto the Patient Health Questionnaire-9 (PHQ-9) scale yielded an absolute reduction (3.86 points) that fell below the pre-specified minimally important difference (MID) of 5 points. No clear difference was found between e-CBT and active face-to-face or bibliotherapy CBT (k = 3; g = 0.08; 95% CI: -0.59 to 0.75); the wide interval means equivalence could not be established.Adding e-CBT to usual care gave an imprecise, non-significant estimate (k = 3; g = 0.32; 95% CI: -1.41 to 2.05; p = 0.511) that neither confirms nor excludes a benefit. The network geometry revealed zero head-to-head RCTs comparing e-CBT monotherapy directly with antidepressant medication.Nineteen of the 22 trials (86%) were at high overall risk of bias, and funnel-plot asymmetry (Egger's test; p = 0.0056) suggested small-study effects, possibly including publication bias; most GRADE certainty ratings were very low.e-CBT is a scalable option that showed no clear difference from faceto-face CBT in a small number of trials, but its efficacy against inactive controls may be overestimated because of small-study effects and methodological limitations.A critical evidence gap exists regarding its comparative efficacy against pharmacotherapy, necessitating rigorous blinded head-to-head non-inferiority trials.
Public doubt about the usefulness of antidepressants appears to have grown, even as scientific agreement about their efficacy has become more settled. This paper examines the role that figures associated with critical psychiatry have had in promoting this doubt. It traces the long entanglement of antidepressants and their critics over the last eighty years, culminating in a 2022 review of the serotonin hypothesis, which, though it made no claims about efficacy, was widely interpreted as showing that antidepressants do not work. Drawing on Oreskes and Conway's concept of 'manufactured doubt', the paper explores how certain philosophical commitments have shaped these actors' views, at times impeding their engagement with new evidence. In doing so, it marks a distinction between productive and unproductive doubt. Whilst external scrutiny has played a significant role in shaping psychiatry, some positions are held so rigidly that the resulting critiques obscure the scientific consensus and risk undermining psychiatric care.
Purpose To evaluate trends in psychotropic prescription rates (PRs) in a Peruvian general hospital during and after the COVID-19 pandemic. Materials and methods We conducted an interrupted time-series (ITS) study using routinely collected clinical data from hospitalized patients with psychiatric diagnoses between September 2020 and December 2024. Psychotropic PRs, defined as the number of weekly psychotropic prescriptions per 1,000 psychiatric consultations, were calculated overall and by subclass: antidepressants (N06A), antipsychotics (N05A), anxiolytics (N05B), and mood stabilizers (N05AN01/N03A). For ITS analysis, the study period was divided into six predefined phases (two waves, three relaxation phases, and one post-pandemic phase) based on COVID-19 mortality and transmission trends in Peru. Segmented regression models were applied to estimate phase-specific level and slope changes. Results A total of 6,151 patients were included. The overall psychotropic PR was higher during the post-pandemic period compared with the pandemic period (1,308 vs. 1,254; p = 0.023). ITS analyses showed a significant downward trend in overall psychotropic prescribing during phase 1 (post-first wave stabilization, β = −25.53; 95% CI = −43.82 to −7.01; p = 0.007), followed by a structural break in phase 2 (second pandemic wave) with an abrupt level increase (β = 295.53; 95% CI = 60.48 to 530.57; p = 0.014) and slope reversal (β = 23.79; 95% CI = 3.33 to 44.26; p = 0.016), after which prescribing stabilized. A similar temporal pattern was observed for antidepressant PR. Anxiolytic PR displayed a transient upward slope change in phase 2 (β = 14.15; 95% CI = 1.80 to 26.50; p = 0.01), whereas antipsychotic and mood stabilizer prescribing remained stable across phases. Conclusions Overall psychotropic prescribing showed phase-specific disruption followed by structural rebound and subsequent stabilization across the COVID-19 and post-pandemic periods. These findings provide real-world evidence and underscore the need to monitor psychiatric prescribing and mental health service adaptation in middle-income settings.
While psychedelic experiences are often reported as positive, adverse psychological reactions can occur. We present the case of a 19-year-old Caucasian male who presented to the ER with suicidal thoughts after taking lysergic acid diethylamide (LSD) alone. He was diagnosed with anxious state associated with panic attacks, and suicidal thoughts without ideation or behaviors. He was admitted to the psychiatric ward and treated with trazodone, a serotonin antagonist and reuptake inhibitor antidepressant, and brexpiprazole, an atypical antipsychotic. He responded well and was discharged after 12 days with recommendations to continue therapy with brexpiprazole and attend follow-up at the territorial mental health services. Five days later, he engaged in alcohol consumption with a roommate met during the psychiatric admission, and suicidal thoughts re-emerged. He presented again to the ER but was not admitted to the inpatient unit, since he improved with supportive conversation and medication. One month later, he returned to the ER a third time due to pervasive doubt and confusion that distressed him to the point of triggering unstructured suicidal thoughts. After one week of hospitalization, he was discharged and citalopram (a selective serotonin reuptake inhibitor [SSRI]) was added to the therapy. Following the third admission, the clinical suspicion strengthened that he may have an underlying obsessive functioning pattern that became clinically apparent following LSD use, although the contribution of other vulnerability factors could not be excluded. This case highlights the importance of set and setting, underscores the relevance of harm reduction strategies for both recreational and clinical users, and highlights a potentially negative interaction between serotonergic psychedelics and alcohol.
