Multidimensional transdiagnostic inventory of psychopathology: development and initial validation

Mark A. Blais; Hannah Cai; Till Julius Adam; Christoph U. Correll Frontiers in Psychology, 2026, Vol. 17, pp. 1917064 https://doi.org/10.3389/fpsyg.2026.1917064

Traditional diagnostic systems, such as the DSM-5 and ICD-11, suffer from well-documented limitations, including high comorbidity rates, arbitrary diagnostic thresholds, and within-disorder heterogeneity. Consequently, the field is evolving towards hierarchical, transdiagnostic, dimensional models of psychopathology. This study reports on the development and initial validation of the Multidimensional Transdiagnostic Inventory of Psychopathology (MTIP). This new broadband inventory aligns traditional DSM disorders with contemporary dimensional models of psychopathology. We conducted initial development and basic psychometric analyses in a mixed sample of 301 adults. We then validated the MTIP in a second independent sample (N = 826), of which 640 who self-reported no history of past mental health treatment were used as our normative/reference sample. The final MTIP contained 13 clinical scales and seven supplemental scales. All the MTIP scales met core psychometric assumptions at the item, scale, and transdiagnostic levels. A series of exploratory and second-order factor analyses supported the MTIP hierarchical dimensional structure. Concurrent validity correlations with the DSM-5 level 1 cross-cutting symptom measure were strong, ranging from 0.89 (depression) to 0.51 (drug misuse). Regression analyses supported the construct validity of the higher-order transdiagnostic scales. Twelve of the 13 clinical scales differentiated individuals with a history of mental health care from those without past care, with effect sizes ranging from 0.20 (Alcohol Misuse) to 1.06 (Anxiety). Effect sizes for the transdiagnostic scales ranged from 0.33 (Thought Dysregulation) to 1.07 (Emotion Dysregulation). The clinical implications and potential applications of the MTIP are discussed.

Long-term outcomes of daridorexant in those with comorbid insomnia

Soumya Saladi; Hatem Jouda; Teck Yong; Kirstie N. Anderson BJPsych Bulletin, 2026, pp. 1-6 https://doi.org/10.1192/bjb.2026.10298

AIMS AND METHOD: Daridorexant is a dual orexin receptor antagonist recently licensed for chronic insomnia disorder. Guidelines recommend it as a second-line treatment for insomnia, after cognitive-behavioural therapy. Initial trials excluded common comorbidities despite comorbid insomnia being the norm, not the exception. The long-term benefit from daridorexant remains unclear, with few long-term trials. We aimed to assess long-term outcomes in those with comorbid insomnia. This was a retrospective analysis of 38 patients with insomnia who commenced daridorexant. Mental health disorders were common in those referred. Adverse events and a range of insomnia outcomes were assessed. RESULTS: The mean follow-up period was 10.5 months. Of the 38 patients in the study, 95% had comorbid insomnia; 55% experienced benefit from the daridorexant, but 24% discontinued because of side-effects, including vivid dreams, fatigue and worsening anxiety. Three patients experienced worsening mood, one of which included suicidality. CLINICAL IMPLICATIONS: Daridorexant can be effective for those with comorbid insomnia, but side-effects may be more common than previously reported.

Richards Sarah-Jane; Stephen Bruce Dunnett; Graeme D. Hughes BJPsych Bulletin, 2026, Vol. 2026, pp. 1-9 https://doi.org/10.1192/bjb.2026.10297

AIMS AND METHOD: An analysis of coronial inquest data provides contextual information rarely available in routine surveillance systems, including a nuanced understanding of proximal circumstances preceding death of value to mental health services. Inquest records of more than 750 suicides occurring within the South Wales Central Coroner's jurisdiction between 2016 and 2023 were reviewed, including the COVID-19 pandemic years, to assess the effects of lockdown and social isolation. Detailed information was extracted, including medical records, toxicology, police reports, witness statements and suicide notes. RESULTS: Several patterns emerged relating to age, gender, method of death, trigger factors, local authority area, and engagement with mental health services. Hanging was the predominant method, particularly among men. Depression was the most common trigger factor, followed by relationship problems, alcohol/drug misuse, health concerns, psychotic illness and bereavement. CLINICAL IMPLICATIONS: Key structural and psychosocial risk factors identified highlighted the need for tailored, context-specific suicide-prevention strategies, with circumstances preceding suicide integrated into public health planning.

Artificial Intelligence in suicide prevention: promise, peril, and clinical implications for clinical practice

Bénédicte Nobile; Philippe Courtet Dialogues in Clinical Neuroscience, 2026, Vol. 29, Issue 1, pp. 15-23 https://doi.org/10.1080/19585969.2026.2745684

The integration of Artificial Intelligence (AI) into psychiatry is progressing rapidly, with promising yet underexplored applications in suicide prevention. While most existing studies have focused on suicide risk prediction, AI could be applied more broadly across the suicide prevention continuum. This review explores AI's potential in improving the detection of suicidal ideation (SI), enhancing clinician training, and expanding patient access to psychotherapeutic support. Studies suggest that AI models may identify SI more sensitively than clinicians by detecting subtle linguistic or semantic cues. Furthermore, AI-based virtual patients have been shown to improve clinicians' confidence and accuracy in suicide risk assessments, particularly when used alongside traditional training methods. On the patient side, CBT-inspired AI tools offer scalable, on-demand support, but most remain unregulated and clinically unvalidated. Despite their perceived empathy and availability, such tools pose ethical and clinical risks. Many patients use AI tools without professional guidance, a trend clinicians often underestimate. This disconnect calls for urgent regulatory oversight and professional engagement. Without clear clinical validation, AI-based tools may evolve outside medical frameworks, exposing vulnerable individuals to preventable harm. Psychiatry must not only adapt but also lead, ensuring that innovation is harnessed responsibly to serve patients with empathy, safety, and scientific rigor.

Perceived Physical Health and Depressive Symptoms Among Older Nepali Adults: The Moderating Role of the Components of Hope and Self-Compassion, and Place of Residence

Pralhad Adhikari; Lewis '; Danielle Sulikowski; Suzanne McLaren Clinical Gerontologist, 2026, pp. 1-15 https://doi.org/10.1080/07317115.2026.2741255

OBJECTIVES: This study examined whether the components of hope (agency and pathways) and self-compassion (self-warmth and self-coldness) moderate the relationship between poorer perceived physical health and depressive symptoms, and whether these moderating effects are conditional on place of residence. METHODS: = 8.16) completed the Adult Hope Scale, Self-Compassion Scale, physical health items of the Duke Health Profile, and short form of the Geriatric Depression Scale. RESULTS: Almost half (46.3%) of the sample had sub-threshold depressive symptoms. Pathways weakened the association between poorer perceived physical health and depressive symptoms for urban older adults. Self-coldness strengthened the association between poorer perceived physical health and depressive symptoms for rural older adults. CONCLUSIONS: Pathways are a protective factor for Nepali older adults experiencing poorer perceived physical health from urban areas, whereas self-coldness is an additional risk factor for those living in rural areas. CLINICAL IMPLICATIONS: Interventions aimed at reducing depressive symptoms among Nepali older adults with poorer perceived physical health should focus on increasing pathways in urban areas and reducing self-coldness in rural areas.

Holding multiplicity: narrative and trauma-informed couples therapy for dissociative identity disorder

Annemarie Sohn; Eman Tadros; Josh Bolle Sexual & Relationship Therapy, 2026, pp. 1-24 https://doi.org/10.1080/14681994.2026.2741916

Couples therapy involving a couple with Dissociative Identity Disorder (DID) promotes the incorporation of relational treatment modalities that have historically been regarded as adjunctive in DID care. Through narrative therapy, trauma-informed practice, and a neurodiversity-affirming framework, the case study addresses individual dissociative experiences and relational dynamics while centering client strengths, agency, and meaning-making. In contrast to integration, the therapy respected and honored multiplicity, supporting partners in constructing empowering narratives about their experiences and shared futures. The findings emphasize the need for therapists to master complex therapeutic interventions that enable co-constructed healing for misunderstood populations. Policy and clinical implications promote couples therapy for those with or systems impacted by DID, the enhancement of trauma training for relational therapists, and the encouragement of treatment methodologies that elucidate intimate partnerships as avenues of healing. This case contributes to emerging discourse positioning couples therapy as essential rather than ancillary in comprehensive DID treatment.

Rapid-acting and neuromodulatory interventions for treatment-resistant OCD: clinical implications in stress reduction

Johan Saelens; Franziska Stadler; Victoria Forschauer; Juan Carlos Baldermann; Ioline D. Henter; Carlos A. Zarate; … Translational Psychiatry, 2026 https://doi.org/10.1038/s41398-026-04480-x

Obsessive-compulsive disorder (OCD) is a highly debilitating and often chronic condition. A large percentage of patients do not respond to first-line treatments with selective serotonin reuptake inhibitors (SSRIs) or cognitive behavioral therapy (CBT), underscoring the need for novel therapeutic approaches. In this context, promising innovative treatments have recently emerged for OCD; these include pharmacological options such as ketamine and psilocybin—which may exert rapid-acting effects—as well as neuromodulatory treatments such as repetitive transcranial magnetic stimulation (rTMS) and deep brain stimulation (DBS). In addition, growing evidence links early life stress with neuroinflammatory pathways and dysregulation of the hypothalamic–pituitary–adrenal (HPA) axis in OCD, opening novel treatment targets. While preliminary clinical trials of novel rapid-acting substances in OCD are encouraging, results are mixed and evidence of repeated dosing is scarce. This narrative review synthesizes current evidence on rapid-acting and neuromodulatory treatments for OCD, with a particular focus on stress-related mechanisms, offering new therapeutic possibilities as well as avenues for future research.

Does More Speech Therapy Lead to Better Results in Childhood Apraxia of Speech? A Systematic Review and Meta‐Analysis of the Available Evidence

Patricia McCabe; Donna Thomas; Xue Ming Jessica Choo; Robert C. Heard; Denise Ng International Journal of Language & Communication Disorders, 2026, Vol. 61, Issue 6, pp. e70337 https://doi.org/10.1111/1460-6984.70337

BACKGROUND: In recent years there has been a significant increase in the number and quality of childhood apraxia of speech (CAS) treatment research published. Much of this research has focussed on establishing treatment efficacy and identifying treatment components. A smaller subset of the literature has looked at treatment intensity, such as comparing therapy delivered twice per week with four times per week, however this literature has not been compared across interventions. AIMS: This systematic review and meta-analysis examines cumulative intervention intensity and its components (dose per session, dose frequency, intervention duration-number of weeks) and reported treatment effects to understand whether there is a relationship between dose and effect in specific treatments or overall. METHODS: A systematic search and review was registered with Prospero (CRD420251107259). Twelve electronic databases were searched supplemented by handsearching references lists. Included studies were coded for CAS diagnosis confidence, methodological quality, dose characteristics, and treatment outcomes. Standard mean difference (SMD), an effect size metric equivalent to Cohen's d, was calculated for 25 included papers and weighted Pearson's correlations performed to identify relationships between dose characteristics and treatment outcomes. MAIN CONTRIBUTION: This paper is the first to (1) create omnibus standard effect sizes across diverse interventions and (2) show the relationship between intensity and treatment outcome in children with CAS. The omnibus SMD across studies with three or more participants for treated items was 1.22 (confidence interval 0.76-1.67), a large effect, and for generalisation items was 0.66 (confidence interval 0.46-0.85) which is a moderate effect. These results show that CAS treatment works in well-controlled studies when delivered at least twice per week for at least 45 min per session. The relationship between intensity variables and generalisation SMD was less clear with weeks of therapy being moderately correlated (r = 0.39) and none of the other variables showing any relationship with study SMD. These results should be interpreted carefully as there was significant missing data and limited range in some variables. Researchers need to include more standard intensity information in reporting their results. CONCLUSIONS: Treatment for CAS is effective and leads to moderate generalisation; more weeks of therapy leads to stronger gains than fewer weeks of therapy. WHAT THIS PAPER ADDS: What is already known on this subject CAS is a neurological-based motor speech disorder that originates in childhood for which there are a number of effective treatments. For this population, high intensity treatment is recommended-over 100 production trials per session, 2-5 sessions per week, across an extended period. In studies which directly compare intensity of variables, more therapy (e.g. 20 sessions) results in greater improvement than less therapy (e.g. 10 sessions). There are no previous meta-analyses of intensity variables-dose, dose frequency, treatment duration, or cumulative intervention intensity in CAS What this study adds to the existing knowledge This study compiles data from 25 studies with a low risk of bias and high CAS diagnostic confidence in a meta-analysis of the dose effects of CAS treatment. The meta-analysis shows that CAS treatment in general works. Most studies had 2-4 sessions per week across 3-12 weeks. Within this narrow band of intensity, a significant correlation was shown between the number of weeks of therapy and outcome. There is no significant evidence for CAS therapy less frequently than twice per week at this time. Eight different treatments demonstrated efficacy for improving speech outcomes in children with CAS. What are the clinical implications of this study? Provided they are delivered at least twice per week for at least 12 sessions, with greater improvements when treatment extends over more weeks, selection of any of the included treatments should result in improved speech where the child being treated is similar to children in the research. The following treatments were shown to be effective for CAS: Integral stimulation treatments including Dynamic Temporal and Tactile Cueing, Kaufman-Speech to Language Protocol, Motor Speech Treatment Protocol, Nuffield Dyspraxia Programme-3rd Edition, Rapid Syllable Transition Treatment, Speech Motor Chaining and Ultrasound Biofeedback alone and the combination of Speech Motor Chaining with Ultrasound Biofeedback. PRACTITIONER POINTS: Therapy must be sufficiently frequent and sustained: Evidence shows that CAS treatment is effective when therapy is provided at least twice per week, over a minimum of 12 sessions, for 45 min per session with total weeks of therapy being the only intensity variable consistently associated with treatment outcomes. Dose matters, both within and across sessions: Clinicians should prioritise the number of production trials and ensure adequate cumulative intervention intensity. Sessions with low active practice time or insufficient trials are unlikely to produce meaningful change. Eight treatments are effective for CAS: Integral Stimulation including Dynamic Temporal and Tactile Cueing (DTTC), Kaufman Speech to Language Program (K-SLP), Motor Speech Treatment Protocol (MSTP), Nuffield Dyspraxia Programme (3rd ed; NDP3), Rapid Syllable Transition Treatment (ReST), Speech Motor Chaining (SMC), Ultrasound Biofeedback, and SMC + Ultrasound Biofeedback combined. Clinicians should consider the client's speech needs and overall profile when selecting from the treatments that have demonstrated effectiveness.

درمان‌های مؤثر برای CAS عبارت‌اند از: درمان‌های تحریک یکپارچه شامل Dynamic Temporal and Tactile Cueing، Kaufman-Speech to Language Protocol، Motor Speech Treatment Protocol، Nuffield Dyspraxia Programme-3rd Edition، Rapid Syllable Transition Treatment، Speech Motor Chaining و Ultrasound Biofeedback به تنهایی و ترکیب Speech Motor Chaining با Ultrasound Biofeedback. هشت درمان برای CAS مؤثر هستند: تحریک یکپارچه شامل Dynamic Temporal and Tactile Cueing (DTTC)، Kaufman Speech to Language Program (K-SLP)، Motor Speech Treatment Protocol (MSTP)، Nuffield Dyspraxia Programme (نسخه سوم؛ NDP3)، Rapid Syllable Transition Treatment (ReST)، Speech Motor Chaining (SMC)، Ultrasound Biofeedback و ترکیب SMC + Ultrasound Biofeedback.
