Background: Individuals with complex trauma may continue to experience emotional distress, rumination, and maladaptive response patterns despite pharmacological treatment. Integrative interventions addressing affective responses, maladaptive patterns, and adaptive functioning warrant further clinical investigation. Case presentation: This report describes a 23-year-old woman diagnosed with complex trauma who her psychiatrist referred because she was not responding adequately to prescribed antidepressant and anti-anxiety medication. She received an eight-session intervention delivered in three phases: one session of Wiring Affect with ReAttach (W.A.R.A.®) on Day 1; one session of Brain-Switch 2.0® on Day 2 to address maladaptive brain patterns; and six subsequent ReAttach® sessions, emphasizing Cognitive Bias Modification (CBM) 3, followed by CBM 1 and CBM 2. Clinical observations: Following the Day 1 W.A.R.A.® session, the client reported a quieter mind, reduced rumination, greater calmness, and sleeping that night without sleeping medication. Brain-Switch 2.0® was administered on Day 2, which further enhanced her self-agency. Subsequent ReAttach® sessions integrated her sense of identity and improved boundary setting with her parents. At one-month post-therapy follow-up, the client reported improved management of rumination, greater emotional regulation, anxiety within normal ranges, preserved cognitive functioning despite ongoing family difficulties, and greater satisfaction with herself. Pre–post quantitative electroencephalography (qEEG) data showed changes in several recorded measures. The qEEG data indicated the absence of previous mental sluggishness, a clinically significant improvement in the trauma-related marker and emotional regulation within the normative range, and the absence of a depression-related marker.
This study explored the academic validation experiences of internal migrant college students and their implications for proposed psychosocial support activities. Specifically, it examined forms of academic validation, their influence on psychosocial well-being and academic persistence, and challenges in obtaining institutional support. Using a qualitative research design and Interpretative Phenomenological Analysis, the study examined interview accounts from eight internal migrant college students at Cavite State University–Main Campus during the academic year 2025–2026. Findings revealed that students experienced academic validation through faculty recognition, constructive feedback, peer encouragement, and academic achievements. Such experiences fostered motivation, confidence, belonging, and continued academic engagement. Conversely, limited recognition contributed to discouragement, self-doubt, and emotional distress. Participants also described homesickness, social isolation, language barriers, unfamiliar cultural practices, communication difficulties, and hesitation in seeking academic or emotional assistance. Despite these challenges, personal resilience, supportive classmates, and determination helped students adjust and persist in their studies. Based on the findings, the study proposed the BRIDGE Program, emphasizing peer mentoring, support circles, cultural integration, wellness activities, academic assistance, counseling, and opportunities for emotional expression. The study concluded that academic validation and accessible psychosocial support are important to the adjustment and educational persistence of internal migrant students. It aligns with Sustainable Development Goal 3, Good Health and Well-Being; Goal 4, Quality Education; and Goal 10, Reduced Inequalities. Its potential sustainability contribution lies in guiding inclusive institutional support practices that strengthen student welfare, social belonging, and equitable participation in higher education. These practices may also encourage more responsive and supportive campus communities.
The increasing accessibility of online pornography has led to earlier exposure among children and adolescents, raising concerns about its potential long-term effects on sexual development, behaviour and mental health. This systematic literature review synthesises current evidence on the relationship between early exposure to pornography and later pornography consumption. A comprehensive search of six electronic databases (MEDLINE, PsycINFO, Sociological Abstracts, Embase, ASSIA, Web of Science) identified 17 studies that met the inclusion criteria, which required individuals to have experienced early exposure to pornography and reported subsequent pornography consumption. Eligible quantitative studies (longitudinal, cohort, case-control, cross-sectional), published in English from 1993 onwards, were identified and screened against inclusion and exclusion criteria. Across studies, earlier age of first exposure was consistently associated with higher frequency of use, greater risk of compulsive or problematic consumption and progression towards more extreme or deviant content in adolescence and adulthood. Developmental and gender differences emerged, with males generally exposed earlier and exhibiting stronger associations between early exposure and problematic use, although similar attitudinal effects were noted among females. Mediating mechanisms included internalisation of sexual scripts, perceived realism, shame and emotional distress, while moderating factors encompassed impulsivity, sensation-seeking, family stability, religiosity and cultural context. Overall, findings indicate that early exposure to pornography may shape enduring behavioural and cognitive trajectories, with outcomes influenced by individual, developmental and social factors. Findings and future directions are discussed in relation to implications for practice, policy and future research.
Parental loss is a severe stressor for individuals with schizophrenia, yet research on resilience in schizophrenia tends to focus on internal individual factors or caregiver burden rather than the survivors’ own experiences. This study aims to understand the process of resilience in individuals with schizophrenia following parental loss. A qualitative case study design was used, involving one primary informant (AW) and one secondary informant (PP) a significant other. Data were collected through semi-structured in-depth interviews guided by Reivich and Shatté’s (2002) aspects of resilience, then analyzed thematically with source triangulation. The findings reveal six forms of resilience in AW: active diversion from emotional distress, self-awareness and medication adherence, pursuing concrete life goals, providing reciprocal support to fellow survivors, openness to new relationships and experiences, and meaning-making of the loss. These findings indicate that resilience develops through a continuous adaptive process involving self-awareness, social support, and meaning-making.
Trichotillomania (TTM) is a body-focused repetitive behavior characterized by compulsive hair-pulling and an association with psychiatric comorbidities and profound emotional distress. Despite its significant impact on well-being, TTM remains understudied, underdiagnosed, and without established treatment guidelines. These deficiencies are particularly evident for ethnic and racial minority groups with TTM, who experience unique factors related to the cultural significance of hair, distinct physical hair properties, stigmas surrounding psychiatric care, and differences in clinical presentation. Additionally, therapies for TTM have been developed primarily from non-Hispanic White samples, potentially contributing to disparities in treatment outcomes. While emerging therapies aim to integrate cultural perspectives, more representative research is needed to address knowledge gaps and develop effective care for TTM across all groups. This review will address differences in the presentation, diagnosis, and treatment of TTM in ethnic and racial minority populations, and discuss barriers to management and future directions of equitable care.
Informal carers play a vital role in supporting individuals with mental health difficulties. To provide an overarching synthesis of available evidence, this umbrella review aimed to synthesise qualitative review evidence on informal carers’ (including family members, friends, or other unpaid supporters) experiences of mental health services regarding supporting the individuals with mental health difficulties they care for and seeking support for their own wellbeing. Following the Joanna Briggs Institute (JBI) methodology, a meta-aggregative umbrella review was conducted. Five databases were searched up to 9th May 2025: CINAHL, MEDLINE, PsycInfo, the Cochrane Library, and Epistemonikos. The quality of primary systematic reviews was assessed using a framework adapted from the work of SmithBattle and colleagues. Findings were extracted from eligible reviews and aggregated into categories, from which higher-order synthesised findings were developed. Twelve eligible qualitative reviews were identified covering a wide range of mental health service contexts. Six synthesised findings were identified: (1) emotional complexity in the caregiving journey; (2) recognition and coping with their loved ones’ mental health difficulties; (3) multiple barriers to accessing mental health support; (4) persistent responsibilities and roles in supporting service users; (5) complex experiences of engaging with services; and (6) relational strain during the caregiving process. While some carers reported positive experiences, most described exclusion, emotional distress, and structural barriers to support. This review highlights the urgent need for mental health services to recognise, involve, and support informal carers.
Background Emotional distress is common around clinical procedures, cancer supportive care, and health-professions training. Aromatherapy and music are easy to deliver, yet it remains unclear when combining them is worthwhile and whether the combination offers more than either component alone. Objective To synthesize experimental studies of aromatherapy combined with music or sound published from January 2019 through September 7, 2026; identify the populations and settings in which benefits were most consistent; and separate benefit over inactive care from incremental benefit over a single sensory intervention. Methods We searched PubMed, Embase, CINAHL, APA PsycINFO, Scopus, Web of Science Core Collection, CENTRAL, CNKI, and Dentistry & Oral Sciences Source from January 1, 2019, to September 7, 2026. Randomized trials and controlled nonrandomized or quasi-experimental studies were eligible when they included an identifiable arm combining aromatherapy with music or sound and reported emotional-distress or related physiological outcomes. Two reviewers independently screened records, extracted data, and assessed risk of bias. Clinical and methodological heterogeneity precluded meta-analysis; findings were synthesized narratively using SWiM guidance. Results Twenty-four studies involving 2,530 participants from 12 countries or regions were included. Of these, 21 were randomized studies and three were controlled nonrandomized or quasi-experimental studies. The evidence covered procedural and perioperative care, dentistry, nursing education and occupational stress, and cancer or palliative care. Results were most consistent in pediatric dentistry, magnetic resonance imaging, nursing examinations or skills testing, and some repeated cancer-support programs. Two lithotripsy trials and the adult endodontic study did not show a clear benefit. Although combined treatment often outperformed inactive care, it frequently failed to outperform aromatherapy or music alone. Adverse events, acceptability, and economic outcomes were poorly reported. Conclusion Combined aromatherapy and music or sound showed the most consistent reductions in anxiety or distress in pediatric dentistry, MRI examinations, nursing examinations or skills training, and some perioperative and cancer-support settings. Clear benefit was not demonstrated in lithotripsy or endodontic care; in general surgery, pregnancy non-stress testing, coronary angiography, palliative care, and outpatient oncology, the combination generally did not outperform a single modality. Future adequately powered factorial trials should standardize the interventions and use prespecified between-group comparisons, longer follow-up, and systematic reporting of adverse events and costs. روشها: پایگاههای PubMed، Embase، CINAHL، APA PsycINFO، Scopus، Web of Science Core Collection، CENTRAL، CNKI و Dentistry & Oral Sciences Source از ۱ ژانویه ۲۰۱۹ تا ۷ سپتامبر ۲۰۲۶ جستوجو شدند.
Although clinicians often report significant emotional challenges in psychotherapy for personality disorders, we still know relatively little about how these difficulties are enacted and reorganized in moment-to-moment patient-therapist interaction. We examine whether interactional resonance and mediopassive agency can help illuminate relational difficulties and change in a good-outcome long-term psychodynamic psychotherapy involving a patient with severe interpersonal pathology, chronic depressive symptoms, and somatic complaints. The patient met the diagnostic criteria for borderline and avoidant personality disorders, as well as depressive and somatic symptom disorders. For much of the treatment, the therapist described sessions as highly distressing.Over time, sessions were experienced as less distressing and more productive. We examined whether this shift could be accounted for by changes in how patient and therapist took up and developed each other's contributions in interaction.Following a preliminary screening of therapist reports, case notes, and session recordings, we selected 10 sessions for multimodal conversation analysis.Using exploratory conversation analysis, we specified analytic indicators for interactional resonance and then compared five high-distress sessions with five lower distress sessions.High-distress sessions showed lower interactional resonance, whereas lower distress sessions showed higher resonance. In lower distress sessions, patient and therapist more often engaged in mediopassive interactions-that is, they jointly participated in the stepwise construction of meaning across turns.Psychotherapy for severe interpersonal pathology may involve working with persistently low levels of interactional resonance that constrain spontaneous, collaborative meaning-making.The findings further suggest that a dyad's capacity for interactional resonance may increase over the course of long-term treatment. Clinical Impact StatementQuestion: How can we explain the emotional challenges that often arise in psychotherapy with patients with severe personality disorder?Findings: Sessions with subjectively high emotional distress lacked interactional resonance, while lower distress sessions showed higher interactional resonance as well as more mediopassive agency-collaborative meaning-making that is not fully predictable or controllable by either participant.Meaning: Very low interactional resonance may be linked to severe emotional difficulties in therapeutic relationships, but long-term therapy may foster increases in a dyad's capacity for resonance.Next Steps: Future research should explore how therapists can foster the development of interpersonal resonance in patients with severe personality disorder and how clinical training might better prepare clinicians to effectively recognize and work with noninterpretive mechanisms of change.
Motherhood is inherently demanding, and some of these demands may be amplified for women with borderline personality disorder (BPD). The exacerbation of BPD symptoms from parenting-related stressors can have adverse impacts on mother-infant relationships and infant development. This qualitative study explores Australian mothers' views of the impacts of mother-infant dialectical therapy plus (MI-DBT+). MI-DBT+ is an innovative therapeutic intervention which integrates mother-infant dialectical behavior Therapy (MI-DBT), a known effective intervention for the mental health of mothers with BPD, with attachment and biobehavioral catch-up (ABC) therapy. Specifically, eight mothers completed semi-structured interviews about their experiences of intervention following completion of the MI-DBT+ program. A thematic analysis of this interview data found that mothers appreciated learning more skills to enhance their understanding of their infants' needs, behaviors and emotions. Mothers also reported improved reflective functioning and better control of their emotional responses. These skills assisted in managing emotional distress at both the individual and dyadic level. The study was limited due to available demographic data, with only maternal and child age available. Discussion also focuses on the fit between DBT and ABC, longer term developmental concerns and sustainability.
Psychological flexibility and inflexibility are core processes in the Acceptance and Commitment Therapy (ACT) model and are consistently associated with well-being and emotional distress. However, the validity of widely used measures, particularly the Acceptance and Action Questionnaire – II (AAQ-II), has been questioned due to potential overlap with indicators of distress and neuroticism. This study examined the psychometric properties of the Acceptance and Action Questionnaire – III (AAQ-III) and the PsyFlex in a general population sample from Colombia ( N = 785) using network psychometrics. We estimated the dimensionality of each scale via Exploratory Graph Analysis (EGA) and evaluated replicability via bootstrap EGA (bootEGA). Discriminant validity was examined using a joint EGA that included the AAQ-III, PsyFlex, and measures of emotional distress (DASS-21) and neuroticism (BFI-2-Neuroticism). Convergent validity was assessed through correlations with the Multidimensional Psychological Flexibility Inventory – Short Form. EGA identified unidimensional structures for both instruments, and bootEGA confirmed perfect structural consistency and item stability. The joint EGA yielded five distinct communities, with the AAQ-III and PsyFlex forming separate communities from emotional symptoms and neuroticism, providing structural evidence consistent with the discriminant validity of both measures. Both instruments showed the expected patterns of correlations with flexibility and inflexibility processes. These findings provide initial validation evidence that the Spanish versions of the AAQ-III and PsyFlex are psychometrically sound measures of psychological inflexibility and flexibility.
Artificial intelligence (AI)-assisted digital mental health interventions are being increasingly discussed as a potential strategy for improving access to psychological support among adolescents with anxiety symptoms and anxiety disorders. Their possible advantages include flexible availability, reduced barriers to help-seeking, psychoeducation, symptom monitoring, CBT-informed exercises, conversational interaction, and personalized feedback. However, their clinical role remains insufficiently defined, particularly regarding efficacy, safety, ethical governance, and the distinction between digital support and psychotherapy in the traditional clinical sense. This narrative review examines current evidence and conceptual debates regarding AI-assisted digital interventions relevant to adolescent anxiety disorders, with a focus on usability, acceptability, preliminary efficacy, safety, ethical and legal considerations, and human clinical oversight. Available literature suggests that conversational agents and CBT-informed digital tools may provide low-threshold support and may reduce the symptoms of anxiety, depression, or general emotional distress in some users. Nevertheless, existing evidence remains heterogeneous and preliminary, often relying on young adult, mixed-age, non-clinical, or transdiagnostic samples, short follow-up periods, and self-report outcomes. Direct evidence for AI-assisted interventions specifically in adolescents with formally diagnosed anxiety disorders remains limited, and findings from broader youth, young adult, mixed-age, or transdiagnostic samples should not be directly generalized to this clinical population. It should not be considered a replacement for therapist-delivered psychotherapy, clinical assessment, or specialist child and adolescent mental health care. Their most appropriate current role is as adjunctive or supportive tools within supervised, stepped-care, or blended-care models. Future research should include adolescent-specific clinical trials, anxiety-focused outcomes, longer follow-up, systematic safety reporting, and clear evaluation of human oversight models. AI-assisted digital interventions may help reduce parts of the adolescent mental health treatment gap; however, their use must remain evidence-informed, developmentally sensitive, ethically regulated, and clinically supervised.
Objectives: Internet Gaming Disorder (IGD) is associated with emotional distress and functional impairment in adolescents. This study evaluated the effectiveness of Acceptance and Commitment Group Therapy (ACGT) in reducing problematic gaming symptoms and associated depression, anxiety, and stress among high school students. Methods: In this randomized controlled trial, 40 adolescents with problematic gaming symptoms, identified using a prespecified 7-item Game Addiction Scale (GAS-7) screening threshold of ≥14, were randomly assigned to either ACGT (n=20) or a control group (n=20).Participants in the intervention group received eight weekly sessions of ACGT, whereas the control group received no structured intervention.Outcomes were assessed at baseline, post-intervention, and three-month follow-up using GAS-7 and the Depression Anxiety Stress Scale-Youth version. Results: Participants who received the intervention demonstrated reductions in problematic gaming symptoms that were maintained at follow-up.The median GAS-7 scores decreased from 16.5 at baseline to 11.0 post-intervention and 11.5 at the three-month follow-up, whereas scores increased in the control group (all p<0.001).In the adjusted analyses, a significant "treatment group×baseline GAS-7" interaction was observed at both post-intervention (F[1,33]=10.36;p=0.003) and follow-up (F[1,33]=6.79;p=0.014), indicating that between-group differences varied according to baseline problematic gaming severity.Secondary emotional outcomes were exploratory and should be interpreted with caution. Conclusion: A group-based intervention that incorporated Acceptance and Commitment Therapy was associated with a sustained reduction in problematic gaming symptoms among adolescents. These findings support the need for further evaluation of this approach in larger randomized trials with active control conditions and diagnostic confirmation of IGD.
