Background: Insomnia is an important public health concern and may be particularly relevant among security personnel because of irregular work schedules, occupational demands, sustained vigilance, and inadequate rest. This study was conducted to determine the prevalence of insomnia and its association with selected sociodemographic and health-related factors among security personnel in a tertiary care government hospital in south India. Methods: A cross-sectional study was conducted among 100 security personnel. Sociodemographic characteristics, including age, gender, income, education, and marital status, and health-related characteristics, including body mass index, diabetes, and hypertension, were assessed using a structured questionnaire. Insomnia was assessed using the Insomnia Severity Index (ISI). Associations between insomnia and selected variables were examined using the chi-square test. Results: Sub-threshold insomnia was present in 32% of participants, while no cases of moderate or severe insomnia were identified. Significant associations were observed between insomnia and gender (p=0.019) and income level (p=0.020). No significant associations were observed with age, education, marital status, or metabolic health status. Self-reported Diabetes and Hypertension were present in 10% and 4% of participants, respectively, while 74% were classified as obese. Conclusions: Although moderate or severe insomnia was not observed, nearly one-third of security personnel had sub-threshold insomnia. Workplace-based sleep hygiene interventions may be beneficial, particularly among women and lower-income employees. Further studies incorporating objective sleep assessments and detailed evaluation of shift schedules are warranted.
Background Insomnia and poor sleep are highly prevalent among university students and are linked to impaired mental and physical health, reduced cognitive functioning, and lower academic performance. Existing interventions for insomnia include cognitive behavioural therapy for insomnia (CBT-I), mindfulness, and sleep hygiene. CBT-I is widely recognised as effective but remains difficult to access for the general population and lack scalability, long-term follow-up, and feasibility in non-clinical settings. Pragmatic, group-based approaches that can be delivered within the university and are tailored to help students develop self-endorsed routines and competencies for better sleep, creating sustainable changes that extend beyond the program period, is needed. Methods This study is a pragmatic, two-arm randomised controlled trial evaluating the effectiveness of a group-based sleep program for university students with insomnia symptoms. A total of 118 full-time students aged 18–30 meeting diagnostic criteria for chronic insomnia disorder will be randomized (1:1) to either the intervention or a wait-list control group. The intervention comprises 12 weekly 90-minute group sessions followed by nine monthly follow-up meetings, facilitated by trained coaches. The program integrates core elements of CBT-I with motivational and coping strategies, delivered in an interactive and peer-supported format. The primary outcome is change in insomnia severity measured by the Bergen Insomnia Scale. Secondary outcomes include subjective and objective sleep measures, mental health, cognitive performance, academic attainment, health behaviours, physical fitness, and biological markers. Assessments will be conducted at baseline, post-program (12 weeks), and 12 months. A mixed methods process evaluation will assess fidelity, acceptability, mechanisms of impact, and contextual influences. Discussion This trial will be among the first pragmatic evaluations of a structured, group-based sleep intervention tailored to university students with insomnia. Key strengths include the randomised controlled design, comprehensive outcome assessment, 12-month follow-up, and embedded process evaluation. If effective, the program could provide a feasible, scalable, and cost-effective model to improve sleep and therein health, and academic performance in young adults in higher education.
Aim and background: Sleep is a biological necessity essential for physical and mental well-being, yet many young adults, particularly college students, experience sleep disruptions due to major life changes and poor sleep hygiene. This study aimed to identify the determinants of insomnia among undergraduate female students in a rural area of Haryana.Materials and methods: A mixed-method study was conducted between May 2024 and May 2025 involving 340 undergraduate female students.Quantitative data were collected using a semi-structured socio-demographic questionnaire and the insomnia severity index (ISI).Qualitative insights were gathered through in-depth interviews (IDI) with 15 students identified with clinically significant insomnia.Statistical analysis was performed using SPSS software version 26, with significance set at p < 0.05.Results: The prevalence of clinically significant insomnia in the cohort was 5.6% (n = 19).Quantitative analysis revealed that the participant's state of origin was the only statistically significant socio-demographic determinant (p = 0.031).Students from states other than Haryana reported a significantly higher insomnia prevalence (12.5%) compared to those from Haryana (4.2%).Factors such as age, socio-economic status, and type of occupancy showed no significant correlation.Qualitative themes highlighted academic performance, overuse of electronic devices, social environment, and psycho-social stress as primary triggers. Conclusion: While the prevalence of insomnia is relatively low, it is significantly driven by academic transitions, excessive screen time, and communal living challenges.A notable awareness-action gap exists, as students recognize the importance of sleep but prefer self-management or mindfulness over professional clinical help.
Abstract Objectives We hypothesized that benzodiazepine/z-drug (BZRA) dose, insomnia symptoms, and obstructive sleep apnea (OSA) risk are associated with cognitive performance above and beyond other modifiable dementia risk factors. Methods In this cross-sectional study, we used baseline data from recruited patients without dementia from one VA site and one academic site to analyze regression models based on previously published risk factors for cognitive impairment, adapted to include sleep and benzodiazepine usage interactions. Changes in R 2 and model fit (F-tests) were examined for four nested regression models predicting Mini-Mental State Examination (MMSE), Trail Making Test (TMT)-A, TMT-B, and Digit Symbol Substitution Test (DSST). Base Models included common dementia risk factors. Model-2 = Base + Insomnia Severity Index; Model-3 = Model-2 + dose; Model-4 = Model-3 + OSA risk; and Model-5 = Model-4 + OSA-BZRA. Results R 2 for the base model predicting MMSE was 0.139 (n = 313), 0.241 for TMT-A (n = 298), 0.229 for TMT-B (n = 287), and 0.189 for DSST (n = 285). Model-5 R 2 was 0.142 for MMSE, 0.246 for TMT-A, 0.238 for TMT-B, and 0.190 for DSST, with all F-test p-values >.145. Conclusions Sleep disorder variables did not significantly explain additional variation in cognition performance. Studies with a wider range of cognitive performance are needed.
This study aimed to investigate the association between insomnia severity and systemic inflammatory indices, specifically neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and systemic immune-inflammation index (SII), in geriatric individuals aged 80 years and older, a subgroup for whom evidence on this relationship remains scarce. This cross-sectional study included 384 patients aged ≥ 80 years attending a geriatric outpatient clinic. Insomnia severity was assessed using the Insomnia Severity Index (ISI). Inflammatory markers, namely the NLR, PLR, and SII were derived from routine hematological parameters and analyzed via correlation analyses and multivariable linear regression. Insomnia was identified in 38.5% of participants. ISI scores showed significant positive correlations with NLR, PLR, SII, chronic disease burden, and medication count (all p < 0.001). Inflammatory marker levels were significantly elevated in those with insomnia ( p < 0.001). After full covariate adjustment, SII (β = 0.07, p < 0.001) and NLR (β = 0.666, p = 0.004) remained independent predictors of insomnia severity, whereas PLR lost significance. Insomnia is highly prevalent in the oldest-old and independently associated with systemic inflammatory burden. SII and NLR, as accessible and non-invasive parameters, may serve as pragmatic biomarkers for stratifying insomnia severity in geriatric practice. Longitudinal studies are warranted to establish causality and evaluate anti-inflammatory interventions.

Chronic insomnia frequently co-exists with nocturia, contributing to impaired sleep quality and reduced health-related quality of life. A recent randomized cross‑over study found that daridorexant improved sleep duration, insomnia severity, daytime functioning, and nocturnal voiding in patients with chronic insomnia and nocturia. To complement these quantitative outcomes, the study incorporated qualitative free‑text surveys capturing patients’ lived experiences, addressing an existing gap in patient‑reported insights for this multifaceted condition. Here, we report these qualitative findings assessed at baseline and post‑treatment. This double-blind, placebo-controlled, two-way crossover trial of patients with insomnia disorder and comorbid nocturia included 60 adults ≥ 55 years with ≥ 3 months of insomnia symptoms, an Insomnia Severity Index ≥ 13, and ≥ 3 nocturnal voids per night. Patients were randomized to daridorexant 50 mg or placebo for 4 weeks, followed by crossover after a washout period. Free‑text surveys at baseline and after each treatment period captured patient‑reported impacts of their conditions on daily life and perceived changes during treatment. At baseline, patients reported broad negative impacts across physical, emotional, cognitive, and daily functioning domains. During treatment, more patients reported improvements in sleep (87% vs. 50%) and nocturia (79% vs. 32%) on daridorexant than on placebo, and improvements in functioning, particularly physical, cognitive, emotional, and daily activities. Patients reported meaningful improvements with daridorexant 50 mg, including better sleep, reduced nocturia, and enhanced physical, cognitive, and emotional functioning. Daridorexant consistently elicited more positive patient‑reported changes than placebo. These findings highlight the value of free‑text surveys for capturing patient‑centered outcomes. NCT05597020.
Primary insomnia is a common sleep–wake disorder characterized by impaired sleep initiation and maintenance with associated daytime dysfunction, while current pharmacological treatments are often limited by safety and tolerability concerns. This study examined the independent and combined effects of a sensory-mediated non-pharmacological intervention using Viola odorata L. and an oral botanical preparation derived from Papaver somniferum L. seeds in adults with primary insomnia. This exploratory, prospective, randomized, three-arm, component-based integrative trial enrolled 90 participants aged 18–65 years, randomized equally to Viola odorata –based Natūl therapy, Papaver somniferum seed powder, or combination therapy and treated for four weeks. Sleep outcomes—including Insomnia Severity Index (ISI), Pittsburgh Sleep Quality Index (PSQI), daytime dysfunction (VAS), and number of nocturnal awakenings—were assessed at baseline and post-intervention and analysed using a per-protocol framework with non-parametric methods. Recurrence rate has been assessed after three months of treatment. All three interventions were associated with significant improvements across all sleep parameters (all p < 0.001), with large within-group effect sizes (r ≈ 0.86 – 0.89). Between-group comparisons of change scores demonstrated significantly greater improvements with combination therapy compared with either monotherapy (η 2 ≈ 0.21–0.26). Viola odorata –based Natūl monotherapy produced improvements comparable to oral Papaver somniferum therapy for most outcomes. No clinically meaningful safety concerns were observed. Sensory-mediated therapy using Viola odorata L. and oral administration of Papaver somniferum L. seeds are effective in the management of primary insomnia, with combination therapy providing additive sustained with less recurrence rate and supporting a graded, personalized integrative treatment approach.
Background and Objective The baseline sleep phenotype of early Alzheimer’s disease (AD) patients entering lecanemab therapy is poorly characterized. Methods Forty-four consecutive early AD patients (mild cognitive impairment or mild dementia) awaiting lecanemab underwent attended polysomnography, the Epworth Sleepiness Scale, Insomnia Severity Index (ISI), Pittsburgh Sleep Quality Index (PSQI), and Beck Depression Inventory (BDI). Forty-four controls without documented cognitive disorder, referred for polysomnography by the study’s sleep specialist over the same period, were balanced on sex, age and apnea–hypopnea index. Comparisons used independent-samples tests with Benjamini–Hochberg false discovery rate correction within pre-specified families. Results Obstructive sleep apnea was prevalent (61.4% moderate-to-severe) despite a body mass index of 22.5 kg/m2; groups were balanced (all p>0.15). Early AD patients showed greater wake after sleep onset (114.0 min vs. 78.7 min; q=0.006), lower sleep efficiency (67.7% vs. 77.2%; q=0.006) and shorter total sleep time (284.0 min vs. 321.1 min; q=0.039) with comparable recording time, and lower REM arousal index (13.9/h vs. 21.3/h; q=0.012). Differences were moderate (|g| 0.49–0.72); sleep-stage distribution and total arousal index did not differ. ISI (3.8 vs. 13.8) and PSQI (4.1 vs. 10.0) were nonetheless lower in AD (both q<0.001; |g| 1.52–1.86); BDI did not differ. Conclusions Early AD patients awaiting lecanemab therapy had a high prevalence of sleep apnea and reduced objective sleep continuity despite reporting fewer subjective sleep complaints than controls, a contrast that may partly reflect the symptomatic sleep-clinic comparator. Objective sleep assessment before initiating lecanemab therapy may therefore be considered.
Background and Objective The Adaptive Cognition and Behaviors about Sleep-6 (ACBS-6) comprises two factors, adaptive sleep-related cognitions (Items 1–4) and behaviors (Items 5 and 6). We aimed to develop a shortened version. Methods Four anonymous online surveys of Korean adults were analyzed. In a general-population derivation sample (n=600), items most relevant to adaptive sleep cognition and most strongly correlated with the Insomnia Severity Index (ISI) were selected. Their reliability and validity were checked among three independent validation samples such as the general population (n=600), respondents reporting insomnia (n=600), and respondents reporting insomnia and taking hypnotics (n=500). Internal consistency was estimated with the Spearman–Brown-corrected split-half coefficient. Mediation analyses explored whether adaptive cognition mediates the association between insomnia severity and the two-item questionnaire version of the discrepancy between desired time in bed and desired total sleep time (DBSTq-2).Results ACBS-6 Items 2 and 4 were selected as the Adaptive Cognition about Sleep-2 (ACS-2). The split-half coefficient ranged from 0.59 to 0.75. The ACS-2 score was inversely correlated with the ISI in all four samples (r=-0.14 to -0.23, all p<0.01), and its correlation with the Dysfunctional Beliefs and Attitudes about Sleep-6 items ranged from -0.11 to 0.08. The ACS-2 did not mediate the association between the DBSTq-2 and the ISI.Conclusions The ACS-2 is a feasible brief rating scale measuring adaptive cognition about sleep. Adaptive cognition appears to be distinct from dysfunctional beliefs about sleep, and not the opposite pole of the same dimension.
Objectives This study aimed to describe shift work patterns in Europe using a data-driven approach and evaluate their impact on adverse sleep outcomes. Methods We analyzed 6245 participants from the SHIFT2HEALTH survey across eight European countries, who provided information on work schedules, shift work metrics, sleep quality and timing (MCTQshift) across different shifts, and chronic insomnia (Insomnia Severity Index). We used mixtures of von Mises-Fisher models to identify shift clusters based on shift start and end times and evaluated differences in shift-specific sleep duration and quality using Bayesian multilevel models. Associations of shift metrics with short average sleep duration and chronic insomnia, adjusting for confounders, were evaluated using Bayesian logistic regression. Results We identified five shift clusters: day, evening and night (mean shift lengths: 7.8, 7.9 and 9.5 hours, respectively), long day and long evening (mean lengths: 10.5 and 12.8 hours, respectively). Sleep duration and quality varied between clusters, with the shortest duration [mean 6.00, 95% credible interval (CI) 5.95–6.06 hours] and poorest quality (29.3% “poor”/“very poor”) sleep reported between night shifts. Compared to day workers, greater chronic insomnia odd ratios (OR) were observed among participants with >90% night shifts (OR 1.26, 95% CI 1.01–1.58), rotating shifts (OR 1.42, 95% CI 1.22–1.65) and with longer night work history (OR ≥16 years 1.28, 95% CI 1.08–1.52). These variables and longer weekly working hours [OR 6.10h increase 1.13, 95% CI 1.04–1.22) were associated with short sleep. Conclusions Data-driven analyses of work schedules may provide a more comprehensive evaluation of the impact of real-world shift work patterns on sleep.
