Background: Patients with chronic kidney disease undergoing hemodialysis frequently experience physical and psychological complaints that may affect their sleep quality. Uremic pruritus, restless legs syndrome, anxiety, and post-hemodialysis fatigue are potential factors that may disrupt sleep quality. Objective: This study aimed to identify the factors influencing the sleep quality of patients with chronic kidney disease undergoing hemodialysis at Bahteramas Regional General Hospital, Southeast Sulawesi Province. Methods: This study employed a quantitative analytical approach with a cross-sectional design. The sample consisted of 83 respondents. Data were collected using the Pittsburgh Sleep Quality Index (PSQI), 5-D Itch Scale, International Restless Legs Syndrome Rating Scale (IRLS), Hospital Anxiety and Depression Scale-Anxiety (HADS-A), and a post-hemodialysis fatigue assessment instrument. Data were analyzed using the Chi-square test and binary logistic regression. Results: Bivariate analysis showed that uremic pruritus (p = 0.046), restless legs syndrome (p = 0.015), anxiety (p = 0.029), and post-hemodialysis fatigue (p = 0.049) were statistically significant factors associated with sleep quality. Simultaneously, the four variables had a statistically significant effect on sleep quality (p = 0.007), with a Nagelkerke R Square value of 0.217. In the multivariable analysis, only anxiety showed a statistically significant independent effect (p = 0.019). Conclusion: Uremic pruritus, restless legs syndrome, anxiety, and post-hemodialysis fatigue were statistically associated with sleep quality in the bivariate analysis. The four variables were jointly significant in the multivariable model, whereas only anxiety remained statistically significant after adjustment for the other variables.
Background: Prolonged screen exposure, after-hours connectivity, and sedentary work are linked to poor sleep and occupational burnout among Indian working professionals. Ayurvedic Dinacharya regulates waking, Vyayama, Bhojana Vidhi, and Nidra Vidhi; however, digital-era disruption of these domains among working adults remains unstudied. This study aimed to assess digital lifestyle behaviours mapped to Dinacharya principles as behavioural markers of Pragyaparadha, in relation to sleep quality and occupational burnout among working adults, and to identify behaviours independently associated with poor sleep and higher burnout. Methods: A cross-sectional analytical survey (STROBE-reported) was conducted at the All-India Institute of Ayurveda (AIIA), New Delhi, during (January 2026 to July 2026), among 400 working adults aged 25-40 years with ≥1 year of occupational experience. A structured proforma captured sociodemographic details and Dinacharya-mapped digital behaviours across five domains (morning routine, bedtime device use, after-hours connectivity, screen breaks/sedentary time, mealtime practices). Sleep quality and burnout were assessed using the PSQI and Copenhagen burnout inventory. Results: Of 400 participants, 56.8% had poor sleep quality, 61% personal burnout, and 55.8% work-related burnout. Bedtime device use and inadequate screen breaks/sedentary behaviour independently predicted poor sleep (aOR 1.76 and 1.98). After-hours connectivity, sedentary behaviour, and mealtime digital engagement predicted personal burnout; bedtime device use, sedentary behaviour, mealtime digital engagement, and digital overload predicted work-related burnout (all p<0.05). Conclusions: Digital lifestyle behaviours reflect patterns of Pragyaparadha in daily digital conduct. Interpreted through Dinacharya and Nidra, the findings support a culturally grounded, preventive Digital Dinacharya framework for digital-era occupational health.
Purpose: This study assessed the prevalence of sleep disturbance and associated factors in patients with hepatocellular carcinoma (HCC) receiving hepatic arterial infusion chemotherapy (HAIC) combined with targeted therapy and immunotherapy. Patients and Methods: A cross-sectional survey enrolled 230 patients undergoing HAIC combined with targeted therapy and immunotherapy at Sun Yat-sen University Cancer Center (July–November 2025). Sleep, psychological distress, pain, appetite, and quality of life were assessed using the PSQI, HADS, NRS, VAS, and EORTC QLQ-C30, respectively, along with prior HAIC cycles. Logistic regression identified factors associated with sleep disturbance. Results: The prevalence of sleep disturbance (PSQI > 5) was 72.6% (167/230). Univariate analysis identified a greater number of prior HAIC cycles, higher levels of depressive and anxiety symptoms, and lower quality of life and appetite as significant factors. In multivariate analysis, four factors remained independently associated with sleep disturbance: prior HAIC cycles (OR = 1.66, 95% CI: 1.25– 2.20, P < 0.001), anxiety symptoms (OR = 1.27, 95% CI: 1.12– 1.44, P < 0.001), depressive symptoms (OR = 1.23, 95% CI: 1.03– 1.47, P = 0.021), and appetite sensations (OR = 0.93, 95% CI: 0.91– 0.96, P < 0.001), which showed an inverse association with sleep disturbance. The model showed an apparent area under the ROC curve of 0.92 (95% CI 0.87– 0.96), with an optimism-corrected AUC of 0.91 after bootstrap resampling. Conclusion: Sleep disturbance was highly prevalent and was independently associated with cumulative treatment burden, psychological distress and impaired appetite. These cross-sectional findings support routine sleep screening together with psychological and nutritional assessment in this population; whether modifying these factors improves sleep quality requires prospective interventional studies. Background and Purpose: Non-suicidal self-injury (NSSI) is a global concern in adolescent mental health. Rural boarding middle school students, as a unique and vulnerable population, remain understudied regarding the potential heterogeneity and longitudinal trajectories of NSSI. This study aimed to identify latent classes of NSSI among this group, examine their transition patterns over time, and evaluate the effectiveness of an emotion management group intervention. Methods: Study 1 cross-sectionally surveyed 927 adolescents; we split samples into non-NSSI and NSSI subgroups and ran latent profile analysis (LPA) only among participants with NSSI ( n =285). Study 2 conducted a 6-month follow-up of 550 participants and applied latent transition analysis (LTA) to examine class shifts and predictive factors. Study 3 was a randomized controlled trial in which 30 students from the High-NSSI received an 8-week emotion-management group intervention. Intervention effects were evaluated using repeated-measures ANCOVA with demographic covariates. Results: Within the NSSI subgroup ( n = 285), two classes were identified: a High-NSSI class ( n = 21, 7.4%) and a Low-NSSI class ( n = 264, 92.6%). Poor sleep quality and bullying victimization independently predicted High-NSSI membership. Longitudinal analysis revealed high stability in the Low-NSSI class (91%) and high fluidity in the High-NSSI class (83% transitioning to low-risk). The emotion-management intervention significantly reduced both NSSI frequency and negative affect. Discussion: These findings demonstrate that clinically meaningful NSSI subtypes exist among rural boarding adolescents and that sleep quality and bullying are key modifiable risk factors. The results support a stepped-care approach in school settings, where the High-NSSI class requires intensive intervention and the Low-NSSI class may benefit from less intensive, universal supports. The emotion-management group intervention is an effective school-based program for the most severe NSSI subtype. Chronobiology studies indicate that individuals are regulated by three timers: social, solar, and biological. Environmental and behavioral changes can modify the sleep–wake cycle. Many university students adopt nighttime study habits, interfering with the circadian rhythm. Sleep restriction or poor sleep quality compromises memory and significantly reduces academic performance. This study aimed to verify whether there is an association between sleep quality and academic performance among nursing and medical students at a public university in Alagoas, Brazil. This is an observational, cross-sectional, and quantitative study conducted with 137 Nursing and Medical students, selected through stratified probabilistic sampling. Data collection included a sociodemographic questionnaire to characterize the sample, the Pittsburgh Sleep Quality Index (PSQI), the Munich Chronotype Questionnaire (MCTQ), and academic self-assessment. Data were analyzed for normality, and associations were tested using Pearson and Spearman correlations (SciPy – Python). Among the 137 students, 65.7% were women, 70.1% were between 19 and 24 years old, 67.9% rated their academic performance between 7.0 and 8.9, and 66.4% presented poor sleep quality. Negative correlations between the PSQI and self-assessment variables (r = -0.368; p < 0.001 for Pearson; r = -0.360; p < 0.001 for Spearman) reinforce the importance of sleep quality in the academic context. The findings highlight the urgency of interventions that promote sleep hygiene education and strategies to improve rest quality, reducing the likelihood of problems that may negatively impact students’ academic performance.
ABSTRACT Type 2 diabetes mellitus is a chronic metabolic disease with a rapidly increasing prevalence worldwide and is frequently accompanied by microvascular complications, particularly diabetic neuropathy. Diabetic neuropathy often presents with sensory symptoms such as pain, paresthesia, and numbness, which tend to worsen at night and may interfere with sleep quality. Disturbed sleep in patients with type 2 diabetes mellitus can adversely affect daily functioning, glycemic control, and overall quality of life. However, previous studies examining the relationship between diabetic neuropathy and sleep quality have reported inconsistent findings. This study aims to determine the relationship between the occurrence of diabetic neuropathy and sleep quality among patients with type 2 diabetes mellitus at RS Siti Khodijah Sepanjang. This study employed an analytical observational design with a cross-sectional approach. Data were collected using primary data from questionnaires and secondary data from medical records of patients with type 2 diabetes mellitus. Diabetic neuropathy was assessed based on clinical evaluation, while sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI). The findings demonstrate an association between the occurrence of diabetic neuropathy and impaired sleep quality in patients with type 2 diabetes mellitus. Patients experiencing diabetic neuropathy tended to have poorer sleep quality, characterized by prolonged sleep latency, reduced sleep efficiency, increased sleep fragmentation, and shorter sleep duration. Diabetic neuropathy is associated with decreased sleep quality in patients with type 2 diabetes mellitus. These findings highlight the importance of considering sleep quality as part of comprehensive management in patients with diabetic neuropathy to improve overall health outcomes. Background Insomnia and poor sleep are highly prevalent among university students and are linked to impaired mental and physical health, reduced cognitive functioning, and lower academic performance. Existing interventions for insomnia include cognitive behavioural therapy for insomnia (CBT-I), mindfulness, and sleep hygiene. CBT-I is widely recognised as effective but remains difficult to access for the general population and lack scalability, long-term follow-up, and feasibility in non-clinical settings. Pragmatic, group-based approaches that can be delivered within the university and are tailored to help students develop self-endorsed routines and competencies for better sleep, creating sustainable changes that extend beyond the program period, is needed. Methods This study is a pragmatic, two-arm randomised controlled trial evaluating the effectiveness of a group-based sleep program for university students with insomnia symptoms. A total of 118 full-time students aged 18–30 meeting diagnostic criteria for chronic insomnia disorder will be randomized (1:1) to either the intervention or a wait-list control group. The intervention comprises 12 weekly 90-minute group sessions followed by nine monthly follow-up meetings, facilitated by trained coaches. The program integrates core elements of CBT-I with motivational and coping strategies, delivered in an interactive and peer-supported format. The primary outcome is change in insomnia severity measured by the Bergen Insomnia Scale. Secondary outcomes include subjective and objective sleep measures, mental health, cognitive performance, academic attainment, health behaviours, physical fitness, and biological markers. Assessments will be conducted at baseline, post-program (12 weeks), and 12 months. A mixed methods process evaluation will assess fidelity, acceptability, mechanisms of impact, and contextual influences. Discussion This trial will be among the first pragmatic evaluations of a structured, group-based sleep intervention tailored to university students with insomnia. Key strengths include the randomised controlled design, comprehensive outcome assessment, 12-month follow-up, and embedded process evaluation. If effective, the program could provide a feasible, scalable, and cost-effective model to improve sleep and therein health, and academic performance in young adults in higher education.
Background Sleep problems are common among university students due to academic stress, lifestyle factors, and psychological distress and may adversely affect physical and mental health, academic performance, and social functioning. Mindfulness-based interventions (MBIs) have increasingly been used as a non-pharmacological approach to improve sleep health; however, previous findings remain inconsistent, and evidence specifically addressing sleep quality in university students remains limited. This study aimed to systematically evaluate the effects of MBIs on sleep quality in university students and explore factors that may account for variations in intervention effects. Methods We searched seven databases—PubMed, Web of Science, EBSCO, the Cochrane Library, Scopus, Embase, and ScienceDirect—from inception to June 26, 2026. Randomized controlled trials (RCTs) evaluating MBIs for sleep quality in university students were included. Two reviewers independently conducted study selection, data extraction, and risk-of-bias assessment using the RoB 2 tool. Meta-analysis was conducted in Stata 17.0 using a random-effects model. Subgroup analyses, sensitivity analyses, publication bias assessment, and GRADE assessment were also performed. Results Ten RCTs involving 638 university students were included, with 335 participants in the intervention groups and 303 in the control groups. MBIs significantly improved sleep quality [SMD = −0.97, 95% CI (−1.43, −0.51), p < 0.001], although substantial heterogeneity was observed ( I 2 = 85.1%). Subgroup analyses revealed statistically significant between-group differences with respect to sleep-outcome measurement instrument ( p = 0.001), intervention type ( p = 0.009), and session duration ( p = 0.030), indicating that these variables may partially account for the observed heterogeneity. Sensitivity analyses supported the robustness of the pooled results. Egger’s test revealed no evidence of significant publication bias ( p = 0.270). The certainty of the evidence was rated as low according to the GRADE framework. Conclusion MBIs may improve sleep quality in university students. However, given the substantial heterogeneity and low certainty of evidence, further high-quality RCTs are needed to confirm these findings. Aim and background: Excessive daytime sleepiness (EDS) is reported far less often than poor nocturnal sleep quality in Indian medical students. We aimed to estimate EDS prevalence and its independent correlates (primary), and to compare these with poor sleep quality correlates in the same cohort and relate both to examination performance (secondary). We hypothesized that sleep hygiene behavior, not global sleep quality category, would dominate.Materials and methods: Prospective cross-sectional study of 354 of 450 eligible MBBS students (78.7%) at a Southern Indian college, sampled by stratified proportional randomization.Under supervised standardized administration, they completed the Epworth Sleepiness Scale (ESS; EDS >10), Pittsburgh Sleep Quality Index (PSQI), and Sleep Hygiene Index (SHI), with screen time, stress, mood, lifestyle, clinical covariates, and the latest university examination percentage.Excessive daytime sleepiness correlates were examined by logistic regression with a pre-specified confounder-adjusted sensitivity model. Results: Excessive daytime sleepiness affected 21.5% [95% confidence interval (CI) 17.5-26.0],more often among poor than good sleepers (27.7% vs 18.6%).Adjusted [area under the receiver operating characteristic curve (AUC) 0.773, 0.714-0.832],only poorer sleep hygiene remained independent [odds ratio (OR) 1.12 per point, 1.07-1.18],unchanged after adjustment for all measured confounders (1.12, 1.06-1.17).Examination percentage did not differ by sleep status (67.33 ± 5.16 vs 68.07 ± 5.72; p = 0.229); academic stage and sex, not sleep, predicted marks (R² = 0.152).Conclusion: In this cross-sectional sample, EDS was associated more closely with sleep hygiene behavior than with global sleep quality category, and the two had distinct correlation profiles.Screening for sleepiness alongside sleep quality, and addressing bedtime device use, merit consideration in student health services. These are associations only; no direction of effect can be inferred.Clinical significance: Measuring EDS alongside global sleep quality identifies a partly distinct at-risk group, for whom brief counselling on bedtime device use may be more specific than generic sleep hygiene advice.
Background: Patients with pulmonary tuberculosis (TB) undergoing treatment may experience poor sleep quality and fatigue, potentially affecting daily activities and quality of life. Objective: This study aimed to examine the relationship between sleep quality and fatigue levels among patients with pulmonary TB undergoing treatment. Methods: This observational analytic study employed a cross-sectional design. The study was conducted in April 2026 at Primary Health Center X, Deli Serdang Regency, Indonesia, involving 21 participants selected through total sampling. Sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI), while fatigue was measured using the Fatigue Severity Scale (FSS). Spearman's correlation test was planned with a significance level of α = 0.05. Results: According to the data presented in the manuscript, 14 participants (66.7%) had poor sleep quality, while 7 participants (33.3%) had good sleep quality. Moderate fatigue was the most frequent category, reported in 8 participants (38.1%), followed by severe fatigue in 7 participants (33.3%) and mild fatigue in 6 participants (28.6%). Cross-tabulation indicated that all participants classified as having severe fatigue belonged to the poor sleep quality group. Conclusion: The descriptive findings suggest a tendency toward greater fatigue among patients with poor sleep quality. However, the statistical significance of this relationship remains unconfirmed pending verification of the analysis using individual participant data
Background Stroke is a leading cause of disability worldwide and is frequently accompanied by psychological and functional impairments. Acceptance and Commitment Therapy (ACT), a third-wave behavioral intervention focusing on psychological flexibility, has been increasingly applied in stroke rehabilitation. Although previous reviews have suggested beneficial effects of ACT in stroke patients, whether these benefits persist in the short term after the intervention ends, and the certainty of the evidence, remain unclear. Methods A systematic review and meta-analysis was conducted according to PRISMA guidelines and registered in PROSPERO (CRD420261325108). Randomized controlled trials evaluating ACT in stroke patients were included. Pooled effects were calculated using random-effects models, with separate analyses for post-intervention and follow-up outcomes. Sources of heterogeneity were explored using subgroup analyses and meta-regression. Risk of bias was assessed using the Cochrane Risk of Bias 2 tool, and certainty of evidence was evaluated using the GRADE framework. Results Twenty-two RCTs involving 1,975 participants were included. ACT may be associated with reductions in depressive symptoms (post-intervention: SMD = −1.32, 95% CI: −1.80 to −0.84; follow-up: SMD = −0.76, 95% CI: −1.07 to −0.45) and anxiety symptoms (post-intervention: SMD = −1.36, 95% CI: −1.90 to −0.82; follow-up: SMD = −0.51, 95% CI: −1.42 to 0.40, not significant); depressive symptoms remained statistically significant at follow-up. Heterogeneity was considerable for both outcomes at post-intervention ( I 2 = 93.68% for depression and 93.60% for anxiety), so the pooled estimates represent average effects across studies. ACT may also be associated with improvements in psychological flexibility, quality of life, and sleep quality; however, these outcomes were based on a limited number of studies and showed considerable heterogeneity ( I 2 = 74.68 to 92.26%), so they should be interpreted cautiously. However, no statistically significant effects were observed for activities of daily living, neurological function, or hope. ACT did not appear to increase dropout rates in the available studies ( K = 16; RR = 0.99, 95% CI: 0.58–1.69). Meta-regression analyses indicated that intervention duration may not be a significant moderator of treatment effects. The certainty of evidence ranged from very low to moderate, mainly due to risk of bias and inconsistency. Conclusion Acceptance and Commitment Therapy may be a useful adjunctive intervention for improving psychological outcomes in people with stroke. However, evidence for functional recovery remains insufficient. Future high-quality RCTs with standardized protocols, larger samples, and longer follow-up are needed to confirm the long-term effectiveness of ACT. Sleep quality is fundamental to college students’ physical and psychological health and academic functioning. Amid increasing digitalization and stress, sleep problems remain prevalent among college students. Previous research has shown that physical activity, cognitive processing styles, and behavioral regulation patterns are associated with sleep outcomes, but these variables have often been examined separately, with limited attention to their integrated association structure. Based on cross-sectional survey data, this study used latent-variable structural equation modeling to examine the sequential indirect association pattern among physical activity, negative attentional bias, bedtime procrastination, and sleep quality. Using cluster sampling, data were collected from 921 college students from four universities in East China (439 males, 482 females; M_age = 20.34, SD = 1.59). Measures included the Physical Activity Rating Scale (PARS-3), Negative Attentional Bias Scale, Bedtime Procrastination Scale, and Pittsburgh Sleep Quality Index (PSQI). Structural equation modeling and bootstrap methods examined indirect associations. Results indicated that physical activity was significantly associated with lower PSQI scores, indicating better sleep quality (B_total = −0.072, β _total = −0.440, p < 0.001). Negative attentional bias and bedtime procrastination both showed significant partial indirect associations. The simple indirect associations through negative attentional bias and bedtime procrastination accounted for 20.8 and 15.3% of the total association, respectively. The sequential indirect association “physical activity → negative attentional bias → bedtime procrastination → sleep quality” was also significant, although its proportion was relatively small (4.2%). Overall, the total indirect association accounted for 40.3% of the total association. Multi-group structural analysis indicated that the association structure was broadly comparable across gender groups, although associations involving bedtime procrastination appeared stronger among female students. By integrating cognitive processing and behavioral regulation perspectives, this study provides a more integrative view of the association between physical activity and sleep quality. The findings suggest that college students’ sleep problems may be embedded within a broader system of health behaviors and psychological processes. Given the cross-sectional design, future longitudinal or experimental studies should examine the dynamic associations among these variables.
This study evaluated the reliability and validity of a context-aware sleep holistic cycle gate algorithm and explored its potential for artificial intelligence (AI)-based personalized sleep management in digital health-care. A 6-week prospective observational study was conducted in 25 adults aged 20–65 years. Participants recorded daytime activities, eve-ning behaviors, presleep environmental factors, sleep outcomes, and feedback adherence using a mobile application. Five sleep-related indices were calculated, and a context-aware gate algorithm comprising five domains was applied. Reliability was assessed through internal consistency and reproducibility analyses, while validity was examined using correlations with sleep quality indicators and receiver operating characteristic curve analyses. Data completion and adherence rates were 80.3%±9.8% and 72.7%±11.6%, respectively. The sleep readiness index was positively correlated with subjective sleep quality (r=0.53, P<0.01). The algorithm demonstrated moderate predictive performance (area under the curve=0.73), with the environment and presleep gates showing the highest accuracy. Following personalized feedback, sleep onset latency decreased by 12.6%, and perceived sleep quality improved significantly (P<0.05). In conclusions, the context-aware sleep holistic cycle gate algorithm demonstrated acceptable reliability and validity for predicting sleep quality. These findings suggest that the proposed framework may serve as a foundational platform for the development of AI-driven personalized sleep management systems and future digital healthcare applications related to sleep rehabilitation and health promotion.
Shift work is associated with irregular eating behaviors, increased energy intake, and impaired sleep quality. Poor dietary patterns may elevate systemic inflammation, contributing to long-term metabolic and cardiovascular risks. The aim of this study was to investigate differences in the Dietary Inflammatory Index (DII), sleep quality, dietary intake, and anthropometric and body composition measures between fixed daytime workers and rotating shift workers and to examine the associations among these parameters. This cross-sectional study included 52 male workers employed at a defense industry factory in Ankara, Türkiye, who worked either fixed daytime or rotating shifts. Dietary intake was assessed using 24-h dietary recalls for three non-consecutive days, from which macronutrient and micronutrient intakes were calculated. DII scores were computed to assess the inflammatory potential of the diet. Sleep quality was evaluated using the Pittsburgh Sleep Quality Index (PSQI). Anthropometric measurements, body composition, blood pressure, and biochemical parameters, including fasting plasma glucose, lipid profile (total cholesterol, HDL-C, LDL-C, and triglycerides), C-reactive protein, and homocysteine, were measured using standardised protocols. Rotating shift workers had significantly higher total energy intake and higher daily intakes of fructose, total fat, saturated and monounsaturated fatty acids, cholesterol, and several micronutrients, including vitamin A, folate, iron, calcium, and phosphorus compared with fixed daytime workers (p < 0.05). Mean DII scores did not differ significantly between the groups (Fixed daytime: 0.2 ± 1.86; rotating shift: −0.2 ± 2.25). DII scores were positively correlated with global PSQI score (r = 0.278, p = 0.046), the PSQI sleep duration component score (r = 0.319, p = 0.021), and daytime dysfunction component score (r = 0.287, p = 0.039); however, the association between DII and overall sleep quality was no longer statistically significant after adjustment for potential confounding factors (age, BMI, smoking status, physical activity, and work schedule). These findings suggest that dietary habits, pro-inflammatory dietary patterns, and poor sleep quality may represent co-existing factors associated with metabolic risk, and addressing these factors may contribute to better overall metabolic health.
BackgroundSleep disruption is common during postoperative hospitalisation and may be influenced by pain, overnight clinical care, and environmental factors. This quality improvement project assessed sleep quality among postoperative paediatric inpatients and identified potentially modifiable contributors to sleep disturbance. MethodologyA clinical audit was conducted on a paediatric inpatient ward between April and June 2026.Children aged 0-18 years admitted following elective or emergency orthopaedic surgery and requiring at least one overnight postoperative stay were eligible.Parents or carers completed a questionnaire after the child's first postoperative night assessing sleep quality, nighttime awakenings, causes of sleep disturbance, ward noise and lighting, clustering of overnight care, assessment of sleep needs, and parent/carer sleep.Responses were analysed descriptively using frequencies and percentages. ResultsIn total, 19 questionnaires were included: 14 from children accommodated in multi-bed bays and five from children in individual cubicles. In bays, eight of 14 (57.1%)children were reported to have fair sleep and six (42.9%) woke more than five times overnight. In cubicles, two of five (40.0%) children were reported to have good sleep and one (20.0%)woke more than five times.Nursing observations were reported as a contributor to sleep disturbance in 10 of 14 (71.4%)children in bays and four of five (80.0%) in cubicles.Pain contributed to sleep disturbance in 10 of 19 (52.6%) children overall. Most respondents reported that the ward was quiet overnight and that lights were kept low. However, only seven of 19 (36.8%) families reported being asked about their child's sleep needs.Parent/carer sleep was also frequently disrupted. ConclusionsPostoperative sleep disruption was common, with nursing observations and pain among the most frequently reported contributors.Despite generally favourable perceptions of overnight noise and lighting, opportunities for improvement were identified in assessment of individual sleep needs, coordination of overnight care where clinically appropriate, and optimisation of pain management.A subsequent audit cycle following implementation of these measures will assess their impact on sleep-related outcomes.
Background Diabetes self-management requires consistent daily behaviors. Social jetlag, the mismatch between workday and free-day sleep timing, may disrupt daily behavior routines. This study examined the association between social jetlag and self-management in adults with type 2 diabetes (T2D), and whether sleep quality and depressive symptoms statistically accounted for portions of this association. Methods This cross-sectional study recruited employed adults aged 18–60 years with T2D from the endocrinology departments of three tertiary public general hospitals in Ningxia Hui Autonomous Region, China, from January to April 2026. Social jetlag was calculated from the micro Munich Chronotype Questionnaire; sleep quality, depressive symptoms, and diabetes self-management were assessed using the Pittsburgh Sleep Quality Index, Patient Health Questionnaire-9, and Summary of Diabetes Self-Care Activities, respectively. Multivariable linear regression and serial mediation analysis with 5,000 bootstrap resamples were performed. Results A total of 339 participants were analyzed. Each additional h of social jetlag was associated with higher PSQI score ( B = 1.352, 95% CI 1.042 to 1.663), higher PHQ-9 score ( B = 3.772, 95% CI 3.314 to 4.229), and lower SDSCA score ( B = −6.623, 95% CI −7.566 to −5.679). The direct association remained significant after PSQI and PHQ-9 scores were included ( B = −3.830, 95% CI −5.048 to −2.611). The specific indirect association through depressive symptoms alone was −2.230 (95% bootstrap CI −3.053 to −1.469), and the serial indirect association through sleep quality and depressive symptoms was −0.314 (95% bootstrap CI −0.534 to −0.150). Conclusion Greater social jetlag was associated with poorer diabetes self-management. Significant indirect associations were observed through depressive symptoms alone and through the serial pathway from sleep quality to depressive symptoms. These associations do not establish temporal ordering or causal mediation because the data were cross-sectional.
Introduction: Polysomnography technologists perform overnight sleep studies requiring continuous patient monitoring, technical management of diagnostic equipment, and sustained attention.Although night-shift work has been associated with adverse health outcomes among healthcare workers, the occupational health burden among polysomnography technologists remains poorly characterized in India. This study evaluated sleep quality, excessive daytime sleepiness, burnout-related symptoms, musculoskeletal symptoms, and selected cardiometabolic indicators and examined their associations with years of night-shift work experience. Methods: This cross-sectional observational study used an online structured questionnaire distributed through professional groups to polysomnography technologists working in private hospitals and sleep centers in South India.Participants were graduates with at least six months of night-shift experience.Of 196 responses, 16 were excluded for less than six months of night-shift experience and 30 for incomplete questionnaires, leaving 150 participants for analysis. The questionnaire included the Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), Copenhagen Burnout Inventory (CBI), and musculoskeletal assessment.Height and weight were measured by participants.Blood pressure was manually assessed, while fasting blood glucose and glycated hemoglobin (HbA1c) were obtained from recent laboratory reports.Poor sleep quality was defined as PSQI >5 and excessive daytime sleepiness as ESS >10.The CBI was analyzed continuously and categorized operationally as 0-49, 50-74, and 75-100.Years of night-shift work experience were analyzed as a continuous exposure and categorized as ≤5 versus >5 years.Multivariable logistic regression was adjusted for age, sex, and BMI.Results: Mean age was 29.97 ± 4.43 years, and BMI was 24.16 ± 3.19 kg/m².Mean PSQI, ESS, and CBI scores were 7.09 ± 2.95, 9.20 ± 3.39, and 45.99 ± 20.95, respectively.Poor sleep quality occurred in 97 (64.7%), excessive daytime sleepiness in 56 (37.3%), and musculoskeletal symptoms in 126 (84.0%).CBI scores were categorized as 0-49 in 86 (57.3%), 50-74 in 48 (32.0%), and 75-100 in 16 (10.7%).Hypertension-range blood pressure was identified in 21 (14.0%) and prediabetes in 11 (7.3%).Each additional year of night-shift work experience was associated with higher odds of poor sleep quality (adjusted odds ratio (aOR): 1.433, 95% CI: 1.118-1.835),excessive daytime sleepiness (aOR: 1.205, 95% CI: 1.001-1.450),CBI ≥50 (aOR: 1.810, 95% CI: 1.350-2.428),musculoskeletal symptoms (aOR: 1.372, 95% CI: 1.003-1.878),lower-back symptoms (aOR: 1.296, 95% CI: 1.051-1.598),neck symptoms (aOR: 1.272, 95% CI: 1.043-1.552),and hypertension-range blood pressure (aOR: 1.387, 95% CI: 1.052-1.829).No significant associations were observed with shoulder symptoms or prediabetes. Conclusion: Polysomnography technologists reported frequent poor sleep quality, excessive daytime sleepiness, burnout-related symptoms, and musculoskeletal symptoms.More years of night-shift work experience were associated with several adverse health outcomes after adjustment for age, sex, and BMI.These findings should be interpreted cautiously given the cross-sectional design and potential residual confounding.Prospective multicenter studies are warranted.

Abstract Introduction: Sleep disturbance is common among hospitalized patients and may adversely affect recovery and well-being. This study aimed to determine the proportion of patients hospitalized for at least 3 days, with unsatisfactory sleep, describe perceived sleep disruptors, and assess their association with socio-demographic, clinical and hospital-related factors. Materials and Methods: A hospital-based descriptive cross-sectional study was conducted among patients admitted to five randomly selected in-patient departments of a tertiary medical college, Kolkata, from 10 April to 20 May 2026. Eligible patients admitted for at least three days were recruited. Data were collected through face-to-face interviews using a pre-designed, pre-tested schedule. Perceived sleep quality and perceived sleep disruptors were assessed using 5-point Likert scales. Participants reporting fair, poor or very poor sleep were categorised as having unsatisfactory sleep. Associations were assessed using Chi-square/Fisher's exact test and Mann–Whitney U test, with p<0.05 considered significant. Results: A total of 183 patients were included in the final analysis. The median age was 40 years (IQR: 26–46.5), 50.8% were male, and median length of stay was 7 days (IQR: 4–13.5). Unsatisfactory sleep was reported by 101 (55.2%) participants, including 36 (19.7%) with fair, 39 (21.3%) with poor and 26 (14.2%) with very poor sleep. Acute medical conditions related to the current admission were the most commonly reported significant disruptor of sleep (77, 42.1%), followed by pre-existing medical conditions (45, 24.6%), noise (40, 21.9%) and light (38, 20.8%). Unsatisfactory sleep was significantly associated with perceived disruption from noise (p<0.001), light (p=0.006), frequent vital-sign monitoring (p=0.016), personal care routines (p=0.037), existing medical conditions (p=0.033), and acute medical conditions (p=0.010). Age, sex, department of admission, diagnosis and length of stay showed no significant association with sleep quality. Conclusion: More than half of hospitalized patients experienced unsatisfactory sleep. Clinical conditions and modifiable hospital-related factors, particularly noise, light and interruptions related to routine care, were associated with poorer sleep. Measures to minimize unnecessary nighttime disturbances may improve sleep among hospitalized patients.
Insomnia disorder (ID) is a common sleep-wake disorder marked by persistent dissatisfaction with sleep quality or quantity. Previous studies have shown that sleep plays a critical role in emotion regulation, particularly in fear extinction. This study aims to characterize ID using estimates of the sympathetic arousal state derived from skin conductance (SC)-based observations, and to compare two observation models: the marked point process (MPP) model and the MPP coupled with continuous (MPP+Cont) model. We utilize SC data from a fear conditioning and extinction procedure involving 24 individuals with ID and 24 good sleepers (GS). The SC measurements are deconvolved to extract autonomic nervous system (ANS) activations and the tonic component. The MPP observation model uses only the extracted ANS activations, while the MPP+Cont model incorporates both ANS activations and the tonic component. Both models are employed to estimate the underlying arousal state over time and compute a high arousal index (HAI). We compare the estimated arousal states and HAI between the ID and GS groups to assess alterations in arousal associated with ID. Our results reveal that ID may be associated with impaired fear extinction learning and recall. Notably, the distinction between ID and GS emerges only under the MPP+Cont model, indicating the critical role of the tonic component in arousal estimation. Overall, this study highlights the potential of arousal state estimation for understanding autonomic dysregulation in ID, thereby supporting interventions for fear-centered comorbidities and improving quality of life.
This study investigates the relationships among sleep quality, social inclusion, and psychological resilience in older adults and examines whether social inclusion moderates the association between sleep quality and psychological resilience. Grounded in the Main Effect Model, social inclusion is conceptualized as a key psychosocial resource in later life. A cross-sectional study was conducted among 455 older adults in Türkiye. Data were collected using the Pittsburgh Sleep Quality Index, the Social Inclusion Scale, and the Psychological Hardiness Scale. Hierarchical multiple regression and moderation analyses were performed while controlling for demographic variables. Findings revealed a strong positive association between social inclusion and psychological resilience (r = .664, p < .001) and a negative association between sleep quality scores and resilience (r = –.305, p < .001). In regression analyses, social inclusion emerged as the strongest predictor of resilience (β = .60, p < .001), whereas sleep quality did not make a significant independent contribution. The interaction between sleep quality and social inclusion was not significant. The results highlight social inclusion as a primary psychosocial factor supporting resilience in later life and underscore the importance of strengthening social participation to promote psychological well-being among older adults.
Background: Sleep disturbances impact outcomes in major depressive disorder (MDD). This cross-sectional study evaluated subjective sleep quality and habits in adolescents with MDD and their associations with suicide attempt (SA) and non-suicidal self-injury (NSSI).Methods: Sixty-eight adolescents aged 13–18 years with MDD were classified as NSSI-only, SA-only, and combined. Clinical severity and depressive symptoms were measured with Clinical Global Impression– Severity scale and Children’s Depression Rating Scale–Revised (CDRS-R). Self-harm and suicidality were evaluated using Inventory of Statements About Self-Injury and Columbia-Suicide Severity Rating Scale. Pittsburgh Sleep Quality Index (PSQI) and Children’s Sleep Habits Questionnaire (CSHQ) assessed past-month sleep quality and past-week sleep habits. Results: Participants had poor sleep quality (89.7%) and disturbed sleep habits (83.8%), without group differences. CSHQ scores correlated moderately with lifetime NSSI frequency (r = .36, P = .003), whereas PSQI scores correlated weakly (r = .28, P = .023). PSQI scores were modestly related to past-month suicidal ideation severity (r = .28, P = .022). In hierarchical regressions controlling for sedative medications and CDRS-R scores, CSHQ (β = 0.299, P = .022) and PSQI (β = 0.248, P = .047) accounted for incremental variance in lifetime NSSI frequency. Conclusion: Sleep problems are common among adolescents with MDD, suggesting a clinical feature rather than a subgroup-specific trait. Sleep disturbance was associated with lifetime NSSI burden beyond depression severity, indicating a clinically relevant assessment domain. Prospective longitudinal studies should clarify temporal and bidirectional relationships with self-harm outcomes.
Inventory of Statements About Self-Injury و Columbia-Suicide Severity Rating Scale ارزیابی گردید.

Sleep disturbance was associated with lifetime NSSI burden beyond depression severity, indicating a clinically relevant assessment domain. Prospective longitudinal studies should clarify temporal and bidirectional relationships with self-harm outcomes.
Inventory of Statements About Self-Injury و Columbia-Suicide Severity Rating Scale ارزیابی گردید.
Background: Classic psychedelics, including LSD, psilocybin, and N,N-dimethyltryptamine (DMT)/ ayahuasca, have shown promise as rapid-acting treatments for psychiatric disorders. Because sleep plays a central role in emotional regulation and neuroplasticity, psychedelic-induced changes in sleep architecture are of interest. Methods: A systematic review was conducted following PRISMA guidelines. PubMed was searched for preclinical and clinical studies published up to May 1, 2026, investigating psychedelics and sleep-related outcomes. Twelve eligible studies evaluated LSD, psilocybin, or DMT/ayahuasca and assessed sleep architecture, sleep quality, electroencephalographic activity, or dreams. Study validity was assessed using appropriate methodological tools. Results: Preclinical studies consistently demonstrated acute REM sleep suppression, delayed REM onset, increased wakefulness, and transient disruption of non-REM sleep. Human findings were more heterogeneous, although randomized trials reported prolonged REM latency after psilocybin and reduced REM duration after ayahuasca. Evidence regarding slow-wave activity (SWA) was mixed, with ayahuasca increasing early-cycle SWA and psilocybin reducing absolute delta power during the first sleep cycle. Subjective sleep quality showed little acute change in controlled trials, whereas one large naturalistic study reported reductions in sleep disturbances following psilocybin. Administration close to bedtime appeared more disruptive than daytime dosing. In general, quality assessments indicated fair-to-poor validity and notable risk of bias. Conclusions: Classic psychedelics produce acute but transient alterations in sleep physiology, particularly affecting REM sleep. Whether these changes contribute to therapeutic effects remains uncertain. Larger, well-controlled studies using standardized polysomnographic methods are needed to clarify the relationship between psychedelic-induced sleep modulation and clinical outcomes. This review was not prospectively registered.
Anxiety is a psychological condition that college students may experience, particularly when they face exams that require clinical skills, such as the Objective Structured Clinical Examination (OSCE). Increasing anxiety leading up to the OSCE can affect students’ sleep quality. The purpose of this study was to analyze the relationship between anxiety levels and sleep quality among nursing students at Muhammadiyah University of Semarang leading up to the OSCE. This study was conducted as a cross-sectional study with random sampling. The sample consisted of 100 undergraduate students in the Nursing program at Muhammadiyah University of Semarang who were scheduled to take the OSCE. Data were collected via a questionnaire assessing anxiety levels and sleep quality. The data were analyzed univariately and bivariately using the chi-square test and Spearman’s rank correlation. The results showed that the majority of respondents experienced high anxiety (50 individuals, 50%), while the sleep quality of most respondents fell into the “poor” category (38 individuals, 38%). The results of the Spearman’s rank correlation test showed a correlation coefficient (r) = 0.509 with a p-value (p < 0.001), indicating a significant moderate-strength relationship between anxiety levels and sleep quality among students prior to the OSCE exam.
BACKGROUND: Poor sleep quality is a global health concern among nurses, particularly in high-intensity settings. Unpredictable workloads, extended shifts and high patient acuity compromise sleep quality, negatively affecting nurses' well-being and patient outcomes. AIM: To describe the perceived sleep quality of nurses working in high-intensity settings. STUDY DESIGN: A qualitative descriptive design was used. Purposive sampling was used to select participants who met the inclusion criteria. Data were collected through three semi-structured focus group discussions with 21 nurses working in high-intensity units of an academic hospital in Gauteng, South Africa. Thematic analysis was used to analyse data manually, and the COREQ guidelines were followed. FINDINGS: Three major themes emerged: barriers to quality sleep, consequences of poor sleep and coping mechanisms. Participants described environmental, occupational, organisational and domestic barriers to quality sleep, including community noise, long working hours, staff shortages, inadequate resources and family responsibilities. Poor sleep was associated with emotional exhaustion, impaired concentration and perceived patient safety risks. Participants reported using pharmacological and non-pharmacological strategies to manage fatigue and sleep difficulties. CONCLUSIONS: This study highlights that sleep disruption among nurses working in high-intensity settings is shaped by environmental, occupational, organisational, community and family-related factors. Poor sleep was perceived to be associated with emotional exhaustion, impaired concentration, patient safety concerns and reliance on adaptive and maladaptive coping strategies to manage fatigue and insomnia. These findings suggest that sleep disruption is a systemic issue requiring organisational and context-sensitive interventions to improve nurses' well-being and patient care outcomes. RELEVANCE TO CLINICAL PRACTICE: Improving nurses' well-being and patient safety requires interventions that address the organisational, environmental and social contributors to sleep disruption. Supportive practice environments, adequate staffing, fatigue management initiatives and psychosocial support may improve sleep quality, reduce emotional exhaustion and clinical errors and enhance nurse well-being and patient care outcomes.

Chronic insomnia frequently co-exists with nocturia, contributing to impaired sleep quality and reduced health-related quality of life. A recent randomized cross‑over study found that daridorexant improved sleep duration, insomnia severity, daytime functioning, and nocturnal voiding in patients with chronic insomnia and nocturia. To complement these quantitative outcomes, the study incorporated qualitative free‑text surveys capturing patients’ lived experiences, addressing an existing gap in patient‑reported insights for this multifaceted condition. Here, we report these qualitative findings assessed at baseline and post‑treatment. This double-blind, placebo-controlled, two-way crossover trial of patients with insomnia disorder and comorbid nocturia included 60 adults ≥ 55 years with ≥ 3 months of insomnia symptoms, an Insomnia Severity Index ≥ 13, and ≥ 3 nocturnal voids per night. Patients were randomized to daridorexant 50 mg or placebo for 4 weeks, followed by crossover after a washout period. Free‑text surveys at baseline and after each treatment period captured patient‑reported impacts of their conditions on daily life and perceived changes during treatment. At baseline, patients reported broad negative impacts across physical, emotional, cognitive, and daily functioning domains. During treatment, more patients reported improvements in sleep (87% vs. 50%) and nocturia (79% vs. 32%) on daridorexant than on placebo, and improvements in functioning, particularly physical, cognitive, emotional, and daily activities. Patients reported meaningful improvements with daridorexant 50 mg, including better sleep, reduced nocturia, and enhanced physical, cognitive, and emotional functioning. Daridorexant consistently elicited more positive patient‑reported changes than placebo. These findings highlight the value of free‑text surveys for capturing patient‑centered outcomes. NCT05597020.
Primary insomnia is a common sleep–wake disorder characterized by impaired sleep initiation and maintenance with associated daytime dysfunction, while current pharmacological treatments are often limited by safety and tolerability concerns. This study examined the independent and combined effects of a sensory-mediated non-pharmacological intervention using Viola odorata L. and an oral botanical preparation derived from Papaver somniferum L. seeds in adults with primary insomnia. This exploratory, prospective, randomized, three-arm, component-based integrative trial enrolled 90 participants aged 18–65 years, randomized equally to Viola odorata –based Natūl therapy, Papaver somniferum seed powder, or combination therapy and treated for four weeks. Sleep outcomes—including Insomnia Severity Index (ISI), Pittsburgh Sleep Quality Index (PSQI), daytime dysfunction (VAS), and number of nocturnal awakenings—were assessed at baseline and post-intervention and analysed using a per-protocol framework with non-parametric methods. Recurrence rate has been assessed after three months of treatment. All three interventions were associated with significant improvements across all sleep parameters (all p < 0.001), with large within-group effect sizes (r ≈ 0.86 – 0.89). Between-group comparisons of change scores demonstrated significantly greater improvements with combination therapy compared with either monotherapy (η 2 ≈ 0.21–0.26). Viola odorata –based Natūl monotherapy produced improvements comparable to oral Papaver somniferum therapy for most outcomes. No clinically meaningful safety concerns were observed. Sensory-mediated therapy using Viola odorata L. and oral administration of Papaver somniferum L. seeds are effective in the management of primary insomnia, with combination therapy providing additive sustained with less recurrence rate and supporting a graded, personalized integrative treatment approach.
Postpartum women constitute a vulnerable population in whom insomnia comorbid with emotional disorders such as anxiety and depression is disproportionately prevalent, attributable to hormonal fluctuations, environmental changes, and social role transitions. This comorbidity significantly increases their long-term risk of mental health disorders. Given the safety concerns of sedative-hypnotic medications, non-pharmacological therapies are the first-line treatment for this population. We report the case of a 34-year-old woman with chronic postpartum insomnia comorbid with emotional disorders, who provided written informed consent for the publication of her de-identified clinical data and received 4 weeks of combined acupuncture and transcutaneous auricular vagus nerve stimulation (taVNS) treatment. Clinical assessments were performed at baseline, week 2, week 4 (end of treatment), and week 8 (4-week post-treatment follow-up) using validated scales including the PSQI, ISI, and HAMA, among others. After 4 weeks of treatment, the patient demonstrated significant improvements in objective sleep quality and anxiety symptoms, while subjective insomnia perception and depressive symptoms remained partially unresolved. Notably, all clinical outcomes continued to improve to near-normal levels during the follow-up period without any additional intervention, and no adverse events were recorded. This case demonstrates that combined acupuncture and taVNS is a safe and effective intervention for chronic postpartum insomnia comorbid with emotional disorders, with a favorable sustained therapeutic effect after treatment cessation, offering a valuable alternative for non-pharmacological management in this population.
Background and Objective The baseline sleep phenotype of early Alzheimer’s disease (AD) patients entering lecanemab therapy is poorly characterized. Methods Forty-four consecutive early AD patients (mild cognitive impairment or mild dementia) awaiting lecanemab underwent attended polysomnography, the Epworth Sleepiness Scale, Insomnia Severity Index (ISI), Pittsburgh Sleep Quality Index (PSQI), and Beck Depression Inventory (BDI). Forty-four controls without documented cognitive disorder, referred for polysomnography by the study’s sleep specialist over the same period, were balanced on sex, age and apnea–hypopnea index. Comparisons used independent-samples tests with Benjamini–Hochberg false discovery rate correction within pre-specified families. Results Obstructive sleep apnea was prevalent (61.4% moderate-to-severe) despite a body mass index of 22.5 kg/m2; groups were balanced (all p>0.15). Early AD patients showed greater wake after sleep onset (114.0 min vs. 78.7 min; q=0.006), lower sleep efficiency (67.7% vs. 77.2%; q=0.006) and shorter total sleep time (284.0 min vs. 321.1 min; q=0.039) with comparable recording time, and lower REM arousal index (13.9/h vs. 21.3/h; q=0.012). Differences were moderate (|g| 0.49–0.72); sleep-stage distribution and total arousal index did not differ. ISI (3.8 vs. 13.8) and PSQI (4.1 vs. 10.0) were nonetheless lower in AD (both q<0.001; |g| 1.52–1.86); BDI did not differ. Conclusions Early AD patients awaiting lecanemab therapy had a high prevalence of sleep apnea and reduced objective sleep continuity despite reporting fewer subjective sleep complaints than controls, a contrast that may partly reflect the symptomatic sleep-clinic comparator. Objective sleep assessment before initiating lecanemab therapy may therefore be considered.
Background Evidence regarding whether exercise timing differentially affects sleep quality and mood remains inconsistent. This study examined the effects of acute and regular exercise performed at different times of day on sleep and mood. Methods Eight English and Chinese databases were searched from inception to June 2026. Parallel and crossover randomized trials were synthesized using three-level meta-analysis with non-diagonal sampling variance-covariance matrices. Exercise vs. Control and direct Time vs. Time comparisons were analyzed separately, with CR2 cluster-robust inference used as a robustness check. Results Thirteen studies contributed 81 effect sizes (40 sleep and 41 mood). For sleep, Exercise vs. Control showed a large effect in the conventional model (g = 0.86, 95% CI [0.26, 1.46], p = 0.005), but CR2 inference was non-significant ( p = 0.109). Direct Time vs. Time comparisons were non-significant (g = 0.15, 95% CI [−0.08, 0.39], p = 0.209; CR2 p = 0.127), with no stable advantage for morning, afternoon, or evening exercise. For mood, Exercise vs. Control was large but non-significant (g = 0.95, p = 0.056). Time vs. Time was significant conventionally (g = 0.30, p = 0.027) but not under CR2 inference ( p = 0.097). Afternoon vs. Evening favored afternoon exercise (g = 0.50, p = 0.001), although based on only two studies. Conclusion Exercise itself may be more important than its specific timing for sleep, whereas mood may show timing-related signals that remain insufficiently robust for definitive recommendations.
Objectives: Given the limited pediatric-specific evidence in this area, we aimed to examine the association between federated-level gymnastics participation and sleep quality and sleep timing in adolescents. The global Pittsburgh Sleep Quality Index (PSQI) score was defined as the primary outcome, sleep-timing measures as secondary outcomes, and individual PSQI component analyses as exploratory. Methods: This observational, cross-sectional comparative study included adolescents aged 12 to 16 years participating in federated gymnastics and controls who did not participate in federated sports.Demographic variables included age, sex, and parental educational level, while training-related variables included weekly training duration, number of training sessions, and training schedule.Sleep quality was assessed using the PSQI, and bedtime, wake-up time, sleep duration, and sleep latency were derived from the corresponding PSQI items.Multiple linear regression models included age, sex, maternal educational level, and paternal educational level as covariates. Results: The study included 60 gymnasts and 37 controls, with baseline differences in age, sex distribution, and parental educational level between groups.Gymnasts trained a median of 12.0 hours/week (Q1-Q3: 8.25 to 20.0) over five sessions/week (Q1-Q3: 4 to 6), with a median training-session end time of 20:30 (range: 18:00 to 22:00).In adjusted analyses, gymnast status was associated with later bedtime (B=0.392hours, 95% CI 0.043 to 0.740, p=0.028), whereas no statistically significant association was observed between study group and global PSQI score (B=0.311,95% CI -0.720 to 1.343, p=0.550).Increasing age was associated with later bedtime (B=0.231hours, p=0.004) and shorter sleep duration (B=-0.398hours, 95% CI -0.560 to -0.236, p<0.001).Among gymnasts, training-session end time was not associated with global PSQI score (B=0.141,95% CI -0.344 to 0.625, p=0.563;R²=0.006).Overall, 19/97 participants (19.6%) met the conventional PSQI >5 threshold for poor sleep quality, although an optimal adolescent-specific clinical cutoff has not been definitively established. Conclusion: No statistically significant difference in adjusted global PSQI score was detected between federated adolescent gymnasts and controls.Gymnast status was associated with later bedtime.Approximately one in five participants met the conventional PSQI >5 threshold for poor sleep quality.Given the cross-sectional design, these findings should be interpreted as associations, and larger longitudinal studies incorporating objective sleep measures are warranted.
Objectives This study aimed to describe shift work patterns in Europe using a data-driven approach and evaluate their impact on adverse sleep outcomes. Methods We analyzed 6245 participants from the SHIFT2HEALTH survey across eight European countries, who provided information on work schedules, shift work metrics, sleep quality and timing (MCTQshift) across different shifts, and chronic insomnia (Insomnia Severity Index). We used mixtures of von Mises-Fisher models to identify shift clusters based on shift start and end times and evaluated differences in shift-specific sleep duration and quality using Bayesian multilevel models. Associations of shift metrics with short average sleep duration and chronic insomnia, adjusting for confounders, were evaluated using Bayesian logistic regression. Results We identified five shift clusters: day, evening and night (mean shift lengths: 7.8, 7.9 and 9.5 hours, respectively), long day and long evening (mean lengths: 10.5 and 12.8 hours, respectively). Sleep duration and quality varied between clusters, with the shortest duration [mean 6.00, 95% credible interval (CI) 5.95–6.06 hours] and poorest quality (29.3% “poor”/“very poor”) sleep reported between night shifts. Compared to day workers, greater chronic insomnia odd ratios (OR) were observed among participants with >90% night shifts (OR 1.26, 95% CI 1.01–1.58), rotating shifts (OR 1.42, 95% CI 1.22–1.65) and with longer night work history (OR ≥16 years 1.28, 95% CI 1.08–1.52). These variables and longer weekly working hours [OR 6.10h increase 1.13, 95% CI 1.04–1.22) were associated with short sleep. Conclusions Data-driven analyses of work schedules may provide a more comprehensive evaluation of the impact of real-world shift work patterns on sleep.
